2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3, If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TERMS:
Mountain Roofing LLC proposes to furnish all materials and labor in accordance with the specifications below for the Initial sum of: REPLACEMENT COST
$ 8018.37 (INSURANCE PROCEEDS) plus any supplements to your claim. All work to be completed in a substantial, workmanlike manner per
manufacturer's Installation instructions and in accordance with local building codes.
Upon approval of your insurance claim, Mountain Roofing LLC will file for any and all supplements which may include permits, building code items, unforeseen items and missed
or omitted Items on the scope of work provided to you by your insurance company. These items will be paid per your policy provisions and will become part of this contact as if
contained herein. Mountain Roofing LLC as your general contractor requests a fair and equitable settlement agreeable to both your insurance company and Mountain Roofing
LLC based on competitive market rates for labor and materials with no additional costs to you except your deductible, plus any upgrades or additional work authorized by you
that are not part of your approved insurance claim.
Colorado law does not permit Mountain Roofing LLC to pay, waive or rebate any insurance deductible. Should your Insurance company deny a full roof replacement, you will
not be obligated to this contract and may enter Into an acceptable repair agreement with Mountain Roofing LLC. You, the buyer, may rescind this contract and receive a refund
of your deposit anytime within 72 hours after execution of this contract. You may also rescind this contract within 72 hours after you receive written notice that the Insurance
claim has been denied. Mountain Roofing LLC shall hold in trust any payments until the roofing contractor has delivered materials to the job site or has performed a
majority of the work. Mountain Roofing LLC maintains workers comprehensive and liability coverage through its agent Mountain Insurance Brokers 303-420-4774.
Mountain Roofing LLC will not be held liable for any structural movement, settling, cracks in drywall, driveway, or damage to siding during the loading process. All interior damage to
buildings is excluded 45 days after completion date of the project Any damage to the interior of the building or its contents will be the responsibility of the owner and the insurance
company. Finance charge of 1.5% per month will be added to any unpaid balance over 30 days. Failure to pay may result in a mechanics lien being filed against the property. In the
event that legal or collection actions are taken, Mountain Roofing LLC is entitled to reasonable attorney's fees and costs. All refunds will be made available 10 days after written

Enclosed are four copies of our survey of the lot line between 4205 and 4185
Balsam St. As you know we had previously surveyed this line in May of 2006.
Mrs. George, who was our client at that time asked that we remove the survey
pins set on that Survey.
The survey pins set on this survey mark the same line as the 2006 survey. The
east pin is at the same location as in 2006, the west pin was a one foot offset on
the 2006 survey since the old fence comer prevented us from marking the true
corner. However the new fence allowed my survey crew to mark the actual
corner.
My 2006 court testimony regarding my determination of the location of the line,
the use of prorating or proportioning the excess within the area shown on plat of
the Wheatridge View would still apply to this survey.
