On January 7", 2020, Randy Eckhart of Structel Consultants, LLC was onsite to observe the
above-mentioned residence for the placement of a new egress window. The placement of the
proposed egress window will only be extending an existing basement window opening downward
in the desire location. This existing window opening has been in place since the home was
originally constructed. Since the opening will only be extended downward and not widened, no
additional structural reinforcing is required. Once the opening is cut, the window may be trimmed
out with approved lumber and install the new window which will provide the appropriate egress
per code.
The window installed must meet the minimum code requirements:
• Minimum width of opening of 20 inches.
• Minimum height of opening of 24 inches.
• Minimum net clear opening of 5.7 square feet.
• Sill height no higher than 44 inches above the floor.
The contractor should be aware of the possibility of unknown or hidden conditions that were not
able to be observed. All conditions in this letter are assumed until actual construction begins and
the elements discussed are revealed. At that time the contractor shall contact Structel Consultants
if any condition appears to be in discrepancy of this letter and requires additional investigation. If
conditions prove to be accurate as stated, then the contractor may proceed with construction
without any further structural actions required.

the bulk sample materials submitted to this facility on 9/04 /2013.
Bulk samples are analyzed in accordance with USEPA test method EPA/600/M4-82-020 and
EPA/600/R-93/116, which requires that distinctly different materials or layers present within a
sample be analyzed and reported separately. (For measurement uncertainty, refer to table 2.1
in EPA/600/R-93/116 method.) Sometimes it is not possible to completely separate thin or
strongly adhering layers, such as paint or adhesive, and a combined result is given. For some
complex materials, such as drywall/tape/mud joints, a composite or overall asbestos content
may be reported in addition to individual layer results. RNET, Inc. is currently accredited for
bulk asbestos analysis by the National Voluntary Laboratory Accreditation Program (NVLAP) of
the National Institute of Standards and Technology (NIST). Our NVLAP laboratory code number
is 200448-0.
The analytical results provided in this report apply only to the samples actually submitted. This
report is considered confidential. Details of this report will not be discussed with any person or
agency not associated with you or your organization. This report must be reproduced in its
entirety and shall not be copied in part or used by the client to claim product endorsement by

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,

We are pleased to provide a written proposal for several plumbing repairs to the waste
and vent system in your home at 3230 Jay Street, Wheat Ridge, CO. 80033. Our bid is
based on a site inspection of the existing conditions on December 31m, 2015.
Listed below are the INCLUSIONS and EXCLUSIONS to our proposal.
THIS QUOTATION INCLUDES:
LEAKING WASTE LINE
1. Cut section of 2" waste line above the 2" ABS street 45 fittings and remove.
2. Clean joint thoroughly.
3. Re -fit new 2" ABS pipe into sink line drain and couple with 2" ABS coupling
fitting or No -hub band.
Total cost for this work will be $130.00
Code Issue — This joint was never glued and is a code violation. It probably was never
tested either which is another issue because the test would have proven it was not glued.
This is a $130.00 pipe repair.
CAP OLD WASTE LINES
1. Chip floor around the 1 /2" lead waste line that goes into the cement slab.
2. After finding a clean pipe that has not been smashed, we will cut it in two.
3. Cut out the other 1/z" lead waste line that is above the floor on the same wall.
4. Cap both lines with an approved No -hub or Mission cap.
5. Cement the floor over the new hole.
6. Test caps by running waste line.
7. Jobsite cleanup.
Total cost for repairs is $584.00
Code issue: Leaving open drains have always been a code violation. Smashing the lead
together is not a proper cap for the line.
Access cost is $$470.00. Plumbing repair cost is $114.00
FLOOR DRAIN SEALING
1. Chip concrete from old floor drain that is in the middle of the room.
2. Chip concrete from the 1 /2" bottom of the drain.
3. Install 1 '/2" expansion plug inside floor drain.
4. Test drain.
5. Seal up with concrete to top of floor drain bowl and flush with floor.
6. Jobsite clean up
Total cost of repair is $442.00
Code issue: Floor drain was not properly capped and this is a code issue for any open
waste line.
Access cost is $388.00. Plumbing repair is $54.00
WASTE LINE SUPPORT
1. Cut off old straps
2. Install new pipe support and tie to wood 2 X 4's in the proper spacing
Total cost will be $145.00
Code issue — ABS waste line was not supported properly in it horizontal run from the
washing machine drain. Workmanship and spacing are the code violations
Plumbing repair is $145.00
PROPER CLEANOUT FOR WASHING MACHINE DRAIN
1. Cut into 2" ABS waste from washer.
2. Install 2" ABS test tee on its back.
3. Install 2" MIP inside the test tee.
4. Install 2" Fitting FIP adaptor with plug (so waste line can be cleaned in both
directions).
5. Run 318" sink cable in both directions as far as possible so that line is cleared.
Currently line is partially plugged. Run water during cleaning of the line.
Total cost is $394.00
Code Issue: No proper cleanout is available to clear the waste line. Cleaning the drain
through the p -trap is not a good practice.
Access cost is $130.00. Plumbing repair is $264.00
THIS QUOTATION EXCLUDES:
1. All other work not mentioned above.
2. Repairs to any equipment.
3. Permits and inspection fees.
4. Breaking -up the bid into smaller pieces. Our pricing is based on all work being
done at the same time. Breakout prices have been provided for the insurance
carrier, per owners' request.

Mr. Simmons & Ms. St. Onge,
As you are aware, despite a final approval by the City of Wheat Ridge Building Division, an array of deficiencies
in the work performed by Devex Properties prior to your purchase of the structure has been identified. Below is a
comprehensive list of deficiencies identified during an investigative inspection conducted by the Building
Division on Friday, April 25, 2014. The Building Division believes this list to be representative of all code
deficiencies identifiable and existing at this time.
The applicable codes, which are the 2006 International Residential Code and the 2005 National Electrical Code,
state that "It shall be the duty of every person who performs work for the installation or repair of building,
structure, electrical, gas, mechanical or plumbing systems, for which the code is applicable, to comply with this
code." This requirement exists despite of any approval by the Building Division or any exemption of work from
permitting requirements. Based on applicable codes, the Building Division holds Devex Properties responsible
for correction and compliance of the below list of code deficiencies.

1) Install continuous handrail at 34-38 inches above stair nosings on stairs to basement.
2) Insure that basement exhaust fan is ducted to exterior of structure.
3) Provide inspection and recommended repair by a State of Colorado Licensed Structural Engineer for
improperly cut or notched floor joists of main floor (approximately eight(8) joists).
4) Electrical junction box in soffit in basement is required to be covered and accessible.
5) Provide sediment trap between gas line shut off valve and flexible gas connector of furnace.
6) Install electrical wire staples within 12 inches of electrical boxes and properly secure remaining electrical
wiring in the mechanical room.
7) Fire block openings in the mechanical room chase where it adjoins the floor/ceiling above and where the
combustion air ducts enter the mechanical room.
8) The low combustion air duct must be no more than 12 inches from the floor to the duct opening.
9) Seal around duct on main trunk line Oust above the water heater).
10) Install light switch cover in mechanical room.
11) Lower level bedroom closet light is required to be a minimum of 12 inches from closet shelf.
12) Air admittance valves in master bathroom vanity should extend to the underside of counter top.
13) Install a waste vent within a maximum of 5 feet downstream of toilet in master bath.
14) Secure shower head in main bathroom.
15) GFCI on left side of kitchen sink has a ground short and does not function properly during test.

inspectors inspected the basement the walls were covered and deemed to be an existing condition based on the
approved plans.
Subsequent inspections turned up numerous code violations and another permit was issued for repairs to the
basement bathroom and some electrical boxes brought flush to ceiling in the basement bedroom. From my
investigation, none of the code violations listed were for items behind drywall in the basement except for the
bedroom and bath areas.
