2. This.permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180, days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and��procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Contract Terms: Properkey will do the above stated work for $ _7%4 .3 aj USD or insurance proceeds. This is only an estimate
and any insurance provider adjustments to the scope of work will supersede this estimated amount. This proposal may be withdrawn if not
accepted within days. By signing this contract the owner agrees to authorize ProperKey to negotiate with owners insurance company
on all matters pertaining to roof repairs and replacement. Owner agrees to allow ProperKey to perform all roofing repairs and replacement
per insurance companies specifications and pricing. Payment due in full upon completion of job unless prior arrangements have been made.
Partial payment is required upon material delivery. All insurance proceeds due upon receipt including all approved supplements in addition
to the contracts price above and made payable to ProperKey. Properkey shall hold in trust any payments received from you until roofing
materials have been delivered at the site or Properkey has performed a majority of the work on your property. You have the right to re-
scind this contract and obtain a full refund of any deposit within 72 hours after entering the contract. If you plan to use the proceeds of a
property and casualty insurance policy to pay for the roofing work, you may rescind this contract within 72 hours after you receive written
notice from the property and casualty insurer that your claim has been denied in whole or in part. However, Properkey is entitled to retain
payments or deposits to compensate Properkey for roofing work actually performed in a workmanlike manner consistent with standard
roofing industry practices. Properkey cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible
applicable to the insurance claim for payment for roofing work on your property. Property Owner is / is not intending to
make payment from the proceeds of a property and casualty insurance policy. All work is scheduled to be completed within 90 d ays. Sub-
ject to delays beyond our control, scheduling, materials, labor and weather conditions. This proposal becomes a legal and binding contract
from the date of acceptance. Workmanship warranty is not in effect until payment is received in full and does not include weather related
damage. Properkey is not responsible for property damage from delivery or removal of materials, including concrete or asphalt driveways.
Interior damage will not be covered unless it was inspected prior to starting the job. Reasonable attorney fees, and all collection fees may
be charged on accounts past due over 30 days. Failure to pay may result in a mechanics lien being filed against the property. Homeowner
acknowledges that Properkey will be entitled to 10% Overhead and 10% Profit as allowed by Insurance Industry Standards.

This agreement (the "Contract") is between Properkey ("Company") and the custonmer(s) identified on the front side of this Agreement
("Customer.") The teens and conditions contained on the front side of this contract are expressly made part of hereof and Company and Customer
hereby agree upon and to be bound by the follomming additional terms and conditions: and will be subject to all appropriate la%N. regulations and
ordinances of the State of Colorado and the local jurisdiction and the following- terms and conditions.
MECHANIC'S LIEN NOTICE: Properkey is required by law to provide Customer with NOTICE.
(A) ANY PERSON OR COMPANY SUPPLYING LABOR OR MATERIALS FOR THIS IMPROVEMENT TO YOUR PROPERTY
MAY FILE A LIEN AGAINST YOUR PROPERTY. IF THAT PERSON OR COMPANY IS NOT PAID FOR THE CONTRIBU-
TIONS. (B) UNDER COLORADO LAW, YOU HAVE THE RIGHT TO PAY PERSONS WHO SUPPLIED LABOR OR MATERI-
ALS FOR THIS IMPROVEMENT DIRECTLY AND DEDUCT THIS AMOUNT FROM OUR CONTRACT PRICE, OR WITH-
HOLD THE AMOUNTS DUE IMPROVEMENT DIRECTLY AND DEDUCT THIS AM OUNT FROM OUR CONTRACT PRICE,
OR WITHHOLD THE AMOUNTS DUE THEM FROM US UNTIL 120 DAYS AFTER COMPLETION OF THE IMPROVEMENT
UNLESS WE GIVE YOU A LIEN WAIVER SIGNED BY PERSONS WHO SUPPLIED ANY LABOR OR MATERIAL FOR THE
IMPROVEMENTS AND WHO GAVE YOU TIMELY NOTICE.
1. This Contract is binding upon the Customer and Properkey upon execution, subject to: (1) Approval by Customer's Insurance Company.
2. Customer agrees and promises to pay to Properkey all reasonable and documented costs and expenses, including reasonable attorney's
fees, paid or incurred or to be paid or incurred by Properkey in collecting the sums due and owing under this Contract, whether or not
legal suit is filed with respect thereto and whether or not such costs and expenses are paid or incurred, or to be paid or incurred, prior
to or after entry of judgment.
3. Properkey shall have no responsibility for damages arising out of or related to fire, rain, tornado, windstorm, or other acts of God, as
are normally contemplated to be covered by Homeowners Risk Insurance or Business Risk Insurance, unless specifically agreed to by
Properkey prior to commencement of work.
4. In the event of any default made by Customer in the payment of this Contract, Customer agrees that interest shall be added on the
unpaid balance from the date of default AT A RATE OF ONE AND ONE HALF PERCENT PER MONTH ( 18% PER ANNUM) OR
MAXIMUM AMOUNT ALLOWED BY APPLICABLE LAW.
5. Properkey is not responsible for ice dams (thawing and refreezing of ice, water or snow) or any other damage on or below the roof line
due to water leaks by winds of 55 MPH or greater.
6. Any and all warranties, guarantees or representations made by Properkey hereunder or non -transferable by Customer.
7. If the Customer cancels the Contract after the work has begun by Properkey the Customer shall pay Properkey the full value of the
work done as of the date of cancellation, plus the total material costs incurred by Properkey.
8. This Contract is composed of the front and backsides of this document and shall be considered the entire agreement
9. Properkey is not responsible for and undetwkes no duty, expressed or implied, to evaluating the Customer's home venting and circula-
tion system status. Properkey will replace or install vents and soffit according to Customer's work order.
10. Properkey is not responsible for the realignment of satellite dish and will take every reasonable effort to reinstall it in the same or simi-
lar condition. Customer bears full responsibility for re -aligning, unless agreed otherwise in writing by Properkey.
11. Wherever possible, each provision of the Contract will be interpreted so that it is valid under the applicable Law. If any provision of
this Contract is to any extent invalid under the applicable Law, that provision will still be effective to the extent it remains valid. The
remainder of the Contract also will continue to be valid, and the entire Contract will continue to be valid in other jurisdictions.
12. The Customer may cancel this Contract by notifying Properkey in writing within 3 days of Customer's execution of the Contract. If the
Contract is canceled by the Customer at any time after 3 days from the date of Customer's execution, Customer shall pay to Properkey
an amount of $1,000.00 as liquidated damages not as a penalty, and Properkey agrees to accept such payment as a reasonable and just
compensation for Customer's cancellation of the Contract.
13. Notwithstanding Section 11 hereof, if material has to be reordered or restocked by Properkey due to cancellation of the Contract by the
Customer, Customer shall be responsible for a pay a restocking fee equal to 20% of the total Contract price.
14. Any upgrade or additional work requested by the Customer that is not approved by the insurance provider will be the Customer's fi-
nancial responsibility and will become part of the Contract.
15. During the duration of the work performed, Properkey will take all reasonable efforts to protect the Customer's property from further
damage. Properkey shall have no responsibility for interior damage and Customer or Customer's insurance provider shall be responsi-
ble for any interior damage that may have occurred or may occur.
16. To the fullest extent permitted by law, and notwithstanding any other provision of this Agreement, the total liability of Properkey to
Customer and anyone claiming by though, or under Customer for any and all claims, losses, costs or damages whatsoever arising out
of, resulting for or in any way related to the Contract or the work performed by Properkey, from any cause or causes, including but not
limited to tort, negligence, strict liability, breach of contract, or breach of warranty expressed or implied by Properkey, shall not ex-
ceed the total compensation received by Properkey under this Contract.
17. Properkey has the right to order excess materials. These materials will not be charged above the agreed upon Contract price and
terms.
18. This Agreement contains the entire Agreement between Properkey and Customer and supersedes all previous and contemporaneous
oral negotiations, commitments, writings and understandings between the parties concerning the matters in this Contract. ANY REP-

20. The Homeox%ner acknowledges that Properkey has been retained as the Homeowner's General Contractor based on their expertise and
experience in the construction industry. The Homeowner understands and acknowledges that Properkey, as a general contractor, is entitled
to 10% overhead and 10% profit for the work performed on behalf of the homeowner.
21. Replacement of deteriorated decking. fascia boards, roof jackets. %entilators. flashing or am other materials. unless otherwise expressly
stated in this Contract. are not included in the price and terms herein and N% ill be charged as additional expenses on a time and material basis.
22. All direct costs, overhead and profit and/or supplement amounts paid to the Customer by the insurance provider are part of the negoti-

Monday through Friday to process these types of requests.
