1) Please provide the added valuation — the valuation on the original permit application (which was adjusted by City of
Wheat Ridge) was only for a 75 ft sewer repair. You can either provide a new project value or submit a copy of the
contract showing the cost of the work being done at 3893 Field Drive.
2)As I stated below, the entire scope of work, including the electrical will be on one permit.
We have many emails going back and forth regarding this project and we need to move forward with getting this
corrected by the contractor and the revision issued.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

to City Hall to submit an application in person. Permits are processed and issued in

B & M Roofing proposes to do the following work at the above referenced property:
SURFACE PREPARATION
Remove and dispose of one layer of existing roofing material.
• Inspect plywood roof deck and re -secure any loose sheets.
• Replace rotten/water damaged plywood after approval by Owner. If this work is required, a separate time
and material price of $60.00 per sheet will be added.
UNDERLAYMENT
• Install one course of ice & water Sheild membrane at eaves.
• Install one layer of 30 -pound felt underlayment over entire roof surface.
• Install Tarco valley liner in valley areas, closed style.
FLASHINGS
• Install new 2"x 4" prefmished drip edge flashing at lower eave edge, and rake edges of roof.
• Install new step flashings along sidewall areas, as necessary.
• Install and paint new vent flashings on pipe penetrations.
• Install and paint new attic vents to facilitate proper attic/roof ventilation.
• All flashings to be 26 gauge prefmished, unless otherwise noted.
ROOFING SYSTEM
• Install new OWENS CORNING STORM IR CLASS 4 ROOFING SYSTEM,
as manufactured by OWENS CORNING, over underlayment, including all starter, hip and ridge.
• Color and manufacturer will be chosen by Owner, prior to material order by B&M Roofing.
AND MISCELLANEOUS
• Debris caused by the roofing operation will be properly disposed of by B&M Roofing.
• Liability insurance is carried by our firm from Continental Western Insurance for your protection.
• Workmen's compensation insurance is carried by our firm on all of our employees also.

Dear Mr. Ponder:
I have reviewed your plans to remove the existing fireplace at the above referenced
property, to create a doorway into the existing covered porch area. As drawn in your
plans, a header will be required to carry the load of the roof rafters and ceiling joists to
both sides of the doorway. The doorway opening is to be 6' -0" wide. It is my
understanding that the existing walls are of double wythe brick masonry.
HEADER SPECIFICATION:
Use a double l-3/4"X7-1/4" MicroLam (E=1.9) beam. The beam may bear directly onto
the outer wythe of the brick masonry wall. The "pony wall" above the beam should have
1/2" plywood sheathing attached to one side with 8d nails at 3" o.c. maximum, and the
plywood should lap down over the beam by 4" and be nailed to the beam similar to the
pony wall.
No modifications or reinforcements to the foundation walls or footings are considered
necessary.
