On October 9, 2003 Elite Systems, Inc. (under the direct supervISIOn of KEMWest, Inc.)
performed the abatement/clean-up and disposal of approximately 3 square feet of cementitious
door paneling, and approximately 140 square feet of asbestos containing plaster texture from the
East wall of the South room of 3281 Chase Street, Wheatridge, Colorado. All friable asbestos
containing building materials, or non-friable building materials that may become friable during
demolition that KEMWest, Inc. identified at the property have been properly removed and
disposed in an EP A Approved Landfill.

(6) The issu nee of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor an approval of, any violation of the

To Whom it May Concern. On June 6. 2003. I purchased an electrical permit tor work to
be performed at 3281 Chase St.[permit included in fax). After this date and prior to any
electrical work being done the house was condemned and is to be demolished.
If possible I would like to get a refund of the permit fee and use tax fee. Any
