Both the front and back of this permit are required to be posted on the job site at all
times. If the complete permit is not present, inspections WILL NOT be performed.
Work shall comply with 2018 IRC & 2020 NEC. Smoke alarms shall comply with 2018 IRC Section
134. Interior alterations, repairs, fuel -fired appliance replacements, or additions, any of
which require a building permit, occurs or where one or more rooms lawfully used for
sleeping purposes shall have an operational carbon monoxide alarm installed within fifteen
feet of the entrance to each room lawfully used for sleeping purposes.
I, by my signature, do hereby attest that the work to be performed shall comply with all accompanying approved plans and specifications;
applicable building codes, and all applicable municipal codes, policies and procedures, and that I am the legal owner or have been authorized

I . This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a changeof the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer&439;s technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Roofing & Gutters Inc reserves the right to file for supplemental insurance claims
when necessary, which shall become a part of this agreement. KAPE Roofing & Gutters
Inc, as the contractor, is entitled to the full O&P provided by the insurance company.
Building Owner (s) (Customer) hereby retains Kape Roofing & Gutters, Inc. (KAPE) to repair/replace the roof and/or gutters upon the above stated property, contingent upon
KAPE' s receipt of Customer's insurance company (Carrier) approval and payment of said work. Customer understands and agrees that KAPE Roofing & Gutters, Inc. will
review said work proposal with Carrier's representative. Work will commence only after KAPE and carrier agree upon the extent of damages and the cost of repairs and
replacements. This agreement is for full scope of insurance proceeds not excluding deductible, depreciation, overhead, and profit, therefore CUSTOMER'S OUT OF
POCKET EXPENSE WILL BE CUSTOMER'S INSURANCE POLICY DEDUCTIBLE AND ANY NON RECOVERABLE DEPRECIATION for work agreed upon
between Carrier and KAPE. AS PER CO LAW SB38, KAPE CANNOT PAY WAIVE OR REBATE THE DEDUCTIBLE IN PART OR IN WHOLE. Customer agrees to
provide all necessary documentation for the claim, and any documentation necessary to facilitate payment from the Carrier or Mortgage Company.
Carrier only pays for installed materials. All surplus material is property of KAPE. Replacement of deteriorated siding, decking, fascia board, ventilators, flashing, or other
materials is not included and will be charged as extra unless otherwise stated above.
Customer may be required to supply all electricity necessary for the work. Lien waiver submitted upon final payments. KAPE is not responsible for interior personal items .
All work is to be completed in a workmanlike manner according to standard practices. The Customer hereby acknowledges that KAPE may be subject to delays due to
inclement weather or material shortages, which are each beyond the control of KAPE and therefore accepts any delays that may occur by one or all of these circumstances
upon completion of said work. Customer grants access to the property as is necessary for the setup and execution of work. Any changes from the above specifications
involving extra costs will be executed only upon written order and will become and extra charge over and above the estimate.
WARRANTY TERMS: KAPE Roofing & Gutters, Inc., agrees to repair any defect in workmanship for a period of two (2) years following work completion. KAPE' s
liability is limited to its cost to affect any warranty repairs. Warranty does not cover damage to roofs caused by lightning, gale (>50mph), hurricane, tornado, hailstorm, or
other violent storm, impact of foreign objects or damage to roofs due to settlement, distortion, failure or cracking of the roof deck, walls, or foundation of building. This
WARRANTY is null and void if payment, or any other terms of the contract have not been met. THIS WARRANTY IS NON -TRANSFERABLE. All statements on reverse
side apply to contract.

This contract and any agreement made pursuant thereto is between KAPE Roofing hereinafter referred to as the "Co." or "Company" and the
customer(s) named herein on the reverse side will be subject to all appropriate laws, regulations, and ordinances of the states of CO, KS, NE and the
terms and conditions:
I FUNDER THE RIGHT TO RESCIND, YOU HAVE UNTIL MIDNIGHT OF THE THIRD BUSINESS DAY TO CANCEL THE WORK TO BE PERFORMED, DAY ONE
BEGINS AFTER THIS CONTRACT IS SIGNED. If this contract is signed under insurance porceeds, the homeowner has the right to cancel this contract within 72 hours
after receiving notice from their insurer that the claim is denied in part or in whole. Any monies paid by the homeowner will be refunded in full within 72 hours.
2.SHOULD DEFAULT BE MADE IN PAYMENT OF THIS CONTRACT, CHARGES SHALL BE ADDED FROM THE DATE THEREOF AT A RATE OF ONE AND ONE
HALF (1 112) PERCENT PER MONTH (18% PER ANNUM) WITH A MINIMUM CHARGE OF $2.00 PER MONTH, AND IF PLACED IN THE HAND OF AN ATTORNEY
FOR COLLECTION, ALL ATTORNEY'S FEES AND LEGAL FILING FEES SHALL BE PAID BY CUSTOMER ACCEPTING SAID CONTRACT.
3.The Company is not responsible for unseen site conditions.
4.The Company shall have no responsibility for damages from rain, fire, tornado windstorm, or other perils, as is normally contemplated to be covered by HOME
OWNERS INSURANCE or BUSINESS RISK INSURANCE, or unless a specified written agreement be made therefore prior to commencement of the work.
5.The quotation or the face hereof does not include expenses or charges for bond insurance premiums or costs beyond normal insurance coverage, and any such
additional expenses, premiums or costs shall be added to the amount of the contract.
6.Replacement of deteriorated decking, fascia board, roof jacks, ventilators, flashing or other materials unless otherwise STATED IN THIS CONTRACT, are NOT
INCLUDED and will be charged as an extra on a time and material basis.
%.The Company shall not be liable for failure of performances due to labor controversies, strikes, fires, weather, inability to obtain materials from usual sources, or any
other circumstances beyond the control of the company, whether of a similar or dissimilar nature.
8. If roofing and sheet metal work is involved, it is understood and agreed to that our standard roof guarantee, a copy of which is available in our office, shall be

9.The Company is not responsible for any damage on or below the roof due to leaks by excessive wind driven rain, ice, or hail during the period of the warranty.
EXCESSIVE WIND IS 50 OR GREATER M.P.H. THE WARRANTY IS NONTRANSFERABLE.
10. If material has to be reordered or restocked because of a cancellation by the customer there will be a RESTOCKING FEE equal to fifteen percent (15%) of the
contract price.
I I .This contract or warranty shall not be assigned except by or with the written permission of the company.
12. IF THIS CONTRACT IS CANCELED BY THE CUSTOMER, customer shall pay the company fifteen percent (15%) of the contract price as liquidated damages, not
as a penalty, and the company agrees to accept such as a reasonable and just compensation for said cancellation.

commencent of work, will be hold in trust until the materials are delivered and the work begins.
15. If any provision of this contract should be held to be invalid or unenforceable the validity and enforceability of the remaining provisions of the contract shall not be
affected thereby.
16.ANY REPRESENTATIONS, STATEMENTS, OR OTHER COMMUNICATIONS NOT WRITTEN ON THIS CONTRACTARE AGREED TO BE IMMATERIAL, and not
relied on by either party, and do not survived the execution of this contract.

21.The Co. is not responsible for construction problems of your home. If pointed out and notified to our Co., we will try to assist you on correcting them on a time and

inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection

This permit shall expire If (A) the work authonzed is not commenced within sixty (60) days from issue date or (B) the building authorized is suspended or
abandoned for a period of 120 days.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
(4) No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
(5) Contractor shall notify the Building Insreetor twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceediing with successive phases 0 the job.
(6) The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor an approval of, any violation of the provisions
