The following requirements are made under the 2003 International Fire
Code adopted by the Fire Board of the Wheat Ridge Fire Protection District
and the City Council of the City of Wheat Ridge, Colorado.
NOTE: ALL CONTRACTOR VEHICLES AND DELIVERS FOR
CONTRACTORS SHALL NOT BE PARKED IN THE FIRE LANES AT ANY
TIME. A CITY TRAFFlC SUMMONS WILL BE ISSUED AND THE
VEHICLE WILL BE TOWED AT THE OWNERS EXPENSE.
Demolition
1. Any use of trash dumpsters shall be .greater than 20 feet to any
structure.
2. All exit paths shall remain clear of construction materials.
3. Upon removal of the ceiling tiles all wiring shall be attached to
structure. There shall be no wiring hanging below drop ceiling height.
4. Fire Lanes shall remain clear at all times of construction.
5. An emergency phone shall be on site during all construction. (Cell
phone will substitute for land line.)
6. Emergency contact numbers will be posted on the front door during
construction.
7. A Fire Extinguisher with a minimum rating of 2A-40BC shall be
within 10 feet of all "HOT WORK" operations.
CONSTRUCTIO..N:
1. The A.D.A. restroom shall have components of the fIre alarm system
as required by the Fire Code.
2. All exit signs shall have battery back up.
3. Sign shall be posted on the exit door stating "THIS DOOR TO
REMAIN UNLOCKED WIDLE THE BUILDING IS OCCUPIED".

Letter shall be a minimum 1" in height and have a contrasting

4. All paths of egress shall have emergency lighting (Incorporated in the
main entry on all floors and the back exit path.)
5, Plans submitted show total coverage to all areas. Addresses shall be
added to all devises in the tenant finish.
6. See approved plans for changes.
7. All permits shall be posted and visible from the street of address.
8. Plans shall be submitted to the Fire District for the fire alarm permit
and approval.
9. Any and all room with storage of medical gases shall be marked as
such. (Quantity and amounts shall be submitted as they apply to the
tenant finish.)

This permit was issued in accordance with the provisions set forth in your application and is subject to the laws of the
State oEColorado and to the zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable
ordinances of the City.
This permit shall expire 180 days from the issue date. Requests for an extension must be received prior to expiration
date. An extension may be granted at the discretion of the Building Official.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no
changes have been or will be made in the original plans and specifications and any suspension or abandonment has not
exceeded one (1) year. If changes have been or if suspension or abandonment exceeds one (1) year, full fees shall
be paid for a new permit.
No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive
written approval on inspection card before proceeding with successive phases of the job.
The issuance of a permit or the appr al of drawings and specifications shall not be construed to be a permit for, nor
an approval of, any violatio210 h ovisions of the building codes or any other ordinance, law, rule or regulation.

IV. Terms and Conditions of Installation:
PermitsNariances: Price includes all building permits required,
Electrical Requirements: 110 volt AC power, single-phase, dedicated circuit. The
Owner must provide the electricity, Electricity must be available prior to installation of the
lift,
Color Selection: The enclosed pricing for the lifts are based on manufacturers standard
finish and color,
Construction/Demolition: By others,
Labor to be provided: All labor and incidental materials necessary for the delivery, set-
up, installation, and training to the owner of the complete lift system shall be provided by
Freedom Lifts, Quoted price includes installation by qualified technicians during normal
working hours as scheduled with the Owner in advance,
Materials/Services: Freedom Lifts, Inc, agrees to furnish all labor and materials for the
completion of all work described in the above Proposal/Contract dated 2,26,08
incorporated herein by reference, for the stated sum of $ , Materials that
are not accepted upon an attempt to deliver will be stored and scheduled for redelivery at
the purchaser's expense, Invoices are payable upon presentation, Title to all equipment
shall remain with Freedom Lifts, Inc, until all invoices are paid in full,
Delivery: In accordance with the project phasing schedule, but not earlier than 8 weeks
from approval of submittals or shop drawings, Shop drawings may be expected within 3
weeks of acceptance of all parties of the proposal or other form of contracVpurchase
order. These time estimates are provided for planning purposes only and do not
represent a contractual obligation or commitment.
Change Orders: All changes to this agreement shall be by written change order signed
by both parties in advance of the start of the work noted in the change order.
Delays: If Freedom Lifts, Inc, is delayed in the progress of work by any act or neglect of
the Owner or by any changes ordered in the job or by any labor dispute, fire, unforeseen
delay in deliveries or in obtaining building permits, or by adverse weather conditions, or
any causes beyond Freedom Lifts, Inc, control, then the contract time shall be extended
for such reasonable time as necessary,
Mediation: If a dispute arises between the parties concerning this contract or
performance thereof and the parties cannot resolve said dispute between the, the parties
agree to attempt to settle the dispute by mediation through the American Arbitration
Association, The cost of mediation shall be divided equally by both parties, If the parties
are unable to resolve any dispute within thirty (30) days after written notice by either party
that wishes to invoke mediation, then either party may purse all legal remedies,
Warranty: Garaventa warrants the lift to be free from defects in materials and
workmanship under normal use and service for a period of One (1) Years from the date
of installation, reasonable wear and tear excluded, Proof of the date of installation is
required to determine the warranty period, This warranty does not extend to any unit
which has been subjected to misuse, neglect, accident, modifications, or damage from

fire, lightning, or water, or used in violation of the instructions furnished, nor units repaired
by other than an authorized Garaventa Dealer, Garaventa liability hereunder is limited to
repair and replacement only, Transportation and labor charges are not covered,
Garaventa accepts no liability for any loss or damage resulting from the use or misuse of
Elevator whether such loss or damage is direct, indirect or consequential. All other
express or implied warranties, statutory or otherwise, are excluded, Freedom Lifts will
extend the term of the warranty to five years upon execution of a Preventative
Maintenance agreement by and between Customer and Freedom Lifts, Inc,

, Lifts, Inc, together will take reasonable precautions to safely protect the premises of work,
contractor equipment, tools and facilities during the construction period, The Owner will
provide Freedom Lifts, Inc, with prompt notification of any circumstances which may
interfere or delay the work, including (but not limited to) activities within the building or the
grounds, which are incompatible with the nature of the work and required electrical power
is supplied to the site and required for completion of job,
Terms: 60% of total contract price is due at the time of order. 20% of total contract price
is due upon delivery of equipment to project location, Remaining 20% is due upon

Contract Cancellation: In the instance the Owner indicates in writing that the contract is
