Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

I . This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and maybe subject to a fee equal to one-half of the originalpermitfee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

If insurance proceeds are to be used to pay for the contracted work, USA Roof Restoration LLC cannot
pay, waive, or rebate all or part of the insurance deductible applicable to the insurance claim for work to be

Property Owner has the right to rescind this contract and obtain a full refund of any deposit within 72
hours after entering the contract. Refund of any deposit or payment following rescission of this contract will
be made within 10 days of rescission. If you plan to use the proceeds of a property and casualty insurance
policy to pay for the roofing work, you may rescind this contract within 72 hours after you receive written
notice from the property and casualty insurer that your claim has been denied in whole or in part. However,
USA Roof Restoration LLC is entitled to retain payments and deposit to compensate USA Roof Restoration
LLC for roofing work performed in a workmanlike manner consistent with standard roofing industry
practices. USA Roof Restoration LLC is also entitled to compensation for costs incurred for any custom or
non -returnable materials that may have been ordered prior to any contract rescission.

This agreement and any agreement pursuant hereto is between USA ROOF Restoration, LLC hereinafter referred to as "company' and the
buyer(s) named herein on the reverse side will be subject to all appropriate laws, regulations, and ordinances of the state of residence, the local
jurisdiction and the fallowing term and conditions.
1. All agreements are subject to the approval of a manager of the company for this agreement to be effective under any conditions.
2. Should default be made in payment of this agreement, charges shall be added from the date thereof at the rate of one and one-half (1-1/2)
percent per month (18% per annum) with a minimum charge of $2.00 per month, and if placed in the hands of an attorney for collection, all
attorney's fees and legal filing fees shall be paid by buyer accepting said agreement.
3. The company shall have no responsibility for damages from rain, tornado, windstorm and other perils, as is normally contemplated to be
covered by homeowner's insurance or business risk insurance, or unless a specified written agreement be made prior to the commencement of
the work.
4. The quotation on the face hereof does not include expenses or charges for bond or insurance premiums or costs beyond normal insurance
coverage and any such additional expenses, premiums or costs shall be added to the total agreement amount.
S. Replacement of deteriorated decking, fascia boards, roof jacks, ventilators, flashing or other materials, unless otherwise stated in this
agreement are not included and will be charged as an extra on a time and material basis.
6. Company reserves the right to revoke this proposal 90 days from the date accepted. After 90 days, Company reserves the right to revise its
price in accordance with costs in effect at the time.
7. The Company shall not be liable for the failure of performance due to labor controversies, strikes, weather, inability to obtain materials from
usual sources, or any other circumstances beyond the control of the company, whether of a similar or dissimilar nature.
8. If roofing and sheet metal work are involved, it is understood that the company's standard roof guarantee, a copy of which is available in the
company's office, shall be acceptable and that all terms and provisions therein shall prevail, unless otherwise specifically agreed to in writing
prior to the commencement of the work. 90# roll roofing is not a warrantable product and will not be warranted against leaks by the company.
However, of specified in writing and signed by both parties, the company will service the installation for the period of one year.
9. The company is not responsible for ice dams (thawing and freezing of ice, water, or snow) or any damage on or below the roofline due to
leaks by excessive snow or wind-driven rain, ice or hail during the period of the warranty. Excessive wind is 50 or greater mph. The warranty is
non-transferable.
10. If the material has to be reordered or restocked because of a cancellation by the buyer there will be a restocking fee equal to twenty percent
(20 %) of the total agreement amount.
11. This agreement or warranty shall not be assigned except by or with the written permission of the company.
12. Payment schedule: Final payment shall be paid upon receipt of the balance of the funds due from the insurance company, or within 30 days
of job completion. All payments must be made to the company. Only payments made to and received by the company will be considered to
satisfy the payment of this agreement. Payment schedule shall not be amended without prior written approval of the company.
13. All direct costs, overhead, and profit paid to the customer by the insurance provider are included in the agreed amount with insurance and
are due to the company in accordance with the payment schedule set forth here.
14. During the duration of the work, the buyer's homeowner's insurance will be responsible for any interior damage as long as the company has
taken appropriate action to protect it if necessary.
15. If there are any solar panels on the roof, the company will not be responsible for damage during the repair, so buyer agrees to have solar
panel company take appropriate action to protect it if necessary.
16. The company is not responsible for pre-existing construction deficiencies that manifest themselves during the construction process, i.e., nail
pops, wood rot, decking deflection, etc. If a construction problem is pointed out prior to construction and company notified in writing, the
company will try to assist the buyer to correct the problem(s) on a time and material basis.
17. The company will not be responsible for plugging gable vents when installing ridge vent or changing ventilation of roof.
18. The company will not be responsible for the slight scratching or denting of gutters, oil droplets in the driveway, hairline fractures in concrete
or damage to plants or shrubbery. If excessive damage is caused by the company, the company will repair or replace damaged area only at
company's expense.
19. The maximurh liability for the company shall be the original cost of labor and materials for the repair which buyer agrees shall be liquidated
sum under any event of default of company herein.
20. Any person or company supplying labor and/or materials for this improvement to buyer's property may file a lien against buyer's property if
that person or company is not paid for the contributions.
21. Any representations, statements or other communications, not written on this agreement or on change form, are agreed to be immaterial,
and not relied on by either party and do not survive the execution of the agreement.
22. If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of the remaining provisions
of this agreement shall not be affected thereby.
23. This agreement is composed of this back page, the front side of the agreement, any change forms and all matters incorporated herein by
reference and shall be considered the entire agreement between the parties.

This permit shall expire if (A) the work authorized is not commenced within sixty (60) days from issue date or (B) the building authorized is suspended or
abandoned for a period of 120 days.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid fur a new permit.

This pannit was issued in accordance with the provisions set forth in yapur application and is subject to the laws of the State of Colorado and to the Zoning
Regulations and Buildin$! Code of Wheat Rid~e, Colorado or any other 8!:,pllcable ordmances 0'- the City.
This permIt shall expire .f (A) the work authonzed IS not commenced withm Sixty (60) days from Issue date or (B) the building authorized is suspended or
abandoned for a period of 120 days.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspenSion or abandonment has not exceeded one (1) year. If changes are made or If suspenSion or abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
Nb work of any manner shall be done that will change the natural flow of water causing a drainage problem.
Contractor shall notify the Building Inspector twenty.four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceediing with successive phases of the job.
The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor an approval of. any violation of the provisions
of the buildmg codes or any other ordmance, law, rule or reguiallon.
