Installation of roof sheathing (new or overlay) requires an approved inspection prior to installation of ANY roof coverings and is require on the entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is required. Eave and rake metal is required. Roof ventilation is required to comply with applicable codes and/or manufacturer installation instructions, whichever is more stringent. The purpose of these visits was to observe and evaluate the foundation repairs being completed at the noted residence. Recommendations for repairs were provided by this office in a letter dated November 21, 2005: Re Project Number 05-00080-001. From our site observations and a review of the pier log provided by the contractor (see attached) it appears that the foundation repairs have been completed in substantial compliance with the recommendations provided in the above referenced letter. The purpose of this visit was to observe and evaluate distresses noted by you, and to provide recommendations to address the cause ofthe observed distresses. From information provided on site, the original portion of the subject home was constructed in the early 1950's and an addition was constructed approximately 5 years ago, The original portion of the subject home appears to he constructed with CMU exterior walls and wood framed interior walls. The exterior walls are clad in brick and wood veneers. The exterior walls of the original home appear to be supported on CMU foundation walls and concrete spread footings. The floor in the original portion of the home is a slab on grade. The addition is a wood framed structure, clad with wood veneer, supported on concrete foundation walls and spread footings. For purposes of this report the home is assumed to face south. Site Observations: During our visit we noted the following (partial list of observations): I. Sheetrock crocks in the ceiling of the living room, north of the fire place. 2, Crocks were noted in the north wall of the living room, particularly in the vicinity of the ceiling crack noted in observation (I) above, 3. Distresses were observed around the fire place - you noted that these distresses are new in the last 5 years. 4. Slab diselevations were noted throughout the living and dining rooms. 5. Cracks were noted around the windows in the bedroom at the northeast corner of the home. 6. Minor distresses were noted around the doors to the northeast and east center bedrooms, 7. Distresses were noted around the windows in the master bedroom. It is our understanding that these distresses have not changed significantly in recent years, 8. You reported that lawn sprinklers against the foundation at the front of the home have been removed and we noted that a small swale has been constructed at the east portion of the front yard to help direct water away from the structure. 9. The grading at the east side and rear of the home is positive (away from the home), although we noted that the flower bed at the northeast corner of the home is lower that the grass around it 10. A sheetrock crack was observed in the ceiling of the laundry room. 12. Slab cracks were noted in the slab of the laundry room. 13. It was noted by you that a leaking sewer pipe was recently repaired beneath the kitchen. Conclusions I Recommendations: From a review of the above noted information, we have concluded the following: I. It appears that the northeast comer of the home has experienced differential settlement related to consolidation of the soils underlying the home. From our observations of the site it appears that the foundation system may have been placed on a relatively small amount of fill at this comer of the home, 2. We recommend that the existing foundation system be stabilized as shown in the attached notes, plans and details, 3. Once helical piers have been installed, an attempt should be made to relevel the northeast comer of the foundation system in order to reduce the severity of distresses at the exterior and interior of the home. The contractor shall adjust the elevation of the foundation at all piers simultaneously; if required 1/8" adjustments may be made one pier at a time but the contractor shall move from pier to pier making adjustments so that the foundation system is raised as unitormly as possible. During the relevel process, the contractor shall make observations of the finish material at the interior and exterior of the home. The relevel process should cease if new distresses are realized. This office should be contacted to observe the relevel process, please provide at least 24 hours notice. 4. It appears that the slab throughout the original portion of the home has experienced minor vertical movements. The majority of this movement appears to be related to settlement of the underlying soils although some of the movement may also be related to minor heave and shrinkage of active soils which likely underlie the home. With the leaking sewer pipe being repaired and the grading improvements being completed at the front of the home it is possible that the interior slab movement may subside as the water content of the soil beneath the home adjusts in relation to the repairs and improvements. We recommend that the slab and interior walls be monitored (by you) for further movement for a period of one year, If an unacceptable amount of movement continues after a period of one year, remedial repairs to address the slab movement may be required. 5. Although not a required portion of the structural repair, we recommend that the ground at the northeast comer of the home be regraded to direct water away from the foundation (i.e. the t10wer bed at this comer of the home should be raised so that it is higher that the grass surrounding it).
