Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

This letter is written to detail the recent steel helical piling installation at this address. Pinnacle Structural
Services installed eight SS5 Chance steel helical piles at the locations shown on the field sketch. These piles
were installed according to the manufacturer's recommendations and structural engineer's specifications to
depths from 10' to 15' with 5,000 ft -lb of torque.
Standard underpinning brackets were installed on top of the piles to transfer the loads from the footings into the
helical piles. These piles have a minimum ultimate load capacity of 50 kips per pile.
In addition, 4" void was installed continuously under the footing in the area shown on the field sketch.

This letter is written to detail the recent steel helical piling installation at this address. Pinnacle Structural
Services installed eight SSS Chance steel helical piles at the locations shown on the field sketch. These piles
were installed according to the manufacturer's recommendations and structural engineer's specifications to
depths from 10' to 15' with 5,000 ft -lb of torque.
Standard underpinning brackets were installed on top of the piles to transfer the loads from the footings into the
helical piles. These piles have a minimum ultimate load capacity of 50 kips per pile.
In addition, 4" void was installed continuously under the footing in the area shown on the field sketch.

On February 20, 2017, I visually inspected the foundation at the subject residence. The purpose was to develop
my professional opinion of the current structural condition of the foundation in order to determine whether
structural repairs are needed at this time; and to recommend a plan of repair if needed. This inspection did not
involve any other system or component of the residence. No soil sampling, testing, or analysis was conducted
by SC Consulting.
INTRODUCTION
This single family residence was constructed in 1948. The east (front) wall is assumed to be parallel to
Fenton St. The original floor plan was a single story ranch over partial basement and crawl space.
—The foundation is poured concrete on continuous footings. No soil reports were made available to us. The
completion paper work from the two previous helical piling installations by D+B Engineering Contractors was
made available.
—The specific item of concern was the structural integrity of the foundation.
OBSERVATIONS and RECOMMENDATIONS
---The shallow depth of the piling installed by D+B shows the hard bedrock to be quite shallow at this location.
The active zone (about 10') above that bedrock has a higher than normal water saturation due to the water being
unable to penetrate into the hard bedrock, and is therefore trapped in the active zone.
—A relative elevation survey of the first floor was conducted and is shown on the attached field sketch.
—In the crawl space area, the west end is about 1" high to the east end. The underpinning recommendations for
both the crawl space and basement areas are detailed below. Numerous significant cracks in the foundation walls
indicate that foundation movement has occurred.
—SC Consulting recommends that six helical piling underpins be installed inside the crawl space (I' Scope of
Work) to stabilize, and possibly lift, a portion of the crawl space foundation. Please note that the underpin
marked "deep" goes under the deeper footing, not under the shallow footing. In addition, the void area marked
on the plan needs to have 4" deep void installed under the frost wall footing in order to isolate it from the
soils below. The second scope of work is to saw cut, demo, remove the slab, install two piles and brackets,
preload, and replace the slab at two basement depth underpins (2nd Scope of Work). The owner desires to have
these two scopes priced separately.

—The helical piling shaft must be solid round corner square bar (RCS). Round shafted helical piling is not
acceptable. Some of the torsion applied to a round shafted pile can go out into side shear along the exterior of the
round shaft in the cohesive soils that exist along the Colorado Front Range. This torsion does not then get applied
to the helix to obtain sufficient bearing into hard bedrock. Any proposed underpinning system specifications
must be submitted to and approved by SC Consulting prior to proceeding with the work. In addition, this
underpinning work must be conducted under the supervision of a licensed professional engineer.

• The helical piling system must be an approved *- International Building Code (IBC) system by virtue of
an ICC -ER Report. olevl
• Approved systems are Chance, Dixie, and Helipile.
• Round corner square solid bar shafted piling (RCS) only.
• Lead section to consist of a :jingle 8" helix with a minimum helix thickness of 3/8".
• Minimum torque to be 5,000 ft -Ib, with equipment having sufficient power to provide continuous
torsion to the pile.
• Minimum depth to helix must be at least 10'. Shallower depths may be appropriate for this site, but only
upon approval by SC Consulting at the time the pile is being installed.
• SC Consulting requests a minimum five business day notice to be on site at the time the first pile is being
installed.
LI HTATIONS and UNDERSTANDING
—All foundations along the Colorado Front Range experience some movement during the life of the structure due
to the "breathing" of the active soil zone above hard bedrock. These soils increase and decrease in volume as
changes in the water saturation occurs in the active zone. This movement is usually measured to be between 1/2"
to 3/4" in most houses. It is important to understand that all of" the footings under this residence are sitting in the
ac.t e zone abc,.Nc the hard bedrock and therefore minor cracks should be expected to occur in the finish materials
due to changes in the water saturation in the active zone.
This inspection was visual in nature (no invasive procedures or material sampling or testing), but represents the
opinion of SC Consulting at this moment in time. Potential future owners of this property should be very
aware that soil and foundation conditions do change over time. SC Consulting must be given the opportunity
to reevaluate this property prior to a potential buyer using this report as a basis for purchasing the property.
—Our assessment could change if there is a change of conditions from the time of this initial assessment.
—No warranties are made or implied as to the continued structural integrity of this property. Soil and moisture
changes are unpredictable.

2. The architect has made an effort to design an addition which is compatible
aesthetically with the existing structure and is compatible with the existing
neighborhood.
3. Structural considerations within the existing foundation prohibit the addition of a
second story to the existing foundation.
4. The second story addition will be minimally visible from the street as it occurs
primarily in the core of the structure. The proposed wood siding on the first floor

record which were accepted by Chair DRDA. She advised the Board there was
jurisdiction to hear the case and reviewed the staff report and digital presentation. Staff
recommended approval of the case for reasons outlined in the staff report.
Ms. Austin entered into the record a letter received the morning of the Board's meeting
from Sharon Maez,3285 Fenton, whose property abuts the applicant's property to the
north. The letter indicated support for the application.
In response to a question from Board Member HOVLAND, Ms. Austin explained that the
applicant could "pop the top" on the existing structure without a variance. However, the
foundation ofthe existing structure would not support a second story. The staffreport
stated that "ifthe proposed addition were to conform to required setbacks, the additional
living space would be located ten feet from the property line and the attached garage
located on the five-foot setback line. This would create an undesirable staggered wall
effect." The R-I C zone district is the only district that requires a 5-foot minimum
setback for the first story and an additional 5 feet for each story.
In response to a question from Board Member ABBOTT, Ms. Austin explained that
because the applicants are located to the north of their neighbors, there would be less
impact to light and shadow on the neighbors.
Board Member ECHELMEYER asked how this situation came to staff s attention. Ms.
Austin explained that it came to light during the building permit review process.
In response to a question from Board Member ABBOTT, Ms. Austin explained that the
staggered wall effect produced by an additional 5-foot setback for the second story would
be an undesirable situation for the applicant rather than the city.
Jim Nienaber
Evergreen, CO
Mr. Nienaber, architect for the applicant, was sworn in by Chair DRDA. Regarding the
turning radius into the garage from the alley, he stated that an 18-foot wide garage door is
planned to give more swing radius into the garage.
Board Member ABBOTT expressed concern about the aesthetic compatibility of the
addition in that the existing house is brick and the addition will be covered with siding.
Mr. Nienaber explained that the existing house and the addition will be painted the same

painted the same color. He further explained that siding was planned in order to meet
budget constraints. Architectural details on the addition, such as windows, are planned to
match the existing house.
Chair DRDA asked if there were those present who wished to address this matter. The

Ms. Ladwick was sworn in by Chair DRDA. She spoke in favor of her neighbor's
application and stated that the planned addition is in keeping with the unique flavor of the
neighborhood.

Mark Bymaster, the applicant, was sworn in by Chair DRDA. He stated their desire was
to provide additional living space that would maintain the character of the house. Many
options were investigated and the proposed addition proved to be the most satisfactory.
Julie Bymaster

Julie Bymaster, the applicant, was sworn in by Chair DRDA. She pointed out that the
proposed siding would be in character with new homes recently built on 33'4 Avenue.
Barry McBride
5801 West 33'" Avenue
Mr. McBride was sworn in by Chair DRDA. He lives directly north across the street
from the applicant. He spoke in favor of the application which would be in keeping with
the diverse nature of the neighborhood. He felt the addition would be an enhancement to
the neighborhood. It also maximizes the landscaping that faces his house. It is also
consistent with other homes in the neighborhood that have "pop-tops" with five-foot
setbacks. He believed a ten-foot setback for the second story would create a
"hodgepodge" effect in the neighborhood.
There were no others present who wished to address this matter.
Board Member DRDA expressed concern that there is a self-imposed hardship because
there are other ways to create additional space without a variance.
Board Member PETERSON commented that R -I C is the only zone district that requires
an additional setback for each story.
Board Member HOVLAND indicated that he was in favor of the application and
commented that a pop-top would have a greater impact on the house to the south than the

proposed application. He also commented that he was very familiar with this
neighborhood and appreciated its diverse architectural nature. He also pointed out that
the comer lot presents setback limitations.
Board Member BLAIR favored the application. He commented that if the view were
taken that the ordinance must be followed as stated, there would be no need for the Board
of Adjustment. He felt it was the Board's function to decide whether the application was
out of character and harmful to the community or whether it was an enhancement to the
community.
Board Member ABBOTT commented that, while there are more "cut and dried"
hardships, he agreed with Board Member BLAIR.
Board Member PETERSON stated that, while he believed there were other alternatives
available to the applicant, he would vote to approve the application because the neighbors
were in favor of the application.
Board Member SCHULZ stated that he believed the proposed addition was a wise and
attractive way to add square footage to the house and is in keeping with other houses in
the neighborhood. He commented that increasing the height of the house would look
awkward.
Upon a motion by Board Member ABBOTT and second by Board Member
HOWARD, the following resolution was stated:
Whereas, the applicant was denied permission by an administrative officer; and
Whereas, Board of Adjustment Application Case No. W A-03-15 is an appeal to this
Board from the decision of an administrative officer; and
Whereas, the property has been posted the fifteen days required by law; and in
recognition that there were no protests registered against it;
Whereas, the relief applied for MAYbe granted without detriment to the public
welfare and without substantially impairing the intent and purpose ofthe
regulations governing the City of Wheat Ridge.
Now, therefore, be it resolved that Board of Adjustment Application Case No. W A-
03-15 be, and hereby is, APPROVED.
Type of Variance: A 5-foot side yard setback variance from the 10-foot side yard

For the following reasons:
1. The two-story addition directly abuts closely only a few feet ofthe existing
dwelling on the property immediately adjoining on the south.
2. The architect has made an effort to design an addition which is compatible
aesthetically with the existing structure and is compatible with the existing
neighborhood.
3. Structural considerations within the existing foundation prohibit tbe addition
of a second story to the existing foundation.
4. The second story addition will be minimally visible from the street as it
occurs primarily in the core of the structure. The proposed wood siding on
the first floor would not be significantly visible from the street view.
5. Testimony was given by some adjacent neighbors in favor of the variance.
6. Staff recommended approval of the variance.
The motion passed 7-1 with Board Member DRDA voting no.
Chair DRDA advised the applicants that their request for variance was approved.
C. Case No. WA-03-16: An application filed by ARC Wireless Solutions for
approval of a variance to Section 26-709 of the Wheat Ridge Code of Laws (Sign
Code) to allow an increase to the maximum sign area allowed for a freestanding
sign on property zoned Planned Industrial Development (PID) and located at
10601 West 1-70 Frontage Road North.
This case was presented by Mike Pesicka. He entered all pertinent documents into the
record which were accepted by Chair DRDA. He advised the Board there was
jurisdiction to hear the case and reviewed the staff report and digital presentation. Staff
recommended denial of the case for reasons outlined in the staff report.
In response to a question from Chair DRDA, Mr. Pesicka stated that if the variance were
denied, the lower portion of the sign would have to be removed.
In response to a question from Board Member HOWARD, Mr. Pesicka stated that the
present sign is 216 square feet and 173 square feet is allowed under the code.
Board Member ABBOTT asked if the code would allow the subject sign to be raised to
50 feet in height. Mr. Pesicka explained that while the previous business was limited to
having a sign 32.5 feet in height, the present business could legally raise the height of the
sign to 50 feet.
Board Member PETERSON commented that the applicant would have had no way of
knowing the sign code had changed and that the existing sign was not in compliance.

I have completed my initial review of the above-mentioned permit application for an addition to the
single-family residence located at 3295 Fenton Street, and I have the following comments:
7 The proposed addition is located five feet too close to the southern property line.
Pursuant to Section 26-208, a two-story structure in the R-lC zone district must be set
back a minimum often feet (10') from the property line. Based on this, the house
addition and garage must be set back ten feet from the southern property line. You do
have the option of applying for a variance to this standard. If you wish to apply for the
setback variancc, you will need to fill out an application, provide the required paperwork
and pay a fee of $390.00. The next available Board of Adjustment meeting will be held
on Thursday July 24,2003. Please submit all required paperwork and the applicable fee
on or before July 3, 2003 to ensure a hearing date on July 24, 2003.
Another option available to you is to lower the structure to a height less than twenty feet
(20'). The Code of Laws states that, for the purposes of defining setbacks requirements,
building height shall be measured from average grade of a building to the highest point
of the building. In the case ofresidential structures, a story is considered every ten feet
(10') of building height.

Mr, Bymaster,
As requested, the existing residence and excavation were observed at the subject site on May 5, 2003.
The purpose of the observation was to evaluate the condition of the existing structure and subsurface
conditions with respect to a proposed one-story addition. The addition will be constructed on a crawl
space with a stairwell to the existing basement. The majority of the foundations are planned at
approximately 3 feet below the ground surface; however, the stairwell foundations will be located at
lower elevations. The test pit was located on the southwestern corner of the garage, in the area of the
proposed addition.
The excavated test pit was located on the southwestern corner of the existing garage and was complete
at the time of observation. The excavation measured approximately 3 feet in the north-south direction
and approximately 3 feet in the east-west direction, The excavation extended approximately 4 feet
below grade, which is about 1.5 feet below the existing garage foundation. Water was not encountered
within the excavation, Materials encountered within the excavation beneath the concrete walkway
consisted of approximately 3,5 feet of moist, brown, mottled sandy clay fill, underlain by moist, brown,
sandy clay that was possible fill to the maximum excavated depth of 4 feet. Hand samples of the sandy
clay that was possible fill were obtained while on site, One of the samples was tested for swell-
consolidation, moisture density, and percent silt and clay, Results of the testing indicated 66 percent silt
and clay. The sample exhibited no movement when wetted under a surcharge pressure of 1000 pounds
per square foot (PSF) as indicated on Figure 1,
The site is located in an established residential area in Wheat Ridge, Colorado. The original portion of
the structure was built in 1948, The structure was observed for evidence of foundation movement.
Foundation cracking was not observed in the majority of the structure, Miuimal foundation cracking
was observed at the southwestern corner of the garage, consisting of cracks measuring no longer than
approximately 2 feet and a maximum of 1/8 inch in thickness, A spread footing was observed on the
southwestern portion of the southern garage wall. Landscaping surrounding the structure consisted of
bushes, shrubs, mature trees and grass.
Subsurface conditions typical of the area consist of wind-blown sandy silts, The depth to bedrock at
this site is approximately 10 to 20 feet below ground surface based on geologic maps from the

Colorado Geological Survey, The Engineering Soils Map for the Denver Metropolitan Area, produced
by the Colorado Geological Survey, indicates soil problems, if any, are localized and varied,
Based on the observed spread footings at the garage, our previous experience in the area, and previous
foundation performance, a spread footing foundation system is recommended provided the owner and
builder can accept similar foundation performance, The footings should be designed for a maximum
allowable bearing pressure of 1500 PSF, The bearing capacity is based on the assumption of a
minimum I-foot embedment for all footings, New footings should be supported by undisturbed
existing sandy clay, The sandy clay may be fill material from the original construction that has likely
settled due to self-weight and can support relatively light foundation pressures, Soil loosened by the
excavation process should be removed prior to placing footings. Foundation walls should be reinforced
to span isolated loose zones, Reinforcing adequate to span an unsupported length of 10 feet is
recommended, Minimum l6-inch wide continuous spread footings and 24-inch square isolated
columns are recommended, Exterior footings should be protected from frost action with a minimum
earth cover of3 feet.
Excavations should not remain open for extended periods of time, permitting wetting or drying of
the bearing materials. Moisture changes of the bearing materials may increase the risk for
movement. The materials encountered within the depth of excavations on site are categorized as
Type B according the Occupational Safety and Health Administration (OSHA). Based on published
OSHA guidelines, the temporary excavations in the Type B soils can have a maximum temporary
slope of 1.0: 1.0 horizontal:vertical (H:V) or 45 degrees. This inclination is reasonable for the
conditions at the site, Exceeding this inclination will increase the chance of deformation, especially
over a long time period, Some localized deformation of the bank may occur, especially during wet
weather, loading or with vibrations, Care should be taken when working near the sides of the
excavation at all times, and the slopes should be monitored by onsite personnel during construction
for evidence of sloughing, bulging or toppling of the sidewalls or cracking at the ground surface,
Surcharge loading at the top of the cut by equipment, materials, or vehicles must be avoided, since
surcharge loading will increase the risk of caving. Spoils of the excavation must be placed a
minimum of2 horizontal feet from the edge of the excavation. A geotechnical engineer should observe
the completed excavation prior to foundation installation.
The existing sandy clay fill observed at anticipated slab elevations is suitable for support of slab-on-
grade construction, with a low risk of slab movement. It is not possible to further qualify the potential
risk for movement based on the limited amount of subsurface data obtained during the investigation.
Slabs should be supported by undisturbed existing sandy clay fill, or structural fill compacted to a
minimum of95 percent of the standard Proctor maximum dry density within 2 percent of the optimum
moisture content. The onsite sandy clay fill is suitable for use as structural fill, Slabs should not be
attached to interior or exterior bearing members. The slabs should be constructed as "floating" slabs,
which are free to move in the vertical direction. Exterior flatwork may settle if supported by backfill
that is not compacted to a minimum of95 percent of the standard Proctor maximum dry density. Non-
structural backfill, such as landscaped areas adj acent to foundation walls should be compacted to a
minimum of90 percent ofthe standard Proctor within 2 percent of the optimum moisture content.

When building additions, it is difficult to prevent minor cracking between the addition and the original
structures. A flexible connection with a trim cover to allow a cosmetic cover over cracking is
recommended, Caulking of connections is recommended, An alternative is a rigid connection where
the new foundations are doweled into the existing foundation walls. It is not recommended rigid
finishes, such as tile, be utilized in areas of transition between new and existing construction, Cracking
is more visible in rigid materials, such as tile, and performance expectations should be considered prior
to installation,
The ground surface should be graded to provide positive drainage away from the structure in all
directions for new and existing foundation walls as part of the new construction. A minimum slope of
12 inches in the first 5 to 10 feet from the structure is recommended and should maintained during the
life of the structure, Down spouts should discharge to the surface a minimum of 5 feet away from the
structure in all areas, Any settlement of backfill should be immediately repaired and positive drainage
away from the structure should be reestablished,
An optional drain may be installed at foundations located greater than 4 feet below ground surface to
reduce the risk of hydrostatic pressure buildup or wet concrete conditions, If a foundation drain is not
installed, similar foundation performance to the existing structure can be expected. If opted, the
foundation drain should lead to a positive gravity outlet or a sump where water can be removed by
pumping. The foundation drain should be installed similar to the detail presented in Figure 2.
It is understood a small portion of the addition will be constructed below grade, Therefore, below
grade walls must be designed for lateral loads, For "active" conditions, the walls should be
designed for an equivalent fluid pressure of 40 pounds per cubic foot (PCF). For "at rest"
conditions, equivalent fluid pressures of 50 PCF should be used. These loads do not include
swelling pressures, hydrostatic loads or surcharge loads such as sloping backfill or vehicles.
Expansive material should not be used for backfill,

This permit shall expire if (A) the work authorized is not commenced within sixty (60) days from issue date or (8) the building authorized is suspended or
abandoned for a period of 120 days.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proce iing with successive phases of the job.
