Zurn, PE X2019 a STRUCTURAL GENERAL CONDITIONS Contractor shall maintain comprehensive general liability, workman's compensation, automobile liability and/or other insurance as appropriate for the work performed which will provide protection form any and all construction or completed operations claims. Certificates of Insurance shall be provided to LEVEL ENGINEERING LLC and/or the Client upon request. The limits will not be less than the minimum mandatory amounts required by the State of Colorado. All construction shall be performed in accordance with all applicable codes and standards, and good construction practices. Contractor shall obtain and pay for any and all permit fees, taxes and licenses required by governing agencies to complete the work. Contractor shall be responsible for all labor, materials, equipment, tools, operations and any other item necessary for proper completion of the work. Contractor should visit site prior to bid submittal to fully understand the scope of the project and the conditions at the site. By submitting a bid for the work proposed in this PLAN of REPAIR, the contractor is implying that they have fully reviewed all associated documents and that they have full understanding of the work to be completed. The Contractor shall conduct a full review of this PLAN of REPAIR prior to beginning work. If during this review or at any time during the construction process the contractor notes any errors or omissions in the plan, work shall be stopped and LEVEL ENGINEERING LLC shall be contacted immediately. Recommendations and specifications contained herein are based on general assumptions of the conditions, some of which cannot be observed during typical nondestructive visual observations. If unexpected conditions are encountered during the course of work, the Contractor shall contact LEVEL ENGINEERING LLC prior to proceeding. The Client shall provide water and electric hook-ups for temporary use. (Contractor must pre -arrange with Owner.) All utility lines and easements shall be located, marked and protected from damage throughout the performance of the work. St WhW Ridge THE REPAIR DESIGN Prepared by LEVEL Engineering, LLC makes no provisions for construction conditions that may arise during the performance of the work. Contractor shall be responsible for maintaining the stability and integrity of existing structures (bracing, shoring, benching, etc.. Contractor shall be responsible for the acts or omissions of his/her agents and subcontractors in the performance of their work. The Contractor shall be responsible for the means, methods, techniques, and sequences of construction. Existing material or property outside the scope of work that is damaged during construction shall be repaired to match their pre -construction condition at Contractor's expense. Unless noted otherwise, no substitutions or deviations from the repair plan and specifications are permitted without the express approval of LEVEL ENGINEERING LLC. Unauthorized modifications to the repair plan and specifications may deem the work unacceptable. All unacceptable work shall be promptly correct at the Contractor's expense under the direction of the LEVEL ENGINEERING LLC. Contractor shall maintain safe jobsite. The Contractor is to stack all necessary equipment and materials in a low traffic rea or in an area that will insure the safety of all individuals potentially present at this site during construction. OSHA standards shall be used as a guideline. Contractor shall keep the jobsite thoroughly clean at the end of each day. All surplus materials shall be safely stacked in a low traffic area or hauled away from the site. Contractor agrees to perform the work in a diligent manner and in full conformance with the repair plan. If a condition arises during the repair process which requires a change in the scope of work, LEVEL ENGINEERING LLC may submit addendums or modifications to the design which may add, delete or modify the scope of repair. Performance of any extra work, or payment for same, shall only be considered upon prior written authorization from Client. This document is for bid purposes only, The Contractor shall be responsible for utilizing a qualified Colorado Licensed Professional Engineer to inspect the repair work and to verify that the repair specifications described within this document are in compliance with local building codes and applicable Manufacturer standards. The proposal shall include a description of the Contractor's Warranty Terms and Conditions was well as Warranty Limitations. The proposal shall also include a description of the system as applicable. STEEL REINFORCEMENT All reinforcement steel shall be deformed bars conforming to ASTM A-615, grade 60. Welding of steel reinforcement shall conform to AWS D-1.4. Provide chairs concrete blocks, spacer bars, ties, etc. As required to securely hold bars in place. When reinforcement must be spliced the bars should overlap at least 24', UNO. Overlapping bars shall be securely tied together with wire ties. Provide a minimum concrete cover of 3" where concrete is cast on the surface of the ground and 1-12" at all other locations, UNO. STRUCTURAL STEEL AND BOLTS All steel fabrication and installation shall conform to the latest addition of the RISC Manual of Steel Construction. The Helical Pile Contractor shall remove any and all material, equipment, tools, building materials, concrete forms, debris or other items belonging to the Contractor or used under the Contractor's direction. EN® OF SPECIFICATION LIMITATIONS This repair design does not represent a complete analysis of the entire structure; it addresses conditions covered under the structural warranty provided by the contractor. The interpretations of the distress to be repaired are based on the visual observations of the structure. No detailed study was made of the structural elements of any part of the building. Therefore, no opinion is presented or shall be implied concerning the structural integrity of those elements not subject to visual inspection. Investigation and analysis of other portions of the structure are beyond the scope of this design at this time. LEVEL Engineering, LLC will not be responsible for 1) knowledge of subsurface conditions away from the existing borings, if any, without extensive geotechnical information supplied by a competent geotechnical engineer, 2) knowledge of distresses, or differential displacements that have occurred in an area which had not been exposed for visual observation during our site visit and 3) any other elements such as joists or beams and other structural members that are not readily visible. LEVEL Engineering, LLC assumes that all information provided by the Client, Contractor, or other Consultants is correct, accurate, and may be acted upon accordingly, and shall have no liability for the validity of information provided. LEVEL Engineering, LLC shall have no liability for acts or omissions of the Contractor or any other agents performing the work on this project or for the failure of the Contractor to carry out the work in accordance with the plans and specifications if not inspect as intended. This PLAN of REPAIR is to be used with the accompanying drawings. WORK DESCRIPTION For the purposes of this report, the front of the home is considered to face East. The structure of the single -story single-family home is a conventionally framed residence. The home has an attached garage at the south side of the home. The home has a finished full basement with a majority of the basement accessible through an interior stair. The full basement has a CMU and concrete foundation placed on spread footings. The foundation system consists of partially reinforced concrete masonry units with basement walls approximately 7 feet in height. The work areas are the southern full height foundation walls. The repairs shall consist of replacing 18 feet of existing CMU foundation with fully reinforced 8 -inch -thick concrete walls. All restoration of finishes exterior and interior flatwork shall be restored in kind and in accordance with the owner's approval. The contractor is responsible for coordinating these type of repair scopes with the contractor. The contractor is required to coordinate these expectations with the owner and include the necessary work items in their bid proposals. PROPOSED COMPLIANCE OBSERVATIONS All observations shall be conducted by a qualified Colorado Licensed Professional Engineer. More than one observation may be conducted during a site visit. The contractor is to determine the number of site visits required to do the observations and include this information in the bid documents. Resitlenfial - 32 SF., 50 LE, equivalent of a 55 -gallon drum (2) Cozamercial or Demolition - 160 square feet (160 SF), 260 linear feet (260 LF), or the equivalent of a 55 -gallon drum). Damaged Condition (DZ Materials are in damaged condition if they have a, surface which is crumbling, blistered, water - stained, gouged, marred, or otherwise abraded over less than one-tenth of the surface if the damage is evenly distributed or less than twenty five percent of the surface if the damage is localized. EPA Lead "Trigger Levels": An inspection must occur if more than 6 SF of interior surfaces; 20 SF of exterior surfaces will be disturbed; or ANY window is being replaced. Friable (jD: Friable materials are materials that can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Good Condition (G): Materials are in good condition if they have a surface with no visible damage or deterioration; or showing only very limited damage or deterioration. Homogeneous Area (HA. Materials can be classified as the same HA if they exhibit similar physical characteristics; and the application of the sampled material can be correlated to the application of un -sampled materials. Miscellaneous Materials (,DM: These materials are other suspect materials, including flooring, ceiling tiles, insulation, and finishing materials. Each homogeneous area/different ALVI material requires a minimum of two (2) samples; or enough samples sufficient to determine the asbestos content, Non -Friable (M. Non -Friable materials are materials that cannot be crumbled, pulverized, or reduced to powder by hand pressure when dry. A NF material may become F if its condition had deteriorated or if it had been impacted by forces that have rendered it F. OSHA "Trigger Levels": 10 SF, 25 SF, or 35 CF; however, OSHA states "no amount of asbestos is safe". Significantly Damaged Condition (SD. Materials are in significantly damaged condition if they have a surface that is crumbling, blistered, water -stained, gouged, marred, or otherwise abraded over at least one-tenth of the surface if the damage is evenly- distributed or twenty five percent of the surface if the damage is localized; or a surface that is hanging from the end, deteriorated, or showing adhesive failure. Surfacing Materials (SIM: These materials are sprayed or troweled onto structural building member. Materials of up to 1,000 SF of material requires a minimum of three (3) samples; between 1,000 SF and 5,000 SF of material requires a, minimum of five (5) samples; over 5,000 SF of material requires a, minimum of seven (7) samples; at least one (1) sample of each patch. Thermal Systems Insulation (TSI. These materials are any type of pipe, boiler, tank, or duct insulation. Each homogeneous area/different TSI material requires a minimum of three (3) samples; at least one (1) sample must be collected from each patch; and collect enough samples sufficient to adequately- assess the material and determine the asbestos content for TSI fittings such as pipe elbows or T's. City of ' Wheat Ridge JEL /, LO INTRODUCTION Advanced Environmental Consulting, LLC (AEO was retained to collect samples of suspect asbestos -containing material(s) (ACM) that could be disturbed at, the above -referenced property- on the above referenced date(s); and provide an asbestos building inspection report including laboratory analysis. Colorado Regulation No. 8 - Part B Section III.A.l, PIIor to airy renovation or demolition which maY disturb greater than the trigger levels of material identified as a suspect ACM puisuant to the EPA, the faeilitJ- component,(s) to be affected br the renovation or demolition shall be inspected to determine if abatement, is required. " See below for further information on identifying trigger levels, sampling protocol, and materials classifications per the regulations. 2.0 EPA LEAD PMULMONS As of April 22, 2010, the EPA instituted guidelines and regulations for the renovation and/or remodel of homes built prior to 1978, the year that the manufacturing of lead-based paint was prohibited. If lead is present within paint on building components above the action level, any activity impacting these components must be conducted using "lead safe" work practices and RRP (Renovation, Repair, and Painting) Certified contractors. As of Alarch 26, 2014, the EPA has instituted new guidelines and regulations for the failure of firms to follow RRP engineering controls, or when remodeling projects are clone by individuals or firms that do not have proper RRP training and certification. When it is suspected that a. lead hazard has been created, EPA will require an inspection, including wipe samples. If the results of the inspection determine that a lead hazard has been created the follow steps will be conducted: ■ The site will be considered lead -contaminator and RRP standards and practices will no longer apply; ■ Both the EPA and the. CDPIIE Lewd iTnit have determined that clean-up is now defined as a lead abatement project; ■ Regulation 19 standards will be required to be followed in order to properly- clean -tip the lead hazards: A state certified abatement firm and individuals will be required to conduct the clean-up - The firm will need to apply for a permit and follow all requirements of Regulation No. 19, Part A, Section V, Abatement A state -certified firm and individual will be required to conduct project clearances, sampling and provide the CDPHE Lead Unit with copies of those reports. Colorado Air Quality Control (bmmission Regulation 19, Part A, states: ■ Lead-based paint hazard means any condition that causes exposure to lead from lead -contaminated dust, lead - contaminated soil, or lead-based paint. Lead-based paint hazard also means hazardous lead-based paint, dust4ead hazard or soil -lead hazard. IM 1 . ' / IM CO -N-1) The inspection and assessment were performed in acconlance with EPA/AIIERA procedures. These procedures call for the visual inspection of all areas of concern including collection and analysis of representative bulk samples of suspect material Random bulk samples, representative of the suspect asbestos -containing materials (ACM) of each homogeneous area (IIA), were collected according to the guidelines published as EPA Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15 USC, Sections 2641 through 2654 and in compliance with 40 CTE, Part 763. Representative sampling is based on the following criteria: • The distribution of the suspect material throughout the HA, • The suspect material's physical characteristics and application, • Random sampling patterns determined for each HA. 40 SAMPLING PROCEDURES Suspect materials alike in aplxwrance and application were sampled as HAs. Those sampled suspect materials are then subcategorized into one of three classifications: SM, TSlor JAIL AEC collected surfacing materials by types of texture. Not every room with each texture may have been tested; however, it is assumed to be positive if it has the same texture as the above -referenced tapes. AEC uses drywaRschool.com4extures to determine the texture label comparisons. For TSI materials, AEC uses images through Google for comparison when naming material types. NINIs are named based on the material they are as seen by the accredited inspector during the on-site inspection. AEC collected at least the minimum member of samples from each HA necessary to meet all regulatory- requirements for the quantity of material to be disturbed. The quantities listed in this report are approximate and on-site verification of the exact quantity- of each material is required. 5.0 ASSESSMENT OF MATERTATS Condition assessments were performed by the accredited AEC inspector(s) at the time of inspection. Condition assessments include ratings of ,6 ", "D", or "SD" per each HA and are meant to indicate the overall condition of each material. Accumulation of powder, dust or debris similar in appearance to the suspect material on surfaces beneath the material can be used as confirmatory- evidence when determining the material condition assessments of G, Dor SD. All suspect materials were divided into For NFcategories at time of sampling as conducted by hand touch and feel when performed by the inspector(s) during the on-site inspection. After classifying each F ACNI(s), the AEC inspector(s) gives each 11A material a hazard rating (HR). There are seven (7) different 11Rs that each material is subcategorized into if the material is deemed F. The following seven (7) numbers are associated with each HR: 1.) "D" or "SD" thermal systems insulation ACNI 2.) "D' friable surfacing ACM 3.) "SD" friable surfacing ACM 4.) "D" or "SD" friable miscellaneous ACD1 5.) ACM with potential for "V 6.) ACNI with potential for SD" 7.) Any remaining friable ACNI or friable suspect ACNI The certified asbestos inspector(s) must provide a. written assessment of the condition of all known or assumed ACNL These assessments are to include specifying: the types of damage, the severity- of damage, the extent of the spread of the damage 'l over each material, the materials potential for disturbance, the known or suspected causes of the damage, any preventive measures that may eliminate the likelihood of the damaged material from becoming a significantly damaged material, and whether the material is accessible; all over the HA. If an ACM is SD and greater than Colorado Regulation No. 8 trigger levels, then the areas) are deemed a major spill. If the materials are less than the trigger levels but greater than 3 SF or 3 LF, then the areas) are deemed a, minor spill. See the spill response sections as outlined below (Section 10.0). 6.0 SITE DESCll IMON AND PPAMEGT DETAILS The above referenced property is a, single-family residential dwelling. AEC research indicates that the building was constructed in 1944. Destructive sampling was conducted on the materials planned to be disturbed at the above referenced property. The asbestos survey was conducted by the above referenced CDPHE and AHERA certified asbestos building inspector. Samples were submitted to Aerobiology & Associates laboratory for PL31 analysis to identify asbestos content, The samples were requested on a. standard turnaround time for results. 7.0 LABORATORY ANALYSIS AND RESUIRS Bulk samples collected were analyzed utilizing the EPA's Method for the Determination of Asbes - Buildin Materials (EPA 600/R/116, July, 1993) and the McCrone Research Institute's The Asbestos ParVele asNiethods references. Analysis of the bulli samples was performed on the date reported,' as listed in the bulli sample analysis port. AEC may be able to request that the laboratory point count trace material sample(s) and/or composite joint compound sample(s) for an additional fee. Please find attached the laboratory report of PLDs results. 8.0 OONCLUSIONS AEC observed seven (7) homogeneous areas that would be disturbed during the planned project as outlined above. The suspect asbestos containing building material(s) were sampled in seventeen (17) locations. Based on the site observations and, bulk sample PLDI laboratory- analysis results collected by AEC, two (2) suspect materials contained asbestos. Colorado Regulation No. 8 requires a project design in with the amount of ACDI to be abated exceeds 1,000 LF on pipes, or 3,000 SF on other surfaces and an asbestos project manager if the amount of F ACM to be abated exceeds 1,000 LF+' on pipes or 3,000 SF on other surfaces. AEC is able to develop a. project design and/or perform project manager services for this project for an additional fee. 9.0 RECOMMENDATIONS AEC recommends that when ACDI is removed/abated, that only a, licensed asbestos abatement contractor using personnel trained in the handling of ACDI be allowed to conduct such activities using appropriate methods (NEPA-filtered vacuuming, wet cleaning methods, respiratory protection, protective clothing, personnel decontamination, negative -air enclosures, air monitoring, etc.). For all spills, AEC recommends that all soft goods (carpet, pad, clothes, couches, etc.) and any ceiling and/or wall surfacing restoration activities be removed from the interior of the building as asbestos waste. AEC also advises that only hard surfaces (plastic, metal, wood, concrete, etc.) are allowed to be left in place before the final visual and final air clearancee tests are completed. AEC urges that electronics and appliances (refrigerators, air conditioners, DVD players, microwaves, toasters, etc.) are removed due to the mechanical openings and working parts that make it difficult to clean thoroughly. 10.0 ASBESTOS SPILL RESPONSES Major Spill Ifasponse Pmtoaols If ACM is significantly damaged and the total quantity exceeds the regulatory trigger levels, the area. is deemed a "Major Asbestos Spill." The area is consequently subject to the requirements in Reg. 8, Section HIT. 1. — Vajor Asbestos SpiHS as outlined below. Additional asbestos air or dust sampling should also be conducted within the remaining areas not directly impacted by the Major Asbestos Spill to determine if asbestos -containing dust/debris has spread to adjacent areas. If asbestos fibers are found within any other areas or on building contents, they should be included in the scope of professional abatement and decontamination. The following response actions must be followed per Colorado Reg. 8 when a Major Asbestos Spill occurs: ■ Restrict access to the area and post warning signs to prevent entry to the area by persons other than those necessary to respond to the incident. ■ Shut off or temporarily modify the air handling system to prevent the distribution of asbestos fibers to other areas. ■ Immediately contact the Division by telephone, submit a. notification in compliance with subsection M.E. (Notifications) and, if in an area of public access, apply for a permit in accordance with subsection M.G. (Permits). ■ Be exempted from the requirements to have a certified Supervisor on-site a,t all times, until such time as the immediate danger has passed. Any cleanup or asbestos abatement that must occur after the immediate danger has passed shall be supervised by a person certified by the Division. ■ Using certified Supervisors and certified Workers in accordance with section H. (Certification Requirements) of this Regulation, seal all openings between the contaminated and uncontaminated areas and establish none -detect air pressure within the contaminated area in accordance with paragraph IILJ (Air Cleaning and None -detect Pressure Requirements). This is to be accomplished using polyethylene sheeting to cover areas such as doorways, windows, elevator openings, corridor entrances, grills, drains, grates, diffusers and skylights. ■ HEPA vacuum or steam clean all carpets, drapes, upholstery, and other non -clothing fabrics in the contaminated area, or discard these materials. ■ Launder or discard contaminated clothing in accordance with subsection M.R. (Waste Handling). ■ HEPA vacuum or wet clean all surfaces in the contaminated area. ■ Discard all materials in accordance with subsection IILR. (Waste Handling). ■ Following completion of subparagraph IILT.La, through IILT.l.i. above, comply with air monitoring requirements as described in subsection M.P. (Clearing Abatement Projects); air samples shall be collected aggressively as described in 40 C.F.R. Part 763, Appendix A to Subpart E (EPA 1995), except that the air stream of the leaf blower shall not be directed at any friable ACM that remains in the area and ■ Comply with any other measures deemed necessary by the Division to protect public health. Minor Spill Rasponse Pmt000ls If ACM is significantly damaged and the total quantity is less than the regulatory trigger levels, however, more than three square feet (3 SF) or three linear feet (3 LF), the area is deemed a "Minor Asbestos Spill." Additional asbestos air or dust sampling should also be conducted within the remaining areas not directly impacted by the Minor Asbestos Spill to determine if asbestos -containing dust/debris has spread to adjacent areas. If asbestos fibers are found within any other areas or on building contents, they should be included in the scope of professional abatement and decontamination. Part 763, Appendix A to Subpart E (EPA 1995), except that the air stream of the leaf blower shall not be directed at any friable ACM that remains in the area and ■ Comply with any other measures deemed necessary by the Division to protect public health. Any samples containing trace amounts of asbestos require following these federal guidelines: federal worker protection regulations (29 CFR 1926.1101 and 40 CFR 763); the National Emissions Standards for Hazardous Air Pollutants (NESHAP, 40 CFR 61 Subpart Al); the IT.S. Environmental Protection Agency (EPA) Guidance for Controlling Asbestos - Containing Materials in Buildings ("Purple Book", EPA 560 / 5-85-04); and the Asbestos Hazard Emergency Response Act (AHERA, 40 CFR 763 Subpart, E). Clomplianee Sp a fics — OSHA Rggaladiona 29 CfB 19,x R01 Class I asbestos work — defined by OSHA as activities involving the removal of Asbestos Containing Materials which are TSI or surfacing material. Class H asbestos work — defined by (SHA as activities involving the removal of ACM which is not TSI or S1I. Class III asbestos work — defined by OSHA as repair and maintenance operations, where ACM TSI or STNl is likely to be disturbed. OSHA requires the following (at a minimum, the contractor should thoroughly review regulation): 1. Establishment of a regulated work area: minimizes access to unauthorized personnel, protects persons outside from airborne asbestos exposure and requires the use of respirators inside of the regulated area (essentially a HEPA filtered negative air pressure containment area). 2. Supervision of the removal work by a competent person: an EPA trained asbestos abatement supervisor. 3. Workershandling ACM shall be properly trained: training requirements as stipulated in OSHA regulations specify a minimum of eight hours (8 hr.) training of required topics. Material and hands-on training more closely fits the EPA Operations and Maintenance Worker sixteen -hour (16 hr.) class. 4. Exposure monitoring of personnel: both eight hour (8 hr.) TWA (time weighted average) and STEL (short term exposure limit). This monitoring will ensure that personnel are wearing adequate respiratory protection. 5. Proper use of engineering controls: namely, wet methods to control airborne asbestos filters, prompt clean up and disposal of debris in leak tight containers, removal of material in an intact state (prohibiter) use of high speed abrasive disc saws without NEPA filtered exhaust, prohibited use of compressed air on ACMs, prohibited by dry sweeping, shoveling or other dry clean-up methods). 6. Removal of ACM in an intact state to the extent feasible. 7. If using tatting machines, they shall be continuously misted: during use, unless a competent person determines that misting decreases worker safety. Dust generated by a, power cutter shall be collected by a NEPA vacuum. 8. Asbestos containing material from a roof must be either hand carried: or lowered to the ground by crane, hoist or in a. dust -tight chute. 9. All waste shall be either wrapped and/or placed into a closed receptacle: in such manner so as to preclude the dispersion of dust, 10. If working on a roof: roof level heating and ventilation air intake sources shall be isolated (sealed) or the ventilation system shut clown. Other Requirements ■ Other appropriate PPE as necessary ■ Implementantion of a Respiratory Protection Program (if necessary) ■ Respirator physical exam for personnel (if issuer) negative pressure respirator) AEC Recommends a final visual and final air clearance monitoring after the material has been removed. 4-r- 12.0 COLORADO DEMOLITION ACTIVITIES Section IH.A.3 — Any asbestos -containing material that is friable or will be made friable during demolition activities in any area, of public access or non-public access area must be removed prior to demolition. Removal, in accordance with Regulation No. 8, is required if the amount of asbestos -containing material that is friable or will become friable during demolition exceeds the trigger levels. Per CDPHE Demolition Permit Demolition of a, building that has non -friable asbestus -containing materials remaining must be completed without causing the asbestos -containing materials to become friable. Burning a building with any asbestos -containing materials is prohibited. Concrete floors covered with floor tile shall be removed in as large sections as possible. Operations such as crushing, pneumatic jacking, etc. of materials containing asbestos are not permitted. Recycling of materials, such as concrete or wood, that are bonded or contaminated with asbestos -containing materials such as floor tile or mastic, is not permitted. 13.0 1 IITATIONS This report describes the data collected and observations made by AEC during the visual inspection and bulk sampling in the above referenced building. AEC represents that its services were performed within the limits prescribed by applicable regulations, within scope of work as approved by the EPA and the State of Colorado and in a manner consistent with the level of care and skill ordinarily exercised by other professional consultants under similar circumstances. AEC accepts no responsibility for data collected, the recommendations made or expressed to the clients and/or other commitments made by persons or firms other than AEC.
