2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing, and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard

Per your contractor's request (Rocky Mountain Steel Peering, Inc), we visited the subject site on August 7 &
17, 2017 to observe the installation of a helical pier and steel frame.
At the time of our observations, we observed the installation of 1 of the 4 piers used for the foundation repair.
Mike Stimac, with Rocky Mountain Steel Peering, Inc., provided us the information on the depth and torque
loads. Based on this information and our observation, it appears that the helical piers have been installed in
general conformance with our recommendation letter dated April 25, 2017. We recommend that the cosmetic
repairs be completed following a 60 day adjustment period.
The scope of our site visits was limited to the items listed above.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

4. CANCELLATION. This contract may be rescinded within seventy-two hours of the date and time of signing for a full

cancelled by Owner other than as contemplated by the preceding two sentences prior to completion of Work by Company,
Owner shall be obligated to (i) provide at least five days prior written notice of cancellation to Contractor and (ii) pay to
Contractor an amount equal to 5% of the Total Price of the Work, plus any actual costs incurred by Contractor through the
termination date.
5. INSURANCE PROCEEDS. If you plan to use the proceeds of a property and casualty insurance policy issued pursuant
to part 1 of article 4 of title 10 of the Colorado Revised Statutes to pay for the roofing work, then pursuant to section 6-22-
105 of the Colorado Revised Statutes Contractor cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part
of any insurance deductible applicable to the insurance claim for payment for roofing work on the covered residential
property.

Dear Jeff,
As requested, we visited the subject site on March 10, 2017 for the purpose of observing reported
structural settlement and to provide necessary repair recommendations to mitigate future settlement.
OBSERVATIONS
We observed a structure that, according to county records, was originally constructed in 1949 of a cast -in-
place concrete foundation with a light framed wood superstructure. The dining room was added on at an
unavailable date, and was constructed of CMU block bearing directly on grade. It does not appear that the
block wall was reinforced or grouted. The block wall appears to be settling away from the main structure,
with cracking occurring at several locations. The settling of this block wall has caused distress in the
finished above.

CONCLUSIONS AND RECOMMENDATIONS
In our opinion, the block wall should be underpinned in attempt to minimize future settlement. As it
appears that the block wall is currently unsupported, we recommend constructing a steel frame at the
exterior perimeter of the wall. The frame it to be constructed of L6x6x3/8 angle material. Cut, grind and
weld the corners for support of the end wall. Provide t4 partial joint penetration weld at this location. Bolt
the frame to the block at 4ft on center minimum.
We recommend that 4 (four) lielical piers be installed, at a minimum, where shown on Page 1 of the
attachment to thi& letter. The helical piers shall be manufactured byMacLean-Dixie HFS. Each pier shall
be designed to support a minimum allowable load of 15,000 pounds. The piers should extend a minimum
of 24ft below the addition, and torques per manufacturer requirements are to be achieved for the desired
load. The piers shall have a positive connection to support angle and to the block wall, using epoxy sleeve
anchors, welding or other means. Refer to the manufacturer for complete installation details.
It may be prudent to isolate the supporting angle from the soils under the wall, and to provide 2" of rigid
XPS insulation around the perimeter walls to help protect against frost heave. This is to be completed at
the discretion of the homeowner.
After the piers have been successfully installed, the drywall, foundation, and other cosmetic repairs may be
completed. We recommend that the block wall cracks 1/ 16" wide or larger be repaired by the use of a low
viscosity, high -bond strength epoxy such as Simpson Crack-Pac or equal. Follow all manufacturer
recommendations for the repair of these cracks, including the use of any adhesives, bonding agents, and
coating materials. Cracks less than 1 /16" wide are not a structural concern at this time.

ALTERNATE RECOMMENDATIONS
We understand that helical piers may be costly. While they are considered a more permanent repair,
alternate repairs with less consistent results can be used. We have attached recommendations for support
of the superstructure that may assist in arresting the movement of the superstructure. We note with the
repairs on Pages 3-6 of the attached recommendations are designed to support superstructure only and
will not support the CMU foundation, which may further deteriorate.
LIMITATIONS
The opinions, conclusions, and recommendations provided in this report are based on visual observations
of the readily assessable areas of the structure. No destructive inspection (i.e. removal of cladding,
sheathing, or covering of any kind, testing or penetration of any wall, floor, or ceiling) was performed
unless specifically noted. As a result of these limitations, latent structural damages may exist which were
not detected. Due to the limitations noted above, and the expansive soils assumed to exist on this site, no
warranties regarding the future performance of the structure, including any repaired area, are expressed,
written, or implied. This evaluation is not intended to determine building code compliance, nor is it
intended to determine the presence of mold or other hazardous material.
The scope of our site visit was limited to the items described above.
