Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

Engineer observation and letter pertaining to the masonry repairs to the single family residence at
3280 Harlan et in Wheat ridge, CO. Reference our original letter dated February 9, 2016.
DISCUSSION
Our observation, took place on March 10, 2016. The purpose of our observation was to review
the masonry repair work. At the time of our observation the concrete masonry units as well as
all of the brick veneer had been replaced, We could see the vertical rebar at the bond beam
which had not been filled. The subcontractor also supplied the general contractor with`many
pictures to veru the rebar placement as they went along with the repair. The pictures provided
ns with enough evidence to the fact that the new concrete masonry units were reinforced as per
the intent of our original letter. There was also a picture of the brick ties for the veneer
indicating its compliance. The neve wall is better than the original due to being reinforced while
the original was not.

Engineer observation and letter pertaining to the vehicle damage to the single family residence at
3280 Harlan Street in Wheat ridge, CO.
DISCUSSION
Our observation took place on February 5, 2016, The purpose of our observation was to assess
y structural member ber d age and make any appropriate :repair recommendations. The
residence is a ranch style structure of masonry and bride veneer wall construction. Accordingto
the county records, the residence was built in 1952. For the sake ofconvenience, the font of the
residence will be'assumed to face west.
The vehicle appears to have taken out the northern most overhead door and impacted the rear
(east) wall. The overall damage appears to be limited to the east., south and west walls of the two
car garage structure attached to the south end of the residence. The garage struct e is roughly
twenty feet in the north to south direction and twenty-eight feet in the east to west direction. The
vehicle impact has knocked down much of the east wall which also had a ' an door an, d single
window in it. As a result of'the damage to the east wall, there is also distress and damage in the
eastern racist flee feet or so of the south wall. The damage to the west wall appears to be limited
to the northern overhead door.The garage wall construction consists of four inch C.M.U.
masonry with brick veneer facing. The C.M.U. mebet°s do not appear to have any steel
reinforcing in the horizontal bed joints or the vertical cells. The east wall man door and window'
had steel angles files over them.. The roof framing consists of 2x6 wood rafters at two foot centers.
There is one brace under the northeast corner hip rafter which appear to kick down to the
damaged east wall. There is also another fourteen feet of patio roof which extends to the east
side of the garage.
RECOMMENDATIONS
First of all we recommend the shoring of the roof" systems un, der the d ina e bearing areas, prior
to initiating any work. We then, recommend the complete removal of the east wall of the garage
along withthe first five feet or so of"the eastern most portion of the south wall. The damaged'
wall sections shall be rebuilt like the original construction as much as possible, with the
following additions, We would also like to see the C.M.U. members reinforced with No.
Durowwal every other bed joint and #4 bar vertically at four feat centers and at all opening jam, bs
and comers. The same size angles can be used for the door and window headers. We
recommend the C.M.U. membrs be tied to the foundation wail with #4 dowels at four feat
centers, The dowels shall be embedded at least eight inches into the top of"the foundation wall
d epoxied. The trip course of the new wall shall be a band beam with a #4 bar horizontally and

3280 Harlan Street in Wheat Ridge, CO, Reference our original letter dated February 9, 2016,

Our observation took place on March 10, 2016. The purpose of our observation was to review
the masonry repair work. At the time of our observation the concrete masonry units as well as
all of the brick veneer had been replaced. We could see the vertical rebar at the bond beam
which had not been filled. The subcontractor also supplied the general contractor with many
pictures to verify the rebar placement as they went along with the repair. The pictures provided
us with enough evidence to the fact that the new concrete masonry units were reinforced as per
the intent of our original letter. There was also a picture of the brick ties for the veneer
indicating its compliance. The new wall is better than the original due to being reinforced while
the original was not.

Engineer observation and letter pertaining to the vehicle damage to the single family residence at
3280 Harlan Street in Wheat Ridge, CO.
DISCUSSION
Our observation took place on February 5, 2016. The purpose of our observation was to assess
any structural member damage and make any appropriate repair recommendations. The
residence is a ranch style structure of masonry and brick veneer wall construction. According to
the county records, the residence was built in 1952. For the sake of convenience, the front of the
residence will be assumed to face west.
The vehicle appears to have taken out the northern most overhead door and impacted the rear
(east) wall. The overall damage appears to be limited to the east, south and west walls of the two
car garage structure attached to the south end of the residence. The garage structure is roughly
twenty feet in the north to south direction and twenty-eight feet in the east to west direction. The
vehicle impact has knocked down much of the east wall which also had a man door and single
window in it. As a result of the damage to the east wall, there is also distress and damage in the
eastern most five feet or so of the south wall. The damage to the west wall appears to be limited
to the northern overhead door. The garage wall construction consists of four inch C.M.U.
masonry with brick veneer facing. The C.M.U. members do not appear to have ally steel
reinforcing in the horizontal bed joints or the vertical cells. The cast wall man door and window
had steel angles over them. The roof framing consists of 2x6 wood rafters at two foot centers.
There is one brace under the northeast comer hip rafter which appear to kick down to the
damaged east wall. There is also another fourteen feet of patio roof which extends to the east
side of the garage.
RECOMMENDATIONS
First of all we recommend the shoring of the roof system under the damage hearing areas, prior
to initiating any work. We then, recommend the complete removal of the east wall of the garage
along with the first five feet or so of the eastern most portion ofthe south all. The damaged
wall sections shall be rebuilt like the original construction as much as possible, with the
following additions. We would also like to see the C.M.U. members reinforced with No. 9
Durowal every other bed joint and #4 bar vertically at four foot centers and at all opening; jambs
and comers. The same size angles can be used for the door and window headers. We
recommend the C.M.U. members be tied to the foundation wall with 44 dowels at four foot
centers. The dowels shall be embedded at least eight inches into the top of the foundation wall
and epoxied. The top course of the new wall shall be a bond beam with a 44 bar horizontally and
one-half inch diameter anchor bolts at four foot center for the wall plate anchorage. We did not
see any roof framing member damage, but if some exists either replace or reinforce the damaged
member with a member of the same size and species as the original. All new rebar shall be of
Grade 60. All new mortar shall have a minimum compressive stress strength of 2500 l'. .1. Any
new masonry units shall match the existing.
