Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
8806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

replacement. Leaks caused be re-using existing skylights are excluded due to the process necessary and disturbance that could cause the failing of existing skylights that may not

Lead: For houses built prior to 1978 there is a high probability lead may have been used to paint the exterior. We are required to use certain techniques to ensure the lead is
correctly removed when the amount disturbed exceeds twenty square feet. Typically the amount of paint disturbed is negligible if just the roof is being replaced but almost always
the case if gutters or siding are being removed. Tiley Roofing is a lead certified renovation firm allowing us to perform this procedure. Prior to contracting please let us know if your
house was built prior to 1978. If the house is pre-1978 we will perform a lead test. If the test is positive for lead we will assess a fee In addition to the above costs. Generally this fee
ranges from $500 to $1000.
Insurance Claims: Owner is responsible to verity insurance settlement payment is sufficient to cover contract amount.

I would like to request an extension on permit number 080699 at 4380 Dover St.
Per discussions with the chief building official upon my re-application for this permit I
was told I could ask for a 180 day extension.
To date I have had two inspections on the new permit and anticipate others shortly. I
currently have two other carpenters working with with me to complete the shingle siding on
the exterior and completion of this will make the exterior of the house more presentable.

regret to inform that your request cannot be granted.
The original permit was issued on May 4, 2000. A Stop Work order for permit expiration was
posted on November 3, 2006. At your request, an extension of the permit for 180 days was
granted on November 15, 2006. On February 9, 2007, a second Stop Work order was posted for
permit expiration. On May 5, 2007, a second extension for 180 days was granted, at half the
original permit cost. On November 15, 2007, the second extension of the permit expired. On June
10,2008, a third Stop Work order was posted.
As of the date of this letter, in excess of eight years has passed since the issuance of the original
permit. In order to continue the work on your project, you will be required to submit an
application and construction documents necessary to obtain a new permit for the project. The new

1 have enclosed product specification sheets for "Icynene" brand spray insulation.
I have also included some of their non-technical information sheets as well.
1 propose using a sprayed in ceiling & wall insulation system in the remodel of
my house @ the above address for the following:
. The house has a 'stick' framed cathedral ceiling, which has inherent
venting problems.
. The use of a sprayed product will provide for increased air infiltration
resistance over standard fiberglass insulation! air barrier (exterior)! poly
vapor barrier (interior) wall system. Although, 1 will still probably use an
exterior house wrap of some sort.

14/3 and 12/2 residential wiring (max.

Tech University has confirmed that
lcynene@is not a source of food for
mold; and as an air barrier, Icynene@
reduces the airborne introduction of
moisture, food, and mold spores into the
building envelope. It has no food value
for insects or rodents.
Environmental! Health! Safety
Icynene" contains no formaldehyde CFC's,
HCFC's, HFA's or volatile organic
compounds. It has been thoroughly
evaluated for in-situ emissions by
industry and government experts. After
10 days there are no detectable
emmisions.
Limitations
Not intended for exterior use. Not to be
installed within 2" [50 mm) of heat
emitting devices, where the temperature
is in excess of 200"F [93"C).
5. INSTALLATIONS
The lcynene Insulation System@ is
installed by a network of Licensed
Dealers, trained in the installation of
Icynene". Installation is generally
independent of environmental conditions.
It can be installed in hot, humid or
freezing conditions. Surface preparation is
generally not necessary. Within minutes,
the foaming process is complete.

Builders of Icynene@ insulated homes,
eliminate most air/vapor retarder
installation and moisture problems.
Icynene provides an effective, continuous
air barrier to both outside humid air in
cooling climates and inside humid air in
heating climates. No building wrap or
additional vapor protection is required
except in the most severe northern
climates or situations where there is a
continuous, year round vapor drive in one
direction, such as encountered in indoor
swimming pool enclosures. In such
situations, a vapor retarder paint is
recommended.
Icynene@ is also the perfect solution for
sealing cavities at joist ends, as these
areas have a high potential for air
leakage.
Because of its' low air permeance,
Icynene@ is not infiltrated by
moisture-laden air. When applied to a
vapor permeable surface, condensation
will not occur within it.

AIR INFILTRATION
According to studies, random air leakage
can account for as much as 40% of total
energy loss. It is not surprising that more
emphasis is now being placed on air
leakage control, where payback on
investment is far higher than the payback
from increasing R-values, (gaps in
fiberglass insulation as small as 3%,
have been found to reduce performance
as much as 35%).
Icynene's@ extremely low air
permeance and its' perfect fit into any
shaped cavity and adhesion to other
building components, minimizes air
leakage and convection within the cavity,
resulting in a higher field performance
than can be expected from conventional
insulation material of equal R-value.

THERMAL PERFORMANCE MYTH
There is a widespread belief that all
insulation materials of equal R-value will
perform equally.
THERMAL PERFORMANCE FACT
While insulations of equal R-value
perform equally in the controlled
conditions of a laboratory, they do not all
maintain that R-value in the walls and
ceilings of a building. Some give off
gasses to the atmosphere and suffer a
decline in efficiency, while the
performance of others are vulnerable to
the installers' shortcomings.
"Thermal Drift" as this phenomenon is
politely called, results from the failure of
materials to fill all cavities completely,
thus leaving air pockets which permit air
movement inside the cavity. Further "drift"
occurs with some materials if the
insulation is not fully protected against air
infiltration, both from the interior and the
exterior. Commonly used insulation may
well perform at a fraction of its' nominal
rating depending on installation, how well
it fits the cavity and how well it's protected
from air infiltration.
The performance of The Icynene
Insulation System" does not "drift." It is
not dependent on the installer, or on other

where Maximum Indoor Air QualityTM is preferred.
SCIENTIFIC BASIS FOR CERTIFICATION
The Envirodesic ™ certification file for The Icynene Insulation System@ is based primarily on tests conducted by the Saskatchewan Research
Council, Saskatoon, Saskatchewan, Canada, described in SRC Report No. 1-4801-9-C-94, May 1994. These tests identified volatile organic
compounds emitted by the insulation, using gas chromatographJmass selective detector analysis under Procedure B of the eGSB Standard
CAN/CGSB.Sl.23.92. Specimens were taken from a test panel after a 24-hour cure period, and offgassing measurements were made after 30
minutes in a 0.33 litre headspace chamber and for 1 hr. and 2 hr. durations at periodic intervals (1 hr., 12 hrs., 1 day, 2 days, 4 days, 7 days, 14
days, and 30 days) in a 51.2 litre dynamic test chamber.
The tests identified specific offgassing products in the headspace test immediately following the 24-hour cure, and tracked the reductions in
concentrations of these and various additional offgassing products in the dynamic chamber tests over time. The results simulate the indoor air
concentrations of volatile organic compounds which may result in a typical 500m3 house with a ventilation rate of 0.3 acth, given full exposure of
the insulation to the indoor air.
In the dynamic chamber tests, several series of offgassing products were observed, all of which declined in concentration to 0.05 mg/m3 or less
within 14 days, and to less than the detection limit of 0.003 mg/m3 within 30 days. Both these levels are well within the Molhave limit of 0.16
mg/m3 of total volatile organics being used presently as the upper limit of the benign range for the general population by the Envirodesic ™
Certification Program.
ADDITIONAL SUBJECTIVE INFORMATION REGARDING ENVIRONMENTAL HYPERSENSITIVITY
Preliminary feedback from subjective testing of partially and fully offgassed insulation samples by select researchers and other individuals
knowledgable about the effects of extremely low chemical emissions on environmentally hypersensitive individuals indicates that The Icynene
Insulation System@ may be suitable for use in installations designed for environmentally hypersensitive individuals.
CONCLUSIONS REGARDING EMISSION LEVELS
The volatile organic compound measurements confirm that emissions from The 1cynene Insulation System@ are well within a range considered
benign for human exposure, within a two-week airing period. In addition, after a 30-day airing period, The Icynene Insulation System@ is
considered likely to be suitable for persons who may be more hypersensitive to chemical exposures than the general population. In cases of
severe chemical sensitivity, individual testing of the individual's sensitivity against aged insulation samples is considered advisable to confirm the
suitability of The Icynene Insulation System@ and the air-out time required under these circumstances.
SUITABILITY FOR NEW CONSTRUCTION
The creation of a tight air barrier is extremely important in low-indoor-pollution construction, to ensure control over intake and exhaust air.
Combining its low offgassing characteristic after the air-out period with its efficient air-tightening properties, The Icynene Insulation System@ is
considered highly suitable for new building construction where Maximum Indoor Air QualityTM is preferred, even for buildings to be occupied by
individuals with moderate to severe chemical hypersensitivity. A 30-day period of ventilation following installation is advised to provide full
airing. This is usually well within common construction timetables.
SUITABILITY FOR RENOVATION
The Icynene Insulation System@ is also considered suitable for retrofit situations where Maximum Indoor Air QualityTM is preferred, and where
environmentally hypersensitive occupants are present. For retrofit situations, an airing period of two weeks is advised for the general public, with
any modification of the airing time depending on the tightness of the air barrier. The time may need to be increased to any occupants who
may be environmentally hypersensitive.

Re: EXISTING FOUNDATION CAPACITY.
Dear Mr. Talmadge:
Prior to the structural design for the proposed addition to your home, the Architect, David
Forman and myself observed the foundation of your home from the crawl space.
It appears that the existing footings are approximately 24" wide. The poured in place
concrete foundation walls appear to be in good condition and there are no signs of any
excessive vertical movement of the existing foundation.
On May 19th, 2000, I received a copy of the Soils Report by Colorado Soil Engineering,
Inc., report No. 00-0713. For the design of the foundation, Complete Structures Co.
assumed a maximum allowable soil bearing pressure at 1,500 PSF. The Soils Report
indicates that a maximum soil bearing pressure of 2,000 PSF may be used.
Following a review of the plans for the proposed addition, considering the existing
structure and the new soils information as provided, it is our opinion that the
existing foundation and structure have adequate capacity to support the proposed
addition.
It is still required that Complete Structures Co provides reinforcing and framing
observations as called for in the Structural General Notes. The contractor shall also
comply with all requirements as noted in the soils report.

the suitable design and construction of an appropriate foundation for the proposed building

straight shaft piers (caissons) drilled a minimum length of sixteen feet and a minimum
penetration into the claystone of six feet or to refusal. These drilled piers should be designed for
a maximum end bearing pressure of25,000 psf, a side shear of2,500 psffor the portion of the
pier in firm claystone and a minimum dead load of 8,000 psf or as high as possible based on pier
end area. The bottom six feet of the holes shall be artificially roughened with a minimum I W'
auger tooth in order to anchor the pier into the claystone soil. A minimum void or clear space
of four inches shall be provided under the concrete grade beams and between the drilled piers
to allow for soil expansion. If water is encountered during the drilling of the piers, casing will
be required and Type II cement shall be used. The drilled pier holes should be thoroughly
cleaned of all loose soil and filled with a low slump concrete immediately after drilling to
prevent infiltration of water or loose soil. The concrete piers shall be reinforced for their full
length with one number five steel rebar for every sixteen inches of pier perimeter, (a minimum
of three number five steel rebars). The reinforcing steel shall extend into the foundation wall a

minimum of two feet. The active case equivalent fluid pressure for this soil is 35 pcf. Iffloor
movement due to expansive soil i8 not acceptable, finished room floor areas should be supported
on wooden or steel joists over a crawl space area.
2. Steel Helical Piers: An alternate type of foundation could be steel helical steel piers. These
piers should be designed for a maximum end bearing pressure of20,000 pounds and a minimum
length of fifteen feet. These piers should be installed by a certified steel helical pier installer
familiar with soil conditions on this site. Piers shall be galvanized and be a minimum of I ;1,"
square steel section with a minimum eight inch diameter helix.
3. Continuous Footings: An open hole inspection is required at this site, If the existing
structure bears on the natural 80il, this addition may bear on the undisturbed natural soil (i.e.
footings, wall on grade). The foundation shall be designed for a maximum soil bearing pressure
of2,000 psffor all depths of the excavated area. The active case equivalent fluid pressure is 35
pcf. All loose and disturbed soil shall be removed before placing of the concrete for the footings.
The bottom of the footings shall be a minimum of three feet below final grade for frost
protection and below any organic material. Settlement at this site may be up to one inch, but will
most likely be less than one-half inch. Soil expansion at this site may be as much as one inch
in some areas, but will probably be less than one-half of this amount.
In general, this test hole consists of clayey sand to the six foot depth and sandy claystone
below the six foot depth. This test hole was drilled to a depth of twenty feet. Free water was
encountered at the eighteen foot depth during the drilling operation.
SITE CONDITIONS:
At the present time there is an existing residence on this site. It is our understanding that
a residential addition is planned for construction.
FIELD AND LABORATORY INVESTIGATION:
One exploratory test hole was drilled on May 9, 2000 at the location on the site as shown
in Figure 1. This test hole was drilled with a four inch diameter power auger.
At specific intervals, the drilling tools were removed from the test hole and soil samples

were obtained with a two inch inside diameter spoon sampling tube. The depths at which soil
sample8 were taken are shown on the Logs of Test Holes, Figure 2.
All soil samples were carefully inspected and classified in the field and laboratory. The
samples were subjected to visual inspection and laboratory testing in order to evaluate the
physical and mechanical properties of the materials encountered. The natural moisture content
and the natural dry density was obtained from relatively undisturbed drive samples of typical
materials encountered. Swell-Consolidation tests were also performed on typical samples of soil,
see Figure 3. These tests indicate the behavior of the soil upon various loading and wetted
conditions.
DESIGN AND CONSTRUCTION DETAILS SLAB ON GRADE CONSTRUCTION:
The natural soils may not be suitable for the support of interior and exterior concrete
slabs. When these soils have a change in the moisture content and become wetted, they may
consolidate in volume and shrink, thus lowering and cracking the concrete slabs, or they may
expand, thus heaving and crack concrete slabs. The builder and future owners should be aware
of and understand that there is a definite risk of future damage with any slab on grade
construction.
If the builder and future owners are willing to accept the definite risk of some damage
due to concrete floor slab movement, the following recommended details should be carefully
followed during construction of the interior floor slabs. Experience with similar soil conditions
has shown that these details will help prevent wetting of the soils under the slabs and will
minimize damage when wetting of the soil does occur.
1. Do not place a gravel layer beneath the concrete slab, unless specified by the foundation
engineer. The slab should be placed directly on the undisturbed natural soils, or well
compacted fill soil.
2. Separate the slab from all load bearing members and utility lines of at least one-half inch
to allow for independent movement of the slab. Provide a positive control or slip joint
at the construction joint between the slab and foundation walls.
3. Provide frequent scoring of the slabs to provide control joints for possible cracking of the
slab. Control joints should be placed to provide approximate slab areas of 150 square
feet, with a maximum dimension between joints of twelve feet.

4. A minimum void or clear space of three inches should be provided at or near the bottom
of all non-bearing partitions and furring strips placed over the concrete slabs, see Figure
4. In finished room areas, all drywall and paneling should be stopped approximately three
inches above the top surface of the slab. This will allow some space for upward
movement of the slab before pressures are applied to the wall and the upper levels of the
structure. One should provide a one-half inch space at the bottom of all door jambs to
allow for limited movement of the floor slab. The owner is hereby notified that it is their
responsibility to maintain these void spaces.
5. In the event a hot water heating system is used, the piping should not be placed beneath
the concrete floor slab. In the event a forced air furnace is used, a two inch collapsible
connection should be provided between the furnace and the heat ducts.
6. The soils that will support the concrete slabs should be kept moist during construction
by occasional sprinkling of water and especially a day or so prior to pouring of the slab.
This procedure will help maintain the moisture content of the underlying soil.
7. In the event the builder or future owner desires to provide decorative gravel or bark
around the addition, we have attached Figure 5 to illustrate one acceptable method of
installation. The method will prevent ponding of water near the foundation and provide
for proper drainage away from the addition.
SURFACE DRAINAGE:
The backfill soil around the addition should be moistened and well compacted to prevent
future settling. The backfill material should be free of trash, frozen soil and large rocks. The
future owner should be advised to fill in any settled area to eliminate ponding of water near the
addition and to provide adequate slope for proper drainage away from the addition and off the
site at all times.
The finished grade or ground surface near the foundation walls should have adequate
slope to provide proper drainage away from the addition and off of the site. A minimum slope
often percent for at least the first five feet out from the addition is recommended.
Roof downspouts and sill cocks should discharge onto long concrete splashblocks,
minimum of five feet long, or into metal gutter extensions to deposit runoff water further out
from the addition and beyond the limits of backfill soil near the foundation walls.
The future owners should be cautioned and advised against the installation of a lawn

sprinkler system within five feet of the foundation walls. If a sprinkler system is installed, the
sprinkler heads should be placed so that water spray from the heads under full pressure does not
fall within five feet of the foundation walls. Lawn irrigation should be controlled to prevent
excess wetting of the subsurface soils. Lawn, flowers, shrubs, and other plantings within five
feet of the foundation walls should be hand watered and this watering should be minimized. If
a drip irrigation system is used, one should limit the amount of water to sustain the plantings.
One is also advised that any irrigation lines can leak and/or break and release excessive amounts
of water near foundations and can cause damage to slabs and foundation walls.
SUBSURFACE DRAINAGE:
Free water was encountered during the drilling of the test hole. The requirement for
subsurface (perimeter) foundation drains will be determined at the time of the open hole
observation. See Figures 6, 7 & 8 for a suggested method of installing the foundation drainage
system. Ground water should not affect construction or be a deterrent to the construction of the
addition.
REINFORCING:
The concrete foundation walls should be well reinforced at the top and bottom of the
walls to clear span at least a distance often feet. This will give the walls, beam strength, to span
or bridge over any soft or loose pockets of soil not found in our exploratory hole or that may
develop during construction. This will also help prevent differential movements of the
foundation system. Refer to the foundation plan for specific details of reinforcement. The
foundation plan is not a part of this soils report, and exact requirements are a function of the
design of the structure.
TOPOGRAPHY:
The topography of the site at which the test hole was drilled is relatively flat.
WEATHER:
The weather at the time of the soil examination consisted of sunny skies with warm
temperatures.

CONSTRUCTION DETAILS:
In any soil investigation it is necessary to assume that the subsurface soil conditions do
not vary greatly from the conditions encountered in our field and laboratory testing. Our
experience has been that at times soil conditions do change and variations do occur and may
become apparent to the time of excavation for the foundation system. If soil conditions are
encountered which appear different from the test borings as presented in this report, it is
required that this office be called to inspect the open excavation. This inspection is not a part
of this report and is not included as any part thereof.
OPEN HOLE OBSERVATION:
The owner, or a representative of the construction firm, shall contact Colorado Soil and
Engineering, Inc., a minimum of twenty-four hours prior to the completion of the foundation
excavation to schedule an open hole observation. This observation must be performed before
the placement of concrete, and preferably to the setting of the forms. This observation will
insure soft spots, anomalies, etc., are not present in locations which will affect the foundation.
At the time of the observation, the requirement for subsurface drainage will be
established. Additionally, the maximum allowable bearing pressure of the soil may be revised
according to the soil conditions exposed.

Regulations and Building Code of Wheat RIdge, Colorado or any other applicable ordinances of. the City.
This permit shall expire if (A) the work authorized is not commenced within SIXty (60) days from Issue date or (8) the building authorized is suspended or
abandoned for a period of 120 days.
If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes have been or will be made in the
original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit
No work of any manner shall be done that wilt change the natura! flow of water causing a drainage problem.
Contractor shall notify the BUilding Inspector twenty~four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceediing With successive phases of the job.
