Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturergems installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

'KAPE Roofing & Gutters Inc reserves the right to file for supplemental insurance claims
when necessary, which shall become a part of this agreement. KAPE Roofing & Gutters
Inc, as the contractor, is entitled to the full O&P provided by the insurance company.
Building Owner (s) (Customer) hereby retains Kape Roofing & Gutters, Inc. (KAPE) to repair/replace the roof and/or gutters upon the above stated property, contingent upon
KAPE' s receipt of Customer's insurance company (Carrier) approval and payment of said work. Customer understands and agrees that KAPE Roofing & Gutters, Inc. will
review said work proposal with Carrier's representative. Work will commence only after KAPE and carrier agree upon the extent of damages and the cost of repairs and
replacements. This agreement is for full scope of insurance proceeds not excluding deductible, depreciation, overhead, and profit, therefore CUSTOMER'S OUT OF
POCKET EXPENSE WILL BE CUSTOMER'S INSURANCE POLICY DEDUCTIBLE AND ANY NON RECOVERABLE DEPRECIATION for work agreed upon
between Carrier and KAPE. AS PER CO LAW S1338, KAPE CANNOT PAY WANE OR REBATE THE DEDUCTIBLE IN PART OR IN WHOLE. Customer agrees to
provide all necessary documentation for the claim, and any documentation necessary to facilitate payment from the Carrier or Mortgage Company.
Carrier only pays for installed materials. All surplus material is property of KAPE. Replacement of deteriorated siding, decking, fascia board, ventilators, flashing, or other
materials is not included and will be charged as extra unless otherwise stated above.
Customer may be required to supply all electricity necessary for the work. Lien waiver submitted upon final payments. KAPE is not responsible for interior personal items .
All work is to be completed in a workmanlike manner according to standard practices. The Customer hereby acknowledges that KAPE may be subject to delays due to
inclement weather or material shortages, which are each beyond the control of KAPE and therefore accepts any delays that may occur by one or all of these circumstances
upon completion of said work. Customer grants access to the property as is necessary for the setup and execution of work. Any changes from the above specif icatio:,a
involving extra costs will be executed only upon written order and will become and extra charge over and above the estimate.
WARRANTY TERMS: KAPE Roofing & Gutters, Inc., agrees to repair any defect in workmanship for a period of two (2) years following work completion. KAPE' s
liability is limited to its cost to affect any warranty repairs. Warranty does not cover damage to roofs caused by lightning, gale (>50mph), hurricane, tornado, hailstorm, or
other violent storm, impact of foreign objects or damage to roofs due to settlement, distortion, failure or cracking of the roof deck, walls, or foundation of building. This
WARRANTY is null and void if payment, or any other terms of the contract have not been met. THIS WARRANTY IS NON -TRANSFERABLE. All statements on reverse
side apply to contract.

This contract and any agreement made pursuant thereto is between KAPE Roofing hereinafter referred to as the "Co." or "Company' and the
customer(s) named herein on the reverse side will be subject to all appropriate laws, regulations, and ordinances of the states of CO, KS, NE and the
terms and conditions:
I .UNDER THE RIGHT TO RESCIND, YOU HAVE UNTIL MIDNIGHT OF THE THIRD BUSINESS DAY TO CANCEL THE WORK TO BE PERFORMED, DAY ONE
BEGINS AFTER THIS CONTRACT IS SIGNED. If this contract is signed under insurance porceeds, the homeowner has the right to cancel this contract within 72 hours
after receiving notice from their insurer that the claim is denied in part or in whole. Any monies paid by the homeowner will be refunded in full within 72 hours.
2. SHOULD DEFAULT BE MADE IN PAYMENT OF THIS CONTRACT, CHARGES SHALL BE ADDED FROM THE DATE THEREOF AT A RATE OF ONE AND ONE
HALF (1 112) PERCENT PER MONTH (18% PER ANNUM) WITH A MINIMUM CHARGE OF $2.00 PER MONTH, AND IF PLACED IN THE HAND OF AN ATTORNEY
FOR COLLECTION, ALL ATTORNEY'S FEES AND LEGAL FILING FEES SHALL BE PAID BY CUSTOMER ACCEPTING SAID CONTRACT.
3.The Company is not responsible for unseen site conditions.
4.The Company shall have no responsibility for damages from rain, fire, tomado windstorm, or other perils, as is normally contemplated to be covered by HOME
OWNERS INSURANCE or BUSINESS RISK INSURANCE, or unless a specified written agreement be made therefore prior to commencement of the work.
5.The quotation or the face hereof does not include expenses or charges for bond insurance premiums or costs beyond normal insurance coverage, and any such
additional expenses, premiums or costs shall be added to the amount of the contract.
6. Replacement of deteriorated decking, fascia board, roof jacks, ventilators, flashing or other materials unless otherwise STATED IN THIS CONTRACT, are NOT
INCLUDED and will be charged as an extra on a time and material basis.
%.The Company shall not be liable for failure of performances due to labor controversies, strikes, fires, weather, inability to obtain materials from usual sources, or any
other circumstances beyond the control of the company, whether of a similar or dissimilar nature.
8.1f roofing and sheet metal work is involved, it is understood and agreed to that our standard roof guarantee, a copy of which is available in our office, shall be
acceptable and that all terms and provisions therein shall prevail, unless specifically agreed to in writing prior to the commencement of the work.
9.The Company is not responsible for any damage on or below the roof due to leaks by excessive wind driven rain, ice, or hail during the period of the warranty.
EXCESSIVE WIND IS 50 OR GREATER M.P.H. THE WARRANTY IS NONTRANSFERABLE.
1 0.If material has to be reordered or restocked because of a cancellation by the customer there will be a RESTOCKING FEE equal to fifteen percent (15%) of the
contract price.

12. IF THIS CONTRACT IS CANCELED BY THE CUSTOMER, customer shall pay the company fifteen percent (15%) of the contract price as liquidated damages, not
as a penalty, and the company agrees to accept such as a reasonable and just compensation for said cancellation.
13.THIS CONTRACT CANNOT BE CANCELED ONCE WORK IS COMMENCED EXCEPT BY MUTUAL WRITTEN AGREEMENT OF THE PARTIES.
14.PAYMENTS ARE TO BE MADE PAYABLE TO KAPE ROOFING & GUTTERS, INC: 1/2 UPON COMMENCEMENT OF WORK AND BALANCE UPON
COMPLETION OF WORK. OTHER ARRANGEMENTS, MUST BE DISCUSSED WITH CREDIT MANAGER. Any pyment s made by the homeowner prior to the
commencent of work, will be hold in trust until the materials are delivered and the work begins.
15. If any provision of this contract should be held to be invalid or unenforceable the validity and enforceability of the remaining provisions of the contract shall not be
affected thereby.
16.ANY REPRESENTATIONS, STATEMENTS, OR OTHER COMMUNICATIONS NOT WRITTEN ON THIS CONTRACT ARE AGREED TO BE IMMATERIAL, and not
relied on by either party, and do not survived the execution of this contract.
1 %.The maximum liability for the Co. shall be the original cost of labor and materials for the repair which customer agrees shall be a liquidated sum, under any event of
default of Co. herein.
I $.During the duration of the work the customer's homeowner's insurance will be responsible for any interior damage as long as the Company has taken appropriate
action to protect the roof during the repair of the roof.
19.If there are any solar panels on the roof, the Co. will not be responsible for damage during the repair, so homeowner agrees to have solar panel company take the
appropriate action to protect it if necessary.
21.The Co. is not responsible for construction problems of your home. If pointed out and notified to our Co., we will try to assist you on correcting them on a time and
material basis.
22.THIS CONTRACT IS COMPOSED OF THIS PAGE AND THE REVERSE SIDE OF THIS PAGE AND SHALL BE CONSIDERED THE ENTIRE CONTRACT BY THE
PARTIES.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

How to Prepare for a Roofing Job & What to Expect During the Construction Process
1. Be Prepared for the noise. There will be constant hammering for two or more days, depending on
IF e size of your
roof.

4. Remove item' from round the exterior that could be damaged by falling shingles or debris.
5. Be aware, leaves or blooms may be knocked off the plants near your home during our work. We do
our best to protect your plants, but many times the layout of the landscape prevents us from doing so
6. During the roofing project, dust may fall from the rafters and undersides of the decking. Any clothing
or fabric items that could be damaged should be protected.
7. We will need access to your driveway to load shingles, and other equipment and to clean up
afterwards. Please park all cars, motorcycles, boats etc. away from the driveway.
8. Occasionally, we will use power tools to cut decking or shingles. When we do, please check your
power breakers to make sure they are operating correctly and have not been tripped. Nobody wants to
deal with the freezer the garage unknowingly losing power 9
9. Check heater vents upon completion of your roof to make sure they have not been loosened in the
attic.Also check the hot water heater and stove vents. It is highly recommended that all home, have
working CO2 detector.
10. When we remove two or more layers of the roof, your roof line will drop. This will be
evident along any wall. The change in appearance will be minimal and will not affect the
integrity and utility of the roof and/or flashing. However, if the siding is tarred, it may be
damaged and HIS Colorado is not responsible for unavoidable siding damage that may take
place
11. When you, old roof is removed, a tremendous amount of weight is taken off the frame
of your home. This shifting can sometimes cause small stress cracks in your ceiling along
with paint popping off of the nails heads. This is an unavoidable occurrence which EIS
Colorado Inc. is not responsible for.
12. Roof mounted satellites will need to be detached and reset. We will do aur best re -align
the satellite, but we highly recommend scheduling an appointment with your satellite
provider for the day following repair, in the event we are unable to property re -align It.
13. In rare occurrences, upon the installation of a new roof, a previously existing skylight

Windows
8 Replacement Windows (10): Remove existing 10 steel windows of the first floor of the house
except the garage (include large front picture to be replaced w/ new fixed center-2 operable side
lite windows). Prepare openings and existing sills. Provide and install new replacement windows,
pre-finished, vinyl, min. 2 1/8" frame, double glazed, low-e glass, residential windows with screens
(manufacturers: Cascade, Custom Gard II, 6500 Series, Xact, or a rehab specialist approved
equal-submit specs to the rehab specialist). New windows shall have the required U-factor (0.30)
to qualify the Owner for a rebate, if eligible. New windows shall have releases to lift out for easy
cleaning. For windows w/ existing muntins, new windows shall have grids to match. Bath windows
shall have obscure glass.
All windows are to be installed according to the manufacturer's recommendations; insulated and
caulked around the openings, adjusted and lubricated. Owner to select color from stock colors
(up-charge for color other than white-bid white). Note: The bedroom windows shall meet egress
