move out. I will look behind walls and under floors when you are ready to scrape the house to be sure

(1) PROPERTY DESCRIPTION AND PURPOSE OF REPORT
This report provides information regarding an asbestos building materials survey performed on a 42 square
foot bathroom in a house built in 1953 was inspected as part of the due diligence process in conjunction
with the proposed bathroom remodel.
Every attempt was taken to identify all asbestos containing suspect materials and the quantity of each
material that would be regulated in the demolition project, according to the CDPHE Regulation 8, The
Control of Asbestos, Part B.
The inspection was performed to encompass all asbestos suspected materials that will have to be removed
before demolition can take place. All suspect materials in the bathroom were tested or assumed to contain
asbestos. The location of each sample was clearly indicated where the samples were collected. These
specific materials were categorized into separate homogenous materials and analyzed for asbestos
content. There was only one homogenous area that was tested. There is no regulated asbestos in the
bathroom.
(2) EXTENT OF INSPECTION
Perform visual, tactile inspection of suspect ACM.
Identify, locate and materials to be sampled or presumed to contain ACM
Collect samples and quantify friable and non -friable ACM and presumed ACM
Provide sample analysis by polarized light microscopy
Provide project report with laboratory results and conclusions.
(3) INSPECTION REQUIRED PROCEDURES
The asbestos survey was conducted by an Environmental Protection Agency (EPA) accredited AHERA and
Colorado State Certified asbestos building inspector. The inspection included a physical assessment of
suspect materials, identification and location of homogeneous areas; assessment of the condition of the
ACM; and collection of samples for laboratory analysis. All procedures were conducted in general
accordance with the guidelines of the National Emissions Standards Hazardous Air Pollutants (NESHAPS) 40
CFR Part 61 and the Occupational Safety and Health Administration (OSHA) 29 CFR 1926.1101.
(4) HOMOGENOUS AREA CATEGORIES
Suspect ACM were grouped into homogeneous areas based on physical characteristics and functional
applications. Homogeneous areas were divided into one of three categories based on the definitions as
designated by US EPA 40 CFR 763 and The CDPHE mandated Regulation 8, The Control of Asbestos, Part B.
1. Thermal systems insulation — (TSI) Insulations to control thermal conductivity pipe/ duct wrap,
boiler & chiller insulations and tanks etc.
2. Surfacing material — Sprayed or troweled—on onto structural members or walls (i.e. re-pr%rmg
structural steel or drywall finishes)

3. Miscellaneous — Remaining suspect materials (i.e. flooring, gaskets, shingles, ceiling tiles, fixtures
etc.)
All samples were submitted to Aerobiology, 780 Simms Street, Golden Colorado 80401, under proper chain -
of -custody procedures. Analysis of the suspect asbestos Bulk sample building materials was performed by
polarized light microscopy (PLM) according to EPA 600/11/116, July 1993 methodology. Aerobiology
is in the National Voluntary Laboratory Accreditation Program (NVLAP) for asbestos
A visual inspection of the house was performed and bulk samples of materials suspected of containing
asbestos acquired. The condition of the materials was assessed, and the content analyzed to determine if
those materials contained regulated asbestos. Estimates of the quantities of those suspect materials were
made to discern if they were above the commercial project trigger level of a 55 -gallon drum, 32 square
feet, or 50 linear feet on piping
The inspection was performed to encompass all regulated asbestos suspected materials that will have to be
removed before the demolition in association with the remodeling can take place. Only the suspect
materials in the bathroom were tested. The location of each sample was clearly indicated where the
samples were collected. These specific materials were categorized into separate homogenous materials
and analyzed for asbestos content. There was one homogenous area tested.
(5) SURFACING MATERIAL (ONE) THAT WAS SUSPECT OF CONTAING ASBESTOS

(6) THERE WERE NO NUSCLLANEOUS SUSPECT ASBESTOS CONTAINING MATERIALS

Sample analytical results are reported in percentages of asbestos and non -asbestos components, The EPA
defines any material that contains greater than one percent (1%) asbestos as being an asbestos containing
material. Materials that are identified as "none detected" are specified as not containing asbestos.
There were no asbestos containing materials found in this limited inspection.
(8) MATERIALS ASSUMED TO CONTAIN ASBESTOS
Common asbestos materials like window caulking, floor tile, and asphalt impregnated roofing materials are
considered to be non -friable asbestos materials if they are in good condition. These materials have special
regulations with respect to their removal. To wit: REG 8, The Control of Asbestos-LB.67. Non friable
asbestos containing materials. (Non -Friable means material which, when dry, may not be crumbled,
pulverized, or reduced to powder by hand pressure. This material must not be rendered friable during
demolition.
From the CDPHE mandated Regulation 8,
LA.3. Any asbestos -containing material that is friable or will be made friable during
demolition activities in any area of public access or non-public access area must be remove'd� pr or to
demolition. Removal, in accordance with Regulation No. 8, Section 111, Effective: Ja(le
ry 30,,008.
Regulation No. 8 requires if the amount of asbestos -containing material that is friai ill
become friable during demolition exceeds the trigger levels. dtR;age

Category I non -friable ACM must be inspected and tested for friability if it is in poor condition before
demolition to determine whether or not it is subject to the Asbestos NESHAP. If the ACM is friable, it must
be handled in accordance with the NESHAP. Asbestos -containing packings, gaskets, resilient floor coverings
and asphalt roofing materials must be removed before demolition only if they are in poor condition and are
friable. *
All Suspect Materials were tested.
The CDPHE Regulation 8, The Control of Asbestos Part B., provides:
III.S.4.b. Remove the materials using hand removal methods or power tools that do not
subject the material to cutting, grinding, sanding, bead blasting, sandblasting,
or otherwise damage the material in such a way as to render it friable.
I.B.80. Regulated asbestos -containing material means (a) friable asbestos -containing material, (b)
Category I nonfriable ACM that has become friable, (c) Category I nonfriable ACM that will be or
has been subjected to sanding, grinding, cutting, or abrading or (d) Category 11 nonfriable ACM
that has a high probability of becoming or has become crumbled, pulverized, or reduced to
powder by the forces expected to act on the material in the course of demolition or renovation
operations regulated by this regulation.
All materials in this bathroom are in good condition.
(9) TRACE SAMPLE RULE:
* The CDPHE Regulation 8, Part B, requires Trace sample to be point counted.
III.A.I.e. The analysis of samples collected during these inspections must be
performed as required in subsection IV.E. (Analysis) of this regulation with one
exception: if the asbestos content of a sample of friable asbestos is estimated to be I%
asbestos or less, but greater than 0%, by a method other than point counting (such as
visual estimation), the determination shall be repeated using the point counting
technique with polarized light microscopy. If a result obtained by point count is
different from a result obtained by visual estimation, the point count result must be
used. Tar impregnated samples do not have to be point counted.
(10) ASBESTOS REMOVAL PERMIT OR NOTIFICATION REQUIREMENTS:
When the quantity of friable ACBM (Asbestos Containing Building Materials) to be removed exceeds ">50
linear feet on pipes or 32 square feet of surfaces or a 55 -gallon drum, in commercial projects the material
must be removed in compliance with the CDPHE Regulation 8, The Control of Asbestos. In addition a permit
or notification and a ten working day waiting period are required by the CDPHE.
There may be materials under the ones tested. The walls and ceilings were systematically examined but no
such materials were found during the inspection. That does not eliminate the possibility that asbestos
containing materials were overlooked during the inspection or that there may be other asbestc containing
materials somewhere hidden in the building. e"6'

The results of this survey pertain only to materials tested or assumed to contain asbestos. The laboratory
reports supporting these findings above are attached below. Please contact 5280 Environmental Consulting
if you have any questions or concerns regarding this report.
(11) FINDINGS:
For the purposes of this survey there is no asbestos in the walls or ceiling in the 42 square foot bathroom.

(13) HIDDEN OR UNDISCOVERED MATERIALS:
5280 Environmental Consulting warrants that the findings contained herein have been with the level of
care and skill exercised by experienced and knowledgeable environmental consultant who is licensed or
otherwise trained to perform asbestos inspections pursuant to the scope of work required on this project.
The asbestos inspection included inspection of accessible materials such as above or behind
suspended ceilings or other non -permanent structures. 5280 Environmental Consulting made reasonable
effort to access behind walls and within ductwork to survey otherwise inaccessible areas. For the purpose
of this warrant, inaccessible is defined as areas of the building that could not be tested (sampled) without
unauthorized destruction of the structure or a portion of the structure.
The quantities of materials identified in this study as ACM are approximate. Contractors providing
asbestos remediation services must verify all field conditions and estimated quantities.
(14) SAMPLING RATIONALIINVESTIGATION METHODOLOGY:
Sample locations were selected in a random manner. The number of samples was collected of
each homogeneous material identified as suspect ACM in accordance with the referenced
CDPHE Regulation 8, The Control of Asbestos, Part B protocol.

(15) INACCESSIBLE AREAS:
Reasonable efforts were made to locate and inspect concealed areas mi accordance with
Colorado Regulation 8 Part B. Only the areas and the materials listed were surveyed as part of
this inspection. There is the possibility that there are walls behind walls or ceilings behind ceilings that may
have been missed upon original inspection. If discovered, please contact me so that I may return to test the
material

Fortress Roofing Corporation will complete the work for the amount of my insurance carrier's estimate, including any supplemental

7UUnforeseen items will be discussed and billed accordingly. If plywood is needed to provide a solid surface prior to installing shingles
(building code). Fortress Roofing Corp. will notify homeowner immediately if there are additional layers of roofing or unforeseen damages that need
tobe ressed. If there is a code item needed that is not covered by my insurance company, I agree to pay for the item out of pocket.
Property owner is aware that any siding that is within 2 inches of the existing shingles may need to be trimmed to have new flashing
installed. All reasonable care will be taken to minimize any damage to the siding or paint however homeowner is aware that damage may occur and
will be at the owners expense for any needed repairs.
Fortress Roofing Corp. shall hold in trust any payment received from you until Fortress Roofing Corp. has delivered roofing materials at the
site or has performed a majority of the roofing work on your property. Any unpaid balance will be due upon completion of the work. I acknowledge
that any unpaid bills, outstanding for a period of thirty (30) days from the final billing date, may be subject to a Mechanic's Lien, with Fortress
Roofing Corp. being entitled to recover all costs of collections, including reasonable attorney fees. All past due balances should be subject to a
finance charge of 24% per annum. Regardless of delays in inspection by the associated permitting jurisdiction, payment is due upon completion of
the job.
Fortress Roofing Corporation carries workers' compensation, automobile and commercial general liability (bodily injury and property

Rough Framing and Wood Requirements
11 The complete rough inspection is to be made after the rough framing, rough electric, rough plumbing, and
rough heat and ventilation are complete. The building should be fully enclosed with the roof covering and siding
in place.
1.3Accessible under -floor areas shall be provided with a minimum 18 -inches x 24 -inches opening through the
floor or 16 -inches x 24 -inches opening through the perimeter wall. When the access is through a below -grade
wall an areaway of not less than 16 -inches x 24 -inches must be provided. Where equipment is located beneath
the floor, the access opening must be within 20 -feet of the equipment and allow the removal of the largest piece
of equipment. (IRC R408.4 and M1305.1.4)
14 Basements greater than 200 square feet and every sleeping room shall have at least one operable emergency
and rescue opening or a door to the exterior.
15 Emergency escape and rescue openings shall meet the following requirements: A minimum clear width of
20 -inches, a minimum clear height of 24 -inches, with a total net clear opening of 5 square feet for grade level
windows and 5.7 square feet for all others. A sill height of not more than 44 inches measured from the
finished floor to the bottom of the clear opening. Window wells for emergency escape and rescue shall provide a
minimum net area of 9 square feet with minimum horizontal projection and width of 36 -inches. The area of
the widow well shall allow the emergency escape and rescue opening to fully open. (IRC R310)
16 Columns must be restrained to prevent lateral displacement at the bottom end to insure against uplift. (IRC
R301.2.1, R407.3)
17 Fire blocking shall be installed to cut off all concealed draft openings in the following locations: (IRC
302.11) A. In concealed spaces of stud walls and partitions, including furred spaces, vertically at the ceiling and
floor levels, and at 10 -feet horizontal intervals. B. At interconnections between concealed vertical and horizontal
spaces such as occur at soffits, drop ceilings, and cove ceilings. C. In openings around vents, pipes, ducts, cable,
wires, chimneys, fireplaces, and similar openings that afford a passage for fire at ceiling and floor levels. Il. In
concealed spaces between stair stringers at the top and bottom of the run, and between studs in line with the run
of the stairs at unfinished walls.
18 At least one egress door shall be provided for each dwelling unit. The egress door shall be side -hinged, and
shall provide a minimum clear width of 32 -inches. Egress doors shall be readily openable from inside the
dwelling without the use of a key or special knowledge or effort. (IRC R311, R311.2)
19 There shall be a floor or landing on each side of each exterior door, The landing must be no lower than 73/ -
inches below the top of the threshold of the door provided the door does not swing over the landing. (IRC
R311.3)
2 P Handrails shall be provided on at least one side of stairways with 4 or more risers and ramps with a slope
exceeding 1:12. Handrails shall be mounted 34 -inches to 38 -inches above the nosing of the treads. Handrails
must be continuous for the entire length of the stairs and provided with a minimum space of 1 %2 -inch between the
wall and the handrail. Ends shall be returned or terminate in newel posts or safety terminals. See the
requirements and more information. (IRC R311.7.8)

sides of stairs with a total rise of more than 30 -inches above the floor or grade below shall have guards not less
than 34 -inches high. Intermediate rails shall not allow the passage of a 4 -inch sphere. (R312)Window fall
protection must be provided for window openings less than 24 inches above the finished floor and more than 72
inches above grade or surface below. (R312.2)
Ceiling heights for habitable rooms, hallways, bathrooms, toilet rooms, laundry rooms, and basements shall
be a minimum of 7 -feet measured from the finished floor to the lowest ojection of the ceiling. Non -habitable
basements may have a ceiling height of 6 -feet, 8 -inches with clearance below beams and ducts of 6 -feet, 4 -
inches. Bathroom may have a ceiling height of 6 -feet, 8 -inches at the fixture front clearance area and over the
fixture. (IRC R305.1)
x 4 Attic access must be provided for attics that exceed 30 square feet and that have a vertical height of 30 -inches
or greater. The access must be located in a hall or readily accessible location with a minimum 22 -inches x 30 -
inches access opening. The access must provide a minimum of 30 -inches headroom. (IRC R807) zs� Trusses
must be designed and fabricated in accordance with ANSUTPI 1. A licensed professional shall prepare the truss
design. Trusses must be legibly marked within 2 -feet of the center to identify the manufacturer, the design load,
and the spacing. Truss engineering and layout must be provided at the site at the rough framing inspection. (IRC
R502.11 and R802.10)
?'Cy Roofing material Wood roofs in the plain areas are limited to minimum Class "B" roof coverings. Shingle
installation shall meet the manufacturer's high -wind installation procedures in areas where wind load is greater
than 105 mph (V3S). Chapter 9, R301.2.1, R325, and manufacturer's listing
_K All habitable rooms shall be provided with aggregate glazing area for natural light of not less than 8% of the
floor area of such rooms. The glazed areas need not be installed in rooms where section M1507 is satisfied and
artificial light is provided at an average of 6 foot candles at a height of 30 inches.
,� ! Safety glazing shall be installed in the following hazardous locations and be permanently labeled as safety or
tempered glazing: glazing panels in doors; glazing enclosing spas, bathtubs or showers; glazing panels adjacent
and within 24 -inches of the edge of a door; glazing panels greater than 9 square feet within 18 -inches of the
floor; glazing used for railings; glazing used for barriers for swimming pools and spas; and glazing enclosing
stairways, landings, and ramps within 36 -inches horizontally of a walking surface. (R308.4 through 308.4.7).
�1 Crawl spaces shall be provided with conditioned air in compliance with Section N1102.2.10. Crawl space
walls shall be insulated. Crawl space wall insulation shall be permanently fastened to the wall and extend
downward from the floor to the finish grade level and then vertically and/or horizontally for a least an additional
24 -inches. Exposed earth in unvented crawl space foundations shall be covered with a continuous Class I vapor
retarder with a minimum thickness of 10 -mil. All joints of the vapor retarder shall overlap by 6 -inches and be
sealed and taped. The edges of the vapor retarder shall extend at least 6 -inches up the stem wall and shall be
attached to the stem wall. N1102.2.9.2.2 Ventilated crawlspaces. Ventilated crawlspaces shall be provided with
insulation. (IRC R303.1)
Habitable rooms must be provided with outdoor openings for ventilation equal to at least 4% of the floor
area and shall have heating facilities capable of maintaining 68° at 3 -feet above the floor and 2 -feet from exterior
walls. H. The windows need not be openable in a room where an opening is not required to be an egress window
and a mechanical ventilation system is capable of producing 0.35 air change per hour in that room, or a whole
house mechanical ventilation system provides 15 cubic feet of outdoor air per minute.
,15 Bathroom exhaust fans shall exhaust directly to the exterior and must be provided in bathrooms where
windows of at least 3 square feet are not provided. The mechanical ventilation must be capable of providing an
exhaust capacity of 50 cfin intermittent or 20 cfin continuous exhaust. (IRC R303.3, M1507.4)

equipped with a back draft damper, and sized and terminated per the manufacturer. The maximum length of the
exhaust duct shall be 25 -feet (Unless approved under the manufacturer's listing) from the connection to the
transition duct from the dryer to the outlet terminal. Where fittings are used, the maximum length of the exhaust
duct shall be reduced in accordance with (IRC Table M1502.4.4.1) Flexible transition ducts are limited to 8 -feet,
shall not be concealed within construction, and shall be listed and listed per UL 2158A. (IRC M1502)

38 Appliances shall be accessible for inspection, service, repair and replacement without removing permanent
construction. 30 -inches of working depth and width shall be provided in front of the control side of an appliance.
(IRC M1305.1) 40 Combustion air is required for all liquid fuel, solid fuel and gas appliances. (IRC M1701.1,
G2407.1) Air for combustion, ventilation and dilution of flue gases for appliances installed in buildings shall be
provided with the requirement of (G2407.6 through G2407 .9).One opening shall be 12" below the ceiling and
one opening shall be 12 " above the floor.
Electrical Requirements
44 All electrodes present at each structure shall be bonded together to form one grounding electrode system to
include: Concrete Encased Electrodes (UFER), water piping, ground rings, grounding rods, and plate electrodes.
Bonding shall ensure electrical continuity. The electrode conductor must be connected in an approved manner
and inspected prior to concealing. (IRC E3607, E3608, E3609, E3610, E3611)

Tamper -Resistant receptacle outlets are required in every kitchen, family room, dining room, living room,
parlor, library, den, sunroom, bedroom, recreation room or similar dwelling areas spaced so that no point along
the wall line exceeds 6 -feet from an outlet. (IRC E3901, E3902, and 4002.1-4002.14)

crawlspaces, unfinished basements, kitchen counters, within 6ft. of laundry, utility and wet bar sinks, boathouses,
hydro massage, spa and hot tub locations. Receptacles must be in a readily accessible location. (IRCE3902,
E3902.11)
Combination type arc -fault circuit interrupters must be installed on all 120 -volt 15 and 20 -amp branch
circuit outlets in receptacle outlets are required in every family room, dining room, living room, parlor, library,
den, sunroom, bedroom, recreation room, hallway, closets, and similar dwelling areas. (IRC 3902.12)
r At least one wall switch -controlled lighting outlet shall be installed in every habitable room, bathroom,
hallway, (I Oft) stairway, attached garage, detached garage with electric power, and at the exterior side of each
outdoor egress door having grade level access. Attics, under floor spaces, utility rooms and basements containing
equipment or used for storage must be provided with a switch operated light. (IRC E3903)
Smoke alarms shall comply with UL 217. Smoke alarms are to be powered by the building's wiring with
battery backup and shall be interconnected so the actuation of one alarm will activate all alarms in the dwelling
unit.(R314.5) Smoke alarms shall be installed in the following locations: (IRC R314) :a. In each sleeping room;
�4 Outside each sleeping area in the immediate vicinity of the bedrooms; and One on each additional story
including basements.
s;,,.'Carbon Monoxide alarms are required in a single-family dwelling that has fuel -fired appliance(s), or a
fireplace, or an attached garage. The alarm must be installed within 15 -feet of the entrance to each sleeping
room. Must be listed by a nationally recognized, independent lab. Plugs into a dwelling's electrical outlet and has
a battery backup. They may be combined with a smoke -detecting device if the combined device complies with
applicable law. (IRC R315)
Clothes closet lighting shall be limited to surface -mounted or recessed incandescent fixtures with completely
enclosed lamps, and surface -mounted or recessed fluorescent fixtures.
Recessed fluorescent fixtures shall have a minimum 6 -inch clearance to the nearest point of storage.
Working space must be provided around energized equipment for servicing and maintenance. The minimum
dimensions shall not be less than 36 -inches in depth and 30 -inches in width in front of the equipment. Working
space shall extend to at least 6 -feet, 6 -inches above the floor. (IRC E3405.2)
7 Electrical fixtures installed in wet or damp locations shall be installed so that water cannot enter or
accumulate n wiring compartments or other electrical parts. Fixtures installed in wet locations must be labeled
"suitable for wet locations". Fixtures installed in damp locations must be labeled "suitable for damp
locations" or "suitable for wet locations". Switches or circuit breakers located in wet locations or outdoors
must be enclosed in a weatherproof enclosure. Outdoor receptacles must have a weatherproof enclosure and an
in -use cover. Cord -connected fixtures, hanging fixtures, track lighting, pendants, and ceiling fans shall not be
located within3-feet horizontally and 8 -feet vertically from the top of a tub or shower rim. (IRC E4003.9,
E4002.8, E4002.9,E4002.10, E4003.9, E4003. I 0,E4003.1 1)

other than plastic by air with no evidence of leakage. Either test shall be applied to the drainage system in its

60 Bathtubs, walls above bathtubs with shower heads, shower floors, and shower compartments shall be
finished with a non-absorbent surface which extends to a height not less than 6' above the floor. Showers must
have a minimum 900 square inches of interior area and a minimum dimension in each direction of 30 -inches.
Shower valves must be pressure balance/thermostatic mixing valves. Shower receptors shall have a curb
threshold not less than 1 -inch below the sides and back of the receptor. The curb shall measure a minimum of 2 -
inches to a maximum of 9 -inches above the top of the drain. (IRC R307, P2708, P2709)

conditions and to examine the proposed construction. The following statements are based on our observations

State of Colorado and to the zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable
ordinances of the City.
2. This permit shall expire 160 days from the issue date. Requests for an extension must be received prior to expiration
date. An extension may be granted at ,the discretion of the Building Official.
3. If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no
changes have been or will be made in the original plans and specifications and any suspension or abandonment has not
exceeded one (1) year. If changes have been or if suspension or abandonment exceeds one (1) year, full fees shall
be paid for a new permit.
4. No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
5. Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive
written approval on inspection card before proceeding with successive phases of the job.
6. The issuance of a permit or the appr al of drawings and specifications shall not be construed to be a permit for, nor
an approval of, any violatio of h 'ovisions of the building codes or any other ordinance, law, rule or regulation.
