Observations of Suspect Asbestos -Containing Materials at the Site was conducted by a Colorado Department of
Public Health and Environment (CDPHE) Certified Asbestos Firm, and Asbestos Building Inspector. On the
exterior of home there was no debris observed on the ground and to the dumpster area. No suspect debris was
found inside the dumpster at the time of inspection. However, in the bathroom area, the existing drywall tested
and analyzed was found to contain 2% asbestos material. Approximately 50 square feet of the purple drywall
and associated texture was removed. The existing flooring located in the bathroom and hall area will remain in
place for renovation, or it may be removed. The floor tile did not contain asbestos material.
MTS transported and submitted samples to EMSL Laboratory for analysis by Polarized Light Microscopy (PLM)
methods on a Rush turnaround analysis to identify whether the suspect material(s) contains asbestos. Sampling
and Analytical Procedures for this inspection and assessment was performed in accordance with the
Environmental Protection Agency (EPA) and Asbestos Hazardous Emergency Response Act (AHERA)
recommended procedures. Please fmd attached the EMSL laboratory report of PLM results.
If asbestos -containing (>I%), the material(s) would then be removed properly by a licensed and certified General
Asbestos Abatement Contractor (GAC) per the Colorado Department of Public Health and Environment
(CDPHE) Asbestos Regulation #8. If samples are found to contain <1% asbestos Federal Protection laws apply.

These procedures call for the visual inspection of all areas of concern and collection and analysis of representative
bulk samples of suspect material. Random bulk samples, representative of the suspect asbestos -containing
building materials (ACBM) of each homogeneous area (HA), were collected according to the guidelines published
as Environmental Protection Agency (EPA) Final Rule: Title II of the Toxic Substances Control Act (TSCA), 15
USC, Sections 2641 through 2654 and in compliance with 40 CFR, Part 763. Representative sampling is based
on the following criteria:
1. The distribution of the suspect material throughout the HA.
2. The suspect material's physical characteristics and application.
3. Random sampling patterns determined for each HA.
Suspect materials sampled and analyzed should be considered representative of materials in each HA if
1. They exhibit similar physical characteristics; and
2. The application of the sampled material can be correlated to the application of un -sampled material.
Friable ACM means any material that contains asbestos and when dry can be crumbled, pulverized, or reduced to
powder by hand pressure and that contains more than one percent asbestos by weight, area or volume. A nonfriable
material may become friable if its condition had deteriorated or has been impacted by forces that have rendered
it friable. Bulk samples collected were analyzed utilizing the EPA's Method for the determination of Asbestos in
Bulk Building Materials (EPA 600/R/116, July, 1993) and the McCrone Research Institute's The Asbestos
Particle Atlasas methods references.
Analysis of the bulk samples was performed on the "date reported," as listed in the bulk -sample analysis report.
Sampling Quantities MTS collected at least the required minimum number of samples from each homogeneous
area necessary to meet all regulatory requirements for the quantity of material to be disturbed. The quantities
listed in this report are approximate and on-site verification of the exact quantity of each material is required. The
following outlines the minimum sample quantities required per homogeneous area: Surfacing Materials (SM):
up to 1,000 ft2 of material requires a minimum of three (3) samples; between 1,000 Wand 5,000 ft2 of material
requires a minimum of five (5) samples; over 5,000 ft2 of material requires a minimum of seven (7) samples; one
(1) sample of each patch Thermal System Insulation (TSI): each homogeneous area requires a minimum of three
(3) samples; at least one (1) sample must be collected from each patch; and collect enough samples sufficient to
adequately assess the material and determine the asbestos content for TSI fittings such as pipe elbows or T's.

Inspection Protocol
Suspect materials alike in appearance and application were sampled as HAs. Suspect materials were divided into
three classifications:
Surfacing material (SM): sprayed or troweled onto structural building member. Thermal systems insulation (TSI):
any type of pipe, boiler, tank, or duct insulation. Miscellaneous (Misc.): other suspect materials, including
flooring, ceiling tiles, insulation, and finishing materials. Condition assessments were performed by the accredited
inspectors at the time of inspection. Condition assessments are listed in the following section. Ratings of "good,"
"damaged," and "severely damaged" are meant to indicate the overall condition of the material. A material in
"good" condition has no visible damage or deterioration, or showing only very limited damage or deterioration.
A material in "damaged" condition has the following characteristics: The surface is crumbling, blistered, water -
stained, gouged, marred or otherwise abraded over less than one-tenth of the surface if the damage is evenly
distributed. Accumulation of powder, dust or debris similar in appearance to the suspect material on surfaces
beneath the material can be used as confirmatory evidence. A material in "severely damaged" condition has one
or more of the following characteristics: The surface is crumbling or blistered over at least one-tenth of the surface
if the damage is evenly distributed. One-tenth of the material is hanging from the surface, deteriorated, or showing
adhesive failure. Water stains, gouges, or mars are over at least one-tenth of the surface if the damage is evenly
distributed. Accumulation of powder, dust or debris similar in appearance to the suspect material on surfaces
beneath the Material can be used as confirmatory evidence.
Laboratory results received on February 4, 2019, document that none of the sampled materials submitted
contains Asbestos material.
Section III.A. Lc — Abatement, Renovation and Demolition Projects - of Colorado Regulation 8 requires that all
samples of friable asbestos is estimated to be 1% asbestos or less, but greater than 0%, by a method other than
point counting, the determination shall be repeated using the point counting technique with PLM. Samples
containing trace amounts of asbestos require following these Federal guidelines: federal worker protection
Regulations (29 CFR 1926.1101 and 40 CFR 763); the National Emissions Standards for Hazardous Air
Pollutants (NESHAP, 40 CFR 61 Subpart M); the U.S. Environmental Protection Agency (EPA) Guidance for
Controlling Asbestos -Containing Materials in Buildings ("Purple Book", EPA 560 / 5-85-04); and the Asbestos
Hazard Emergency Response Act (AHERA, 40 CFR 763 Subpart E).

Conclusions and Recommendations:
Laboratory results received on September 16, 2019, document that the existing drywall associated texture
sampled contains 2% Asbestos material. The flooring is Non -Detect for asbestos
MTS observed Two (2) homogenous areas that would be disturbed during a planned renovation project in the
Bathroom only.

MTS recommends asbestos abatement contractor to decontaminate surrounding area and bathroom, the
existing drywall texture is 2% asbestos containing and has been deemed a "Major Spill" per the Colorado
Department of Public health and Environment Regulation #8. Contractor to follow the Regulations set
forth Section T.111.1 for decontamination with a third -party air clearance at completion.
Section III.A.3 — Any asbestos -containing material that is friable or will be made friable during demolition
activities in any area of public access or non-public access area must be removed prior to renovation or demolition.
Removal, in accordance with Regulation No. 8, is required if the amount of asbestos -containing material that is
friable or will become friable during demolition exceeds the trigger levels. Per CDPHE Demolition Permit:
Demolition of a building that has non -friable asbestos -containing materials remaining must be completed without
causing the asbestos -containing materials to become friable. Burning a building with any asbestos -containing
materials is prohibited. Concrete floors covered with floor the shall be removed in as large sections as possible.
Operations such as crushing, pneumatic jacking, etc. of materials containing asbestos are not permitted. Recycling
of materials, such as concrete or wood, that are bonded or contaminated with asbestos -containing material such
as floor tile or mastic, is not permitted. Based on the site observations and, bulk sample PLM laboratory analysis
results collected by MTS, one (1) suspect material containing Regulated by Colorado, Friable asbestos above the
regulatory limit; and Zero (0) areas of non -friable asbestos containing material. Therefore, Regulated Asbestios
abatement contractor is needed.

that contains Greater than the trigger levels (i.e. commercial 160 sq. /ft., 260 Ln. /ft., equivalent of a 55 -gallon
drum) (i.e. Residential 32 sq. /ft., 50 In. /ft., equivalent of a 55 -gallon drum). Colorado Regulation No. 8 requires
a project Design in with the amount of ACM to be abated exceeds 1,000 In. /ft. on pipes, or 3,000 sq. /ft. on other
surface

This report describes the data collected and observations made by MTS during the visual inspection and bulk
sampling in the above referenced building at the time of inspection only. MTS represents that its services were

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

1. This permit was issued in accordance with the provisions set forth in your application and is subject to the laws of the
State of Colorado and to the Zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable

2. This permit shall expire 180 days from the issue date. Requests for an extension must be received prior to expiration
date. An extension may be granted at the discretion of the Building official.
3. If this permit expires, a new permit may be acquired for a fee of one -half the amount normally required, provided no
changes have been or will be made in the original plans and specifications and any suspension or abandonment has not
exceeded one (.1) year. If changes have been or if suspension or abandonment exceeds one (1) year, full fees shall
be paid for a new permit.
4. No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
5. Contractor shall notify the Building Inspector twenty -four (24) hours in advance for all inspections and shall receive
written approval on .inspection card before proceeding with successive phases of the job.
6. The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor
an approval of, any violation of the provisions of the building codes or any other ordinance, law, rule or regulation.
