Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if (/•
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

INSURANCE AGREEMENT. The client authorizes Endeavor Exteriors to represent their best interest in pursuit of full replacement costs for damages believed
to be covered under homeowner's insurance and allows Endeavor Exteriors to deal directly with the insurance company regarding this claim. This contract does not obligate
the client to Endeavor Exteriors until the claim is approved by the insurance company, and accepted by Endeavor Exteriors. Upon approval of claim. Endeavor Exteriors
will complete all specified work at a price agreeable to the insurance company (RCV) and Endeavor Exteriors with no additional cost to the client, except for the deductible

1. If the client plans to use the proceeds of a property and casualty insurance policy pursuant to part 1 of
article 4 of title 10, C.R.S., to pay foi- the roofing work, pursuant to section 6-2-105, the rooting contractor
cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part of any insurance deductible
applicable to the insurance claim for payment for work on the covered residential property.
2, All Contracts subject to approval by Endeavor Exteriors financial department.
3. Contract price is valid for thirty (30) days.
-1. Endeavor Exteriors reserves the right to perform specified work in accordance with generally accepted
construction guidelines, while adhering to local codes in effect at the time of contract.
5. All checks must be made payable to: Endeavor Exteriors.
h. All excess materials remain the property of Endeavor Exteriors.
Client is responsible for Homeowners Association approval of products and colors.
;. Client acknowledges Endeavor Exteriors as a general contractor, as such Endeavor Exteriors will be
entitled to ten percent (10%) overhead, and ten percent (10%) profit, (O&P) if approved by the insurance
company, if this project is part of a claim.
9. An account is defined as past due when 10 days have elapsed from the date of completion of work. Past
due accounts will earn interest at a rate of one and one-half percent (1.5%) per month or eighteen percent
(15(1/o) annually. If Endeavor Exteriors is required to turn the matter over to an attorney for collection, the
client also agrees to pay all costs incurred, including attorney's fees.
10, Should default of payment occur on this contract, Endeavor Exteriors reserves the right to place a
mechanic's lien on the clients' property.
11. Endeavor Exteriors shall not be held liable for failure of performance due to labor controversies, strike,
fire, hail, weather, inability to obtain material from usual source, or any other circumstance beyond the
control of the company.
12. Our standard workmanship warranty for roofing is non -transferable limited lifetime, and one (1) year for all
other trades, and the company will provide a service call for roof inspection, as long as the client owns the
property.
13. Endeavor Exteriors is not responsible for any leaks or water damage below the roofline caused by
excessive wind, ice or hail during the period of the warranty.
14. Endeavor Exteriors shall refund any deposit if this contract is canceled within 72 hours (3 days).
I.S. If this contract is canceled by the client, after three (3) or more days of execution, the client agrees to pay
Endeavor Exteriors twenty percent (20%) of the contract price, set by the insurance company, only if
materials have been delivered or work has been performed, as liquidated damages. 'I his payment is not a
penalty, but is fair and just compensation for services rendered to that point.
16. "The homeowner may cancel this contract within 72 hours if the insurer has denied, in whole or part, the
homeowners claim that relates to this contract.
17, While we make every effort to ensure there are limited consequences of construction, Endeavor Exteriors
will not be held responsible for slight scratching or denting of gutters, droplets of oil in the driveway,
cracks in concrete, or damage to plants and trees. Furthermore, Endeavor Exteriors will not be held
responsible for exterior damages such as deteriorated trim pieces, crown molding, stucco, solar panels,
evaporative coolers, existing skylights, or interior damage such as falling dust and debris, cracked plaster,
sheet rock or nail pops, as a result of normal re -roofing.
18. Replacement or repair of the deteriorated decking, fascia boards, swamp coolers, ventilators, chimneys,
counter flashing, or any other work completed for the homeowner not explicitly stated in this contract,
are not included in quoted price and will be charged as extra on a time and material basis.
19. Endeavor Exteriors holds the right to photograph all projects in which it is involved, and the right to
publish throughout all Endeavor Exteriors' social media outlets.

I am writing to request an extension on permit number 080495, which was issued on May
20, 2008 for a fencing project in our yard at 3500 Hoyt St. in Wheat Ridge last year.
We intend to continue that project this year, extending the fence to enclose the portion of
our yard that our driveway runs through. I understand that, if you are able to grant this
extension, that we will be able to use our existing permit and hence avoid charges for an
additional permit.

comments were left on the letter; Can only extend to 5120/2009 without fee otherwise half of the permit fees are

Colorado, with improvements thereon known as tbe Wheat Ridge High School ("Lessor's Proper.y"); md
WHEREAS, Lessees are the owners of residential property I"caled at 3500 Hoyt Slrut, 'Vheat
Ridge, Colorado ("Lessees' Properly") whieb is adjacent to a portion of the northern bOlmdary Iir,e of
Lessor's Property; and
WBEREAS, Lessor has constructed a chain link fence on Le.ssor's. Property that runs ger.eral1/
parallel to the northern boundary of Lessor's Property at 0 distance approximslely Iwenti-seven (n) f",1 to
the south of the northern boundary line of LCBsor's Property; and
WHEREAS, the area between the chain liDk fence on Lessor's Properly and Ibe portion of U,e
northern boundary line of Lessor's Property that is adjacent to the sontbern bound.ry line uf Lessees'

NOW, THEREFORE, in consideration of Ihe foregoing recitals and the mutual covcntlnts

3. TERM.
3.1 The initialtenn of this Agreement .hall be for a period of ten (10) years
commencing with the effective date hereof. If at the expiration of the initial or any rMewal tenn the
Lessee. are not in default of this Agreement and neither Ihe Lessor nor the Lessees bave elected to
terminate this Agreement as provided in this Section, the tenn shall be automatically renewed for .nother
ten (10) year tenn,
3.2 If the Lessees fail 10 maintain the Leased Proporty in good condition aDd repair as
approved p~rsuant to this Agreement and suoh failure to so maintain lhe Leased Proper!}' materially
adversely affeo" the Lessor's Property (excluding tho Leased Property) and Le.."". fail to commence the
appropriate maintenance or repair of lbe Leosed Propelty WId/at Lessor's Property to repair such tuteriel
adverse effect and prevent further occurrences within thirty (30) days after roceipt of written notice from
the Lessor speoifying U,e required maintenance or repair and thereafter to diligently continuo and complete
such mninlenoocc or repair, or if such mainfenance or repair caonot be commenced within thirty (30) days,
Lessees fail to undertake as soon as practicable after receipt of liuch written notice the necessa.ry acHon to
commence tho required maintenance or repair or thereafter fail to diligently coutlnue and complete such
maintenance or repair. Lessor may tenninatc "this Agrooment by giving written notice thereof to L'~ssee.;.
3.3 Notwit!ultanding anything herein to the contrary, Lessor may tel minawIllis
Agreement upon thirty (30) days' advance written notice to Lessees if(a) Lessor decides to sell the Leased
Property, eilher as a separate parcel or as part of LeSSOI"S Properly, (b) Lessor decides thaI it needs to uso
the Leased Property in furtherance of its purposes and objectives, or (c) the Board of Education or Les:;rr
decides as a matter of policy that none of the real property owned by the Les.or shall be leased OIliccn..,d
to adjacent property owners who are non-governmental parties unles~ such lease or license is specifically
approved by the Board of Education,
3.4 Lessees may terminate this Agreement at any time by giving written notk:.e tho ~Qf
10 Lessor. Such termination shaU be effective upon the date stated in the notice or if a termina.tion datf: is
not slaled, then upon the date of Lessor's receipt of the notice.
3.5 Upon termination or expiration of this Agreement. either party lDay prepare, sign,
acknowledge, and record W1 acknowledgment of termina.tion or expiration. Suoh document upon
recordation in the Teal estate records of Jefferson Counly, Colorado shall serve 3!:1 conclusive e\lidenGc v:-
such termin"lion or e.pitalion and may be relied upon by all third parties.
4. USE AND OCCUPANCY.
4.1 Le",ees shall have exclusive use of the Lea.ed Property provided Uta! suoh uoz. is

4.8 Lessor and its directors, officers, employee., and agents sball b., permitted tn

nnreasonably withheld, condiHoned, or delayed,
5. EQUIPMENT AND OTHER PERSONAL PROPERTY. All eqnipment and othe,

6. UTILITIES. Lessees shall pay all gas, electricity, water, and any other utihty charg'" 0'

destroyed or damaged by fire or any other casualty.
8. INDEMNIFICATION.
8.1 Lessees shall indemnify and hold Lessor and its dlreelors, offic""s, employees.
and .gents harmless from all liability, claims, dem.nd., losses, damages. or expenses (including reason.ble
attorneys' fees) resulting fTom or related to any injury, los&, or damage, including, without limitation,
claims arising from bodily injury, personal injury, sickness, death, disease, property loss or damage., OT amy

9.1 Lessees shall procure and maintain the minimum insurance coverage listed in Ulis

Colorado and which maintaiD at least aD A- rating in Best's Insurance Report. All policies shall be issued
as prima!)' policies and Lessees shall provide thirly (30) days' written notico to Lessor bofore cancellation
ofanypolicy, Lessees shall pwvide Lessor with a certificate of insurance for each policy required unde:

for renewal or new policies shall be provided to Lessor not less than ten (10) days before expiration of the

9.2 Lessees :snail procure and maintain homeowners' insurance with personrlliabiJity
coverage and, if desired. a general personalliabilily umbrella insurance policy provided that the "ggregt te
liability limit of such insurance coverage is not less than $1,000,000 and insures against allliabiUty of
