by any Sanitation District representative. Inspection and approval of work by any

**For trenchless sewer replacements - The contractor will verify proper slope of new sewer
piping in conformance with the IRC and IPC of 1/4 inch per foot unless otherwise allowed

STRUCTURAL OPINION REPORT AND REPAIR DESIGN
Dear Tena:
On June 6, 2018, Doug Dahl of this firm inspected the above referenced property at your request. The pur-
pose of the inspection was to visually observe the front and rear foundation walls of the basement to deter-
mine the requirements for structural repairs. The inspection did not involve any other system or component
of the residence. No soil or other material sampling, testing, or analysis was performed by Coyle Structural
Inspection Engineers.
General Information:
• The home is a 4 level single family home structure, assumed to be facing East. The home was reportedly
constructed in 1969.
• The foundation is of poured concrete construction.
• The perimeter walls of the upper structure are of conventional wood frame construction.
• No soils report or other architectural/engineering documents were available for review.
• Provided for our use was a copy of the Proposal by Mike Lombardo of Pinnacle Structural, proposing 5
wall anchors at each of the front and rear walls..
Observations, Opinions and Recommendations:
• Structural displacements have occured to the front and rear walls.
• Restraint with wall anchors is a reasonable repair method.
• Please refer to the attached diagrams and specifications.
• With the wall anchors installed the proposed egress window can be installed.
Additional Comments:
All foundations are subject to movement over time, due to v��ations in soil and moisture conditions. The
home should be monitored for additional future movement 4-n o eaent should be re-

FOUNDATION DRAIN (if needed):
• Drain Pipe shall be 4" rigid PVC pipe with drain holes at the 4:00 and 8:00 positions.
• Trench to be a minimum of 8" wide and 8" deep (at the high point).
• Gravel to be 3/4" washed rock.
• Trench Liner to be filter fabric at sides and bottom of trench
• Depth - top of pipe to be 4" below crawlspace soil elevation at the high point of the drain system.
• Slope - the trench bottom and pipe shall slope downward from the high point to the sump pit at a
continuous minimum slipe of 1/8" per foot.
• Pit/Pump - pit to be preformed corrugated plastic, or otherwise approved material, 24" diameter
+/- and with bottom at least 18" below the bottom of the lowest inlet pipe. Pump to be 3/4 HP
submersible. Permanent electrical supply to be supplied to the pump without need for extension
cord. Pit location to be determined by agreement between the contractor and owner.
• Discharge Pipe shall be 1.5" diameter (minimum) PVC and contain a check valve no more than
24" above the top of the pit. Discharge location to be determined by the contractor but shall be a
minimum of 10 feet from the foundation at a point where ground slop .La4 ow the discharge
to continue flowing away from the foundation. Some municipaliti(Ii6Whged

placed into the sanitary sewer system, and if allowed in this muniality this wodld be the pre-
ferred method of discharge. A special permit and connection by a plurnber may be re-

STRUCTURAL REPAIR NOTES FOR THE HOMEOWNER:
• CONTRACTOR SELECTION and VERIFICATION: Structural repairs should only be performed
by qualified contractors with significant experience. The homeowner is solely responsible for bidding,
contractor selection, verification of contractor's experience, licensing, insurance, warranty program, etc.
This contractor selection and verification process is not part of the scope of services provided by Coyle
Structural Inspection Engineers.
• FOUNDATION STABILIZATION: It should be understood that underpinning/stabilization of part of
the foundation will minimize future movement in the area stabilized, but will not necessarily prevent
movement elsewhere. Movement elsewhere (if it occurs) may in fact become more apparent once part of
the foundation is stabilized, as cracking may develop at the transition points between the stabilized area
and the non -stabilized area.
• FOUNDATION ADJUSTMENT: Some incremental adjustments (lifting/lowering/realignment) of the
affected area may be attempted with the owner's approval. It must be understood that adjustments could
cause additional damage to finished surfaces and foundation walls. The owner is encouraged to be
present at the time of adjustments in order to authorize the amount of adjustment to be attempted. Coyle
Structural Inspection Engineers shall not be responsible for adjustment authorization or for any damages
caused by attempted adjustments.
• GOOD DRAINAGE PRACTICE: Poor drainage has the potential to cause structural damage and
water intrusion. The owner is advised to improve and/or maintain drainage per the guidelines below, in
order to promote long term structural integrity for the property.
GOOD DRAINAGE PRACTICE (for the homeowner):
• Providing and/or maintaining good drainage is recommended in Colorado, in order to promote stabiliza-
tion of moisture content in the foundation bearing soil. Stability of moisture conditions at foundation
depth is considered essential toward promoting long term structural integrity of all homes.
• These drainage remarks are general in nature to all homes in the area, and not necessarily site specific, as
each site has unique constraints. In some cases, consultation with a land surveyor may be necessary in or-
der to determine a detailed scope of drainage improvement, based on site constraints.
• Provide/maintain positive drainage away from the foundation. Building Codes typically require a mini-
mum of 2% slope (1/4" per foot) and should be considered an absolute minimum. Ideally, 6" of fall in the
first 5' is recommended by most soils engineers in the Colorado Front Range area with expansive soil
conditions, for landscape areas, and then 2% (1/4" per foot thereafter). However, care must be taken not
to raise the soil too high relative to siding/framing. Several inches of clearance should be maintained to
deter moisture and pest access. If clearance cannot be maintained, then cutting the soil down and away
from the foundation may be necessary in lieu of adding soil. In this case landscape drainage swales and/or
french drains may need to be created in order to convey the drainage in a continuous manner. In some
cases, addition of sidewalk "skirts" or hardscape patios may be an alternative.
• Hardscape areas such as driveways, patios, sidewalks, etc. should slope away from the foundation, at a
minimum of 1% slope (1/8" per foot) and should be installed at an elevation that does not act as a barrier
to drainage. Where abutting the foundation, the hardscape should be well sealed to prevent moisture pene-
tration. It is not recommended to attach hardscapes to the foundation via rebar pinning.
• Construct or improve landscape swales, as needed, to route the drainage around the foundation. The flow -
lines of the swales should have a slope of at least 2% (1/4" per foot). In areas where 2% cannot be at-
tained, a minimum slope of 1% is acceptable if a French drain is incorporated in the flowline. Swale flow -
lines should be located no closer than 5 feet from the foundation walls. The swales sWixld be itinued
to a points where natural drainage is possible.
• Maintain/repair/replace roof gutters and downspouts, as needed, to prevent leaka d�/OE eZspi ing
around the foundation. Install and/or repair/maintain downspout extenders, as nee A, to discharg a min-
imum distance of 5 feet from the foundation. Determine the discharge locations o y�biuie&7, spout

pipes and test with a hose to ensure unimpeded flow and visible (daylight) discharge locations. If the dis-
charge locations are not evident, and/or if blockage exists, the pipes may need to be extended or replaced.
• Determine the locations of any and all active sprinkler heads, and move as needed to a distance of 5 feet
from the foundation as a minimum, and orient so that spray does not fall within 5 feet of the foundation.
Drip irrigation and/or "misting" heads are generally considered to be acceptable as long as they are used
sparingly. Maintain or create a 5 foot wide non -irrigated landscape buffer around the foundation.
• Replace or modify any solid landscape edging to allow drainage to pass thru the edging. Remove any
plastic weed barriers adjacent to the foundation (within 5 feet). If a weed barrier is desired, use a geotex-
tile mesh landscape fabric that allows the soil to evaporate its excess moisture.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process

Enterprises cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible applicable to the insurance claim for payment for roofing
