Dear Customer
As we try to build our customer base and ensure your complete satisfaction on every project
United Test and Balance would like to extend a 180 Day warranty policy for every project The
terms of the warranty are listed below:
United Test and Balance does hereby warrant the before said completed project for a period of
180 Days from the date of the issued Certified Test and Balance Report. That said work shall
remain free from all defects in workmanship and material, and that it shall comply with all the
specific requirements of the Specifications and other Contract Documents governing the
Testing, Adjusting, and Balancing.
It is understood and agreed upon that in the event of defects and the necessary of making repairs,
the Owner and/or Contractor will immediately notify United Test and Balance in writing of its
conditions and shall give the contractor reasonable time, 30 days, in which to make said repairs.
If any person, firm, or corporation other than United Test and Balance has, since the completion
of the above work, performed or attempted to perform any repairs to the Mechanical Systems
then this warranty will become null and void This warranty does not cover any repairs made by
anyone other than United Test and Balance or one of its authorized representatives.

Building One reviewed the Building Envelope at the Outlook Clear Creek property, specifically at the 4050
Clear Creek Drive address. The weather resistive barrier was observed at all exterior wall assemblies.
Building One initially noted some inconsistencies, but it appears that all our concerns have been addressed.
It is our understanding that the exterior wall assemblies are in general conformance with the construction
documents and industry standards.

Dear Mrs. Grady,
Per our contract, our office reviewed tendon elongation reports dated May 13, 2020 (Revised May 26, 2020) and
May 30, 2020 provided by Cole Garner Geotechnical for the above referenced project. The reports indicate that
the special inspector from Cole Garner observed as the tendons were stressed using the following equipment:
Ram No. PT Puller #206 & Jack & Associates #302
Gauge No. #206 McDaniels, 10,000 PSI, 4" & #302 McDaniel, 10,000 PSI, 4"
Pump No. SPX Power Team, #206, S/N 371136 & Enerpac, #302, S/N 1403
The equipment #206 was last calibrated on February 14, 2020 and equipment #302 was last calibrated on May 15,
2020. The corresponding stressing gauge pressure is 5,385 & 5,460 pounds per square inch respectively when
applying 33 kips of tension to the tendon. A copy of the associated calibration curve was provided to our office
for our records and review.
Reports dated May 13, 2020 (Revised May 26, 2020) was for all of the tendons in the pour. After review of this
report tendons #6A, #7A, #713, #7C, #17A, #23A, #23B, #23C, #25A, #25B, #28A, #29A, #29B, #30A, #30B,
#31A, #31B, #34A, #41B, #43A, #43B, #46A, #49A, #58A, #7113, #73A, #76B, #77A, #78A, #78B, #80A, #80B,
#81 A, #81 B, #82A, #82B & #83A did not meet the code allowed minus 10% deviation and needs to be re -
stressed. Report dated May 30, 2020 was the elongation report for the tendons that needed to be re -stressed.
Based on our review of the stressing records, calibration curve, PTI allowances of +/- 1/4" elongations for short
tendons, and the knowledge of the measurements taken to the nearest 1/8", it is our opinion that all the tendons
included in these reports have been adequately stressed and can be burnt off to a distance of 3/4" inside the
exterior surface of the concrete. Stressing pockets should be packed with grout as outlined in the contract
documents.
