Dear Mr. Kunz:
Family Environmental Compliance Services, Inc. (Family Environmental) is pleased to submit
this report summarizing our limited Asbestos Containing Materials (ACM) inspection. This
report details the fieldwork completed and analytical results of samples collected. This work was
authorized by you on September 18, 2013.
It is our understanding that you are planning renovation activities at the above referenced
address. Per the United States Environmental Protection Agency (EPA) procedures published in
Code of Federal Regulations (CFR) Title 40 Part 763, Subpart E — Asbestos -containing
Materials in Schools and Colorado Regulation 8 (Regulation 8), suspect Asbestos Containing
Building Materials (ACBM) must be properly sampled and analyzed prior to any renovation or
demolition activities. The purpose of this sampling was to evaluate for the presence of any ACM
in the areas identified. The inspection was conducted in accordance with Regulation 8. These
inspection protocols specify requirements for the inspector (Section 763.85), laboratory (Section
763.87), and number of samples collected during an inspection (Section 763.86).
1.0 FIELD ACTIVITIES
The inspection was conducted by a Colorado State Certified Asbestos Building Inspector on
September 20, 2013 and on October 8, 2013. This inspection involved identification and
sampling of suspect ACM in the planned renovation area. Bulk samples of suspect ACM were
collected by the Inspector in general conformance with Asbestos Hazardous Emergency
Response Act (AHERA) protocols as outlined in Regulation 8. Random samples of suspect
materials were collected in each homogeneous area (see Table 1 - enclosure). Samples were
placed in sealable containers and labeled with unique sample numbers using an indelible marker.
2.0 LABORATORY ANALYSIS
Fifty -Four (54) samples were submitted under chain of custody procedures to River North
Environmental Testing (RNET), for analysis by polarized light microscopy (PLM) with
dispersion staining techniques per EPA methodology (40 CFR 763, Subpart F). Visual
estimations were used in obtaining the percentage of asbestos in bulk samples. RNET is
accredited under the National Voluntary Laboratory Accreditation Program (NVLAP) Lab Code:
200448-0. A copy of the original laboratory report is attached to this summary report.
3.0 FINDINGS
Suspect ACM includes nearly all building materials except bare concrete, glass, wood, masonry,
metal or rubber. ACM contains asbestos greater than 1 percent, (> 1% asbestos) as confirmed by
an accredited laboratory. ACM is categorized as Friable or Non -friable asbestos. Friable
asbestos can be crumbled, pulverized, or reduced to a powder by hand pressure when dry. Non -
friable asbestos is any material that cannot be crumbled, pulverized or reduced to a powder by
hand pressure when dry.
Our Inspector observed the following suspect ACM during this inspection:

• Light Orange Peel Surfacing Material;
• Tape Joint Compound;
• Block Filler;
• Linoleum;
• Wall Glue;
• Carpet Glue, and
• Ceiling Tile.
Non -Friable:
• Wallpaper;
• Cove Base and Mastic, and
• Floor Tile.
Table 2, enclosed, contains a summary of samples and results.
3.1 Identified Asbestos Containing Materials
Based on the completed PLM results, asbestos was identified in some of the floor tile samples
collected.

4.0 CONCLUSION & RECOMMENDATION
All identified asbestos containing materials must be removed by a Colorado state certified
General Abatement Contractor if they are to be affected during restoration activities. Final air
clearance testing must be performed according to Colorado Regulation 8 Part B: for Asbestos
following the removal of the above mentioned materials. Only materials listed were tested or
quantified as part of this inspection. If materials are encountered that have not been discussed in
this report, stop all activities in the area and contact a Colorado Certified Asbestos Inspector to
test those materials prior to continuing with activities in the general area of the materials.
5.0 LIMITATIONS
This asbestos inspection was conducted in a manner consistent with the level of care and skill
ordinarily exercised by members of the profession currently practicing under similar conditions
in the same locale. The results, findings, conclusions and/or recommendations expressed in this
report are based on conditions observed during our limited inspection of the areas in question.
The information contained in this report is relevant to the date on which this survey was
performed, and should not be relied upon to represent conditions at a later date. This limited
asbestos inspection report is not considered a comprehensive asbestos inspection report and it
must be understood that the existence of unique or concealed ACM may be possible.
This report has been prepared on behalf of and exclusively for use by the client for specific
application to the project as discussed. Contractors or consultants reviewing this report must
draw their own conclusions regarding further investigation or remediation deemed necessary.

The detection limit for the method is <1 % by visual estimation and 0.25% by 400 point counts or 0.1 %
by 1,000 point counts.
Due to the limitations of the EPA 600 Method, nonfriable organically bound materials (NOBs) such as
vinyl floor tiles can be difficult to analyze via polarizing light microscopy (PLM). EPA recommends that
all NOBs analyzed by PLM, and found not to contain asbestos, be further analyzed by Transmission
Electron Microscopy (TEM). Please note that PLM analysis of dust and soil samples for asbestos is not
covered under NVLAP accreditation.

the bulk sample materials submitted to this facility on 10/08/2013.
Bulk samples are analyzed in accordance with USEPA test method EPA/600/M4-82-020 and
EPA/600/R-93/116, which requires that distinctly different materials or layers present within a
sample be analyzed and reported separately. (For measurement uncertainty, refer to table 2.1
in EPA/600/R-93/116 method.) Sometimes it is not possible to completely separate thin or
strongly adhering layers, such as paint or adhesive, and a combined result is given. For some
complex materials, such as drywall/tape/mud joints, a composite or overall asbestos content
may be reported in addition to individual layer results. RNET, Inc. Is currently accredited for
bulk asbestos analysis by the National Voluntary Laboratory Accreditation Program (NVLAP) of
the National Institute of Standards and Technology (NIST). Our NVLAP laboratory code number
is 200448-0.
The analytical results provided in this report apply only to the samples actually submitted. This
report is considered confidential. Details of this report will not be discussed with any person or
agency not associated with you or your organization. This report must be reproduced in its
