Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

Roof Restoration Agreement
The Owner(s) listed above hereby jointly and severally agree to hire LifeTime Exteriors LLC, a Colorado LLC (herein referred to as
"LifeTime Exteriors"), to contract with owner(s) to install at owners' property (stated above) by LifeTime Exteriors as follows:
Section A. Scope of work authorized by Owner for LifeTime Exteriors to perform:
Remove existing shingles to original decking. Install a new Tamco Heritage laminated shingle roofing system with 301b underlayment,
drip edge, new step flashing, new pipe vent flashings, flashing around chimney, and other accessories in accordance with local building
codes. Remove, replace, repair misc. collateral damage from hail.
Total materials and labor: $6677.00 PLUS APPROVED SUPPLEMENTS
LifeTime Exteriors shall remove all debris and trash created by work that LifeTime Exteriors personnel create during course of removal
and replacement of owners' roof, and/or gutters.
All work to be completed according to local building codes.
LifeTime Exteriors, LLC shall hold in trust any payment received from homeowner until LifeTime Exteriors, LLC has delivered
roofing materials at the site or has performed a majority of the roofing work on homeowners' property.
LifeTime Exteriors' liability coverage insurer is Western Pacific and can be contacted through Ahbe Group 7167 S. Alton Wy., Centennial,
CO 80112.
You have the right to rescind this contract and obtain a full refund of any deposit within 72 hours after entering the contract. If you plan to use the
proceeds of a property and casualty insurance policy to pay for the roofing work, you may rescind this contract within 72 hours after you receive written
notice from the property and casualty insurer that your claim has been denied in whole or in part. However, LifeTime Exteriors, LLC is entitled to retain
payments or deposits to compensate LifeTime Exteriors, LLC for roofing work actually performed in a workmanlike manner consistent with standard
roofing industry practices. LifeTime Exteriors, LLC cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible
applicable to the insurance claim for payment for roofing work on your property.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

Jones Devish Consulting Engineers met with Jesse on Friday, March 23, 2007
to review the framing in the kitchen-living room Area. There are four items
that need to be corrected in'order to be code compliant. These items are as
follows.
1) The beam that supports the roof load from the small lean-to
addition that was added on the West elevation several years ago will
need to be replaced.
A) Replace the existing beam with (2) 1 3/4"x5 1/2" X1.9E
LVL members. This beam will bear on a single trimmer on both
ends.
B) The nailing pattern between the sloping rafters and the
ceiling rafters need to have (3) 16D nails @ each end. The
short vertical post that attaches to the short column that
bears on the beam and supports the roof rafters will also need
(3) 16d nails between the column and rafter. The end bearing
on the beam should be toe nailed on each side.
2) The beam that supports the roof and floor load of the original
house will also need to be replaced. This beam is located next to the
previous beam and does not carry the roof load from the lean-to
addition.
A) Replace the existing beam with (2) 1 3/4' x 7 1/2" X 1.9E

Narrative-IR ON 022103, WHILE ON ROUTINE PATROL, I. OBSERVED SEVERAL CODE VIOLATIONS AT THE
PROPERTY LOCATED AT 3917 SHERIDAN BLVD. I ATTEMPTED TO CONTACT THE RESIDENT. NO
ONE ANSWERED THE DOOR. IT APPEARED THE HOUSE WAS VACANT. I OBSERVED SEVERAL
PILES OF CONSTRUCTION DEBRIS LAYING ON THE PROPERTY. I OBSERVED A SIGN ON THE
FRONT WINDOW OF THE RESIDENCE THAT SAID, "IN CASE OF EMERGENCY OR TO PURCHASE
CONTACT KATHY WERKMEISTER 303-458-3058". I ATTEMPTED TO CONTACT WERKMEISTER. NO
ONE ANSWERED THE TELEPHONE. I CONTACTED BOB GASPAR, BUILDING INSPECTION, AND
ASKED IF THE PROPERTY HAD BEEN ISSUED A BUILDING PERMIT. GASPAR ADVISED ME THE
PROPERTY WAS LAST ISSUED A PERMIT IN 2000. I EXPLAINED TO GASPAR THE BUILDING
APPEARED TO BE VACANT AND IN DISREPAIR. GASPAR STATED HE WOULD CHECK THE
PROPERTY THIS AFTERNOON.
Narrative-Supplemental 1 ON 022203, I RESPONDED TO THE RESIDENCE TO ATTEMPT CONTACT WITH
SOMEONE RESPONSIBLE FOR THE PROPERTY. UPON ARRIVAL, I OBSERVED TWO VEHICLES IN
THE DRIVEWAY. I CONTACTED S1 DWAYNE WAYMIRE AND S2 JESSE WAYMIRE, THE PROPERTY
OWNER. I ASKED DWAYNE WHAT WAS GOING ON AT THE PROPERTY. DWAYNE STATED THEY

WERE REPLACING CARPET AND OTHER ITEMS IN THE RESIDENCE. DWAYNE STATED SOMETIMES
HE AND JESSE STAY OVERNIGHT AT THE RESIDENCE. JESSE STATED BE WAS STAYING AT THE
RESIDENCE. DWAYNE INVITED ME INSIDE THE RESIDENCE. AS I ENTERED THE RESIDENCE,
IT APPEARED AS THOUGH THE RESIDENCE WAS BEING REMODELED. THE FLOORS WERE BARE
AND A LARGE AMOUNT OF CONSTRUCTION DEBRIS WAS LAYING ON THE FLOORS. I DID NOT GO
UPSTAIRS. THE MAIN FLOOR OF THE RESIDENCE LOOKED AS THOUGH IT WAS UNINHABITABLE.
DWAYNE STATED HE AND JESSE WERE ONLY REPLACING THE CARPET AND DRYWALL. DWAYNE
STATED HE WAS ALSO REPLACING SOME OF THE STUCCO. I ASKED DWAYNE IF HE HAD
OBTAINED A BUILDING PERMIT FOR RENOVATING THE RESIDENCE. DWAYNE STATED HE DID
NOT NEED ONE.. DWAYNE STATED JESSE WAS A HVAC PROFESSIONAL AND KNEW WHAT HE WAS
DOING. I ADVISED DWAYNE BOB GASPAR FROM THE CITY'S BUILDING DEPARTMENT HAD
VISITED THE RESIDENCE YESTERDAY. I ADVISED DWAYNE I WOULD MEET WITH GASPAR TO
DISCUSS THE RESIDENCE. I ADVISED DWAYNE THE DEBRIS AND CONSTRUCTION MATERIAL ON
THE OUTSIDE OF THE PROPERTY NEEDED TO BE DISPOSED OF BY 022903. JESSE STATED HE
WOULD HAVE A DUMPSTER PLACED ON THE PROPERTY BY NEXT WEEKEND. I WILL MEET WITH
GASPAR ON MY RETURN TO WORK NEXT WEEK TO DISCUSS THE BUILDING PERMIT NEEDED, IF
ANY, AND THE LIVING CONDITIONS IN THE RESIDENCE.
Narrative-Supplemental 2 ON 030603 I RECHECKED THE PROPERTY. THE PILES OF TRASH, DEBRIS
AND LITTER HAD BEEN REMOVED FROM THE PROPERTY. THE PROPERTY WAS IN COMPLIANCE.

(5) Contractor shall notify the Building lnspector twenty four (24) hours in advance for all inspections and shall receive written approval on inspection card before
