written approval from the City of Wheat Ridge BUILDING DIVISION, regardless of inspection
by any Sanitation District representative. Inspection and approval of work by any
Sanitation District representative DOES NOT grant authority to cover work without the
approval of the Building Division.
**For trenchless sewer replacements - The contractor will verify proper slope of new sewer
piping in conformance with the IRC and IPC of 1/4 inch per foot unless otherwise allowed
specifically by the applicable code. The plumbing contractor will provide verification of
proper slope and drainage in writing as a condition of approval of the permitted work.**

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

I/We authorize American Heritage Restoration LLC. (AHR) to work directly with my insurance company on my behalf and
furnish material and labor as listed below for the Agreed Insurance Replacement Cost Value.
Property owners out of pocket expense will not exceed their insurance deductible in the amount of $ 1000

No down payment is required. First payment is due upon delivery of materials and start of work. All insurance

NOTICE
THE CONTRACT ON THE FACE HEREOF AND ANY AGREEMENT MADE PURSUANT THERETO BETWEEN AMERICAN
HERITAGE RESTORATION LLC. (THE "COMPANY") AND CUSTOMER(S) WILL BE SUBJECT TO ALL APPROPRIATE LAWS,
REGULATIONS AND ORDINANCES, AND TO THE FOLLOWING SPECIAL TERMS AND CONDITIONS.
1. All Agreements shall be subject to approval by our credit department and office without exception. Approval must be obtained from the office for this
contract to be effective under any conditions.
2. American Heritage Restoration LLC. has the right to order excess material. These materials will not be charged above the agreed price. All excess material
belongs to American Heritage Restoration LLC. If materials need to be restocked or reordered due to a cancellation from the customer, there will be a
RESTOCKING FEE of $500.00.
3. Supplement paid by the Insurance Company for the additional labor and materials needed beyond the original scope of repairs shall be paid directly to
American Heritage Restoration LLC for work that AHR has completed.
4. All Material is guaranteed as specified. All work to be completed in a workmanlike manner according to standard practices. Any alterations or deviation
from above specifications involving extra costs will be executed only upon written orders, and will become an extra charge over and above the estimate. All
agreements contingent upon strikes, accidents or delays beyond our control. American Heritage Restoration LLC. shall have no responsibility for damages
from rain, fire, tornado, windstorm, or other perils. Homeowner to carry fire, tornado and other necessary insurance.
5. This Agreement constitutes the entire agreement between the parties. It may be changed only by written instrument signed by both parties.
6. If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceable of the remaining provisions of this agreement
shall not be affected hereby.
7. Any representation, statements or other communications not written in this contract is agreed to be immaterial, and not relied on by either party, and do not
survive the execution of this contract.
8. The company is not responsible for any damage below the roof, due to leaks by winds in excess of 45 mph, ice dams, hail, pre-existing or future construction
defects caused by storms or lack of maintenance during the period of the warranty or depressions in driveway caused by trailers or dumpsters. The company
is also not responsible for nail pops.
9. The company will take appropriate means necessary to protect the roof during the repair of the roof and during the entire duration of the work, the
homeowner's insurance will be responsible for ANY interior damages.
10. The dollar amount of the contract is the amount approved by the Insurance Company for the scope of loss, plus overhead and profit (if applicable) and any
agreed to upgrades and deductible. ANY replacement of damaged or deteriorated decking, flashing, roof jacks, fascia boards, vents or other materials unless
included in this contract, are considered NOT INCLUDED and will be charged for material and labor.
11. The final payment in full shall not be held up while waiting for the city to inspect the work the company agrees to meet or exceed all city inspection
requirements.
12. First payment is to be one half of contract price or balance of first check from insurance company (No Cash). Additional payments will be paid in full upon
completion of each segment. Any unpaid balance shall be due upon final completion. Any balance unpaid after thirty (30) days of completion will be
deemed in breach of contract and Homeowners held in default.
13. Should Homeowners default on this contract, charges shall be added from date thereof at a rate of eighteen percent per annum (18% PER ANNUM); and if
placed in the hands of an attorney for collection, all attorney fees, costs, and legal filing fees shall be paid by customer accepting said contract.
14. Title to job shall remain in contractor's titles until such a time homeowner shall pay for job in full. Homeowner does hereby authorize contractor or its
designated representative to enter upon the premises of the homeowner and remove the job in the event of default of payment of any of the installment of the
contract price when due. Contractor shall be responsible for any damages that may be caused to the premises of the homeowner of the result of the
installation or the removal of said job and equipment in the event of default as herein set forth.
15. These conditions shall be considered as part of any contract entered into, including the contingent agreement when the work is approved and will be paid for
by the Insurance Company, the same as if they were included herein.
16. The company is not responsible satellite dish removal, resetting, or damage on the house. Homeowner's insurance company will pay to have the satellite
dish removed and reset properly. The company does not take responsibility for damage to solar panels on the roof during repair; homeowner agrees to take
necessary means in removing, protecting, and re -installation of solar panels at homeowner's expense.
17. The company will not take responsibility for construction problems on your home. The company will point out and inform the homeowner on correcting the
problem on a materials and labor basis.
18. Homeowner's agreement to the contingency statement on the front of this contract holds the entire contract as valid and binding as long as the Insurance
Company approves to pay homeowner for the loss.
19. Roof replacement warranty is for 5 years, 2 years siding replacement, and 1 year on gutter installation.
20. You, the buyer, may cancel this transaction at any time prior to midnight of the third working day after the date of this transaction. To cancel this transaction,
mail or deliver a signed and dated copy of this cancellation notice or any other written notice, or send a telegram to: American Heritage Restoration LLC.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mlid-roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.
