On Monday, June 28, 2004, Custom Environmental Services, Inc. mobilized to the above referenced site
to perform the removal and disposal of asbestos containing building materials. The Scope of work
included the removal and disposal of approximately 750 sq. ft. of asbestos containing ceiling texture from
drywall and approximately 750 sq. ft. of asbestos containing overspray on concrete in each of 10 similar
storage rooms. The removal and disposal of approximately 1,200 sq. ft. of asbestos containing ceiling
texture from plaster and approximately 900 sq. ft. of asbestos containing overspray on concrete in one
additional storage room.
The removal work was performed under full containment engineering controls including, the appropriate
work area postings, HEPA equipped negative pressure maintained at a minimum - .02 inches of water
column exhausted to the exterior, a view port capable of providing visual access to the work area no
smaller than 12" X 12", a minimum of 4 air changes per hour, 6 mil critical barriers, an attached waste
load out unit consisting of at least 2 chambers with triple flaps separating the chambers from the work
area and the exterior of the work area, an attached decontamination unit consisting of at least 3 chambers
with triple flaps separating the chambers from the work area and the exterior of the work area, the
decontamination unit was constructed with a fully functioning shower where all water used in the shower
was filtered down to 5 microns and disposed of into a municipal sanitary sewer, 2 layers of 6 mil
polyethylene sheeting on the walls, 2 layers of 6 mil polyethylene sheeting on the floors and waste
disposal in an approved EPA landfill. The work was performed in compliance with applicable EPA,
CDH and OSHA regulations. Project work areas were cleared by representatives of Nova Consulting
Group, Inc. after we were complete with identified scope of work.
All work was complete by the end of the work shift on Monday, November I, 2004, with project
demobilization.

Generated asbestos waste was disposed of at the Denver Arapahoe Disposal Site, 3500 South Gun Club
Road, Aurora, CO 80013.

1.0 INTRODUCTION
Nova Consulting Group, Inc. (Nova) provided on-site air monitoring/project management during
the removal of asbestos-containing ceiling materials located in the utility/laundry rooms off the
various buildings at the Luebke Apartments in Wheat Ridge, Colorado.
Removal was conducted using full containment asbestos abatement techniques. All asbestos-
related work was performed in accordance with federal, state, and local asbestos regulations.
The asbestos abatement/removal work began on approximately June 29, 2004 and was
completed on October 21, 2004. Custom Environmental Services, Inc. conducted the asbestos
abatement/removal.
Sample collection and analysis was completed by Nova Consulting Group, Inc. and Analytica
Solutions, Inc.
The spray-applied ceiling material was removed from the ceilings of eleven (11) individual
utility/laundry rooms located within nine (9) buildings. The ceiling substrate varied per building
but was either sheetrock or plaster. There were specific areas of inaccessible spray-applied
ceiling material that were left in-situ and these areas shall be documented in a future Operations
and Maintenance Plan document. The addresses of the nine (9) buildings subjected to the
abatement actions are:

On-site air monitoring was provided during the various phases of the work by Andy Brummer,
Pete Becofske and Molly Swartz. PCM sample collection and analysis was completed in
accordance with NIOSH Method 7400. Quality control procedures were followed as required by
this method. All final air clearance samples were submitted to Analytica Solutions, Inc. as a
third party analytical laboratory. Air sample analysis results can be found in the appendices.

3.0 WASTE TRACKING
All asbestos waste containers were labeled with the required EPA, DOT, and OSHA labels. All
generated asbestos waste was double wrapped in 6-mil polyethylene disposal bags. The waste
included, but was not limited to, spray-applied ceiling material, any associated wood
components, polyethylene sheeting utilized for the containment and set-up, used duct tape, the
workers' half-faced respirator filters, all rags, TYVEK suits, gloves and other asbestos-related
waste. All asbestos-related waste was adequately wetted before placement in the disposal bags
to reduce the potential for airborne asbestos fibers. The project waste containers were then
loaded onto a Custom Environmental Services, Inc. on-site enclosed dumpster for proper
transportation and disposal.

On September 16, 2004 Cindy Hagerman and I did a housing complaint inspection located
at 3638 and 3632 Depew Street in Wheat Ridge, Colorado. The inspection looked for
violations to the Uniform Housing Code and the Uniform Building Code regulations of
present day and when the apartments where built.
I inspected both units and tested the electrical oudets. All of the oudets worked ok. There
were no GFCI's in the bathrooms or kitchens, which was not required at the time of
construction. The electrical oudet for the stove was on the floor, which was also permissible
at the time of construction as well. The wiring appears copper in the walls from the oudets
that were inspected. The refrigerator and stove of both units worked just fIne at the time of
inspection. The toilets and sinks drained ok. However, I did note that the washing machine
water supply was shut off from the lack of knobs to turn the water on and off at 3638
Depew. 3638 Depew was occupied at the time of inspection and the tenants said they have
had no problems with wiring or plumbing. Exterior stairway lighting is not required under
the 1997 UBC, however exterior lighting will be required under the 2003 International
Building Code/ Residential Code.
The Building Department also inspected the asbestos mitigation work of the buildings both
before and after mitigation. All work being done is in compliance with the City Building
Code. I am waiting to receive letters from an asbestos inspector that all the buildings are
asbestos free.
The conclusion of the inspection is that there is still some asbestos work being done on the
property but all the other building code issues have been taken care of at the time of our
inspection on September 16, 2004.

ASBESTOS ABATEMENT PERMIT
This permit is granted subject to Colorado Air Quality Control Commission Regulation No.8, Part B, adopted December 18,2003,
and effective March 1,2004, the Air Quality Control Act C.R.S. (25-7-101 and 25-7-501 et seq) and the following provisions. It is
only for the purpose of allowing asbestos abatement.
ADDITIONAL PERMIT PROVISIONS:
By performing work under this permit the abatement contractor agrees that the Division may revoke or suspend this permit
should the Division fmd that the contractor:
· has violated or has aided and abetted in the violation of25-7-501 or 25-7-501 et seq., C.R.S. or Regulation No.8, or an order
of the Division or Commission,
· has failed to meet any permit and notification requirement or failed to correct any violations cited by the Division during any

DCM Science Laboratory, Inc. analyzes bulk asbestos samples following procedures developed by the
McCrone Research Institute and in compliance with guidelines established by the Environmental Protection
Agency (EPA-600/R-93/116, July, 1993).
Bulk samples are prepared for analysis using a 10X-60X stereo microscope in a hepa filter hood which provides
a contamination-free environment. The sample is then analyzed by polarized light microscopy (PLM) at 100X.
When the sample consists of more than one layer, each layer is prepared and analyzed separately. Fiber and
matrix materials are identified by the characterization of cptical properties including color and pleochrorism,
form, cleavage, relief, birefringence, extinction, orientation, twinning, interference figure and other distin . .
features. Dispersion staining is also used to further aid in mineral identification. All percentage as stos,
other fibers and non-fibrous constituents are calculated from the values obtained from the stereo a
microscopes analysis. In-house and NIST standards as well as a chart prepared by R.D. Terry and GY
Chilinger for "The Journal of Sedimentary Petrology", (Volume 24, pp. 229-234, 1955) provide a guide for
estimating percentages. All samples are archived for six months unless other arrangements are made by the
