Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

applicable building codes, and all applicable municipal codes, policies and procedures, and that I am the legal owner or have been authorized
by the legal owner of the property and am authorized to obtain this permit and perform the work described and approved in conjunction with

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Per your request, I have prepared a repair plan for the ad ition to t
St., Lakewood, Colorado.
Preliminarv Information
The grade beam foundation supporting the kitchen additio has settled and pulled away from
the original home.
Recommendations
1) Install eight (7) resistance piers at the locations shown n the attached engineering
drawing. It may be desirable to lift the structure during he repair, however the
homeowner should understand this may cause additiona cosmetic damage to finished
area.

shall assume sole and complete responsibility for job site conditions d ring the course of construction,
including safety of all persons and property; that this requirement sha apply continuously and not be
limited to normal working hours; and that the contractor shall defend, ndemnify and hold the engineer
harmless from any and all liability , real or alleged, in conjunction with he performance of work on this
project, excepting for liability arising from the sole negligence of the e gineer. The client agrees to limit the
engineer's liability to the total fee charged for services rendered on thi project.

2) In general, surface water should be managed and direct away from the foundation.
Surface water should be directed away from the foundati n. I recommend obtaining a
10% slope within the first 10 horizontal feet away from t e building to facilitate adequate
drainage away from the foundation. Grass, plants, trees and shrubs with high watering
requirements should not be located within five feet of th foundation. All gutter
downspouts should have functioning downspout extensio s, which extend at least five feet
away from the foundation. Solid lawn edging should be emoved.
The preceding list of repairs is intended to address the issue and concerns observed during
my visit and during our discussions. Other unknown concer s may also exist. I observed the
structural condition of the foundation near the garage. The epetsos Group, Inc. makes no
representations regarding the suitability of other areas, incl ding, but not limited to
mechanical, electrical, plumbing, architectural, geotechnical the roof system, and previous
foundation modifications and repairs.
Regardless of the repairs effected, the homeowner should u derstand additional unforeseen
and unforeseeable issues related to expansive soils, soil se lement, and exposure to
unwanted water may cause additional foundation damage i the future. The proposed repairs
may not prevent all additional detrimental foundation move ent. My analysis and
recommendations are based on my education, experience a d observations made on the date
of my visit. The Lepetsos Group, Inc. makes no representa ions regarding the ability of the
property to meet current or past building code requiremen . Please note it was not possible
to observe every structural detail. Please contact this offic if additional information becomes
available which could affect our observations and recomme dations.
