Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

All material is guaranteed to be as specified. All work to be completed in a workmanlike manner according to standard
practices. All interior damage to buildings is excluded 90 days after completion date and the owner is notified that any
damage to contents will be the responsibility of the owner and his insurance company. All agreements contingent upon
strikes, accidents or delays beyond our control. Owner to carry fire, tornado and other necessary insurance. Scheduling
will be done based upon date signed but limited to availability for materials, crews and weather conditions.
ACCEPTANCE OF PROPOSAL: This proposal becomes a legal binding contract 72 hours from date of acceptance.
Cancellation of this contract can be made with written notice within 72 hours of date of acceptance and will allow the
property owner to rescind the contract for roofing services and obtain a full refund of any deposit. The property owner
may also rescind this contract within 72 hours of notice from the insurance company for a claim that is denied in whole
or part if the work being contracted for is to be funded from the proceeds of a property and casualty insurance claim.
Interest at the rate of 1-1/2% per month (18% per annum) will be added on to all accounts 30 days past due. In addition,
costs and reasonable attomey's fees for collections may be charged on any account past due over 30 days. Payment
will be made as outlined above. Failure to pay can consthute a mechanics lien filed against property. Cancellation of the
contract may be achieved by paying a cancellation fee in the amount of 5% of the contract price. All returned checks are
subject to a returned check charge fee.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Dear Christy,
I am writing you this letter regarding the property on 4095 Harlan Street. This is a house that had
an apartment built into it on the upper level of the home. My name is Rebecca Hill, and I have
lived at this residence from March 15,2003 to April 4, 2004. The purpose of this letter is to
inform you of the problems occurring at this residence, making it hazardous. My landlord's
name is Donna Dire, and I will enclose her information with this letter. On March 28, 2004 we
noticed water leaking through the ceiling in the bathroom, and put a call into Ms. Dire
immediately after noticing it, at about I :30pm. The only telephone number that we had for her
during this whole year was a cell phone number that you had to leave a message on, and she
would return the call at her convenience. We put buckets under the leak because we noticed it
was coming out of the light fixture rather quickly. At approximately 2:30pm, Ms. Dire returned
our call telling us to place buckets under the leak and she would worry about it later, I do not
think that this was sufficient and was worried for the safety of myself being 7 months pregnant,
my 9 year old daughter, and my husband. The water continued to come through the light fixture,
so I called the Fire Department to see if this was a hazard and if we should vacate the premises. I
called the Fire Department at about 7:00pm, and they came over to inspect the property. They
were worried that the bathtub was overflowing in the property upstairs, and needed to get into the
property. The woman who lives upstairs would not answer her door, so the Fire Department
called the landlord. The landlord came to let them in, and they noted that the tub had overflowed
causing the water damage. Upon noticing this, I saw a rather large bubble in the ceiling of my
daughter's room, and noticed a large mold spot on the wall. This must have been done due to the
water damage the property endured. Two days after all of this, we received a letter from the
landlord letting us know we needed to vacate the premises immediately for our own safety. We
moved out of the property in 5 days from receipt of this letter. I believe that this property is very
unsafe, and needs to be inspected, and something needs to be done. I have also noticed that there
are two rooms in the house that have not been insulated, causing them to be very cold in the
winter. My daughter slept in the coldest room in the house, and I do not think this is fair or safe
for a young child. After doing a quick inspection of our own while moving out, we noticed that
the walls on the north side of the house are very soft, giving them the texture of a bag of sugar.
This house has endured severe water damage due to the irresponsibleness of the tenant upstairs,
and is considered very unsafe. Another concern of mine is the heating system in the home. I
know that this residence is an old property. There is a stove in the middle of the living room
which is supposed to provide the whole house with heat. We had noticed a bird fly in through
the vent of the stove, coming into our home through the vent leading to the outside of the
property. This in my opinion is not a sufficient way to heat any residence. We have also had
problem with the toilet, and had a leak on the outside of the property from the sprinkling system.
I to this day do not know if that leak was ever fixed because that part of the water was shut off. It
caused water to leak into the basement where there is a mini heater to prevent the pipes from
freezing in the winter time. My biggest concern is the fact that she has the property up for rent
again, and we have not noticed anyone at the house doing any sort of repairs. Please take a look
and inspect this place, before she endangers the lives of other people.

At 1911 hours on Sunday March 28, 2004 we were dispatched to a water problem. B9 and
E7 was assigned to this incident. Nineteen personnel responded. We arrived on scene at
1918 hours and cleared at 2021 hours. The incident occurred at 4095 N HARLAN SI,
WHEAT RIDGE. The general description of this property is multifamily dwelling. The
primary task( s) performed at the scene by responding personnel was investigation water
leaking from roof. No mutuaVautomatic aid was given or received.

RegulatIOns and Building Code of 'lv'heal Rid~e. Colorado Of any other applicable ordinances of the City.
This pemut shalt expire rf (A) the work authomed is nol commenced within sixty (50) days from issue dale Of (8) the building authorized is suspended or
abandoned, fof a ,period or 120 days.
If this permIt expIres, a new permit may be acquired for a fee of one.half the amounl normally required. provided no Changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded on8 (1) year. If changes are made or if suspension or abandonment
exceeds one (t) year, full fees shall be paid for a new permit.
No work. of any manner shall be done that will change the natural flow of water causing a drainage problem.
Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceedllng with successive phases of thel'Ob.
