Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of ,the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

necessary work described as: Exterior Restoration for services requested at the property listed below. The

will be determined at a later date, and will be determined by a thorough, comprehensive estimate provided in accordance with
standard Industry practices. Kinetic provides pricing In accordance with Xactimate software pricing, and by signing, the Owner
understands that the pricing for the services provided is based on third party pricing guides that are not controlled by Kinetic. Any
deviation from Xactimate pricing shall include justifiable reasons, and agreed upon in advance with the Owner prior to execution.
Owner agrees to make payment directly to Kinetic for the work. This includes any applicable deductible, depreciation (unrecoverable
or otherwise, unless discussed in advance). This may include amounts not covered in Insurance related repairs. Owner agrees to
remove cash, jewelry, firearms, collectibles, or any valuable Items prior to work start. Owner acknowledges the understanding and
agreement to all terms and conditions on reverse side.
Owner Requests that all payments pertaining to work performed by Kinetic be paid directly to Kinetic by Owner's Insurance provider when
applicable, and, in all cases, add Kinetic as additional payee on any insurance related payments to work performed by Kinetic.
PAYMENT TERMS: Non insurance related contracts, or "Direct Contracts" - 50% Down Payment at time of contract, with progress
invoices submitted on a monthly basis for work that has been completed by the end of the month prior, including material

TERMS & CONDITIONS
scope of Work & Additional Work: Kinetic agrees to perform the scope of work described on
the front page of this document. Owner agrees that any supplements or additions to work may
be accomplished verbally or with a written change order. Supplemental work Includes
betterment; owner selected changes, and/ or enforcement of code or
ordinances by municipality or building department.
Lump sum Contract & Pricing: All efforts will be made to draft an Xactimate estimate to define
the scope and pricing. A copy of the approved estimate shall be made available upon request. If
owner chases to reduce the overall scope of work, profit and overhead originally calculated will
still be due to the contractor. Kinetic, at Its option may utilize value engineering to complete the
same work In a more efficient manner which may result in a savings to Kinetic. Kinetic may, at
Its discretion, provide a Pre-Uen on the property to secure Its Interests. A Conditional Release
or Partial Release of Lien shall be provided In consideration for partial or complete payment.
Work quality: All work involving restoration and/or repairs Is for the damaged work only and
for like kind & quality craftsmanship and does not cover unaffected areas and does not cover
restoring existing defidencles. Estimate presumes original walls, floors and framing are plumb,
square, and straight. Construction means and methods are provided by the expert team at
Knetic. All work completed shall abide by governing codes, and any regulatory agency or
generally accepted industry standard.
Construction guarantee: All work to be completed In accordance with"like kind and quality" to
the product in place prior to the repairs unless otherwise specified. Imperfections In substrate
materials (framing, subfloors, roof decks, and other pre-existing conditions) may impact overall
finished product, and are not the responsibility to correct without written change order. Field
conditions should be referenced when assessing finished product. Our work will conform to
existing product and will be governed by references published by the National Association of
Home Builders, "Residential Construction Performance Guidelines'. All construction debris will
be removed from project and areas left in a broom swept condition at the conclusion of the
project. A final clean for re -occupancy may he requested, possibly at an additional charge.
A presonsftirctlon walk shall be conducted by Kinetic and the owner(s) to establish any pre-
existing damage to in-place materials or contents. Any items noted in the pre -walk shag be
photographed, and documented to establish a clear knowledge of the condition of the
property. Should imperfections exist within the work area, Kinetic may elect to include these
repairs within the scope of work, for a nominal fee.
Contract Tlme: Time is always of the essence. Once an agreed upon price has been reached,
Kinetic agrees to diligently pursue substantial completion of the work. Kinetic will provide a
construction schedule as part of the contract package This schedule shall be used as a guideline
for events, and milestones. Kinetic cannot be held liable for delays due to deliveries, weather,
or insurance carrier. other delays include those caused by the owner in selecting finishes,
providing access, or restricting work hours. Kinetic shall not be held responsible for scheduling
conflicts of subcontracted trades or any other conditions established to be beyond contractor's
control. Owner agrees to hold harmless contractor for any additional delays to work, unless
liquidated damages have been agreed upon in advance, with a subsequent performance clause
far a project finished prior to scheduled completion. Owner further agrees not to cause delays
to project for any reason and to provide clear and continuous access to the work site from
7:00am to 5:30pm. Kinetic may, with ownership permission, place a secure lock box at the
property and assumes no responsibility for unauthorized entry, Owner agrees to provide
electricity, heat, water, local telephone (where applicable) and sanitary facilities.
Stdrstao cal cogipletion is the date when the property can be occupied for its Intended use, not
when the work Is 100% satisfactory. Customer agrees to sign Certificate of Satisfaction upon
project completlon-
involce & Payment Method: if the insurance company does not pay Kinetic directly, Owner
agrees to make payment to Kinetic for the Work, whether or not such Work is covered by
insurance. Kinetic may Invoice on a monthly basis for work where portions of work may be
complete. Owner shall pay Kinetic the Invoice in full no later than ten (10) days following
Owner's receipt of the invoice. Interest may accrue on unpaid amounts within such time at the
lesser of (1) the maximum lawful Interest rate or (g) two percent (2%) per month. Any remaining
balance of the Contract Price after the above payments are made shall be paid no later than ten
(10) days following completion of the Work. At the completion of Final Walk Through, owner
must pay Kinetic for all sums less correction Items on list. Items addressed after Final Walk
Through will be placed on a warranty list and will not be subject to subtractions from final
payments subject to limited warranty below.
In the event that an invoice has not been paid within the 10 -day period of work being completed,
Kinetic holds the right to rile fora Mechanic's lien on the property within 120 days of the
Substantial Completion date, which will include any administration and legal fees.
Limited Warranty: Conditioned upon payment In full of all amounts due Kinetic, Kinetic
warrants that the Work will be free from defects for a period of 2 (two) years from the first date
of substantial completion. Substantial Completion as previously defined, Is the date of habitable
occupancy, but may be prior to accepted finishes by either party. Owner shall be transferred all
applicable warranties of manufactures, supplies and/or others. Kinetic's warranty is limited to
repair or replacement, at Kinetles option, of the defective work and specifically excludes any
equipment or materials covered by manufacturers, suppliers at others' warranties. Knetic also
specifically excludes incidental or consequential damages. This warranty specifically excludes
cracking, etc. of any concrete, drywall, plaster, caulking, sealant, tile, or any other product
subject to movement of any kind. This warranty also excludes any lass caused by, or consisting
of any mord or microbial growth which may or may not have been a result of the Work
performed by Kinetic or those contracted to perform on the property under this Contract.
Owner further agrees to hold Kinetic harmless from all claims for personal, professlorwl, or
property damage related to mold or other microbial growth. Except as provided herein, there are
no other expressed or implied warranties.
Certain Owner Obligations: Owner shall make customer selections within Kinetic deadlines as
provided In the construction schedule, or as requested in writing to maintain completion dates.
Selections that are not made In the requested, reasonable tlmellnes may result In additional
overhead fees to Kinetic. The owner agrees to utilize the design center of contractor's cho;ce
select finishes. if owner falls to make timely selections, Kinetic may be required to make
selections on behalf of owner with standard items, after notification to the Owner
that timelines have been elapsed. No response from the owner may imply acceptance.
Owner agrees to allow Kinetic to communicate with Insurance company in order to
fadhtate the processing and payment of the claim. Owner agrees to quickly facilitate the signing
of Proof of Loss and/or mortgage company inspections and/or endorsements to claim
payments, Owner waives any right of recovery or subrogation against Kinetic to the extent of
Owner's Insurance coverage. Owner shall provide and maintain Property, Building, Personal
Property, Builders Risk, "All Risk" and Premises Liability insurance covering the Work,
including all materials and supplies on site but not yet installed. Owner agrees to make claim
for defects in construction pursuant to the Colorado Construction Defect Claims Act.
Hazardous or Other Conditions: kinetic may haft the work upon any finding of hazardous
substances or unsafe conditions. Kinetic will notify owner upon the discovery of any such Items.
owner must notify Kinetic of any known hazardous items at the site Including;
asbestos , chemicals, lead, firearms or ammunition, radon or any other known condition.
Owner shall Indemnify and hold harmless Kinetic from and against any and ag claims arising from
or related to the aforementioned Conditions, and Kinetic shall be entitled to payment from
owner for all costs, expenses and damages, including reasonable attorneys' fees and expenses
incurred as a result of the Conditions. If Conditions cause delay to project for more than one
calendar week from date of discovery or disclosure, Kinetic may terminate this agreement Whlie
Kinetic may perform work related to, and including the removal of mold and mold spores, Owner
agrees to hold Knetic harmless from any and all claims for physical, personal, and/or
psychologica I damages related to or consisting of maid or microbial growth of arty sort or
manner. Kinetic provides no guarantee that all mold or microbial growth will be removed during
the course of the work. Completed scope of work has been achieved at the discretion of the tests
conducted by a properly qualified Indoor Air Quality Specialist, or Industrial Hygienist
Default: If Owner defaults in any of its abhgations hereunder, Kinetic may, at its option, in
addition to other remedies provided in this Contact or pursuant to applicable law or principles
of equity, pursue one or more of the following remedies; suspend some or all of the onsite or
offsite activity related to the Work until all defaults have been cured. Upon three (3) days
written notice to Owner, Kinetic may terminate some or all of its obligations under this
Contract, and/or recover all amounts due under this Contract plus all expenses and reasonable
attorneys' fees and expenses incurred by Kinetic as a result of Owner's breach or Kinetic's
enforcement of this Contract. in the event this Contract or Kinetic's obligations are terminated by
Kinetic pursuant to the terms of this Contract, Kinetic shall be paid for all work performed
through the date of termination in an amount that will compensate Kinetic for all
costs incurred, plus an additional 30% of those costs for the overhead, profit due, and related
administrative and legal costs associated with closing the contract Kinetic retains the right to file
and perfect a Mechanics Lien for services rendered and hereby notifies Owner that Knetic will
file said Bens to protect Kinetic's Interest in the subject property. In such event, owner shall
pay all Interest charges and filing fees for the hen.
Dispute Resolution: Any Controversy or claim arising out of, or relating to this Contract or work
performed pursuant thereto shall be resolved by arbitration in accordance with the
Construction Industry Arbitration Rules of the American Arbitration Association, and judgment
upon the award may be entered In any court having jurisdiction. Kinetic shall be entified to all
reasonable attorneys' fees and other legal associated costs. Limitation, Waiver, and Venue
Owner and Kinetic agree that venue for any legal proceedings shall be Douglas County, CO and
that Colorado law shall govern all disputes relating to this agreement The Arbitration shall be
conducted by the American Arbitration Association. Owner agrees not to libel or slander
contractor for any reason whatsoever and that in doing so to any source, agrees to indemnify
contractor an amount equal to $5,000.00 unless it can be proven that such libel & slander did not
cause any damages. Owner and contractor further waive their right to trial by jury and any claims
for consequential or punitive damages against each other.
Entire Agreement: This Contract, including the Attachments hereto, comprises the complete
agreement of the parties and no representations or agreements have been made by either
party except as expressly stated in this Contract. All modifications to this Contract shall be in
writing and signed by both parties hereto. If any provision of this Contract becomes or Is
determined to be Illegal or unenforceable for any reason, the remainder of the contract shall
remain in full force and effect.

(1) This permit was issued in accordance with the provisions set forth in yopur application and is subject to the laws of the State of Colorado and to the Zoning
Regulations and Building Code of Wheat Ridge, Colorado or any other applicable ordinances of the City.
(2) This permit shall expire if (A) the work authorized is not commenced within sixty (60) days from issue date or (B) the building authorized is suspended or
abandoned for a period of 120 days.
(3) If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
(4) No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
(5) Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceediing with successive phases of the job.
(6) The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor an approval of, any violation of the
provisions of the es or a~~:. law, rule or regulation.
