Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. The Building Division permit counter is open from
7:30 am to 3:30 pm, Monday through Friday to process these types of requests.

EPA, Housing and Urban Development (HUD) and Colorado Department of Public Health and Environment
(CDPHE) define lead-based paint as having a lead content greater than or equal to 0.5% by weight when
analyzed by AAS or 1.0 milligrams/per square centimeter (mg/cm2) when analyzed by a x-ray fluoresces
(XRF) device.
The above listed components that have less than 1.0 mg/cm2 or 0.5% lead content are classed as lead -
containing paint and are subject to Occupational Health and Safety Administration (OSHA) worker protection
regulations.
OSHA Construction Industry Standard for Lead Regulation 29 CFR 1926.62 requires the employer to address
worker protection items including but not limited to: worker training, respiratory protection, medical
surveillance, work practices, personal air monitoring and exposure assessments. OSHA maintains a personal
exposure level of 50 micrograms per cubic meter (pg/M3) over a time weighted 8 hour period. Please also
keep in mind that other OSHA regulations may apply to work that may disturb lead-based and lead-containi
paint such as Hazard Communication and Respiratory Protection. I

repair/renovation areas at the direction of the client. This limited LBP screening report is not

Following the removal of ACM within the contained work area, BuildSafe's Air Monitoring Specialist
(AMS) conducted a final visual inspection and performed FAC procedures according to CDPHE Reg.
No. 8. Part B. If successful, these activities show that the air within the work area is below the
Maximum Allowable Asbestos Level (MAAL) of 0.01 fibers/cc and acceptable for re -occupancy.
Final clearance air sampling was performed and samples were analyzed by a successful AIHA
participating member (see Air Sample Log).
Five (5) air samples were collected using high volume pumps. Sample collection draws a calibrated
amount of air through 25mm mixed cellulose ester membrane (Millipore 0.8 MCEF) filters, which a
housed in three-piece cassettes, equipped with 50mm electrically -conductive extension cowls,
Samples were collected with variable volume pumps. Flow rates were established and checked with
calibrated flow meter. I
Clearance samples were analyzed for airborne fibers by Phase Contrast Microscopy (PCM) per NIOSH
Method 7400 using a positive phase -contrast microscope equipped with a Walton -Beckett graticule
(Type G-22 forA counting rules). A quarter -wedge was cut from each sample filter and was
examined at a magnification of 400x. Fiber's greater than 5 microns in length with a length -to -width
(aspect) ratio equal to or greater than 3:1 were counted. Total fiber counts for each sample filter
were divided by their respective sample volumes. The resulting concentrations were expressed in
terms of total fibers per cubic centimeter of air (fibers/cc),

Phase Contrast Microscopy (PCM) is not asbestos -specific; rather, it counts Total Fibers, showi
worst-case situation because all fibers are counted (carpet, cloth, hair, paper, asbestos, etc). I
In order to pass PCM air sample analysis, microscopic analysis results from the area sampled during
the FAC testing must have fiber counts at or below the MAAL, which is the limit the EPA and the
CDPHE requires all samples to be equal to, or less than, prior to releasing an area to the public.
Based on visual observations, documentation review and air sample results, it is the opinion of
BuildSafe that the air quality within the work area was in compliance with regulatory and industry
standards at the time of this sampling inspection.

BuildSafe Environmental, Ltd. (BuildSafe) was contracted by the client to perform a
limited repair/renovation specific asbestos inspection (inspection) at the above referenced
"Site Address". The inspection was conducted by an Environmental Protection Agency
(EPA) accredited and Colorado Department of Public Health and Environment (CDPHE)
certified Asbestos Building Inspector. The inspection was performed at the direction of
the client and only includes those materials, locations, and quantities specified in the
"Homogenous Areas in Work Scope Table".
The client is planning repair/renovation activities that will disturb suspect Asbestos
Containing Building Materials (ACBM) within the repair/renovation affected areas at the
Site Address. CDPHE Air Quality Control Commission Regulation No. 8, Part B — Asbestos
(Reg 8) requires that all suspect ACBMs be properly inspected and sampled by certified
personnel prior to conducting any repair/renovation activities that will disturb the suspect
ACBMs. All samples were analyzed by a laboratory accredited under the National
Voluntary Laboratory Accreditation Program (NVLAP), The CDHPE has established the
following trigger levels for different types of properties, which specify how suspect and
confirmed ACBMs are inspected and subsequently abated:

Asbestos Inspection Procedure
The inspection procedure was conducted in accordance with EPA Asbestos Hazard and
Emergency Response Act (AHERA), CDPH E Reg 8, and applicable Occupational Safety
and Health Administration (OSHA) guidelines and regulations. These regulations specify
requirements and protocols for conducting asbestos inspections, sample analysis, and
asbestos abatement.

Homogenous Material Types
The inspection involved identification and sampling of suspect ACBMs in the areas
established by the client that would be part of the scope of work. Suspect ACBMs were
grouped into homogenous materials based on color, texture, and date of application.
Homogenous materials were given distinct codes (CDW01, FTCO2, TS05, etc.) based on
the type sequence of the homogenous material, Details about the size, color, condition,
friability, quantity, location within each functional space and other important details for
each homogenous material were reported as part of the inspection. Homogenous
materials are grouped into one of three categories based on intended use:
1. Surfacing Material — material that is sprayed on, troweled on, or otherwise

Homogenous Material Friability
Each homogenous material was given one of the three following friability categories
defined by the EPA and CDPHE:
1. Friable Material —materials that can be crumbled or reduced to powder by hand
pressure.
I Category I Non -friable Material — materials that cannot be crumbled or
reduced to powder by hand pressure including resilient floor coverings, gaskets,
packing materials and asphalt impregnated roofing materials.
I Category II Non -friable Materials — all other materials that cannot be
crumbled or reduced to powder by hand pressure that do not include category I
non -friable materials.
Homogenous Material Condition
Each homogenous material was given one of three condition categories based on visual
observation defined by the EPA and CIDPH:
1. Good Condition — No visible signs of damage were observed during the
inspection.
2. Damaged Condition — Visible signs of damage less than ten percent spread over
the entire material or twenty five percent localized.
I Significantly Damaged Condition — Severe damage greater than ten percent
spread over the entire material or twenty five percent localized.
Sample Collection
Bulk samples of each suspect homogenous material were collected randomly throughout
the homogenous material area within the scope of work. Bulk samples were sampled all
of the way through the material so that all associated layers within each homogenous
material are represented. Bulk samples were placed in closeable sample bags and then
labeled with a permanent marker with distinct sample identifiers (eg., CDW1-1, FTC2-1,
MISC2-3, etc.) for transport to the laboratory. Sample locations and photographs of the

Bulk samples were submitted to the laboratory along with a completed chain of custody
form to the above listed NVLAP accredited laboratory for analysis by polarized light
microscopy (PLM) techniques per EPA methodology (EPA -600/M4-82-020, Dec. 1982).
Microscopic visual estimation was used in obtaining the percentage of asbestos within
each layer of the bulk samples.
Asbestos Containing Building Materials
EPA and CDPHE both define an asbestos containing building material (ACBM) as a
material which has an asbestos content of greater than one percent. In the event that a
friable material is estimated to contain "'trace" or between zero -to -one percent, that
material must undergo a point -count analysis by the laboratory to confirm that the
material is trace asbestos -containing. Further, if a point count analysis is performed on a
bulk sample, the point count result takes precedence over the visual estimation result.
See appendix A for laboratory report and chain of custody documents.
BuildSafe has performed a limited repair/renovation specific asbestos inspection of the
home/facility located at the Site Address, Through visual inspection and laboratory
analysis of bulk samples collected at the Site Address, BuildSafe recommends hiring a
certified general asbestos abatement contractor to remove any ACBMs that may have
been identified within the scope of work. All inspection activities were conducted at the
direction of the client and only include those materials, quantities and locations detailed
in sections I and 2 of this report. See sections I and 2 of this report for information
regarding inspection scope of work, suspect ACBMs, confirmed ACBMs, sample
information and laboratory analysis results,
General Comments and Limitations

within the scope of work. However, for any structure, the existence of unique or
concealed suspect ACBMs is a strong possibility. BuildSafe only inspected and collected
suspect ACBM bulk samples in the repair/renovation areas at the direction of the client.
This limited repair/renovation asbestos inspection report is not considered a
detailed in sections 1 and 2 of this report.
Should further repair/renovation work expand outside of materials and locations listed in
sections I and 2 of this report, those locations must be properly inspected and sampled
by qualified personnel prior to disturbance of suspect ACBMs. Any subsequent abatement
R f ACBMs must be performed in accordance with all federal, state and local regulations
pertaining to abatement, transport, and disposal of ACBMs.
The asbestos inspection was conducted in a manner consistent with the level of care and
skill exercised by professionals of the asbestos inspection profession. The results,
findings, conclusions and recommendations expressed in this report are based on
conditions observed during our limited inspection of the repair/renovation areas. The
information contained in this report is relevant to the date on which this survey was
performed, and should not be relied upon to represent conditions at a later date. This
report has been prepared on behalf of and exclusively for use by the client for specific
application to the project as discussed. Contractors or consultants reviewing this report
must draw their own conclusions regarding further investigation or remediation deemed
necessary. BuildSafe does not warrant the work of regulatory agencies, laboratories or
other third -parties supplying information which may have been used in the preparation

Major or Minor Spill Response
In an event of an asbestos spill involving greater than the trigger levels, the buildiri
owner or contractor shall:
1. Restrict access to the area, and post warning signs to prevent entry to the
area by persons other than those necessary to respond to the incident.
2. Shut off or temporarily modify the air handling system to prevent the
distribution of asbestos fibers to other areas (if applicable)
3. (Only if above establislyed td_q_qer levels) Immediately contact the
CDPHE Air Quality Division by telephone, submit notification in compliance,
and apply for a permit.
4, Be exempted from the requirements to have a certified Supervisor on site at
all times, until such time as immediate danger has passed. Any clean up or
asbestos abatement that must occur after the immediate danger has passed
shall
®+ supervised by a person certified by the Division.
5Using certified Supervisors and certified Workers in accordance with
regulation, seal all openings between the contaminated and uncontaminated
areas and establish negative air pressure within the contaminated areas in
accordance with regulation, This is to be accomplished using polyethylene
sheeting to cover areas such as doorways, windows, elevator openings,
cM rridor entrances, grills, drains, grates, diffusers, and skylights.
6. HEPA vacuum or steam clean all carpets, drapes, upholstery, and other non -
clothing fabrics in the contaminated area, or discard these materials.

10. Following the completion of the work air samples shall be collected
aggressively as per regulation by an AHERA state -certified Air Monitoring
Specialist (AMS), except that the air stream of the leaf blower shall not be
directed at any friable ACM that remains in the area.

State o£. Colorado and to the Zoning Regulations and Building Codes. of Wheat Ridge, Colorado or any other applicable

2. This permit shall expire 180 days from the issue date. Requests for an extension must be received prior to expiration
date. An extension may be granted at the discretion of the Building Official.
3. If this permit expires,' a new permit may be acquired for a fee of one-half the amount normally required, provided no
changes have been or will be made in the original plans and specifications and any suspension or abandonment has not
exceeded one (1) year. If changes have been or if suspension or. abandonment exceeds one (1). year,. full. fees shall
be paid for a new permit.
