REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec. R806. The installation of ridge venting requires the installation or existence of soffit venting. For calculation purposes, one hat or turtle vent equal to one-half of one square foot of opening. Effective December 1, 2014, asphalt shingle installations require an approved midroof inspection, conducted when 25-75 percent of the roof covering is installed, prior to final approval. Installation of roof sheathing (new or overlay) is required on the entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and mid -roofs may be called in at the same time, one hundred percent of the sheathing must be complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave and secured in place is required to be provided for all roof inspections. Roof ventilation is required to comply with applicable codes and/or manufacturer installation instructions, whichever is more stringent. In order to pass a final inspection of elastomeric or similar type roof coverings, a letter of inspection and approval from the manufacturer technical representative stating that "the application of the roof at (project address) has been applied in accordance with the installation instruction for (roof material brand name) roof covering" is required to be on site at the time of final inspection. Coiorado 80301-2475 On October 8, 2000, Drexel, Barrell & Co. conducted a follow up inspection of the drainage system and the Gisness Residence. The following observations have been made: 3034424338 303 442 4373 Fax 1. The berm proposed on the original plan has been installed. This should "heiD" deflect any runoff from the street or a clogged chase/box/pipe junction from flowing into the property. 2. The inlet to the 12" culvert under the driveway and the outlet which is below the invert of the sidewalk chase (refer to DBC letter dated September 26, 2000) needs to be inspected and maintained frequently. This frequent maintenance should prohibit water from' backing up into the swale on the south side of the residence: 3. The perimeter swale around the house was only visually inspected and it appears to drain positively. We did not "as-build" survey this swale. We are relying on the owner to assure the swale drains properly and was constructed to the lines and grades specified on the plans. This needs to be confirmed by the owner just prior to sodding or seeding of the area. 4. In summary, while the original plan was no adhered to exactly, we feel that the owner has taken steps to mitigate the lack of positive drainage. As long as the owner adheres to a routine of frequent inspection and maintenance, and checks the swales for positive drainage prior to sodding, the system should not cause any "major" flooding problems on his property. License: Licensor, without warranting the right, title or privilege, hereby grants Licensee the privilege of relocating a portion of Licensor's ditch and constructing the above described structure(s) in accordance with engineering plans and specifications prepared or to be prepared and submitted to Licensor by Licensee subject to the following terms and conditions: a) Plans and Soecifications: Prior to Licensee's commencement of any work on the ditch or right-of-way, the engineering plans and specifications for such work shall have been submitted to and accepted by Licensor's endorsement thereon and upon acceptance are incorporated by reference herein as Exhibit" A ". , Supervision: All work shall be performed under the supervision of a registered professional engineer and in accDrdance with EXIllBIT "A.."'s plans and specifications, unless a modification thereof is agreed to in writing by Licensor. At Licensor's request, Licensee will supply Licensor with "as constructed" drawings. b) Commencement and Timinl! of Work: Licensee shall not commence any construction on the ditch or right-of-way or affecting ditch operations withol.\t first nDtifying and obtaining Licensor's consent to the commencement and scheduling of such work. thereafter, Licensee shall organize commence, conduct and complete all construction and maintenance activities with all necessary speed and diligence and in such a fashion as to avoid any interference with ditch operation or the full, free flow of water through the ditch. Any and all excavations made shall be immediately leveled off, and any damage to the canals and laterals, embankments, fences, roads or other improvements of Licensor shall be promptly repaired by Licensee at Licensee's sole cost tD satisfaction of Licensor. c) InsDection and Comoletion: Licensor shall have the right to inspect any relocation, construction, installation or removal of any structure by Licensee to assure that such will not interfere with the full free flow of water or otherwise impair operation or use of the ditch system and rights- of-way. Licensee's work shall not be deemed satisfactorily completed until inspection and acceptance thereof by Licensor. Licensor's inspection and Page 4 RMWC License Agreement acceptance shall operate only as approval that the work and structure as performed and installed by License will not interfere with Licensor's use and operation of the ditch system and right-of-way. e) Mechanic Liens: Licensee agrees that all design, engineering, construction and installation or other work permitted hereunder is solely for the benefit of Licensee and Licensee's property. Licensee has no authority to create any lien for labor or material on or against Licensor's property, water rights or ditch system. Licensee agrees to notify all material men, suppliers, contractors or laborers that they must look only to Licensee for payment and that Licensee has no authority whatsoever to bind Licensor or Licensor's property for any costs, charges, or expenses incurred by the work performed hereunder. 7. Construction and Maintenance of Structures: Licensee covenants and agrees to perform all work and construct any structure permitted by this Agreement in a workmanlike manner and with such materials as will safely perform the purpose for which the structure is installed. Licensee covenants to construct and maintain any structure hereunder which encloses, bridges or provides crossing over Licensor's ditch and right-of-way (if any) in a workman1ike manner and with such material as will safely support and carry any weights, loads or traffic that may at any time or in any manner be placed thereon. Licensee covenants and agrees at all times to make, keep and maintain any structure provided for hereunder in a good and safe condition and to repair any deterioration, break, leak and! or damage therein or thereto at Licensee's own expense. Licensee agrees that if by reason of any deterioration, break, leak or damage in or to any structure of Licensee, damage or injury is directly or indirectly caused Licensor, then Licensee will, with all due diligence, repair and place the property of Licensor in the same condition as it was prior to the injury and will pay any and all monetary damages sustained or incurred by Licensor, its stockholders or water users. Licensee shall continue to own and bear full responsibility for the use, operation, maintenance, and if necessary, removal of any crossing or structure pennitted under this Agreement. In the event Licensee fails or refuses to promptly carry out or perform any covenant, duty or obligation to be performed by Licensee, Licensor shall have full right, power and authority, but not the obligation, to perform the same at Licensee's cost and expense without releasing Licensee from any liability hereunder or consequences of breach thereof. Debris: Licensee covenants to continuously and at all times keep that portion of Licensor's ditch and right-of-way covered by this Agreement free and clear of all debris, of every kind and nature whatsoever, so that use of the right-of-way and the full and free flow of water in Licensor's ditch system shall not be interfered with at any time, including times of heavy rain or abnonna11y high water in the ditch caused in any manner whatsoever, and so that no damage can or will result to Licensor, Licensor's ditch system or to the person or property of others by the escape or overflow of water or in any manner whatsoever. 9. Enclosure Gratinll: Licensee agrees that when, if ever, a grate, slat or lattice is required or should be installed, nor or in the future, on the upstream side of any structure covered by this License Agreement, Licensee shall bear full responsibility of the installation or failure to install such grate, slat or lattice, and if installed, for the design, construction and maintenance thereof in such a manner as to avoid any interference with the full and free flow of water in the ditch. Licensee agrees for any grate, slat or lattice required, now or in the future, on any structure covered by this License Agreement to install and keep and maintain the same free and clear of all debris of any kind and nature whatsoever. 10. Indemnification: Licensee covenants and agrees that in the event any penalty, loss, injury or damage is caused Licensor or any other person or persons or their property in any manner whatsoever arising out of the relocation of the ditch right- of-way and/or the construction, maintenance, use existence or removal of the structure (if any) covered by this License Agreement, whether by failure of Licensee to fully carry out and perform any covenant or agreement to be carried out and performed by Licensee or otherwise, Licensee agrees to indemnify and hold Licensor its successors or assigns harmless from any penalties, losses, damages and injuries including, but nDt limited to, actual damages, court costs, attorneys fees, and any other cost and expenses of any kind or nature whatsoever incurred directly or indirectly by Licensor as a result of the design, construction, maintenance, use, existence or removal (if necessary) of any structure hereunder. 11. Ditch Modifications: If in the future Licensor should desire to enlarge, deepen, add to or otherwise change or relocate its ditch system within the right-of-way or to do any other thing incident to the operation and maintenance of its ditch system, right-of-way and easement, then Licensee agrees at Licensee's expense, with all due diligence, to change remove, relay and/or reconstruct any crossing or structure (if any) hereunder so as to comply with such plans and specifications as Licensor Page 6 ',~' RMWC License Agreement may prescribe as being necessary to pennit the proper maintenance and operation of Licensor' s ditch system as altered by Licensor. 12. Tenn and Notice: The License and privilege herein granted Licensee for the use and maintenance of the structures and/or crossing under this agreement shall continue for so long and only so long as Licensee, its successors and assigns comply with the provision hereof. Licensee agrees that in case of any failure on the part of Licensee to fully perfonn the covenants and agreements to be perfonned hereunder, this License may be immediately revoked and terminated and the task perfonned by Licensor at Licensor's sole election. Licensor agrees to give Licensee written notice of its election sent by registered mail to Licensee addressed as follows: James Gisness 3470 Holland Street Wheat Ridge, CO or to such other person or persons as may be designated in writing by Licensee to receive such notice. Licensor's election to terminate this License and performance of Licensee's task or removal of any crossing or structure hereunder shall not relieve Licensee of any liability hereunder for the costs and expenses to Licensor of Licensee's breach or the costs and expense of removal. 13. Covenants Run With Land: the covenants, terms and conditions hereof are intended to run with the land and shall be binding upon and inure to the benefit of the parties hereto and their respective agents, heirs, executors, administrators, personal representatives, successors and assigns. 14. FULL ENVIRONMENTAL TESTS AND ANALYSIS CAN BE COMPLETED FOR AN ADDITIONAL FEE AND EXECUTION OF A SEPARATE CONTRACT. GENERAL: This report presents the results of data obtained during the subsoil investigation of 3404 HOLLAND STREET, JEFFERSON COUNTY, COLORADO. CONCLUSION AND FOUNDATION RECOMMENDATIONS: This house shall be supported on grade beams and straight shaft piers (caissons) drilled a minimum length of sixteen feet and a minimum penetration into the claystone of six feet, or to refusal. These drilled piers should be designed for a maximum end bearing pressure of25,000 psf, a side shear of2,500 psffor the portion of the pier in firm claystone soil, and a minimum dead load of 10,000 psf or as high as possible based on pier end area. The bottom five feet of the holes shall be artificially roughened in order to anchor the pier into the claystone soil. A minimum void or clear space of four inches shall be provided under the concrete grade beams and between the drilled piers to allow for soil expansion. If water is encountered during the drilling of the piers, casing will be required and Type II cement shall be used. The drilled pier holes should be thoroughly cleaned of all loose soil and filled with a low slump concrete immediately after drilling to prevent infiltration of water or loose soil. The concrete piers shall be reinforced for their full length with one number five steel rebar for every sixteen inches of pier perimeter, (a minimum of three number five steel rebars). The reinforcing steel shall 1 extend into the foundation wall a minimum of two feet. The active case equivalent fluid pressure for this soil is 40 psf. If floor movement due to expansive soil is not acceptable, finished room floor areas should be supported on wooden joists over a crawl space area. This test hole consists of a sandy clay to the three foot depth, weathered claystone from the three foot depth to the six foot depth and firm claystone below the six foot depth. One test hole was drilled to a depth of twenty feet. No free water was encountered during the drilling operation. SITE CONDITIONS: At the present time this .site is vacant. It is our understanding that a single family residence with a basement and garage is planned for construction on this site. FIELD AND LABORATORY INVESTIGATION: One exploratory test hole was drilled on June 2, 1997 at the location on the site as shown in Figure I. The test hole was drilled with a four inch diameter power auger. At specific intervals, the drilling tools were removed from the test hole and soil samples were obtained with a two inch inside diameter spoon sampling tube. The depths at which soil samples were taken are shown on the Logs of Test Holes, Figure 2. All soil samples were carefully inspected and classified in the field and laboratory. The samples were subjected to visual inspection and laboratory testing in order to evaluate the physical and mechanical properties of the materials encountered. The natural moisture content and the natural dry density was obtained from relatively undisturbed drive samples of typical materials encountered. Swell-Consolidation tests were also performed on typical samples of soil, see Figure 3. These tests indicate the behavior of the soil upon various loading and wetted conditions. DESIGN AND CONSTRUCTION DETAILS SLAB ON GRADE CONSTRUCTION: The natural soils are suitable for the support of exterior concrete slabs, garage slabs, and basement slabs. However, when these soils have a change in the moisture content and become wetted, they may consolidate in volume and shrink, thus lowering and cracking the concrete slabs, or they may expand, thus heaving and cracking concrete slabs. The builder and future owners should be aware of and understand that there is a definite risk of future damage with any 2 slab on grade construction. If floor movement due to expansive soil is not acceptable, finished room floor areas should be supported on wooden joists over a crawl space area. If the builder and future owners are willing to accept the definite risk of some damage due to concrete floor slab movement, the following recommended details should be carefully followed during construction of the interior floor slabs. Experience with similar soil conditions has shown that these details will help prevent wetting of the soils under the slabs and will minimize damage when wetting of the soil does occur. I. Do not place a gravel layer beneath the concrete slab, unless specified by the foundation engineer. The slab should be placed directly on the undisturbed natural soils, or well compacted fill soil. 2. Separate the slab from all load bearing members and utility lines of at least one inch to allow for independent movement of the slab. Provide a positive control or slip joint at the construction joint between the slab and foundation walls. 3. Provide frequent scoring of the slabs to provide control joints for possible cracking of the slab. Control joints should be placed to provide approximate slab areas of 150 square feet with a maximum dimension between joists of twelve feet. 4. A minimum void or clear space of three inches should be provided at or near the bottom of all non-bearing partitions and furring strips placed over the concrete slabs, see Figure 4. In finished room areas, all drywall and paneling should be stopped approximately three inches above the top surface of the slab. This will allow some space for upward movement of the slab before pressures are applied to the wall and the upper levels of the structure. One should provide a one-half inch space at the bottom of all door jambs to allow for limited movement of the floor slab. The owner is hereby notified that it is their responsibility to maintain these void spaces. 5. In the event a hot water heating system is used, the piping should not be placed beneath the concrete floor slab. In the event a forced air furnace is used, a two inch collapsible connection should be provided between the furnace and the heat ducts. 6. The soils that will support the concrete slabs should be kept moist during construction by occasional sprinkling of water and especially a day or so prior to pouring of the slab. This procedure will help maintain the moisture content of the underlying soil. 3 7. In the event the builder or future owner desires to provide decorative gravel or bark around the house. we have attached Figure 5 to illustrate one acceptable method of installation. The method will prevent ponding of water near the foundation and provide for proper drainage away from the house. SURFACE DRAINAGE: The backfill soil around the house should be moistened and well cQmpacted to prevent future settling. The backfill material should be free of trash, frozen soil and large rocks. The future owner must be advised to fill in any settled area to eliminate ponding of water near the house and to provide adequate slope for proper drainage away from the house and off the site at all times. The finished grade or ground surface near the foundation walls should have adequate slope to provide proper drainage away from the house and off of the site. A minimum slope of ten percent for at least the first five feet out from the house is recommended. Roof downspouts and sill cocks should discharge onto long concrete splashblocks, minimum of five feet long, or into metal gutter extensions to deposit runoff water further out from the house and beyond the limits of backfill soil near the foundation walls. The future owners should be cautioned and advised against the installation of a lawn sprinkler system within five feet of the foundation walls. If a sprinkler system is installed, the sprinkler heads should be placed so that water spray from the heads under full pressure does not fall within five feet of the foundation walls. Lawn irrigation should be controlled to prevent excess wetting of the subsurface soils. Lawn, flowers, shrubs, and other plantings within five feet of the foundation walls should be hand watered and this watering should be minimized. If a drip irrigation system is used, one should limit the amount of water to sustain the plantings. One is also advised that any irrigation lines can leak and/or break and release excessive amounts of water near foundations and can cause damage to slabs and foundation walls. SUBSURFACE DRAINAGE AND BASEMENT CONSTRUCTION: No free water was encountered during the drilling of the test hole. The requirement for subsurface (perimeter) foundation drains will be determined at the time of the open hole observation. See Figure 6 & 7 for a suggested method of installing the foundation drainage 4 system. Ground water should not affect construction or be a deterrent to the construction of the house with a basement. REINFORCING: The concrete foundation walls should be well reinforced at the top and bottom of the walls to clear span at least a distance often feet. This will give the walls, beam strength, to span or bridge over any soft or loose pockets of soil not found in our exploratory holes or that may develop during construction. This will also help prevent differential movements of the foundation system. Refer to the foundation plan for specific details of reinforcement. The foundation plan is not a part of this soils report, and exact requirements are a function of the design of the structure. TOPOGRAPHY: The topography of this site is that of an incline sloping down towards the north at 5%. WEATHER: The weather at the time of the soil examination consisted of partly cloudy skies, with warm temperatures. CONSTRUCTION DETAILS: In any soil investigation it is necessary to assume that the subsurface soil conditions do not vary greatly from the conditions encountered in our field and laboratory testing. Our experience has been that at times soil conditions do change and variations do occur and may become apparent to the time of excavation for the foundation system. If soil conditions are encountered which appear different from the test borings as presented in this report, it is required that this office be called to inspect the open excavation. This inspection is not a part of this report and is not included as any part thereof. 5 OPEN HOLE OBSERVATION: The owner, or a representative of the construction firm, shall contact Colorado Soils and Percolation Test, Inc., a minimum of twenty four hours prior to digging of the foundation excavation to schedule an open hole observation. This observation must be performed before the placement of concrete, and preferably to the setting of the forms. This observation will insure soft spots, anomalies, etc. are not present in locations which will affect the foundation. At the time of the observation, the requirement for subsurface drainage will be established.
