The garage ceiling and roof will be removed for planned addition. The limited asbestos inspection included bulk sampling of suspect asbestos -containing building material (ACBM) in the garage (ceiling) and the mud room attic insulation. MTS transported and submitted samples to EMSL Laboratory for analysis by Polarized Light Microscopy (PLM) methods on a 24-hour rush analysis time, to identify whether the suspect material contains asbestos. Sampling and Analytical Procedures for this inspection and assessment was performed in accordance with the Environmental Protection Agency (EPA) and Asbestos Hazardous Emergency Response Act (AHERA), and the Colorado Department of Public Health & Environment (CDPHE) recommended procedures. Observations of Suspect Asbestos -Containing Materials at the Site was conducted by a Colorado Department of Public Health and Environment (CDPHE) Certified Asbestos consulting Firm & a certified Asbestos Building Inspector, Ms. Sharon Major (Certifications Attached). Some minor destructive sampling was conducted on the above referenced material locations. Two (2) homogenous areas with suspect ACBM was sampled in five (5) locations only. These procedures call for the visual inspection of all areas of concern and collection and analysis of representative bulk samples of suspect material. Random bulk samples, representative of the suspect asbestos -containing building materials (ACBM) of each homogeneous area (HA), were collected according to the guidelines published as Environmental Protection Agency (EPA) Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15 USC, Sections 2641 through 2654 and in compliance with 40 CFR, Part 763. A nonfriable material may become friable if its condition had deteriorated or has been impacted by forces that have rendered it friable. Bulk samples collected were analyzed utilizing the EPA's Method for the determination of Asbestos in Bulk Building Materials (EPA 600/R/116, July, 1993) and the McCrone Research Institute's The Asbestos Particle Atlasas methods references. Analysis of the bulk samples was performed on the "date reported," as listed in the bulk -sample analysis report. Sampling Quantities MTS collected at least the required minimum number of samples from each homogeneous area necessary to meet all regulatory requirements for the quantity of material to be disturbed. The quantities listed in this report are approximate and on-site verification of the exact quantity of each material is required. The following outlines the minimum sample quantities required per homogeneous area: Surfacing Materials (SM): up to 1,000 ft' of material requires a minimum of three (3) samples; between 1,000 ft2 and 5,000 ft' of material requires a minimum of five (5) samples; over 5,000 ft' of material requires a minimum of seven (7) samples; one (1) sample of each patch Thermal System Insulation (TSI): each homogeneous area requires a minimum of three (3) samples; at least one (1) sample must be collected from each patch; and collect enough samples sufficient to adequately assess the material and determine the asbestos content for TSI fittings such as pipe elbows or T's. This report presents the results of that investigation. The investigation was performed at the request of Kevin McAdams in order to comply with local requirements and to determine the allowable foundation construction at this site. FOUNDATION REC0M3MNDAW0NS With respect to the soil investigation performed at this site, and the intended structure as described by the client named above, the following is one or more of the allowable foundation type(s): 1. Footings A footing type of foundation system may be utilized at this site provided foundation components bear on a minimum of 2 feet of properly compacted Structural Fill Material, which in turn bears on undisturbed natural soil_ Foundation components must also bear below all organic material. Some movement of the structure on this type of foundation system is expected, but this movement is anticipated to be less than 1 inch. The maximum soil bearing capacity at this site is 2000 psf. Prior to placing concrete, all bearing surfaces must be cleared of loose material. Disturbed material may be re -compacted in place only after contacting, and with guidance from, this office. To verify the soil conditions and to verify the design bearing pressure for any footings, it is required that an open hole inspection be performed at this ske. This inspection must be performed only after the entire excavation has been dug to final grade. Though unlikely, potentially expansive clay or other unsuitable conditions may necessitate revised construction and foundation recommendations. Failure to complete the open hole inspection voids this soil investigation and may be grounds for denial of the certificate of occupancy for the proposed structure. 2. Drilled Concrete Piers This foundation may consist of a grade beam and concrete pier foundation system. These drilled piers must be a minimum of 20 feet in length and must penetrate a minimum of 10 feet into bedrock, or to refusal. These piers must be a minimum of 10 inches in diameter. The maximum allowable end bearing pressure for these piers is 25,000 psf, and there is a corresponding 2500 psf side shear value available for the portion of the pier within bedrock. The minimum dead load on these piers should be 8000 psf when based upon the pier end area. The bottom 10 feet of the pier hole mmg be grooved (shear rings) in order to anchor the bottom of the pier into the corresponding bedrock. Grooving shall be accomplished by creating ring grooves in the drilled hole, American Geotechnical Inc. Subsurface Investigation Project No. 12-185 Pg3of14 that are a maximum of 2 feet apart, using a tool which projects a minimum of 1 '/z inches out from the side of the auger drill. If water is encountered during or after drilling the piers, measure must be taken to keep the holes dry while placing concrete, or, concrete may be placed by pumping it into the very bottom of the holes and forcing the water out. Casing may be required to eliminate collapsing sidewalls or to minimize any infiltration of water. Piers must be poured as soon as possible after drilling to ensure the integrity of the drilled holes. Ensure all loose material has been removed from the pier holes prior to placing concrete. Piers must be vertically reinforced throughout their length using steel rebar, according to the following schedule: One #5/60 rebar for every 16 inches of pier circumference, or a minimum of 3 #5/60 rebars, whichever is greater. This rebar must extend into the foundation wall a minimum oft feet. There must be a minimum XX inch void space under grade beams to separate the grade beam from the soil. Note: In cases of deep bedrock., piers may ultimately be longer than the specified minimum length, due to bedrock penetration requirements. 3. Helical Piers This structure may be supported on steel helical piers. The helix on these piers must be a minimum of 8 inches in diameter. The piers must be rated for this application and be installed by a qualified contractor. These piers must extend a minimum of 15 feet into undisturbed natural soil. During installation, the rate of advancement of the piers must coincide with the dimension of the flights on the helix, so that there is minimal soil disturbance. At final placement, there must be a minimum of 3,000 foot-pounds of torque developed at the drive head. Using the above minimums, and a factor of safety of 2, each pier would have a maximum service load of 15,000 lbs. Greater installation torque yields higher service load. There must be a minimum 4 inch void space under grade beams to separate the grade beam from the soil. Note - Piers will often be longer than the specified minimum length, due to torque requirements. Ensure that the requirements contained herein are reflected in any construction plans developed for this site. The case equivalent fluid pressure at this site is 40 pc£ Pressure can result from the presence of expansive soils — soils which can exert additional pressures when wetted. When the proposed construction is an addition to an existing structure, the new foundation must be well connected to the existing foundation using rebar doweling. This connection must be comprised of a minimum of two #5 steel rebars located at the top and bottom of the new wall, as well as #4 rebars which are spaced no more than I2 inches apart in mid -wall. Epoxy must be commercial grade auto -mix epoxy, injected into and filling the pre- drilled holes in the existing foundation. These dowels must be a part of the foundation `steel' inspection for this project. In order to minimize the chances of water damage to the structure(s), an exterior perimeter drain is required for any habitable spaces below grade built on this site. This drain may daylight to an exterior outfall location or may flow into a sump pit fitted with a pump. SITE DESCRIPTION The site contains an existing structure. The vegetation consists of conventional landscaping. The site slopes to the west at an overall 5% grade. PROPOSED CONSTRUCTION It is our understanding that a garage addition is planned for construction on this site. This report is written based upon with the type of construction presented by the client. FIELD AND LABORATORY ANALYSIS A test hole was drilled in the vicinity of the proposed structure (see Site Map) using power auger and/or manual equipment. Test holes are generally drilled to a depth of 24 feet. Holes may have been drilled to different depths if restrictions exist or if needed to make bedrock determinations. In-situ soil samples were retrieved from depths appropriate to adequately anticipate foundation requirements. These samples were retrieved utilizing a 2 -inch California sampler. In-situ samples consist of relatively undisturbed soil. Additional loose samples may also have been brought to the laboratory in order to make a determination of the soil types encountered at depths other than those where in-situ samples were retrieved, or to indicate where changes in soil composition occur as a function of depth. These samples were returned to the laboratory for analysis. Laboratory analysis consists of a determination of moisture content, density, swell and consolidation potential, and particle size distribution, where possible. In order to assure valid results, non -cohesive highly granular samples may not have been tested for swell or consolidation potential. Subjective analysis of the soil conditions was also made in the field during the drilling operations. investigations performed on nearby sites may also have been considered in these results. Materials which will also minimize the transfer and/or retention of any water below the slab. Materials with a Liquid Limit below 30 and a Plasticity Index less than 15 are preferred. Slabs shall be isolated from foundation components by slip joints constructed to allow the independent movement of the slab. Stabs shall also be separated from any utility components by isolation joints. Mechanical equipment resting on slabs must be fitted with expandable / collapsible sections in order to allow movement of the slab without damage to the equipment or to the structure. Non-bearing partition walls constructed over floating slabs must be constructed with a minimum of 2 inches of float to allow for movement of the slab without damaging any part of the structure above. Float shall be built into the framing AND drywall AND trim. See the Float Diagram later in this report. All doors similarly built shall be constructed with at least a I inch gap at floor level. Door trim must also be isolated from slab movement. It is the owner's responsibility to monitor and maintain all floats and gaps as necessary. If additional information on float is needed, please contact this or any geotechnical office. Garage door trim must be isolated from floating garage slab movement. Slabs must be appropriately reinforced to resist the anticipated loads as well as the effects of the supporting soils. Floor slabs must also be scored in accordance with the American Concrete Institute (ACI) recommendations in order to control cracking of the slab due to shrinkage or other factors. These scores should be a maximum of 12 feet apart. H some movement of a slab is not acceptable, finished room areas should be constructed utilizing a structural floor system over a void space, rather than a slab. SITE PREPARATION We anticipate that additional grading may be necessary. Site preparation procedures should be performed such that the potential for accumulation and standing of water around the site is minimized. Clay soils should be kept from desiccating (drying and shrinking) by sprinkling the soils occasionally. Drainage swales or channels should be created where necessary in order to route water away from the excavation and construction area. CP American Geotechnical Inc. Depending on the municipality, shoring of the excavation may be required. Where space is limited, site excavation may result in slopes steeper than I to 1. Where site boundaries would otherwise result in cut banks steeper than a I to 1 slope, shoring may be required. On sites of limited size or where local regulations warrant, contact the local building agency for requirements. If shoring IS required, contact this office for an engineered shoring design. Mold has become an issue in building construction. The health risks resulting from the vast majority of mold spores are minimal or non-existent, however, the debate continues and litigation on this subject is increasing. The water content of the soils at this site are not high. The use of proper drainage techniques outside of the structure, and the use of subsurface perimeter drains, should keep water from accumulating in a way which would trigger mold formation. However, the most current building practices which reduce the likelihood of mold formation, should be gathered, studied and followed. LIMITATIONS In any subsurface investigation, limited data is available from which to formulate soil descriptions and generate recommendations for foundations and related construction components. The samples taken are indicative of the subsurface materials at the time and at the location the samples were taken. Precipitation, seasonal changes, and excavating are just a few of the factors which may create changes in the composition .of the site. If conditions are encountered which are significantly different from those described in this report, contact this office before proceeding. This report does not address nor was intended to address any environmental issues, hazardous materials or toxic waste issues, or other subsurface situations or conditions other than those described within this report. The top one foot of the backfill material must be relatively impervious to aid in routing water away from the foundation. The on-site soils, exclusive of topsoil and organics, are suitable for use as backfill. First floor framing should be constructed prior to the backfill process, or the foundation walls should be supported by some other means. SURFACE DRAINAGE The area surrounding the structure should be graded so as to provide adequate drainage away from the structure. This is typically accomplished by utilizing a 10 percent (12 inches in 10 feet) slope down and away from the foundation. Low spots should be filled in order to prevent accumulation and standing of water near the structure. Surface water running toward the structures from up slope areas should be diverted around and away from the structures by means of drainage swales or other similar measures. Decorative edging should be constructed to allow water to flow through, around or under the edging and not so as to cause damming of water behind the edging. Sprinkler systems should be constructed so as to keep the area within 5 felt of the structure as dry as possible. Sprinkler heads or other irrigation components should not be placed in, nor should they release water into, the area that is within S feet of the structure. Downspouts and sill cocks must terminate outside of the backfill area of the structure and a minimum of 6 feet from the structure. Downspouts and sill cocks should discharge into splash blocks that extend well beyond the limits of the backfill. The use of Iong downspout extensions in-place of splash blocks is advisable. See the attached graphical representation of some of the above. SUBSIM ACE DRAINAGE In order to minimize the chances of water damage to the structure(s), an exterior perimeter drain is required for any habitable spaces below grade built on this site. This drain may daylight to an exterior outfall location or may flow into a sump pit fitted with a pump. if an exterior outfall location is selected, ensure that the drain opening is protected by screen. Any surplus materials left over after this contract has been completed are the property of Contractor and will be removed by same. No credit is due Owner on returns for any surplus materials because this contract is based upon a complete job. All salvage resulting from work under this contract is the property of Contractor. 7.Cleanup & Advertising. Upon completion, and after removing all debris and surplus materials, wherever possible, Contractor will leave premises in a neat, broom clean condition. Any debris consisting of dust, dirt, asphalt, or small bits of materials that settle into attics, garage areas, or any other area being worked over with open beam ceilings or no attic, is unavoidable and Contractor shall not be responsible for this cleanup. Contractor recom- mends that Owner lay out drop cloths to protect such areas wherever owner discovers debris infiltration. Owner hereby grants to Contractor the right to display signs and advertise at the job site for the period of time starting at the date of signing of this contract and continuing uninterrupted until fourteen (14) days past the date the job is completed and payment in full has been made. Contractor will inform Owner of any dry rot or other sub -roof deterioration which is concealed and is discovered during the course of the work. Contractor is not responsible to repair any such discovered deterioration and any work done by Contractor to remedy such discovered deterioration will only be done as extra work in a written change order. 9.Termites, Pests & Hazardous Substances. Owner understands that Con- tractor is not qualified or licensed as an inspector or abatement contractor for Hazardous Materials (as defined by the government), or for Pests (in- cluding Termites). Should any such hazardous substances or Pest be sus- pected to be present on the premises, it is the Owners' responsibility to arrange and pay for inspection and abatement. Contractor cannot certify or warrant your building as being free of hazardous substances or pests. id. Collection & Legal Fees.
