Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

1. This permit was issued based on the information provided in the permit application and accompanying polplans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, icies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalppermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issua granting of a permit shall not be construed to be a permit for, or an approval of, an violation of any provision of any
applicable c o an 'ordi r re It f n of this jurisdiction. Approval of work is subject to field inspection.

This application is exclusively for new permits for residential roofs and for licensed c
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR c�
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if 5

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Dear Ms, Williams,
I am in receipt of your letter dated July 29,1997, I will address each item in the order you have listed them, First.
you addressed this letter to Thomas M, Sloan and Terrey Young, You purchased your home from Wycolff Construction of
which I am the President. Ms, Young on the other hand, has no connection with Wycolff Construction. other than she is
my Realtor and should not be addressed in your complaint.
Cracked Driveway
As I have said all along. I will not replace a driveway which has a hairline crack. I said when we first noticed it
that I would keep my eye on it to see if there were any further movement. It has reach the point where some of it needs to
be replaced, I have had my concrete sub out to inspect and measure and he will replace it at his earliest possible
convenience, Remember, this is the busiest time of the year for construction and since you already have a driveway, I
have given him a wide berth as to when he replaces it. If it were to grow larger, it would be of no concern to me as it is to
be replaced,
Cracked Sidewalk
It is the nature of concrete to crack, hence the comment, " Concrete- it' s not all it' cracked up to be," The
sidewalk is not on your property and therefore of no concern to you,
Cracked Patio
The rear patio will be repaired and not replaced when the concrete sub addresses the driveway,
Holes in Garage floor
The holes in your garage floor are similar to the holes in my driveway and almost every driveway, They are
caused by road salts used in the winter. They are not my responsibility,
Inside Cupboards Sagging
You complained about this in a different shelf earlier, My punchlist carpenter repaired this b-f installing the
center support brackets that were missing, I checked the new shelf in question and found the support brackets in place, I
also found the shelf jammed full of canned goods, They are not made to support that kind of weight. Please redistribute
your groceries, This is not my responsibility,
Front Gutter not installed Properly
As I explained to your son earlier this week, the gutters are installed correctly, But the amount of roof vs, the
amount of gutter creates a volume of water that the gutters cannot handle, He suggested an additional downspout and I
agreed to install that and check with my gutter installer for another alternative, My gutter sub has suggested a 6" gutter
and a 4"x 6" downspout vs, the existing 5" gutter and 2" x 3" downspout. This will be changed and painted,
Leak under Kitchen Sink
The City ofWheatridge requires an air gap in the plumbing under the sink, This air gap is where the leak

Wheatridge, This inspection is important not only Wheatridge but to the City of Arvada as well, It is the number one
reason final plumbing inspections fail. I know you had trouble with your dishwasher and it appears your repairman did
not re-install the plumbing correctly, It is not my responsibility, If I'm not mistaken your son is a plumber. I'm not saying
it's his responsibility to repair other peoples work, but he has taken the time to meet me a couple of times regarding this
punch list, yet he has not made an effort to repair the leak and has let it get to the point of damaging other items. If I were
a plumber I'm sure I would have corrected this for my mother by now, and I'm sure she would have insisted I do,
Front Door Drags
I have planed this door twice, Wood expands and contracts with the changing temperature, If I agree to plane it
again, am I to be responsible when air gaps appear in the winter? I will plane it again if you please, but then will take no
further responsibility for the door and will need your signature to that effect if I am to do any work on that door,
Flooded Basement
I met with your son, the soils engineer, and an inspector from the City of Wheatridge last Wednesday, There was
evidence that the leak happened because you did not have your gutter extensions down, There was erosion below the
gutter and next to the window well, mud in the window tracks, mud on the concrete below the window, and mud on the
floor in front of the window, There was no other water in the basement other than what came through the wiudow, That
was my suspicion all along and it was confirmed by both the engineer and the city inspector, The city inspector also
noticed that the window was not in it's proper location. There are two grooves for the window latch and the window was
in the wrong groove thereby creating a gap between the window and frame, The inspector sumized that it was removed to
get your long lengths of building materials in the basement for the basement finish you were doing without a permit.
I have wasted many hours at your home lIying to be attentive, You complained I did not install the telephone or
cable outlet in the second bedroom for which you paid extra, Not only was the cable outlet installed, but your mothers
heart monitor was plugged into the telephone receptacle, You complained the towel bar by the shower was damaged It
was bent more that an inch because someone tried to use it as a grab bar instead off or it's intended purpose, You
complained that you did not get a final touch-up by the painter, I told you that you did and you were standing in the house
while it was being done, And your response was "Yes, But that was before we moved in." I sent the painter in at my
expense to try an appease you, even though I had given you a touch up kit that included both interior and exterior paint. a
tube of caulk, and a paint brush, Then after six months you complain that a bush has died, You might have tried watering
it or keeping your dog off it.
Your letter dated June 19,1997 states that Jody Rheinhart and Ritchy's also have complaints and your complaint
to Channel Nine News said everyone of my buyers have a complaint. I have met with the rest of the buyers and they told
me you tried to enlist there help against me, but they all said they told you they have no prublems with me, Please stop
lying,
As a home owner you have a responsibility to maintain your home, Home ownership is obviously not for
everyone and I am not your maintenance man,
