Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding AM -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€T"s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Additional Provisions
1. Contract must be approved by Reliable Restoration.
2. Reliable Restoration provides a limited warranty for the labor
involved in performing the obligations hereto, as provided in the
attached "Roofing Labor Guarantee." Reliable Restoration makes
no other warranties and specifically disclaims all other warranties
that could be applicable to said labor.
3. Reliable Restoration shall have no liability for fire, wind, rain or
other peril beyond our control during the performance of the
specified work.
4. Reliable Restoration shall have no liability for hidden risks such
as, but not limited to, puncturing or damaging utilities, personal
property or building components below the rook deck during or
after completion of the work.
5. Reliable Restoration shall not be liable for failure of
performance due to labor controversies , strikes, fire, weather,
inability to obtain materials from usual sources or any
circumstances beyond our control.
6. Reliable Restoration shall not be responsible for slight
scratching and denting of gutters, oil droplets in driveways ,
hairlines cracks in concrete, broken branches or flowers on
plants and shrubbery.
7. Replacement of deteriorated decking, fascia boards, roof jacks,
pipe ventilators, flashing or other material unless otherwise stated
in this contract are not included and will be charged as an extra,
on a time and materials basis.
8. Reliable Restoration is not responsible for incidental interior
damage resulting from a roof leak during the warranty period. The
workmanship warranty does not cover damming due to ice, debris
or leakage at the chimney above the flashing.
9. Any alteration or additional work outside the scope of insurance
coverage required by applicable code will be charged on a time
and materials basis and payment for such is due upon completion.
10. In the event that Reliable Restoration presents this agreement as
a bid, the bid amount and terms will expire thirty (30) days from
the date of the bid proposal unless extended in writing by
Reliable Restoration.
11. Property owner may rescind the contract for roofing services and
obtain a full refund of any deposit within 72 hours after entering
into the contract.
12. The property owner may rescind a contract for services, the
payment for which will be made from the proceeds of a property
insurance claim, within 72 hours if they receive notice from their
insurer that the claim is denied in whole or in part.
13. Any cancellation of this contract outside of the three (3) day right
to recision will entitle Reliable Restoration to collect a 15%
cancellation fee and, if applicable, a 20% materials restocking
charge as liquidated damages, as the damages to Reliable
Restoration are uncertain in amount and the parties hereto
acknowledge the reasonableness of said sums.
14. Payments for services are due as specified. There is no provi-
sion, unless otherwise stated herein, for the holding back of funds
pending any inspection (city, county or otherwise) or warranty
issues.
15. Reliable Restoration will provide the property owners with copies
of any documentation for any warranties that may be provided
by the manufacturer of materials. Reliable Restoration makes no
warranty as to materials.
16. If any provisions of this agreement should be held as invalid or
unenforceable, the validity and enforceability of the remaining
provisions of this agreement shall not be affected.
17. The workmanship warranty is transferable only after inspection
and written approval by Reliable Restoration.
18. Property owner is to furnish electricity.
19. All surplus materials belong to Reliable Restoration.
20. No changes, alterations, modifications, amendments, additions or
qualifications to the terms of this agreement shall be made or be
binding unless made in writing and signed by each of the parties
to this agreement.
21. Ice damming is a natural phenomenon that occurs when water,
snow, ice or any combination thereof collects as a result of
continued freezing and thawing. The property owner understands
that ice damming can cause the collection of water, the retention
of water and water leakage through the roofing materials. The
property owner agrees that it is his/her responsibility to keep the
roof clear of water, ice and snow or any combination thereof that
may cause ice damming.
The property owner acknowledges and agrees that Reliable
Restoration is not liable for any ice damming that occurs or any
damage related to or as a result of ice damming. Maintenance of
the roof likewise includes keeping the roof free of debris that aid in
the formation of ice damming.
The property owner understands and agrees that failure to
remediate the causes of ice damming, if such actually results in
damage, shall be a violation which voids any and all guarantees
and warranties, including, but not limited to, the labor guarantee.
The property owner further understands and agrees that it is
the property owner's sole responsibility to monitor and control the
moisture content of his/her house and also understands and
agrees that under no circumstances shall Reliable Restoration
have liability to the property owner or any other person as result of
the formation, existence, discovery, treatment or remediation of
mold on the property, whatever the source of moisture and
wherever the located in or on the property.
22. The state of Colorado implies that warranties for General Con-
tracting are one (1) year for workmanship and materials, two (2)
years for mechanical equipment and ten (10) years for major
construction defects.
23. Reliable Restoration will not be held responsible for siding
damage caused by changing of wall flashing as this is necessary
for a quality roof. Minor damage may occur due to vibration or
movement of your home during installation of your new roof which
