Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Mountain Roofing LLC proposes to furnish material and labor in accordance with the above specifications for the sum of $ 8775.00 All work to be
completed in a substantial workman like manner in accordance with industry practices. Mountain Roofing LLC will not be held liable for any structural movement, setting,
cracks in drywall, driveway, or damage to siding during the loading process. All interior damage to buildings is excluded 45 days after completion date of project. Any
damage to the interior of the building or its contents will be the responsibility of the owner and the insurance company. Finance charge of 1.5% per month will be added
to any unpaid balance over 30 days. Failure to pay may result in a mechanics lien being filed against the property. In the event that legal or collection actions are taken

(1) TItis permit was issued in accordance with the provisions set forth in your application and is subject to the laws of the State of Colorado and to the Zoning
Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable ordinances of the City.
(2) TItis permit shall expire if (A) the work authorized is not commenced within sixty (60) days from issue date or (B) the building authorized is suspended or
abandoned for a period of 120 days.
(3) If this permit expires, a new pennit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in
the original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes have been or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
(4) No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
(5) Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceeding with successive phases of the job.
(6) The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor an approval of, any violation of the
provisions 0 e ildin codes or any other ordinance, law, rule or regulation.

Re: 2651 Kendall Street, Wheat Ridge, Colorado - Approval of
Final Drainage Report, Grading/Erosion Control Plan.
Dear Mr. Mitzelfeld,
This letter is to inform you that the final drainage report, and
the grading/erosion control plan for the above referenced project
have been reviewed and approved for construction. Please be aware
of the following items regarding the construction of the project:
1. The proposed drainage facilities shall be constructed and
approved by the City of Wheat Ridge prior to issuance of the
Certificate of Occupancy.
2. Erosion control for the project shall be placed in conformance
with the approved drainage/erosion control plan and shall be
maintained as needed during the course of construction.
3. All drainage improvements shall be constructed in conformance
with the approved drainage report and plan.
4. Upon completion of the drainage improvements the engineer,
Mr. William Mitzelfeld, P.E. shall provide to the City of
Wheat Ridge a letter of certification stating that the various
improvements, as defined in the drainage report and plan, have
been accurately surveyed to confirm that their construction is
in accordance with these documents. The certification letter
shall be written and stamped by the engineer who prepared the
drainage report, and shall be submitted for review and
approval by the City prior to issuance of the Certificate of
Occupancy.

5. It will be the responsibility of the contractor for the
project to repair any damage to the existing public
improvements on 2651 Kendall Street as a result of related
construction traffic in the area. Also, the contractor will
be responsible for maintaining 2651 Kendall Street on a
regular basis such that they are free of construction debris
and tracking from construction traffic accessing the site.
6. It will be the responsibility of the developer/owner, to
provide the necessary testing, as applicable fro the scope of
this project, for subgrade compaction and other related
material tests for those improvements to be constructed within
public right-of-way.
(SEE ATTACHED MATERIAL SAMPLING & TESTING REQUIREMENTS)
7. Prior to any construction of the public improvements, the
necessary right-of-way construction permit(s) and respective
licensing will need to be submitted for processing by the
City.

MATERIALS SAMPLING AND TESTING REQUIREMENTS
FOR
CONSTRUCTION OF PUBLIC IMPROVEMENTS
DESCRIPTION
Materials sampling and testing for public improvements shall be performed by an independent
materials testing company at the expense of the developer, and/or utility district. Unless
otherwise designated, all referenced specifications, standards or policies shall be the latest edition
as revised or updated by approved supplements published and issued prior to the date of the
building permit.
REQUIREMENTS
All materials sampling and testing shall be performed by certified, experienced and qualified
materials testing technicians who work under the supervision of a registered professional
engineer in the State of Colorado, All materials sampling and testing equipment shall be
serviceable and calibrated,
Soil classifications and moisture-density curves shall be provided to the Department of Public
Works inspector prior to in-place density testing. Materials testing technicians shall furnish
copies of failed test results to the inspector promptly as the results become available. On a
weekly basis when testing is being performed, the developer shall furnish the inspector with
copies of all test results taken during the prior week and a cover letter, signed by the supervising
registered professional engineer, which summarizes the results and discusses any failed tests or
inconsistencies,
The City materials testing requirements are provided in Table 720-1, All testing procedures,
point of verification and central lab requirements shall be as specified in the Frequency Guide
Schedule of the Colorado Department of Transportation Field Materials Manual. One test is
required for any fraction of the specified frequency. The reference to Section 203.07 in the table
is from the Colorado Department of Transportation Standard Specifications for Road and Bridge
Construction.

SUBSURFACE INVESTIGATION
PROPOSED DUPLEX UNITS
2651 AND 2661 KENDAL STREET
WESTMINSTER, COLORADO
PURPOSE
This report presents the results of a subsurface investigation performed December
10, 1999, at the site of the proposed duplex units to be constructed at 2651 and
2661 Kendal Street in Westminster, Colorado. This investigation was made to
provide design criteria for the foundation systems of the proposed structures to
be constructed at this site. A total of four (4) borings were completed during the
course of the investigation. The location of the borings are indicated on the Boring
Location Map (Figure 1).
Factual data gathered during the field and laboratory work is summarized in
Figure 2 and Table 1 attached. The results of this investigation, our opinions
which are based on this investigation, and our experience in the general area are
summarized in this report.
INVESTIGATION DETAILS
The field investigation consisted of drilling four (4) borings. The borings were
completed with 4-inch diameter, continuous flight power augers using a truck-
mounted drill rig.
The augers are utilized to bore and clean the hole to the desired sampling depth.
The augers are then removed, and a 2-inch J.D. California spoon sampler is
inserted to the desired testing depth. The sampler is then driven with blows of a
standard 140 pound hammer falling a distance of 30 inches.
The sampler is driven a total of 12 inches or a maximum of 50 blows. The number
of blows required to drive the sampler 12 inches, or a fraction thereof, constitutes
the penetration test. The test is similar to the Standard Penetration Test described
in ASTM D1586. This test, when properly evaluated, is a measure of the soil
strength and density. The results of these tests are shown on the Graphic Boring
Logs (Figure 2).
All soil samples recovered were inspected, and some samples were selected for
testing by the project engineer. The testing program consisted of performing the
following tests where appropriate:

Consolidation/Swell
· Consolidation/Swell tests were performed to determine the relative
stability of the different subsurface soil types.
Natural Dry Density
. The dry density of the soils provides us with an indicqtion of the relative
compaction of the surficial soils.
Natural Moisture Content
. The moisture content test provides us with information which may
indicate the probability of instability due to consolidation or swell that
may be caused by excessive wetting or drying.
Unconfined Compressive Strength
· The approximate unconfined compressive strength was determined by use
of a calibrated hand penetrometer. The unconfined compressive strength
can be useful in determining the bearing capacity of a soil.
PROPOSED DEVELOPMENT
As currently planned, the structures are to be a 1 to 2 story duplex units of wood
frame construction over garden level basements supported on reinforced concrete
foundation walls. The loadings are anticipated to be light to moderate with no
unusual loading conditions.
If actual building plans differ from the above description, we should be notified
so that our recommendations can be reviewed and revised, if necessary.
SITE CONDITIONS
The site consisted of vacant lots within an existing older subdivision at the time of
our investigation. The ground surface was relatively flat with a slight slope down
towards the south. The site was bordered on the east by Kendal Street and
existing apartment buildings, to the north, south and west by existing residences.
Vegetation on the site consisted of a moderate growth of native grasses and
weeds, along with some medium size deciduous trees along the east boundary.
Also, some debris was piled on the southern portion of the site and consisted
mainly of tree branches.
SUBSOILS
The subsoils on the site generally consisted of approximately 1 foot of dark-
brown, silty, very sandy clay topsoil, containing organics. Underlying the topsoil,
was a deposit of brown, silty, very sandy clay, which extended to depths of
approximately 16 to 21-1/2 feet. Grey to yellow brown, silty, claystone was

encountered below the surficial soils and extended to the maximum depths
explored of approximately 20 to 25 feet.
A detailed description of the soils encountered in this investigation is presented in
the Graphic Boring Logs (Figure 2).
GROUNDWATER CONDITIONS
Groundwater was encountered during our investigation in all of the borings at
depths of approximately 5-1/2 to 7 feet below the existing ground surface. We
are near the time of seasonal low groundwater table, and some rise of the
groundwater table is anticipated. Groundwater will be of concern if any cuts are
greater than approximately 1-1/2 feet. We recommend that the bottom of the
excavations be maintained at least 4 feet above the seasonal high groundwater
levels. Additionally, improper drainage could result in a "perched" groundwater
table. This is discussed further in the "Site Drainage Considerations" section that
is included later in this report.
FOUNDATION RECOMMENDATIONS
The upper level topsoil materials are not considered suitable for foundation
support. The on-site, silty, very sandy clays tested were of low to high expansive
potential. However, where near the groundwater level they were saturated and
of very low strength, which will require special considerations. The claystone
bedrock tested was of moderate to high expansive potential.
It is our opinion that the structures could be placed on drilled piers or on
conventional spread footings with some special design considerations. The two
sets of criteria that we have developed for this site are explained in the following
sections.
Drilled Piers
It is our opinion that if the structure is founded on drilled piers, the piers will
need to be relatively long, (approximately 28 feet). The piers should be designed
for an end bearing of 15,000 PSF and side shear of 1,500 PSF based on bedrock
embedments of greater than 2 feet. The design pressures should be based on the
dead load plus 100% of the maximum anticipated live load. No minimum dead
load is required as the pier analysis has been done assuming a minimum dead
load condition.
The piers should be designed for a minimum bedrock embedment of 10 feet. In
addition, we recommend that a minimum pier length of 20 feet be maintained
under all circumstances. The minimum embedment lengths should be taken
below any weathered portions of the bedrock.

The piers should be reinforced with a minimum of one No.5 bar (grade 60 or
equivalent reinforcement) for their full length, for each 10 inches of pier
perimeter. A 4-inch minimum void space should be provided beneath the grade
beams to assure effective concentration of the loads upon the piers. The grade
beams should be centered upon the piers, and the tops of the piers should not be
enlarged. The grade beams spanning the piers should be designed for
appropriate loading conditions and reinforced accordingly.
We anticipate that casing will be needed for the pier drilling operation and
recommend that the steel reinforcement and concrete be placed immediately after
drilling. If the water cannot be sealed off, then the use of a pump will be
necessary. In no case should concrete be placed with more than 4 inches of water
present in the holes, unless the concrete can be pumped from the bottom of the
holes. Also, a larger than normal drilling rig will be necessary since the piers will
be on the order of 30 feet, and long casing will need to be utilized.
Footings
In general, the overburden silty, sandy clays are of low to high swell potential
under light loads and exhibit consolidation under heavier loading especially
where in close proximity to the groundwater table.
In order to utilize footings at this site, remedial work as outlined will be
necessary. The expansive clays will need to be removed down to low expansive
materials and the low bearing capacity materials should be removed down to a
minimum depth of 2 feet below the footing elevation and replaced with a clean
crushed rock 3/4 to 1-1/2 inch in size or other approved material, compacted to a
minimum of 100% of maximum density. If this remedial work is accomplished,
then the footings can be designed for a maximum soil bearing pressure of 1,000
PSF based on dead load plus full live load.
We recommend that the excavation be done with a large track mounted backhoe
due to some of the soft soils conditions that are anticipated to be encountered
during excavation.
Differential settlement must be considered in the design of the foundation system.
Differential settlement should be kept to a minimum, which can be achieved by
keeping loads as uniform as possible throughout the foundation elements.
Foundation walls supported by footings should be designed as grade beams
capable of spanning a minimum distance of 12 feet. The amount of reinforcing
steel used should not be less than two No.5 bars, both top and bottom of the
foundation wall. Reinforcement should be continuous around corners.
Differential settlement will be minimized by proper reinforcement of foundation
walls.

Any areas of soft loose soil or potentially moderately expansive present within
the proposed footing lines or pad locations should be removed down to
acceptable, undisturbed soil or compacted to a minimum of 100% of maximum
density as determined by the standard mOisture/density relationship ASTM
D698.
An alternative foundation that may be considered would be a post tensioned slab,
we are available to discuss this option if desired.
SLABS-ON- GRADE
The soils anticipated to be beneath most slabs will have some swell potential.
These soils exhibit a change in volume with changes in moisture content which
can cause heaving and cracking of lightly loaded slabs-on-grade.
Slabs-on-grade should be utilized on potentially expansive soils only if the owner
realizes that some slab damage is likely to occur. The use of a structural floor
supported by the foundation walls with an airspace beneath would eliminate the
possibility of floor movement.
However, it is our opinion that significant damage to slabs placed in this site can
be minimized by strict adherence to the recommendations made in the "Site
Drainage Considerations" section of this report. The owner needs to be made
aware they are responsible for future adherence to the above-mentioned
recommendations.
If slabs-on-grade are utilized on potentially expansive soils, the following
construction techniques will help to prevent secondary damage that could be
caused by slab movement.
1. Separate slabs from the foundation elements with a slip joint. One method
of doing this is to use two layers of tempered hardboard with a silicone
lubricant between the boards. A slip joint should be used around the
perimeter of the slab and adjacent to any other structural elements.
2. Slab joints must be provided to control the cracking. The floor joint grid
should be designed to allow no more than 200 square feet of continuous
slab area.
3. Any load bearing partitions or columns must be provided with their own
foundation system and the slab separated as outlined above.
4. Provide a 3-inch minimum air space below any interior non-load bearing
partition to provide for slab movement without damage to the structure. If
unsure of the proper construction methods to achieve the recommended
air space, we should be contacted for further recommendations.

5. Any pipes nsmg through the slab should be provided with flexible
couplings or other means to allow substantial movement without damage
to the piping. Any ducts connecting to equipment founded on the slab
should be equipped with flexible or crushable connections to allow for
some slab movement.
6. Equipment and other building appurtenances constructed on the slab
should be constructed so that slab movement will not cause damage.
Following the recommendations given above will not prevent movement of the
floor slabs in the event that the moisture content of the soil beneath the slab
changes. However, if movement occurs, damage will have been reduced for a
relatively small investment.
Topsoil, debris, existing fill, and organic materials should be stripped prior to
placing any slab. Prior to placing the slab, we recommend that the area on which
the slab is to be placed be carefully inspected to delineate any areas of soft or
loose soil. These areas should be densified in place or removed down to
acceptable soil and replaced with compacted fill. It is our opinion that select site
materials would be satisfactory for this purpose. If fill is required to bring the site
to the desired slab elevation, we recommend that granular fill be utilized and that
the fill be compacted in maximum 9-inch lifts to a minimum of 95% maximum
density, as determined by the standard moisture/density relationship test ASTM
D698. It should be noted that failure to provide adequate fill compaction can
result in settlement which might cause slab damage such as cracking and tilting.
SITE DRAINAGE CONSIDERATIONS
It is essential that site grading be provided to prevent infiltration of surface water
into the foundation system. The following methods of preventing this infiltration
are recommended. These recommendations will also assist in preventing a
"perched" groundwater table.
1. Mechanically compact all fill around the building, including the backfill.
Compaction by ponding or saturation must not be permitted. The backfill
should be compacted to not less than 90% of maximum density as
determined by the standard moisture/density relationship ASTM D698.
Backfill which is to support slabs should be compacted to 95% of maximum
dry density. Note that some moisture may need to be added to the soils in
order to obtain the proper compaction.
2. Provide an adequate grade for rapid runoff of surface water away from the
structure (a minimum of 10 percent for the first 10 feet away from the
structure is recommended, 2 percent if paved).

3. A well constructed, leak-resistant series of gutters, or other roof drainage
system, is essential.
4. Discharge roof downspouts and all other water collection systems well
beyond the limits of the backfill, a minimum of 5 feet.
5. No irrigation within 5 feet of the foundation. Avoid heavy watering of any
foundation plantings.
6. Observe and comply with any other precautions which may be indicated
during design and construction.
It is our opinion that a perimeter drainage system should be installed at this site if
the structure is to have below grade space (basement, garden level or crawlspace).
The perimeter drainage system should consist of 4-inch perforated pipe,
surrounded by 3/4 to 1-1/2 inch washed rock. The drains should be placed a
minimum of 12 inches below the surface of the adjacent concrete slab, crawlspace
level, or the bottom of grade beam, and should drain to a positive gravity
discharge (surface discharge strongly recommended) or to a sump from which
water can be pumped.
Attached to this report (Figures 3 and 4) are drawings which illustrate a typical
perimeter drain configuration for footing and drilled pier foundation systems.
Additional drainage considerations may be necessary (such as an underslab
drainage system) if the excavation is to extend to within 3 feet of the seasonal
high groundwater table, refer to Figures 5 and 6 for typical underslab drainage
system details.
EARTH RETAINING STRUCTURES
At this site, we recommend that the walls (including foundation walls and other
grade beams) be designed using a lateral earth pressure equivalent to that
developed by a fluid weighing 40 pef plus any additional surcharge loads. Use of
this value assumes that the wall will be backfilled with the site soils and that these
soils will not be allowed to become saturated at any time during the life of the
wall. Proper site grading and drainage, and installation of drainage systems at
the base of any walls that are to retain soil above grade, will help to prevent
saturation of the soils behind the walls. This value is valid for walls up to 10 feet
in height.
LIMITATIONS
The borings in this investigation are believed to present a reasonably accurate
knowledge of the existing subsoils. However, variations of subsoils not indicated
by the borings are always possible. In addition, all excavations should be

inspected by an engineer knowledgeable in soils to confirm that the soils actually
are as indicated by the investigation, and to make recommendations if differences
are noted.
Identification of potential hazardous waste material, if any, at this site is beyond
the scope of work for which the activities of this project were intended. It should
be noted that the foundation design and slab design recommendations are
implemented as recommended. Damage to the foundation system and/ or slabs-
on-grade can take place if the soils supporting these elements become inundated
over a substantial period of time due to improper site drainage.
INSPECTION AND QUALITY CONTROL
Placement of any significant thickness of fill, particularly fill which is to remain in
place beneath loaded slabs or other structural elements, should be inspected and
tested in order to verify proper compaction is being obtained. We also
recommend that all foundation excavations be inspected by an engineer
knowledgeable in soils in order to provide final foundation and drainage
recommendations.
