The homeowner has verified in writing to CDPHE that this is their prixry residence (III.E.2.) Due to a planned remodel it removal of asbestos containing materials was necessary. A total of 384 SF of plaster of asbestos containing plaster was removed. All abatement activities were conducted within a secondary containment (enclosure). In addition, one negative pressure air filtration unit equipped with IJEPA filters was installed within the containutent and exhausted filtered air from the interior of the work area to the exterior. The unit operated continuously from the time of barrier construction throughout the acceptable final clearance method. Versatile Environmental, LIX conducted a post abatement visual inspection and final air clearance following the reinoval of the asbestos containing materials. Final clearance activities were performed in accordance with the Environmental Protection Agency (EPA) and Colorado Department of Public Health and Environment (CDPHE) regulations. Final clearance activities were conducted by a CDPI-11's certified Air Monitoring Specialist (AMS). Removal and disposal of the ACM was performed by A.P.R,Inc. FIELD ACTIVITIES Visual InsL)ecfi2n Prior to collecting air samples, for clearance a post abatement visual inspection was conducted to verify the asbestos containing material was removed as specified and that the containment was free from visible dust and debris. With oifly critical barriers in place and the HEPA air filtration its still in operation, the air monitoring specialist (AMS) conducted a walk-through inspection of the regulated work area. Careful examinations of all areas within the containment where debris and water could accumulate and be concealed, were thoroughly inspected. If any area had visible dust or debris, the contractor was required to perform the necessaryte-cleaning until the area met the visual criteria. Once the containment met the visual criteria final air clearance was performed. Aggressive Method Once the containmentn=et the visual criteria the next step in final clearance was performed. Final air clearance sampling was conducted using aggressive techniques as outlined in Appendix A of the EPA Asbestos Hazard Emergency Response Act (Affl-,RA) regulation (40 CFR Part 763), and as referenced in C17111HE Reg No. 8 Part B Asbestos. Aggressive sampling consists of agitating the air using a leaf blower five (5) minutes for every 1,000 square feet of floor space and one(]) stationary box fan per 10,000 cubic feet of volume in containment. For this work area one (1) box fan was strategically placed within the containment and the leaf blower was used approximately five (5) minutes. Air Sampligg Following the aggressive protocol activities, five (5) air samples, were then collected throughout the containment using variable high volume pumps. Flow rates of each pump were established and checked with a calibrated flow meter before and after the collection of each sample using a to. tameter (secondary standard) traceable to a primary standard. Air samples were collected by drawing air at a rate of 15.6 liters per minute (LPM) through 25nim mixed cellulose ester membrane (Millipore 0.35 MCEF) filter, housed in three-piece cassettes and equipped with 50mm, electrically Versatile Environmenta(LLC Page 2 3550 S, Ilmlair &. #132 Denver, CO 80.735 7.10-234-5010 bit (tx tt versaenA, cora C Versatile E"nvironmental LLC 2015 conductive extension cowls.. pre and post calibration of each sample purrip was documented and an average flow rate was calculated and used in the final analysis of each sample collected. Calculations used in the final analysis combines the average flow rate (LPM), the total sample time, and the volume of air collected. PCM Ana Lysis All air samples collected were analyzed by Air Quality Analysis, LLC using the Phase Contrast Microscopy (PCM) N10SH 7400 Method. Samples were analyzed fear airborne fibers using a positive Phase -Contrast microscope equipped with a Walton -Beckett graticule (type 6-22 for "A counting Rules"). Only fibers greater than 5 microns in length with a length -width (as t) ratio equal to or greater than 3:1 were counted. The resulting concentrations were expressed in to of total fibers per cubic centimeter of air (f/cc). Final Clearance Criteria An abatement action is considered complete when the results of samples collected within the abatement work area and analyzed by PCM using the NIOSH Method 7400 show that the concentration of fibers for each of the live samples collected is equal to or less than. 0.01, Fee. The is the level the EPA and the Air Quality Control Conumssion (CL PHE) requires all samples to he equal to or less than in order to have an area released to the public. All final air samples collected liar this project met the clearance criteria, and the containment can be dismntled and the area returned Liar norinal occupancy. CONCLUSION Based on visual inspections; documentation review-, and final air sample results-; it is the professional opinion of Versatile Environmental, LLC, that the air quality within the regulated area following the asbestos abatement activities is in compliance with all regulatory and industry standards. Versatile Environmental, LLC assumes no liability for any loss, injury, and or claim for damages arising directly or indirectly from any use or reliance on this report or the opinions expressed lit -rein. The inspection was limited to those areas that were visually and physically accessible. The inspection, was not a risk assessment, physical inspection of systems, structure, or components of the subject property, as it relates to their serviceability. The inspection was performed in accordance with the Standards of Practice of the Colorado Regulation 8 Part B in effect at the time of this Inspection. This inspection is not intended to be technically exhaustive. The Inspector has prepared this written asbestos Inspection Report for the sole use and benefit of the client. The asbestos inspection report shall identify, report, and make recommendations for future evaluation. Client agrees to read the entire asbestos inspection report when received and shall promptly call the inspector with questions or concerns regarding the report. 71'he use of this report and info nnation contained herein by others, in whole or in part, is not authorized without the written mutual consent of the client and Versatile Environmental. Purpose and Scope Tbe purpose of this inspection was to conduct a visual asbestos inspection and to collect property in the affected areas only. The scope of this report included a visual inspection of all of ected rooms an, d/or areas throughout the subject property for visible suspect ACM (asbestos containing materials). This does not include any materials that were covered or hidden by walls, structures,, carpeting, wood flooring, multiple layers, wallpaper, texturing, etc. However, the inspector made every reasonable effort to access and identify all suspect ACM. Bulk asbestos samples were randomly collected from homogenous areas of suspect ACM by the above named Inspector, an PA/ State of Colorado Certified Asbestos Inspector. Homogenous areas are defined as any area with the same material, texture, and color. If any of these variables differs between materials or if for any reason the inspector feels the materials might be different in any respect (such as materials being installed at different times even though they appear similar), separate sets of samples must betaken from each homogenous area. Representative bulk samples were taken from similar materials but this does not guarantee that materials of similar appearance are of the same composition. Even similar materials are subject to uneven mixing and distribution and random sampling can miss ACM. Versatile Environmental used reasonable diligence and its professional judgment consistent with industry standards in order to identify all suspect ACM. Performance of this asbestos inspection is intended to reduce, but not eliminate, uncertainty regarding the presence of ACM in connection with the above referenced property. No guarantee is expr I essed or implied that all ACM was identified in the inspection. Therefore, Versatile 21812016 Versatile Environmental LLC Page 2 3550,5, Harlan A #132 Denver, CO 80235 720-234-5010 billy@,versaenv,com CC) Versatile Envirvnmenteal 11C 016 or information that remained unknown, or areas that were inaccessible at the time of the inspection. guidelines established by the US EPA (40 CFR Part 763, Subpart F, Appendix A). Asbestos concentrations were visually estimated and/or point counted and reported in percent for each layer of the sample. The assessment was executed in accordance with the authorized scope of work. If the scope of work is changed, the inspector must be informed and an addendum made to this report. This report is solely a record of activities, observations, analytical results, and recommendations performed to date. Regulatory Review EPA and the Colorado Department of Public Health and Enviromn, cut (CDPIIE) regulate all building materials that contain greater than 1% asbestos as determined through PLM Analysis. OSIIA regulates all building materials that contain smaller amounts of asbestos (even trace concentration as determined by PLM analysis). EPA and CDPIJE Regulation No. 8, Part B, requires that suspect ACM in buildings built prior to 1988 be assumed to be asbestos or an inspection be conducted prior to demolition or remodeling activities that could disturb ACM. however, with the North American Free Trade Agreement in effect and trade with countries such as Canada, Mexico, and China, it can never be detennined that a material is not asbestos containing (even if it was produced after 198 8) unless it is tested by a certified state inspector. The EPA and Cl P11E require all friable and non-fiiable ACMs in the affected area(s) be removed prior to renovation activities that may disturb the ACM, and all friable ACM must be removed prior to demolition. Removal or management of regulated, ACM is only required if they may be damaged or disturbed during remodeling, demolition, or burning. In some circumstances, "point co t" 'analysis isrequir W- for bulk samples previously analyzed by PLM. Point counting is a more detailed means of analysis that standard PLK Federal and State agencies define ACM as materials containing greater than I% asbestos. The SII SHAP regulation requires that if standard PLM analysis detennines that a sample contains less than 10% asbestos, the material must be, considered asbestos containing or be point counted. Even if the, sample is less than. 1% by standard PLM, the material either has to be assumed to be ACM or point counted. If the point counting analysis is different than the PLM analysis, the point counting results take precedence over the PLM. If standard PLM analysis determines that a material has no asbestos or that the material contains greater than 10% asbestos, point, counting is not necessary. 2/812016 Versatile Environmental LLC Page 3 3550 S, Mirlan St. 1#132 Denver, CO 80235 7,70-234-5010 Call, 41versaenvxom CO Versatile Environmental LLC 2016 Rachgrougd The in called to thesubiect nroh& bi t4c orneowmer ir-w-lig, 6, kho was i i WN planned remodel in both side of the duplex.
