1, This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180, days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and��procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all requ�ired
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, anviolation of any provision of any
�pplicabje code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof

mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

agreement unless the damage is 'acknowledged' by the insurance company and accepted by CRE. By signing, the
property owner requests that CRE pursue the property owners best interests for a roof / gutter replacement at an
'agreeable price' to the insurance company and CRE, and with no cost (•) to the property owner other than their
legally -required deductible. Once it's determined, the 'agreeable price' shall become the 'final contract price' of this
agreement, and the property owner authorizes CRE to obtain the materials and labor required, according to the

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same,._time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

EXECUTIVE SUMMARY
On December 10, 2010, Mr. Steve Shurtliff with DS Consulting, Inc. (DSC) conducted a final visual
inspection, performed aggressive clearance air monitoring, and analyzed clearance air - quality samples
via Phase Contrast Microscopy (PCM) for an asbestos abatement project at the residence located at 3680
Hoyt Ct, Wheat Ridge, Colorado.
Services were provided at the request of Mr. Scott Tash with ALM Environmental Services in support of
asbestos abatement activities being conducted to remove +/- 350ft of asbestos - containing drywall from
a modified full containment encompassing the lower level laundry room and the connecting storage
room. Monitoring locations and frequencies were determined by Mr. Steve Shurtliff, having EPA and
State of Colorado accreditation No. 15413. PCM analysis was also performed by Mr. Shurtliff (see
Appendix B for certification). The homeowner of the residence chose to opt -out of having the single -
family residence considered an area of public access.
DSC's scope of work for this project included performing a final visual inspection and collection and

PROJECT INTRODUCTION
On December 10, 2010, Mr. Steve Shurtliff with DS Consulting, Inc. (DSC) conducted a final visual
inspection, performed aggressive clearance air monitoring, and analyzed clearance air - quality samples
via Phase Contrast Microscopy (PCM) for an asbestos abatement project at the residence located at 3680
Hoyt Ct, Wheat Ridge, Colorado.
Services were provided at the request of Mr. Scott Tash with ALM Environmental Services in support of
asbestos abatement activities being conducted to remove +/- 350ft of asbestos - containing drywall from
a modified full containment encompassing the lower level laundry room and the connecting storage
room. Monitoring locations and frequencies were determined by Mr. Steve Shurtliff,, having EPA and
State of Colorado accreditation No. 15413. PCM analysis was also performed by Mr. Shurtliff (see
Appendix B for certification). The homeowner of the residence chose to opt -out of having the single -
family residence considered an area of public access.
SCOPE OF WORK
DSC's scope of work for this project included performing a final visual inspection and collection and
analysis of aggressive final air quality samples by NIOSH 582- trained personnel for determination of
airborne asbestos (as total fibers) concentrations in identified areas. DSC is a participant in the Industrial
Hygiene Proficiency Analytical Testing (IHPAT) program with laboratory ID No. 188987 and has a rating
of Proficient. The abatement project passed the final visual inspection and the analytical results of all
samples collected were below the clearance criteria of 0.01 fibers /cubic centimeter (f /cc) (seeAppendixA
for Air Sample Analysis).
AIR MONITORING
Air - quality samples were collected utilizing a modified NIOSH 7400 Method, whereby samples were
collected on 25- millimeter (mm) mixed - cellulose, ester - membrane filters with 0.45- micron pore size and
analyzed via Phase Contrast Microscopy (PCM) as per the customer's requirements.
Sample Collection - PCM samples, when applicable, were collected on 25- millimeter (mm) mixed -
cellulose, ester - membrane filters with 0.45- micron pore size with an effective collection area of 385 mm.
All filters were pre - assembled by the manufacturer in three stages, conductive sampling cassettes with
extension cowls.
Air samples were collected at an average flow rate 12.5 liters per minute (L /m) using Thomas high-
volume pumps with a minimum of 1,200 liters being collected. Flow rates were collected at the
beginning and at the end of the sampling period utilizing an airflow rotameter calibrated against a
primary -flow calibration instrument. Start times and stop times were recorded for all sampling periods.
A field blank and lab blank were also collected and analyzed along with the five (5) clearance samples.
PCM Sample Analysis Method - PCM samples were analyzed according to a modified NIOSH 7400
Method —A Counting Rules— although samples were collected on 25 -mm mixed - cellulose, ester -
membrane filters with 0.45- micron pore size and analyzed via PCM. Air sample results contained in this
report have been calculated with blank sample corrections.
