Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R606. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Owner understand that any person or company has the right to file a lien against your property if not fully paid for materials and
labor supplied for this improvement to your property within 120 days upon the completion of the work. Recon Restoration, LLC
will obtain and furnish lien waivers for each trade upon request. Owner authorizes owner's insurance company to communicate
with Contractor and authorizes owner's insurance company to assign payments directly or jointly to the Owner and Contractor.

TERMS AND CONDITIONS
1. Material Selection. Owner is solely responsible for the material and color selection of all products used by Recon Restoration,
LLC (Contractor) and agrees to ensure that all such selections shall meet or exceed any neighborhood, city, homeowner's association,
zoning, master leases, building code, or any other requirements. Owner shall indemnify, defend and hold Contractor harmless from
any action brought by any person, group, or entity concerning the quality, type, color, or other characteristic of the material selected
by Owner.
2. Agent. Owner acknowledges that Contractor is appointed as Owner's agent and representative in working on their behalf to assist
in Owner's insurance claim negotiation. Owner agrees to fully cooperate with Contractor in this regard and shall share any and all
documents or correspondence received regarding Owner's insurance claim with Contractor. Owner appoints Contractor and its au-
thorized representatives as its attorney-in-fact to take any action necessary to fulfill the terms of this contract.
3. Performance. Contractor shall perform all specified work in compliance with all generally accepted industry standards and guide-
lines adhering where applicable to local building codes and ordinances.
4. Failure to Perform. Owner acknowledges that Contractor will not be responsible for any failure to perform its obligation due to
labor strikes, acts of nature, or inclement weather, material shortage, labor shortage, eminent domain actions, court, or any other gov-
emment agency or authority declaration; or any other circumstances beyond the control of Contractor.
5. Owner's Acknowledgment. Owner hereby acknowledges that Contractor shall not be responsible for minor inconveniences and
damages resulting from Contractor's work at the contract property in accordance with all generally accepted industry standards and
practices generally associated with such work. These include, but are not limited to, scratches on gutters, oil on driveway, cracks in
concrete driveway, landscaping damages, scratched or dented fascia, siding, flashing, rotted or broken or falling molding, stucco or
plaster cracks or chips, noise, vibration, vehicles in driveway, any interior damage resulting from any work, etc. Owner waives and
agrees to hold Contractor harmless from such damages and inconveniences. If there are any solar panels on the roof, Contractor will
not be responsible for damage during roofing work. Owner agrees to have a solar panel company take the appropriate action to protect
it, if necessary.
6. Additional Work. Costs for the replacement or repair of any roofing system components including, but not limited to, deteriorated
decking, fascia, swamp coolers, solar panels, vents, exhaust, HVAC, chimney or chimney flashing, step or headwall flashing, win-
dows, etc. or additional work not specifically states in this contract are not included. In addition to any work or damage uncovered
during the repair or replacement process that was unforeseen, is not included in this contract. Such work will be billed separately as an
extra and billed according to the fair market prices. Failure to remit such payment may result in the work being unreasonable delayed.
Owner agrees to pay such amounts in a timely manner.
7. Access. Owner shall allow Contractor, subcontractors, and employees or agents of Contractor to have continuous and reasonable
access to the premises where the work is to be performed at all times during the construction period. Owner shall notify any tenants,
subtenants, leaseholders or other third -parties of Contractor's work at the contract property and shall indemnify, defend, and hold
Contractor harmless from any claim or liability brought by any third -party against Contractor related in any way to Contractor's work.
8. Payments. Accounts are considered past due if full payment is not received ten (10) days after work is completed for "cash bid"
jobs or two (2) weeks after insurance company has issued payment. Owner must take all steps necessary to pay all accounts in a
timely manner. Any amount past due for thirty (30) days will accrue interest at any rate of one and one half percent (1.5%) per month
(18% annum). Owner acknowledges that it shall be responsible for any and all expenses incurred by Contractor for collection of any
past due amounts including, but not limited to, all attorney fees and costs as well as expert fees, etc. If full payment is not received
within 30 days, Contractor may place a lien or encumbrance on the contract property.
9. Warranty. Contractor will provide a limited workmanship warranty for a period of 5 years for a total roof replacement and one (1)
year for roofing repairs. Such warranty does not extend to leaks or water damage below the roof line or any damage resulting from
lightening, excessive wind (70 MPH or greater), ice, or hail during the warranty period. All applicable written manufacturers' warran-
ties on materials shall apply. The Owner is given completed written warranties following payment in full. Warranties shall not be
effective unless payment is made in full. No other warranties expressed or implied are being provided by Contractor and are hereby
disclaimed.
10. Acceptation. Contractor's work is deemed accepted unless the Owner gives written notification within 10 days of the completion
of such work, specifying any and all deficiencies. All claims not made in writing at such time shall be deemed waived and released by
Owner.
11. Insurance Payments. Owner hereby authorizes the insurance and/or mortgage company to make any checks payable jointly. The
Owner hereby also agrees to assign all amounts due from the Owner's insurers and/or mortgagees for work contracted by Contractor.
This includes, but is not limited to, all direct costs, overhead and profit, and any supplements submitted to and approved by the insurer
for additional work, code upgrades, cost increases, and unforeseen work and/or repairs. The insurance payments from the Owner's
insurer are to be made as follows: (ACV) Actual Cash Value check as a down payment; (RCV) Replacement Cost Value checks and
the deductible will be due upon completion of work.
12. Arbitration. All claims, disputes, and other matters in question arising out of or relating to the contract or the breach thereof, ex-
cept for claims which have been waived by the making and acceptance of final payment, shall be decided by Arbitration. This agree-
ment to arbitrate shall be specifically enforceable under the prevailing arbitration law. The award rendered by the Arbitrators shall be
final and judgement may be entered upon it in any court having jurisdiction thereof. Any award shall provide for payment within 30
days of the date of the award. Notice of the demand for arbitration shall be filed in writing with the other party.
13. Cancellation. If the Owner cancels this agreement after the third business day of signing this agreement and prior to the com-
mencement of physical work, Owner agrees to pay Contractor in full, the total sum of the consulting and administrative services com-
pleted to date and time, the cost of non -returnable materials plus applicable taxes, 25% of the cost of materials purchased and/or deliv-
ered as a restocking fee, and the overhead and profit as authorized by the Owner's insurance company. This contract CANNOT be
cancelled once the physical work has commenced.
14. Representations. Any representations, statements, or other communications not written in this contract are agreed to be immate-
rial and are hereby disclaimed.
