Dear Jordan,
On behalf of Harris Kocher Smith, I, Rachel C. Patton, a Colorado Licensed Professional Engineer, PE
#54450, am providing this letter to certify that the grading constructed for the Outlook Clear Creek
multi -family project Building 5 was constructed in general conformance with the approved civil
construction documents for the project. This certification is based on the review of the as -built survey
information provided by Harris Kocher Smith, Aaron Murphy, registered professional land surveyor in
the State of Colorado, PLS #38162. Below is a presentation of grading comments:
Building 5 - The general intent of the grading plan was followed with slight adjustments for field
conditions. The finished floor elevation for Building 5 was built within construction tolerances. The
landscaped grades around the perimeter of the building slope away from the building in accordance
with the geotechnical and IBC requirements. The paved areas adjacent to the building also slope
away from the building in accordance with the approved paving plan. The landscape drains and
associated pipes have been constructed around the perimeter of the building as well in general
conformance with the approved plans.
Setback Distances -The setback distances for all buildings discussed above were generally constructed
in accordance with the approved construction documents.
The best of my knowledge and belief the above mentioned buildings, perimeter grading and
perimeter drainage systems have been constructed in accordance with the approved construction
documents. Based on my review of the as -built survey information available as of the date of this
letter these buildings generally comply with the design intent and the as -built grading will not
negatively impact the surrounding properties if properly maintained.

Dear Customer,
As we try to build our customer base and ensure your complete satisfaction on every project
United Test and Balance would like to extend a 180 Day warranty policy for every project The
terms of the warranty are listed below:
United Test and Balance does hereby w arrant the before said completed project for a period of
180 Days from the date of the issued Certified Test and Balance Report Thatsaid w ork shall
remain free from all defects in workmanship and material, and that it shall comply with all the
specific requirements of the Specifications and other Contract Documents governing the
Testing, Adjusting, and Balancing.
It is understood and agreed upon that in the event of defects and the necessity of making repairs,
the Owner and/or Contractor w ill immediately notify United Test and Balance in writing of its
conditions and shall give the contractor reasonable time, 30 days, in which to makes aid repairs.
If any person, firm, or corporation other than United Test and Balance has, since the completion
of the above work, performed or attempted to perform any repairs to the Mechanical Systems
then this warranty will become null and void. This warranty does not cover any repairs made by
anyone other than United Test and Balance or one of its authorized representatives.

All roughs to be done at Framing Inspection.
No C.O. may be issued until public improvements are complete as identified in the
Subdivision Improvement Agreement.
All landscaping shall be installed in accordance with the approved landscape plan (Case No.
WZ-19-01) prior to C.O. and pursuant to the phasing identified in the Subdivision

applicable building codes, and all applicable municipal codes, policies and procedures, and that I am the legal owner or have been authorized
by the legal owner of the property and am authorized to obtain this permit and perform the work described and approved in conjunction with
this�permrt. I further attest that I am leg ally authorized to include all entities named within this document as parties to the work to be
performed and that all work to be performed is disclosed in this document and/or its' accompanying approved plans and specifications.

1, This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180, days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and��procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all requ�ired
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.

the special inspector from Cole Garner observed as the tendons were stressed using the following
equipment:

Gauge No. #206 McDaniels, 10,000 PSI, 4"
Pump No. SPX Power Team, #206, S/N 371136
The equipment was last calibrated on February 14, 2020. The corresponding stressing gauge pressure is
5,385 pounds per square inch when applying 33 kips of tension to the tendon. A copy of the associated
calibration curve was provided to our office for our records and review.
Based on our review of the stressing records tendons #7C, #8A, #8B, #12B, #16C, #21B, #33A, #33B,
#34A, #76B, & #78A did not meet the code allowed minus 10% deviation and needs to be re -stressed.
Please re -stress the tendons indicated above as outlined in the contract documents and provide our office
with the re -stressing report. It is our opinion that all other tendons have been adequately stressed and
can be burnt off to a distance of 3/4" inside the exterior surface of the concrete. Stressing pockets
should be packed with grout as outlined in the contract documents.

Per our contract, our office reviewed tendon elongation report dated July 6, 2020 provided by Cole
Garner Geotechnical for the above referenced project. The reports indicate that the special inspector
from Cole Garner observed as the tendons were stressed using the following equipment:
Ram No. PT Jack & Accessories #302
Gauge No. #302 McDaniels, 10,000 PSI, 4"
Pump No. Enerpac, #302, S/N 1403
The equipment was last calibrated on May 15, 2020. The corresponding stressing gauge pressure is
5,460 pounds per square inch when applying 33 kips of tension to the tendon. A copy of the associated
calibration curve was provided to our office for our records and review.
Based on our review of the stressing records, calibration curve, PTI allowances of+/- 1/4" elongations
for short tendons, and the knowledge of measurements taken to the nearest 1/8", it is our opinion that the
tendons have been adequately stressed and can be burnt off to a distance of 3/4" inside the exterior
surface of the concrete. Stressing pockets should be packed with grout as outlined in the contract
documents.
