to be installed in every sleeping room, in hallways outside of sleeping rooms, and on every
level of the structure.

1, Tis permit was issued based on the information provided in thepermit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180.days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permitfee.
I 3. If this permit expires, a new permit may be required to e obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

AGREEMENT
This Agreement ("Agreement") is made on ! . 20 between Family Roofing LLC, a Colorado Limited

Family Roofing will advocate the Property Owner'(s) best interest in restoration of property damage and complete
all work in accordance to insurance company agreed scope of loss ("Loss") and cash value of the insurance
settlement ("Settlement"). Work shall only include what is covered by the Settlement and Family Roofing's
compensation for such work shall be limited to the Settlement's scope of work price for the work actually
performed by Family Roofing. Family Roofing has been materially induced to enter in to this Agreement by
Property Owner's intent to make payment from proceeds of the Settlement. As part of this Agreement, any
supplement sent to the Insurer on the Property Owner(s) behalf will be included in the Contract Price. Upgrades

TERMS AND CONDITIONS OF THE AGREEMENT BETWEEN FAMILY ROOFING AND IV 7- _� t
1 Property Owner's Responsibilities. The Property Owner is responsible to supply electrical utilities unless otherwise agreed to in
writing. Electricity to the site is necessary. Property Owner agrees to allow and provide Family Roofing and its equipment access to the
property. The Property Owner is responsible to remove or protect any personal property and Family Roofing shall not be responsible
for personal property including driveways, lawns, shrubs, etc.
2. Delays. Family Roofing agrees to start and diligently pursue work through to completion, but shall not be responsible for delays for
any of the following reasons: failure of the issuance of all necessary building permits within a reasonable length of time, funding of
loans, disbursement of funds into control or escrow, acts of neglect or omission of Property Owner or Property Owner's employees or
Property Owner's agent, acts of God, stormy or inclement weather, strikes, lockouts, boycotts or other labor union activities.
3. Plans and Specifications. Family Roofing will obtain and pay for all required building permits. if the Property Owner cancels the
agreement, the Owner agrees to pay for any city pen -nit fees already incurred for the property.
4. Subcontracts, Family Roofing may subcontract portions of this work to properly licensed and qualified subcontractors.
55.. Insurance. Family Roofing shall carry General Liability Insurance for the protection of Family Roofing's workers during the
progress of the work. Property Owner warrants that he/she maintains homeowner insurance and will indemnify Family Roofing
against injury to Property Owners)/residents or persons on the job site at Property Owner's invitation.
6. Rightto Stop Work Family Roofing shall have the right to stop work if any payment shall not be made, when due, to Family
Roofing under this Agreement; Family Roofing may keep the job idle until all payments due are received. Property Owner's failure to
make payment, when due, is a material breach of this Agreement.
7. Clean Up. Family Roofing will remove from Property Owner's property debris and surplus material created by the Work and leave
it in a neat and broom clean condition.
8. Limitations. No action of any character arising from or related to this Agreement, or the performance thereof, shall be commenced
by either party against the other more than two years after completion or cessation of work under this Agreement.
9. Validity and Damaaesin case one or more of the provisions of this Agreement or any application thereof shall be invalid,
unenforceable or illegal, the validity, enforceability and legality of the remaining provisions and any other applications shall not in any
way be impaired thereby. Any damages for which Family Roofing may be liable to Property Owner shall not, in any event, exceed the
cash price of this Agreement. Family Roofing is not responsible for any damage to interiors, rafters, and/or any and all housing structures
resulting from removal of excess layers and/or the vibrations caused during the re -roofing process. Family Rooting is not responsible for any
water damage to interiors or contents due to water leaks from roof after installation. In the event of water leaks during warranty period, Family
Roofing liability shall be limited to labor and material costs of repairing defective work on roof only. Family Roofing shall not be liable for
any cracks or damage to driveways, walkways, sidewalks either caused or not caused by Family Rooting and/or its sub -contractors. Family
Roofing shall not be liable for any bowed, cracked, or sagging rafter after installation either caused or not caused by Family Rooting and/or its
sub -contractors.
10 Asbestos Lead. Mold and other Hazardous Materials. Property Owner hereby represents that Property Owner has no
knowledge of the existence on or in any portion of the premises affected by the Project of any asbestos, lead paint, mold (including all
types of microbial matter or microbiological contamination, mildew or fungus), or other hazardous materials. Testing for the existence
of mold and other hazardous materials shall be performed in accordance with Colorado Law. Unless the Agreement specifically calls
for the removal, disturbance, or transportation of asbestos, polychlorinated biphenyl (PGB), mold, lead paint, or other hazardous
substances or materials, the parties acknowledge that such work requires special procedures, precautions, and/or licenses. Therefore,
unless the Agreement specifically calls for same, if Family Roofing encounters such substances, Family Roofing shall immediately stop
work and allow the Property Owner to obtain qualified asbestos and/or hazardous material Family Roofing to perform the work or
Family Roofing may perform the work itself at Family Roofing's option. Said work will be treated as an extra cost under this
Agreement, and the Agreement Term setting forth the time for completion of the Work may be delayed. In the event that mold or
microbial contamination is removed by Family Roofing, Property Owner understands and agrees that due to the unpredictable
characteristics of mold and microbial contamination, Family Roofing shall not be responsible for any recurring incidents of mold or
microbial contamination appearing in the same or any adjacent location, subsequent to the completion of the work performed by
Family Roofing. Property Owner agrees to hold Family Roofing harmless, and shall indemnify Family Roofing harmless for any
recurrence of mold or microbial contamination. Property Owner also agrees that Family Roofing shall not be responsible, and agrees
to hold Family Roofing harmless and indemnify Family Roofing, for the existence of mold or microbial contamination in any area that
Family Roofing was not contracted to test and/or remediate. Further, Property Owner is hereby informed, and hereby acknowledges,
that most insurers expressly disclaim coverage for any actual or alleged damages arising from mold or microbial contamination.
Family Roofing makes no representations whatsoever as to coverage for mold contamination, though at Property Owner's additional
expense, if requested in writing, Family Roofing will inquire as to the availability of additional coverage for such contamination or
remediation, and if available, will obtain such coverage if the additional premium is paid for by Property Owner as an extra.
11. Standards of Materials and Workmanship. Family Roofing shall use and install "standard grade" or "builder's grade" materials
on the project unless otherwise stated in the Scope of Work, the plans, and/or specifications provided to Family Roofing prior to the
execution of this Agreement.
12. interest Overdue payments will bear interest at the rate of 1.5% per month (18% per annum).
13. Chances in the Work. Should the Property Owner, construction lender, or any public body or inspector direct any modification or
addition to the work covered by this Agreement, the Contract Price shall be adjusted accordingly. This includes any requirements
from governing bodies as it pertains to code items and other required construction requirements. Modification or addition to the work
shall be executed only when both the Property Owner and Family Roofing have signed a Change Order Agreement.
14. Excess Materials, Family Roofing has the right to order excess materials. All excess materials belong to Family Roofing.
15. Supplemental Estimates and Charges. Family Roofing reserves the right to file for supplemental insurance claim charges if
insurance adjusters scope of work or recommended repairs are inadequate, incomplete, or unsatisfactory. Family Roofing reserves the
right to submit supplemental insurance claim due to an increase in costs. Supplements paid by the insurance company for additional
costs arising from materials, labor or additional scope of repairs are part of this Agreement as if contained herein and will be paid to
Family Roofing.
16. Warranty. Any warranty from Family Roofing either express or implied shall not be effective until the Contract Price is paid in full.
Warranty applies to roof only. The Workmanship Warranty is for two years from the month of the roof installation. Labor warranty is void in
the event of gale force winds (50 m.p.h.). lightning to roof, hurricane, tornado, hail damage, impacts of foreign objects or other violent storm
or casualty, or damage to roofs due to settlement, distortion, failure or cracking of the roof deck, wall or foundation of a building. Property
Owner agrees that material warranties are limited warranties issued by material manufactures and are subjected to terms and limitations from
each manufacturer.
17. Inspections. The final payment in full shall not be held up, or retained contingent while waiting for the city, county, insurance
company, or lien holder inspection or appropriate building department to complete final inspection and issue satisfactory completion.
Final payments are due upon completion of construction in accordance with the terms of the Agreement.
10Asbestos Lead Mold and other Hazardous Materials. Property Owner hereby represents that Property Owner has no
knowledge of the existence on or in any portion of the premises affected by the Project of any asbestos, lead paint, mold (including all
types of microbial matter or microbiological contamination, mildew or fungus), or other hazardous materials. Testing for the existence
of mold and other hazardous materials shall be performed in accordance with Colorado Law. Unless the Agreement specifically calls
for the removal, disturbance, or transportation of asbestos, polychlorinated biphenyl (PCB), mold, lead paint, or other hazardous
substances or materials, the parties acknowledge that such work requires special procedures, precautions, and/or licenses. Therefore,
unless the Agreement specifically calls for same, if Family Roofing encounters such substances, Family Roofing shall immediately stop
work and allow the Property Owner to obtain qualified asbestos and/or hazardous material Family Roofing to perform the work or
Family Roofing may perform the work itself at Family Roofing's option. Said work will be treated as an extra cost under this
Agreement, and the Agreement Term setting forth the time for completion of the Work may be delayed. In the event that mold or
microbial contamination is removed by Family Roofing, Property Owner understands and agrees that due to the unpredictable
characteristics of mold and microbial contamination, Family Roofing shall not be responsible for any recurring incidents of mold or
microbial contamination appearing in the same or any adjacent location, subsequent to the completion of the work performed by
Family Roofing. Property Owner agrees to hold Family Roofing harmless, and shall indemnify Family Roofing harmless for any
recurrence of mold or microbial contamination. Property Owner also agrees that Family Roofing shall not be responsible, and agrees
to hold Family Roofing harmless and indemnify Family Roofing, for the existence of mold or microbial contamination in any area that
Family Roofing was not contracted to test and/or remediate. Further, Property Owner is hereby informed, and hereby acknowledges,
that most insurers expressly disclaim coverage for any actual or alleged damages arising from mold or microbial contamination.
Family Roofing makes no representations whatsoever as to coverage for mold contamination, though at Property Owner's additional
expense, if requested in writing, Family Roofing will inquire as to the availability of additional coverage for such contamination or
remediation, and if available, will obtain such coverage if the additional premium is paid for by Property Owner as an extra.
11 Standards of Materials and Workmanship. Family Roofing shall use and install "standard grade" or "builder's grade" materials
on the project unless otherwise stated in the Scope of Work, the plans, and/or specifications provided to Family Roofing prior to the
execution of this Agreement.
12. interest. Overdue payments will bear interest at the rate of 1.5% per month (18% per annum).
13. Changes in the Work. Should the Property Owner, construction lender, or any public body or inspector direct any modification or
addition to the work covered by this Agreement, the Contract Price shall be adjusted accordingly. This includes any requirements
from governing bodies as it pertains to code items and other required construction requirements. Modification or addition to the work
shall be executed only when both the Property Owner and Family Roofing have signed a Change Order Agreement.
14. Excess Materials. Family Roofing has the right to order excess materials. All excess materials belong to Family Roofing.
15. Supplemental Estimates and Charges. Family Roofing reserves the right to file for supplemental insurance claim charges if
insurance adjusters scope of work or recommended repairs are inadequate, incomplete, or unsatisfactory. Family Roofing reserves the
right to submit supplemental insurance claim due to an increase in costs. Supplements paid by the insurance company for additional
costs arising from materials, labor or additional scope of repairs are part of this Agreement as if contained herein and will be paid to
Family Roofing.
16. Warranty. Any warranty from Family Roofing either express or implied shall not be effective until the Contract Price is paid in full.
Warranty applies to roof only. The Workmanship Warranty is for two years from the month of the roof installation. Labor warranty is void in
the event of gale force winds (50 m.p.h.), lightning to roof, hurricane, tornado, hail damage, impacts of foreign objects or other violent storm
or casualty, or damage to roofs due to settlement, distortion, failure or cracking of the roof deck, wall or foundation of a building. Property
Owner agrees that material warranties are limited warranties issued by material manufactures and are subjected to terms and limitations from
each manufacturer.
17. inspections. The final payment in full shall not be held up, or retained contingent while waiting for the city, county, insurance
company, or lien holder inspection or appropriate building department to complete final inspection and issue satisfactory completion.
Final payments are due upon completion of construction in accordance with the terms of the Agreement.
18. Leal and Collection Fees. Property Owner shall be liable for all costs of collection, including all court costs, all attorney's fees (both
hourly and contingency fees), and all collection aeency fees in the event this Agreement is placed for collection with an attorney andior

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd

approval of, any violation of the provisions of the building codes or any other ordinance, law, rule or regulation. All plan review
