Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

Dear James,
Per your request to assess the insurance claims paperwork on the three buildings listed above, we have prepared
this proposal to replace the roof coverings & Gutters according to the Scope of Work below. We will coordinate
additional work on siding, windows, downspouts, etc. as requested by the HOA. AVI may exceed the scope
recommended by the insurance claims estimate and will Supplement AmFam for these additions as applicable
... LE attic ventilation is not adequate in all three buildings and will be Supplemented. Scope of work not
considered as covered by the policy or approved by AmFam will be processed as a Change Order in which you
will have final approval before work is done. AVI will use quality materials and install materials in a workmanship
like manner following generally accepted roof covering standards that meet local City's/County's current IBC and
IRC Building Codes and their amendments. Pictures are included for clarification and to highlight a few overview
conditions.
Overview.
1. Condition of the Roofs:
After thoroughly investigating the roofs, we agree with your adjuster that there is severe hail damage to the
asphalt shingles on the buildings listed above and are generally in sync with your insurance company's estimate
of repairs. Please note that there are two claims covering three buildings. We will assist coordinating between
these two claims to identify all damage combined as follows:
• Bldg. 4440 —The roof only is covered under a 6/28/2016 DOL (Date of Loss) claim, We have determined
that there is additional damaoe to the building that are a result of the 5/8/2017 DOL and not reflected in
the most recent DOL insurance scope of damage that we are seeing, AVI will detail these additional
scope items and present to AmFam as follows:
o Screens on the west elevation
o All Gutters and a few downspouts
o Staining of Deck on west end.
• Bldgs. 4440, 4430, & 4420 -- AVI recommends beginning work on the Roof and Gutters immediately
regardless of which claim is covering these trades. AVI will also Supplement items on the Roof and
Gutters as needed and perform other Code required work. Some known Supplementals are as follows:
0 3 Emergency Repairs (4430 C&B; and 4420 D);
o Code Required work that is listed as PWI on your Insurance PW (paid When Incurred)
o Adequate Ventilation correction (both intake and Exhaust venting are inadequate)
o Others as discovered on build
There may be Supplements required after the work is underway. We will address any changes to the scope after
consultation with you and your insurance company to determine pricing and seek agreement on any changes. We
will also review and discuss the additional scope of work items outlined on the AmFam's insurance estimate with
you to determine how we can be most helpful.

• install all new 2x4" Drip Edge on Eaves and Rakes
• Install 90# Valley material in valleys and Ice Guard on all eaves and around all penetrations
• Install rake and eave flashing that is pre -painted .26 gauge 2x4". Color (Initial your agreement)
• Adequately attic ventilation issues (Must install adequate intake and exhaust ventilation)
• Call in final inspection with the City of Lakewood
• Remove & Install new 5" steel gutters
• Downspouts: scoped only as needed (only a few downspouts are damaged and currently included in the
Insurance Scope of Work).
o NOTE: We will do a thorough inspection of all downspouts when the Adjuster is back on-site to
confirm damage from the new storm: date to Bldg 4440.

4. INSURANCE AGREEMENT
The Owner authorizes AVI Roofing, Inc. to represent their best interest in pursuit of full replacement costs for
damages believed to be covered under HOA's insurance and allow AVI Roofing, Inc. to deal directly with the
insurance company regarding this claim. AVI Roofing, Inc. will complete all specified work at a the RCV
(Replacement Cost Value) price agreeable to the insurance company and AVI Roofing, Inc., with no additional
cost to the HOA except for the deductible and any HOA approved Change Orders. Final agreed upon price will
become the final contract price, regardless of the initial estimated contract price above. We request that all
payments also be made payable to AVI Roofing, Inc.

6. Exclusions from Scope
If there are any additional costs for labor or materials that are not included in this bid or agreed by the
lnsurance Company, they will be subject to change orders.
Replacement of deteriorated decking, fascia boards, roof jacks, ventilators, flashing or other materials,
unless otherwise stated in this Proposal & Agreement are not included and will be charged as an extra on
a time and material basis through submission of detailed change order requests. Owner is responsible
for removal of all outdoor furniture, awnings, or other outdoor fixtures prior to the start of the Work. The
contractor shall not be required to perform work relating to asbestos or polychlorinated biphenyl (PCS).
Owner will be responsible for the additional cost of removal or cost to render harmless this material in the

If this permit expires, a new permit may be acquired for a fee of one.half the amount normally required, provided no changes have been or will be made in the

exceeds one (1) year, full fees shall be paid for a new permit.

that a fire hydrant is required for the above referenced address. This letter is to inform you that the
fire hydrant will need to be installed and accepted by these districts prior to a Certificate of
Occupancy being issued for 4430 Jay Street units C and D. Also no Certificates of Occupancy will
be issued for 4440 Jay Street unlil this fire protection requirement is completed. Please contact the
Wheat Ridge Water District for any information related to what they may need to complete the
installation.

Dear Ms. Reckert,
This letter is to confirm water availability at the above
mentioned addresses.
Please be advised that each address is served by a 3/4" tap.
Currently only the tap at 4440 Jay Street is active, the
others are only Stub-ins and are not paid for. Mr. Cook is
aware of the water situation.
We feel that his proposed development will, at best, be
marginally served by the 3/4" taps.
