At that time, the subject area of foundation was found to have experienced historic downward displacement. Recommendations to install helical piers to underpin the subject foundation area were provided in a plan set dated October 4, 2020. An observation of the repairs was performed at the subject residence on October 27, 2020. During this observation, the recommended repairs were found to have been completed in general compliance with the plan set dated October 4, 2020. This report details the fieldwork completed and analytical results of samples collected. It is our understanding that you are renovation activities at the above referenced address. Per the United States Environmental Protection Agency (EPA) procedures published in Code of Federal Regulations (CFR) Title 40 Part 763, Subpart E — Asbestos -containing Materials in Schools and Colorado Regulation 8 (Regulation 8), suspect Asbestos Containing Building Materials (ACBM) must be properly sampled and analyzed prior to any renovation or demolition activities. The purpose of this sampling was to evaluate for the presence of any ACM in the areas identified. The inspection was conducted in accordance with Regulation 8. These inspection protocols specify requirements for the inspector (Section 763.85), laboratory (Section 763.87), and number of samples collected during an inspection (Section 763.86). 1.0 FIELD ACTIVITIES The inspection was conducted by a Colorado State Certified Asbestos Building Inspector on January 10, 2019. This inspection involved identification and sampling of suspect ACM at the above referenced property. Bulk samples of suspect ACM were collected by the Inspector in general conformance with Asbestos Hazardous Emergency Response Act (AHERA) protocols as outlined in Colorado Department of Public Health and Environment (CDPHE) Regulation 8. Random samples of suspect materials were collected in each homogeneous area (see Yable 1 — Homogeneous Materials Table). Samples were placed in sealable containers and lab' led with iidgo unique sample numbers using an indelible marker. �e„;,a__ z jzol� 12s Y N R fll 2 C ENVIRONMENTAL CONSULTING 2.0 LABORATORY ANALYSIS Ten (10) samples were submitted under chain of custody procedures to Eurofins, CEI for analysis by polarized light microscopy (PLM) with dispersion staining techniques per EPA methodology (40 CFR 763, Subpart F). Visual estimations were used in obtaining the percentage of asbestos in bulk samples. Eurofins, CEI. is accredited under the National Voluntary Laboratory Accreditation Program (NVLAP) Lab Code: 101768-0. A copy of the original laboratory report is attached to this summary report. 3.0 FINDINGS Suspect ACM includes nearly all building materials except bare concrete, glass, wood, masonry, metal or rubber. ACM contains asbestos greater than 1 percent, (> 1% asbestos) as confirmed by an accredited laboratory. ACM is categorized as Friable or Non -friable asbestos. Friable asbestos can be crumbled, pulverized, or reduced to a powder by hand pressure when dry. Non - friable asbestos is any material that cannot be crumbled, pulverized or reduced to a powder by hand pressure when dry. Our Inspector observed the following suspect ACM during this inspection: Friable: • Two (2) types of Plaster • Two (2) types of Sheet Vinyl Non -friable: Not Sampled 3.1 Identified Asbestos Containing Materials Based on the completed PLM results, asbestos was not identified in the samples collected. Table 2, enclosed, contains a summary of samples and results. 4.0 CONCLUSION & RECOMMENDATION Only listed materials were tested or quantified as part of this inspection. If materials are encountered that have not been discussed in this report, stop all demolition activities and contact a Colorado Certified Asbestos Building Inspector to test those materials prior to continuing with building demolition. 14 Y N A Ill 2 C ENVIRONMENTAL CONSULTING 5.0 LIMITATIONS This asbestos inspection was conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale. Wall #1: Wall #1 is running east to west and separates the kitchen from the dining room. This wall is carrying approximately 9'-3" of roof and attic load. The length of the opening is to be approximately 7'- 91Y . The west end of the beam is to be face mounted to Beam #2 with a Simpson HU48 hanger with max nailing. The east end of the beam shall bear on a new (2) 2x4 built up column constructed per Figure 1 below with screws through one side. Beam supports are to have a continuous load path to the foundation by either being continuous or having squash blocking installed below the sub -floor to the foundation or beam below. The roof purlin kickers must be replaced and toe nailed into the top of Beam #1 after installation. Beam #2: Install a (2) 2x8 beam to replace the chimney frame out.
