This letter and accompanying sketches are intended to modify or clarify the existing contract documents
based on field conditions or contractor/homeowner requested changes.
1. Reference plan view 1/S-1 and detail 4/S-2: Detail 4/S-2 is showing the required beam pocket
configuration. The beam depicted in the detail is generic and is not intended to supersede the
information on plan view 1/S-1. The beam in question is a (2) 1.75 x 11.25 Microlam as
indicated on 1/S-1
2. Reference Detail 1/S-2: Due to field conditions, the floorjoist detail was modified from bearing
on the foundation to flush mounting to the exterior foundation using a ledger board detail. The
existing installation was directed by J2.26 Engineering. J2.26 Engineering has visited the site to
confirm all installation of the floorjoists and floor beams meet design criteria. All installation is
satisfactory.

3. Reference 1/S-1: see attached Sketch SK -1. All shear walls shall be a minimum of 3'-6". Install
foundation anchors as indicated by SK -1.
4. Reference Detail 1&2/S-1: Due to field conditions, a beam/column structure has been
constructed to support the roof truss system in lieu of using the existing ledger condition.
Reference SK -1 for beam/column information. Note, previous confirmation letters have already
confirmed modified foundation conditions to accommodate this condition.
This sealed letter confirms the observations, considerations, and calculations necessary for the above
modifications were completed by me or done under my direct supervision.

This letter is to confirm all foundation geometry and reinforcement meet or exceed the project
requirements as shown in the construction documents including variances from original construction
documents as noted below. This confirmation is based on observations and report completed by onsite
inspection on 12/02/20 and is limited to the formwork and reinforcement in place on that date.
Variations from construction documents:
1. Prior to placing reinforcement, J2.26 confirmed it is acceptable to modify reinforcement from
#5 bar to #4 bar.
2. A pilaster was added adjacent to the north side of the house due to actual framing conditions
encountered in the field. This formwork geometry and reinforcement has been installed by
Compass Construction as directed by J2.26 Engineering. Reference attached sketch for
additional information.

3. Due to the conditions of the existing home, a cast concrete grade beam/retaining wall was
installed along the existing foundation as directed by J2.26 engineering. This grade beam is an
8" wide 36" deep beam with reinforcing to match new foundation wall reinforcing per project
documents. Reference attached sketch for additional information.
4. J2.26 Engineering observed the UFER grounding rod installed per Wheat Ridge, Colorado
building department requirements.
5. J2.26 Engineering observed the foundation waterproofing material installed.
6. J2.26 Engineering observed the perimeter foundation drain installed.
This sealed letter confirms the observations and considerations were completed by me or done under
my direct supervision.

All roughs to be done at Framing Inspection.
A printed copy of the permit and city stamped on-site plans must be available on-site for
the first inspection.
Approved per plans and red -line notes on plans. Must comply with 2012 IRC, 2017 NEC and all
applicable City of Wheat Ridge Municipal Codes. Work is subject to field inspections.
As part of the foundation inspections and PRIOR TO proceeding with any further
construction/inspections, a completed, signed and sealed, Foundation Setback and Elevation
Certification shall be submitted to the City for review. The certification form shall be
fully completed by a Professional Land Surveyor licensed in the state of Colorado, stating
that the foundation was constructed in compliance with all applicable minimum setback and
elevation requirements of the City.
Owner/Contractor is responsible for locating property lines and constructing improvements
according to the approved plan and required development standards. The City is not
responsible for inaccurate information submitted within the plan set and any construction
errors resulting from inaccurate information.
For one- or two-family dwellings, the first twenty-five feet ( 2 5 ' ) of driveway area from
the existing edge of pavement into the site shall be surfaced with concrete, asphalt, brick
pavers or similar materials. A right-of-way permit may be required; contact Public Works
Department at 303-235-2861.
NOT APPROVED AS AN INDEPENDENT DWELLING UNIT.
Prior to issuance of a CO, 2 tree(s) shall be planted within the front setback.
In addition, no less than twenty-five percent (25°%) of the gross lot area and no less than

Engineer, offers the following in response to the site plan received July 9, 2020, concerning a proposed addition at the
above referenced address. The Wheat Ridge Sanitation District has no objections with the proposed development
conditional on the items identified herein are fully addressed. The following are general requirements for sanitary
sewer service. Rules, Regulations and Standards of the District must be complied with at all times.
The subject lot appears to be entirely within the service and boundary area of the Wheat Ridge Sanitation District.
Treatment of sewage generated within the Wheat Ridge Sanitation District is provided by Metro Wastewater
Reclamation District.
Existing District Sanitary Sewer Mainline(s)
Wheat Ridge Sanitation District has the following sanitary sewer mains in the vicinity of the proposed property:
• 8 -inch mainline running north to south, west of the referenced property within Lamar Street.
It appears the property may be serviced by gravity flow; however, the owner/developer is responsible for determining
horizontal locations and vertical depths of the existing sanitary sewer main(s) and potential conflicting utilities, to verify
if service and gravity flow is feasible or if a private individual sewage ejector (lift station) is required. The issuance of a
sewer tap permit by the District does not in any way guarantee that the property can be served by gravity flow. No
underdrain or storm sump pump connections to District facilities are allowed.
Utility Locates
All projects must meet Colorado Revised Statue Title 9 for proper notification to the Notification Association by the
Developer's professional engineer. It is also the owner's responsibility to include electronic locators on all new
underground facilities, including service lines up to the structure or building being served. Submit details for tracer
wire and associated test stations.
Proposed Addition
If the proposed addition requires a water meter upsizing, the District will need to review and approve plans prior to
construction. If a water meter size increase takes place, the District must be informed immediately, and appropriate
sanitary tap fees must be paid prior to acceptance. In addition, the District will require review to determine if an
additional service connection and associated tap fee is required. Please provide a letter from the water service provider
with a determination of water meter size.

1. Introduction
Schafer Environmental Consulting, Inc. (Schafer) performed asbestos inspection services
at the 2696 Lamar Street in Wheat Ridge, Colorado 80214 (project site) on February 5,
2020. The inspection services were performed in accordance with applicable regulations
and industry standards. The inspection services were performed to facilitate planned
renovation of the kitchen, dining room and living room and demolition of the attached
garage. The building is reportedto be approximately 1,582 square feet constructed in 1941.
Only areas that of planned renovation activities were inspected.
2. Asbestos Inspection
The bulk samples of the suspect asbestos containing materials (ACM) were collected in a
random and representative manner, as determined by the inspector in accordance with
applicable regulations. The inspector collected bulk material samples by taking a
composite core that included all layers within the suspect ACM.
The bulk samples collected were representative of suspect ACM for each of the
homogeneous area Samples were collected in compliance with Colorado Department of
Public Health and Environment (CDPHE) Asbestos Control Regulations and
Environmental Protection Agency (EPA) Asbestos Hazard Emergency Response Act
(AHERA) 40 CFR, Part 763. Representative sampling was based on the distribution of the
suspect material throughout the homogeneous area, and the suspect material's physical
characteristics and application.
Applicable asbestos regulations define ACM as a material containing greater than one
percent (1%) asbestos by weight, volume or visual surface area and also distinguish
between friable and non -friable forms of ACM. Friable ACM can be crumbled or reduced
to powder by hand pressure when dry. Non -friable materials cannot be crumbled,
pulverized, or reduced to powder by hand pressure.
The EPA National Emission Standards for Hazardous Air Pollutants (NESHAP) further
classify non -friable ACM as Category I and IL Category I non -friable ACM include
asbestos containing packings, gaskets, resilient floor coverings, and asphalt roofing
products. Category II non -friable ACM are defined as all other non -friable ACM, such as
cement board, window putty, etc. Regulated ACM is friable ACM or ACM that will be
rendered friable by demolition or renovation activities. During abatement, renovation and
demolition projects, applicable regulations identify removal requirements based on the
quantity and friability classification of an ACM. The EPA NESHAP and CDPHE
regulations require point count analysis for friable materials found to contain detectable
asbestos at 1% or less by Polarized Light Microscopy (PLM) analysis.
The results of sample analysis were considered representative of materials in each
homogeneous area if:
• They exhibited similar physical characteristics.

• The application of the sampled material could be correlated to the application of
un -sampled material.
Suspect materials that are similar in appearance and application were sampled as
homogeneous areas. Suspect ACMs were separated into the following three classifications
with specific sampling requirements:
Surfacing Materials - Trowel or sprayed applied materials
• Three samples of each homogeneous area 1,000 square feet or less.
• Five samples of each homogeneous area greater than 1,000 square feet and less than
5,000 square feet.
• Seven samples of each homogeneous area greater than 5,000 square feet.
Thermal System Insulation
• Three samples of each homogeneous area.
Miscellaneous Materials - Other suspect materials not classified in the above
categories
• Sufficient number to determine asbestos content.
The bulk suspect asbestos samples were submitted to an accredited laboratory for Polarized
Light Microscopy (PLM) analysis. Laboratories used are National Voluntary Laboratory
Accreditation Program (NVLAP) accredited laboratory and is also accredited by the
American Industrial Hygiene Association (AIHA).
The bulk samples were analyzed by PLM coupled with Dispersion Staining as detailed in
the EPA's "Interim method for Determination of Asbestos in Bulk Insulation Samples"
(EPA 40 CFR 763 Subpart F, Appendix A). The total asbestos reported is the average of
all components in the material, with the asbestos content of separate layers also identified.
Unused portions of samples are archived for 60 days unless the client requests special
handling.
Quality Assurance and Quality Control
Schafer performed an internal review of field data and laboratory analysis reports to ensure
that data generated in this report is accurate and complete. Quality control procedures were
performed by CDPHE and EPA certified inspection personnel that included cross
referencing inspector field notes and sample logs with the laboratory PLM bulk sample
analysis data to confirm sample numbers and material descriptions for each sample
submitted for analysis.

Material Condition Assessment
Each suspect material was classified as friable, Category I nonfriable, or Category II
nonfriable, according to the NESHAP definitions.
Schafer performed homogeneous area/material condition assessments as part of this
investigation. The condition of suspect materials was evaluated as "good", "damaged", or
"significantly damaged" using the following parameters:
Good- material with no visible damage or deterioration or showing only very
limited damage or deterioration.
Damaged- material which has deteriorated or sustained physical injury such that
the internal structure (cohesion) of the material is inadequate or, if applicable,
which has delaminated such that the bond to the substrate (adhesion) is inadequate
or which for any other reason lacks fiber cohesion or adhesion qualities. Damaged
materials are those that have <10% scattered or <25% localized damage.
Significantly Damaged- material which has extensive and severe damage.
Significantly damaged materials are those that have >10% scattered or > 25%
localized damage.
The ACM Assessment included documentation of the Type of Damage, if present, as
follows:
Deterioration - (i.e., crumbled, blistered, or loss of adhesion)
Physical Damage - (i.e., scrapes or gouges)
Water Damage - (i.e., water stains)
Air Erosion - (i.e., elevator shaft, fan room, or ventilator air stream)
Vibration - (i.e., room use, motor/engine, or ducts vibrating but no fan in area)
Suspect ACM was also assessed based on accessibility and potential for disturbance. The
AHERA Assessment Categories (1-7), as numbered below, utilize the term Asbestos
Containing Building Material (ACBM). Schafer's condition assessment also referenced
the AHERA categories for friable materials as a guideline for determining appropriate
recommended response actions. Schafer may also recommend removal by a state certified
asbestos abatement contractor, or General Abatement Contractor (GAC).

Appendix A— Suspect Asbestos Material Photographs. Asbestos laboratory analytical data
is presented in Appendix B.
4. Conclusions and Recommendations
Asbestos Containing Materials
The confirmed ACM identified at the project site are listed below with general location,
friability and the approximate quantities.

The following materials were found to asbestos at equal to or less than 1% and are not
considered ACM by EPA and CDPHE. OSHA regulations still apply.
➢ Plaster (PL1) - Smooth Finish Plaster - Gray Granular
o Location — Kitchen and Laundry Room and Throughout
o Quantity -350 SF to be Impacted
o Assessment — Good Condition - Friable
o Potential for Damage — High due to planned renovation
o Recommendation — Removal by GAC or OSHA trained personnel in
accordance with OSHA Trace Asbestos Requirements prior to renovation

The Plasters were found to contain trace amounts of asbestos. Per regulatory requirements,
point count analysis was performed and confirmed that the plasters contained less than
0.25% asbestos. The EPA and CDPHE do not consider materials containing 1% or less as
asbestos containing materials.
Schafer recommends that activities that may impact plaster be coordinated with the
designated asbestos consultant to prevent uncontrolled disturbance of plaster, maintain
regulatory compliance and to prevent asbestos fiber release protecting human health and
the environment. Schafer recommends that an asbestos abatement contractor perform
removal of trace asbestos plaster prior to renovation activities.
Recommendations
Renovation activities should be coordinated with an asbestos consultant to prevent
uncontrolled disturbance of ACM and trace asbestos, maintain regulatory compliance and
to prevent asbestos fiber release protecting human health and the environment. This report
is not an asbestos abatement specification.
Schafer recommends that an asbestos abatement contractor perform asbestos removal
required prior to renovation and that abatement oversight, post removal visual inspection
and air sampling be performed by a qualified asbestos consulting firm.
Schafer can provide these additional Asbestos Consulting Services upon request.
Certain precautions described in OSHA 29 CFR 1926.1101 are universal and apply to all
work with asbestos, regardless of airborne exposures, or asbestos content of materials.
These requirements include wet methods, and to promptly clean up and dispose in closed
containers, waste and debris contaminated with asbestos. OSHA also requires regulated
areas, training and asbestos exposure assessment air monitoring performed by an asbestos
competent person.
General Recommendations
Removal of ACM is not required by regulations unless the potential for an airborne release
of asbestos fibers exceeding allowable levels exists. Typically, the potential for an airborne
release of asbestos fibers exists in three circumstances.
1. The ACM will be impacted during maintenance, renovation, or demolition.
2. The material is significantly deteriorated to the extent that asbestos debris is
present.
3. The ACM will be subject to future deterioration by vibration, airflow, or
weathering.
A compliant alternate to abatement of intact ACM is in-place management through an
Operations and Maintenance (O&M) Program. The EPA has stated in its Summary of

Managing Asbestos In-place (Green Book), - 1990 "Intact and undisturbed asbestos
materials do not pose a health risk."
ACM is required to be abated and/or managed properly during maintenance and renovation
activities that may impact the ACM in order to prevent the release of airborne asbestos
fibers. In accordance with EPA and CDPHE requirements, asbestos abatement permits are
required if greater than trigger levels of ACM will be abated.
The OSHA requires that building owners notify the following persons of the presence,
location, and quantity of ACM and presumed ACM in their buildings and facilities.
1. Prospective employers applying or bidding for work whose employees reasonably
can be expected to work in or adjacent to areas containing such materials;
2. Employees of the owner who will work in or adjacent to areas containing such
materials;
3. On multi-employer worksites, all employers of employees who will be performing
work in or adjacent to areas containing such materials;
4. Tenants who will occupy areas containing such materials.
The OSHA requires contractors and workers be notified of the presence, quantity and
location of asbestos and be protected from exposure to asbestos above the excursion limit
of 1.0 Fibers per cubic centimeter (F/cc) and the permissible exposure limit of 0.10 F/cc
over an 8 -hour time weighted average for more than 30 days per year.

5. Limitations
Schafer warrants that the findings contained herein have been prepared with the level of
care and skill ordinarily exercised by professionals practicing in the industrial hygiene and
environmental consulting industry. Certifications are provided in Appendix D. The scope
of work addressed readily accessible and/or exposed interior building areas. Observation
or sampling of inaccessible areas and materials, such as behind walls, was not performed
in all cases. Schafer did not perform destructive access investigations or sampling that
would excessively damage building materials. Schafer does not imply or guarantee that
every suspect ACM on or in the building has been identified or sampled. In the event of
undiscovered suspect asbestos materials that were not identified during this investigation,
the suspect material is required to be assumed to contain asbestos until evaluation,
sampling and analytical data confirm otherwise.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A;�-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave

This permit was issued in accordance with the provisions set forth in yopur application and is subject to the laws of the State of Colorado and to the Zoning
Regulations and Building Code of Wheat Rid~e, Colorado or any other applicable ordinances of the City.
This permit shall expire if (A) the work authonzed is not commenced within sixty (60) days from Issue date or (8) the building authorized is suspended or
abandoned for a period of 1:20 day's.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection carel before
proceediing with successive phases Ofthe/"Ob.
