evaluation of the loading was based on the site observations and the design criteria listed below.

McGourty Engineering visited the property on October 16th 2018 to review the framing
installation. Further installation documentation was provided via photograph from the contractor
on October 26th 2018.
The completed framing installation as well as required existing conditions were observed at the
time of the site visit and photographs provided, to be in general conformance to the design intent.
No further modification was necessary to the framing modifications made at the time of
observation.
Note: This installation observation is limited to the items identified in the above referenced

Rough Framing and Wood Requirements
3 The complete rough inspection is to be made after the rough framing, rough electric, rough plumbing, and
rough heat and ventilation are complete.
4 Fire blocking shall be installed to cut off all concealed draft openings in the following locations: (IRC
302.11)
A. In concealed spaces of stud walls and partitions, including furred spaces, vertically at the ceiling and floor
levels, and at 10 -feet horizontal intervals. B, At interconnections between concealed vertical and horizontal
spaces such as occur at soffits, drop ceilings, and cove ceilings. C. In openings around vents, pipes, ducts, cable,
wires, chimneys, fireplaces, and similar openings that afford a passage for fire at ceiling and floor levels. D. In
concealed spaces between stair stringers at the top and bottom of the run, and between studs in line with the
run of the stairs at unfinished walls.
5 Appliances and equipment shall be installed in accordance with the conditions of their listing.
Manufacturer's installation instructions must be available at the time of the inspection. (IRC G2408.1,
M1307.1, M1401.1,(M1902.2)
6 Appliances shall be accessible for inspection, service, repair and replacement without removing
permanent construction. 30 -inches of working depth and width shall be provided in front of the control side
of an appliance. (IRC M1305.1)
7 Combustion air is required for all liquid fuel, solid fuel and gas appliances. (IRC M1701.1, G2407)
8 Gas burning appliances may not be located in or obtain combustion air from: a sleeping room, bathroom,
toilet room, or storage closet unless it is a direct vent appliance or installed in an enclosure that opens only
into a bedroom or bathroom in and such enclosure is provided with a solid weather-stripped door, a self-
closing device and all combustion air is taken from the outdoors. (IRC G2406.2)
Electrical Requirements
9 All hot, neutral, and grounding conductors in electrical boxes shall be tied together wit a mechyanical

Recessed lighting fixtures must either be IC -rated or installed in a sealed box in which the insulation is at least
3 -inches from the light fixture. All recessed lights must be tightly sealed or gasketed to prevent air leakage
through the fixture into the ceiling cavity. (IRC N1102.4.5)
10 Tamper -resistant Receptacle outlets are required in every kitchen, family room, dining room, living room,
parlor, library, den, sunroom, bedroom, recreation room or similar dwelling areas spaced so that no point
along the wall line exceeds 6 -feet from an outlet. (IRC E3901, E3902, and 4002.1-4002.14)A. Counters 12 -
inches in width or wider shall have receptacles placed no more than 20 -inches above the counter top and
spaced so no point exceeds 24 -inches from an outlet. B. Island and peninsula counters with a length at least
24 -inches and a width of 12 -inches or greater must be equipped with at least one outlet. C. At least one
receptacle shall be installed in a bathroom within 36 -inches of each basin. D. A least one receptacle outlet
must be installed in each laundry, basement, attached garage and detached garage with power. E. At least
one receptacle outlet that is accessible while standing at grade level and located not more than6-feet, 6 -
inches (1981 -mm) above grade, shall be installed outdoors at the front and back of each dwelling unit having
direct access to grade. Balconies, decks, and porches that are accessible from inside of the dwelling unit and
that have a usable area of 20 square feet. F. Attics or crawlspaces containing equipment must be provided
with at least one receptacle outlet within25-feet of the equipment. G. Hallways 10 -feet or more in length.
11 Two 20 -amp rated branch circuits shall serve receptacles in the kitchen, pantry, breakfast area, and dining
area. At least one 20 -amp branch circuit shall supply bathroom receptacles and at least one 20 -amp branch
circuit shall supply laundry. (IRC E3703.2, E3703.3, E3901)
32 Ground fault protection shall be provided on all 125 -volt, single phase, 15 and 20 -amp receptacles in the
following locations: bathrooms, garages, unfinished grade level accessory buildings, at exterior locations,
crawlspaces, unfinished basements, kitchen counters, within 6ft. of laundry, utility and wet bar sinks,
boathouses, hydro massage, spa and hot tub locations. Receptacles must be in a readily accessible location.

13 Combination type Arc -Fault circuit interrupters must be installed on all 120 -volt 15 and 20 -amp branch
circuit outlets in Receptacle outlets are required in every family room, dining room, living room, parlor, library,
den, sunroom, bedroom, recreation room, hallway, closets, and similar dwelling areas. (IRC 3902.12)
14 Smoke alarms shall comply with UL 217. Smoke alarms are to be powered by the building's wiring operated
light. (IRC E3903)with battery backup and shall be interconnected so the actuation of one alarm will activate
all alarms in the dwelling unit. Smoke alarms shall be installed in the following locations: (IRC R314) A. In each
sleeping room; B. Outside each sleeping area in the immediate vicinity of the bedrooms; and C, One ach

Plumbing Requirements
17 DWV systems shall be tested on compellation of the rough piping installation by water or for piping
system other than plastic by air with no evidence of leakage. Either test shall be applied to the drainage system
in its entirety or in sections after rough piping has been installed. I) Water test 10 feet above the highest fitting
connection or to the highest point of the completed system. The system shall prove leak free for 15 minutes.
2) Air test shall maintain 51hs of pressure for 15 minutes.
18 Piping must be inspected and tested prior to concealing. It shall be protected against physical damage and
it must be approved for use and bear the identification of the manufacturer. (IRC P2503, P2603, P2608.1)

Advanced Environmental Consulting, LLC (AEC) was retained to collect samples of suspect asbestos -containing
material(s) (ACM) that would be disturbed at the above -referenced property on the above referenced date(s); and
provide an asbestos building inspection report including laboratory analysis.
Colorado Regulation No. 8 - Part B Section TMA. 1, Prior to any renovation or demolition which may disturb
greater than the trigger levels of material identified as a suspect ACMpursuant to the Environmental Protection
Agency (EPA), the facility components) to be affected by the renovation or demolition shall be inspected to
determine if abatement is required. " See below for further information on identifying trigger levels, sampling
protocol, and materials classifications per the regulations.
2.0 EPA LEAD REGULATIONS
As of April 22, 2010, the EPA instituted guidelines and regulations for the renovation and/or remodel of homes built prior
to 1978, the year that the manufacturing of lead-based paint was prohibited. If lead is present within paint on building
components above the action level, any activity impacting these components must be conducted using "lead safe" work
practices and ERP (Renovation, Repair, and Painting) Certified contractors.
Trigger levels:
■ An inspection must occur if more than six square feet (6 SF) of interior surfaces;
■ Twenty square feet (20 SF) of exterior surfaces will be disturbed,
■ ANY window is being replaced.
As of March 26, 2014, the EPA has instituted new guidelines and regulations for the failure of firms to follow RRP
engineering controls, or when remodeling projects are done by individuals or firms that do not have proper RRP training
and certification.
When it is suspected that a lead hazard has been created, EPA will require an inspection, including wipe samples. If the
results of the inspection determine that a lead hazard has been created the follow steps will be conducted:
The site will be considered lead -contaminated and RRP standards and practices will no longer apply;
Both the EPA and the CDPHE Lead Unit have determined that clean-up is now defined as a lead abatement project,
Regulation 19 standards will be required to be followed in order to properly clean-up the lead hazards:
- A state certified abatement firm and individuals will be required to conduct the clean-up
- The firm will need to apply for a permit and follow all requirements of Regulation No. 19, Part A, Section V,
Abatement
- A state -certified firm and individual will be required to conduct project clearances, sampling and provide the
CDPHE Lead Unit with copies of those reports.
Colorado Air Quality Control Commission Regulation 19, Part A, states:
■ Lead-based paint hazard means any condition that causes exposure to lead from lead -contaminated dust, lead -
contaminated soil, or lead-based paint. Lead-based paint hazard also means hazardous lead-based paint, dust -lead
hazard or soil -lead hazard.
The inspection and assessment were performed in accordance with EPANAHERA procedures. These procedures call for the
visual inspection of all areas of concern including collection and analysis of representative bulk samples of suspect material.
Random bulk samples, representative of the suspect asbestos -containing materials (ACM) of each homogeneo HA),
were collected according to the guidelines published as EPA Final Rule: Title H of the Toxic Subs es Contra ct
(TSCA), 15 USC, Sections 2641 through 2654 and in compliance with 40 CFR, Part 763. City o

Representative sampling is based on the following criteria:
• The distribution of the suspect material throughout the HA,
• The suspect materials physical characteristics and application,
• Random sampling patterns determined for each HA.
4.0 SAMPLING
Suspect materials alike in appearance and application were sampled as HAs. Those sampled suspect materials are then
subcategorized into one of three classifications:
1. Surfacing Materials (S1VI)' sprayed or troweled onto structural building member.
2. Thermal Systems Insulation (11,Z any type of pipe, boiler, tank, or duct insulation.
3. Miscellaneous Materials (1ll1VI : other suspect materials, including flooring, ceiling tiles, insulation, and
finishing materials.
AEC collected surfacing materials by types of texture. Not every room with each texture may have been tested; however, it
is assumed to be positive if it has the same texture as the above -referenced types. AEC uses drywallsehool.coin/texthures to
determine the texture label comparisons. For thermal system insulation materials, AEC uses images through Google for
comparison when naming material types. 'Miscellaneous materials are named based on the material they are as seen by the
accredited inspector during the on-site inspection.
The following outlines the minimum sample quantities required per HA material classification:
1. Surfacing Materials (W. up to 1,000 SF of material requires a minimum of three (3) samples; between
1,000 SF and 5,000 SF of material requires a minimum of five (5) samples, over 5,000 SF of material requires
a minimum of seven (7) samples, at least one (1) sample of each patch.
2. Thermal System Insulation (M. each homogeneous area requires a minimum of three (3) samples; at least
one (1) sample must be collected from each patch, and collect enough samples sufficient to adequately assess
the material and determine the asbestos content for TSI fittings such as pipe elbows or T's.
3. Miscellaneous Materials (MM. A each homogeneous area requires a minimum of two (2) samples; or enough
samples sufficient to determine the asbestos content.
AEC collected at least the minimum number of samples from each HA necessary to meet all regulatory requirements for the
quantity of material to be disturbed. The quantities listed in this report are approximate and on-site verification of the exact
quantity of each material is required.
Condition assessments were performed by the accredited AEC inspector(s) at the time of inspection. Condition assessments
include ratings of good," damaged," and "significantly damaged" per each HA and are meant to indicate the overall
condition of each material.
Accumulation of powder, dust or debris similar in appearance to the suspect material on surfaces beneath the material can
be used as confirmatory evidence when determining the material condition assessments.
A material in Good "(G) condition has:
- A surface with no visible damage or deterioration; or showing only very limited damage or
deterioration.
A material in Rama LD) condition has:
- A surface which is crumbling, blistered, water -stained, gouged, marred, or otherwise abraded
over less than one-tenth of the surface if the damage is evenly distributed or less than twenty
five percent of the surface if the damage is localized.

All suspect materials were divided into friable or non -friable categories at time of sampling as conducted by hand touch and
feel when performed by the inspector(s) during the on-site inspection.
• Friable CD ACM is a material that contains more than one percent asbestos by weight, area, or volume; and
that can be crumbled, pulverized, or reduced to powder by hand pressure when dry.
• Non -Friable (NM ACM is a material that cannot be crumbled, pulverized, or reduced to powder by hand
pressure when dry. A NF material may become F if its condition had deteriorated or if it had been impacted
by forces that have rendered it F.
After classifying each F ACM(s), the AEC inspector(s) gives each HA material a hazard rating (HR). There are seven (7)

The certified asbestos inspector(s) must provide a written assessment of the condition of all known or assumed ACM. These
assessments are to include specifying: the types of damage, the severity of damage, the extent of the spread of the damage
over each material, the material's potential for disturbance, the known or suspected causes of the damage, any preventive
measures that may eliminate the likelihood of the damaged material from becoming a significantly damaged material, and
whether the material is accessible; all over the IIA.
If an ACM is SD and greater than CDPHE trigger levels (i.e. commercial 160 square feet (160 SIJ), 260 linear feet (260 LF),
or the equivalent of a 55 -gallon drum) (i.e. residential 32 SE, 50 LF., equivalent of a 55 -gallon drum) then the area(s) are
deemed a major spill. If the materials are less than the trigger levels but greater than 3 SF or 3 LF, then the area(s) are
deemed a minor spill. See the spill response sections as outlined below (Section 10.0).
6.0 SITE DESCRIPTION AND PROJECT DETAILS
AEC research indicates that the building was constructed in 1978. The building is a single-family residential dwelling.
Destructive sampling was conducted on the materials planned to be disturbed at the above referenced property.
The asbestos survey was conducted on September 26, 2018 by Colorado Department of Public Health and Environment
(CDPHE) and Asbestos Hazard Emergency Response Act (AHERA) certified asbestos building inspector Mr. Seth
Kisselman (Certification Number 21329).
One (1) homogeneous area(s) with suspect ACM was sampled in three (3) location(s) at the job site.
Samples were submitted to Aerobiology and Associates (Aerobiology) laboratory for Polarized Light Microscopy (PLM)
analysis to identify asbestos content. The samples were requested on a priority turnaround time for results per the client.
7.0 LABORATORY ANALYSIS AND RESULTS
Bulk samples collected were analyzed utilizing the EPA's Method for the Determination of Asbestos in Bulk Building
Materials (EPA 600/R/116, July, 1993) and the McCrone Research Institute's The Asbestos Particle Atlas as methods
references. Analysis of the bulk samples was performed on the "date reported," as listed in the bulk -sample analysis report.
Laboratory results received on September 27, 2018 document that zero (0) sampled material(s) submitted con e as s

&0 CONCLUSIONS
AEC observed one (1) homogeneous area(s) that would be disturbed during the planned project as outlined above. The suspect
asbestos containing building material(s) was sampled in three (3) location(s) at the job site per the above details and scope
of work as outlined by the client/situation.
Based on the site observations and, bulk sample PLV. laboratory analysis results collected by AEC, zero (0) suspect
material(s) contained asbestos; therefore, removal of these area(s) will not require asbestos abatement.

The CDPIIE requires permitting for any removal activities in a commercial or residential building that contains greater
than the trigger levels (i.e. commercial 160 SF, 260 LF, equivalent of a 55 -gallon drum) (Le. residential 32 SF, 50 LE,
equivalent of a 55 -gallon drum); however, OSIIA trigger levels are 10 SF or 25 LE Colorado Regulation No. 8 requires a
project design in with the amount of ACM to be abated exceeds 1,000 LF on pipes, or 3,000 SF on other surfaces.
AEC is able to develop a project design for this project for an additional fee.
10 RECOMMENDATIONS
AEC recommends that when ACM is removed/abated, that only a licensed asbestos abatement contractor using personnel
trained in the handling of ACM be allowed to conduct such activities using appropriate methods (IIEPA-filtered vacuuming,
wet cleaning methods, respiratory protection, protective clothing, personnel decontamination, negative -air enclosures, air
monitoring, etc.).
For all spills, AEC recommends that all soft goods (carpet, pad, clothes, couches, etc.) and any ceiling and/or wall surfacing
restoration activities be removed from the interior of the building as asbestos waste. AEC also advises that only hard surfaces
(plastic, metal, wood, concrete, etc.) are allowed to be left in place before the final visual and final air clearance tests are
completed. AEC urges that electronics and appliances (refrigerators, air conditioners, DVD players, microwaves, toasters,
etc.) are removed due to the mechanical openings and working parts that make it difficult to clean thoroughly.
10.0 ASBESTOS SPILL RESPONSES

Additional asbestos air or dust sampling should also be conducted within the remaining areas not directly impacted by the
Major Asbestos Spill to determine if asbestos -containing dust/debris has spread to adjacent areas. If asbestos fibers are found
within any other areas or on building contents, they should be included in the scope of professional abatement and
decontamination.
The following response actions must be followed per Colorado Reg. 8 when a Major Asbestos Spill occurs:
■ Restrict access to the area and post warning signs to prevent entry to the area by persons other than those necessary to
respond to the incident'.
■ Shut off or temporarily modify the air handling system to prevent the distribution of asbestos fibers to other areas.
■ Immediately contact the Division by telephone, submit a notification in compliance with subsection HI.E.
(Notifications) and, if in an area of public access, apply for a permit in accordance with subsection III.G. (Permits).
• Be exempted from the requirements to have a certified Supervisor on-site at all times, until such time as the immediate
danger has passed. Any cleanup or asbestos abatement that must occur after the immediate danger has passed shall be
supervised by a person certified by the Division.
■ Using certified Supervisors and certified Workers in accordance with section IL (Certification Requirements) of this
Regulation, seal all openings between the contaminated and uncontaminated areas and establish none -detect air pressure
within the contaminated area in accordance with paragraph M.J. (Air Cleaning and None -detect Pressure
Requirements). This is to be accomplished using polyethylene sheeting to cover areas such as doorways, windows, elevator
openings, corridor entrances, grills, drains, grates, diffusers and skylights.
■ HEPA vacuum or steam clean all carpets, drapes, upholstery, and other non -clothing fabrics in the contaminated area,
or discard these materials.
■ Launder or discard contaminated clothing in accordance with subsection M.R. (Waste Handling).
■ HEPA vacuum or wet clean all surfaces in the contaminated area.
■ Discard all materials in accordance with subsection IILR. (Waste Handling).
■ Following completion of subparagraph M.T.l.a. through M.T.l.i. above, comply with air monitoring requirements as
described in subsection M.P. (Clearing Abatement Projects); air samples shall be collected aggressively as described in
40 C.F.R. Part 763, Appendix A to Subpart E (EPA 1995), except that the air stream of the leaf blower shall not be
directed at any friable ACM that remains in the area and

regulations (29 CFR 1926.1101 and 40 CFR 763); the National Emissions Standards for Hazardous Air Pollutants
(NESHAP, 40 CFR 61 Subpart M); the U.S. Environmental Protection Agency (EPA) Guidance for Controlling Asbestos -
Containing Materials in Buildings (`Purple Book", EPA 560 / 5-85-04); and the Asbestos Hazard Emergency Response Act
(AHERA, 40 CFR 763 Subpart E).

Class I asbestos work — defined by OSHA as activities involving the removal of Asbestos Containing Materials which are
TSI or surfacing material. Class H asbestos work — defined by OSHA as activities involving the removal of ACM which is
not TSI or SM. Class HI asbestos work — defined by OSHA as repair and maintenance operations, where ACM TSI or SM
is likely to be disturbed.
OSHA requires the following (at a minimum, the contractor should thoroughly review regulation):
1. Establishment of a regulated work area: minimizes access to unauthorized personnel, protects persons outside from
airborne asbestos exposure and requires the use of respirators inside of the regulated area (essentially a HEPA
filtered negative air pressure containment area).
2. Supervision of the removal work by a competent person an EPA trained asbestos abatement supervisor.
3. Workers handling ACM shall be properly trained: training requirements as stipulated in OSHA regulations specify
a minimum of eight hours (8 hr.) training of required topics. Material and hands-on training more closely fits the
EPA Operations and Maintenance Worker sixteen -hour (16 hr.) class.
4. Exposure monitoring of personnel: both eight-hour (8 hr.) TWA (time weighted average) and STEL (short term
exposure limit). This monitoring will ensure that personnel are wearing adequate respiratory protection.
5. Proper use of engineering controls: namely, wet methods to control airborne asbestos filters, prompt clean up and
disposal of debris in leak tight containers, removal of material in an intact state (prohibited use of high speed
abrasive disc saws without NEPA filtered exhaust, prohibited use of compressed air on AC_Ms, prohibited by dry
sweeping, shoveling or other dry clean-up methods).
6. Removal of AMC in an intact state to the extent feasible.
7. If using cutting machines, they shall be continuously misted: during use, unless a competent person determines that
misting decreases worker safety. Dust generated by a power cutter shall be collected by a HEPA vacuum.
8. Asbestos containing material from a roof must be either hand carried: or lowered to the ground by crane, hoist or
in a dust -tight chute.
9. All waste shall be either wrapped an"r placed into a closed receptacle: in such manner so as to preclude the
dispersion of dust
10. If working on a root roof level heating and ventilation air intake sources shall be isolated (sealed) or the ventilation
system shut down.
Other Requirements
■ Other appropriate PPE as necessary
■ Implementation of a Respiratory Protection Program (if necessary)
■ Respirator physical exam for personnel (if issued negative pressure respirator)
AEC Recommends a final visual and final air clearance monitoring after the material has been removed.
12.0 COLORADO DEMOLITION ACl'1YITIES
Section HI.A.3 — Any asbestos -containing material that is friable or will be made friable during itionwti 'ties in
any area of public access or non-public access area must be removed prior to demolition. Remo 1, i �ccor'6a with
Regulation No. 8, is required if the amount of asbestos -containing material that is friable or wi 01n /fable ring
demolition exceeds the trigger levels.
Per CDPHE Demolition Permit: Demolition of a building that has non -friable asbestos -containing maining
must be completed without causing the asbestos -containing materials to become friable. Burning a building with any

asbestos -containing materials is prohibited. Concrete floors covered with floor tile shall be removed in as large sections as
possible. Operations such as crushing, pneumatic jacking, etc. of materials containing asbestos are not permitted.
Recycling of materials, such as concrete or wood, that are bonded or contaminated with asbestos -containing materials such
as floor tile or mastic, is not permitted.
