applicable building codes, and all applicable municipal codes, policies and procedures, and that I am the legal owner or have been authorized
by the legal owner of the property and am authorized to obtain this permit and perform the work described and approved in conjunction with
this�permrt. I further attest that I am leg ally authorized to include all entities named within this document as parties to the work to be
performed and that all work to be performed is disclosed in this document and/or its' accompanying approved plans and specifications.

1, This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180, days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and��procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all requ�ired
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, anviolation of any provision of any
�pplicabje code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

curb and gutter adjacent to West 38th Avenue. On May 4, 2005, the Community Development
Department received a check from you in the amount of $3,150.00. With the receipt of this escrow,

Dear Ray:
I am the electrical engineer responsible for the design of the electrical instaliation at the
Wheatridge Dental Clinic at 10135 West 38th Avenue.
I have received copies of your Correction Notice dated April 20, 2005 for the project, and I
strongly disagree with your interpretation of NEC ArtiCle 517 wlth reSpect to a requirement
for ground fault interrupting (GFI) receptacles for patient chairs in the treatment rooms of
this dental clinic. Sy the very definition of "wet locations" in NEC Paragraph 517.2 (which
you referenced) and by any practical interpretation, the patient treatment rooms cannot be
Classified as "wet locations".
NEC Paragraph 517.2 defines wet locations aS "locations that are normally subject to wet
conditions while patients are presenY". Furthermore it defines these locations where water
or other fluids are "normaily standing on the floor or drenching the work area". These
definitions clearly do not apply to dental patient treatment areas. The definition concludes
by saying that "routine housekeeping procedures and incidental spillage of liquids do not
define a wet location", a statement that specifically excludes the dental treatment areas
from consideration as wet locations.
Since the dental treatment areas are not wet locations, the GFI requirement of NEC
Paragraph 517.20 (Wet Locations) do not apply to the treatment areas.
In the course of my twenty-seven years of experience in electrical engineering for
construction I have provided electrical drawings for hundreds of similar dental clinics. The
patient treatment areas have never been considered wet locations, and should not be
considered wet focations by any reasonable interpretation.
Please let me know if you have any comments or if you would like to discuss the issue
further.

Dear Ms. Manwaring,
I have been working with Travis Crane in the Wheat Ridge pianning department over the
past several months to have the water main extended on to the Denning property as per the
agreement with the City of Wheat Ridge executed in June 1999. As you are aware the City was
to install an 8 inch diameter water main into the Denning Property a minimum distance of 10
feet and this has not been completed to date. Travis Crane has informed me you will be
handling the installation of the water main and you needed a written request to have the City
install the water main as per the agreement. I have enclosed a copy of the executed
agreement for your convenience.

I am the electrical engineer responsible for the design of the electrical installation at the
Wheatridge Dental Clinic at 10135 West 38th Avenue.
I have received copies of your Correction Notice dated April 20, 2005 for the project, and I
strongly disagree with your interpretation of NEC Article 517 with respect to a requirement
for ground fault interrupting (GFI) receptacles for patient chairs in the treatment rooms of
this dental clinic. By the very definition of "wet locations" in NEC Paragraph 517.2 (which
you referenced) and by any practical interpretation, the patient treatment rooms cannot be
classified as "wet locations".
NEC Paragraph 517.2 defines wet locations as "locations that are normally subject to wet
conditions while patients are presenY". Furthermore it defines these locations where water
or other fluids are "normally standing on the floor or drenching the work area". These
definitions clearly do not apply to dental patient treatment areas. The definition concludes
by saying that "routine housekeeping procedures and incidental spillage of liquids do not
define a wet location", a statement that specifically excludes the dental treatment areas
from consideration as wet locations.
Since the dental treatment areas are not wet locations, the GFI requirement of NEC
Paragraph 517.20 (Wet Locations) do not apply to the treatment areas.
In the course of my twenty-seven years of experience in electrical engineering for
construction I have provided electrical drawings for hundreds of similar dental clinics. The
patient treatment areas have never been considered wet locations, and should not be
considered wet locations by any reasonable interpretation.

1 inch to twenty feet (1:20) which illustrates the fol3owing:

C. Location and extent of areas to be filled and/or excavated.
D. I,ocation of existing waterways and drainage courses indicating any changes. (for a site containing an
established irrigation ditch, a]etter of approval from the appropriate ditch company shall be required).

F. Location and extent of exisUng vegetation, proposed changes in such vegetation and methods of
rehabilitation on site vegetation afrer earthwork operations are complete.

intervals of 2 feet and spot elevations.

site shall have been completed in accordance with the approved plans. Rehabilitation shall consist of
leveling, grading, landscaping or any combination thereof to minimize potentia] erosion and be acceptable
to the City Engineer.
B. Debris: Debris ancllor contaminanu shall not be used except with the approvai of the City Engineer.
"For the purpose of this Section, the term debris shall have the same meaning as the terms "gazbage, trash
or junk" as defined in the Wheat Ridge Code of Laws, Section 15-4."
In no case shall debris or contaminates identified or classified as hazardous waste by local, state or federal
agencies be used as fill in any fill areas within the City of VJheat Ridge.
C. Excavation and/or fill areas shall be gaded to facilitate weed control until final gades are set and site
rehabilitalion and use occur (must be leveled with side slopes not to exceed 3 to 1).

Re: Plan review for WHEAT RIDGE DENTAL, 10135 West 38th Ave.
The following are requirements of the Fire District made under the 1997
Uniform Fire Code as adopted by the city council of the city of Wheat
Ridge, Colorado.
1. Fire hydrants shall be tested and in service prior to combustible
construction.
2. Strobe lights in the treatment area shall be in sync.
3. Knox box location shall be located outside the main entrance.
4. Fire alarm circuit breakers shall be mark, indicating they are for the fire
alarm system.
5. Means of egress shall be maintained at all times of construction.
6. Two means of access to the job site shall be maintained at all times of
construction.
7. Full system test to be completed with the Fire District present during
testing for final sign off.
8. Fire lanes shall be signed and painted according to code. ( Lanes to
include the area next to the islands on the south and north of the parking
lot.)
