Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

herein and that I have the right and power to enter into this agreement. I authorize All Phase Restoration (the "Company") to provide services to the
real property, personal property, and contents of the property described as Incarnation resident owners association (the "Property"). I also hereby
hire and authorize the Company to provide the services (the "Work") that I have initialed below and accept responsibility to the Company for such
services rendered:

for payment for all services rendered and any costs the Company incurs as a result of this decision. I further agree to make arrangements for
completion of the job without cost or penalty to the Company and to release, defend, and hold the Company harmless as a result of my decision.
In addition, in the absence of intentional wrongful acts or wrongful failures to act, I waive the right to bring suit against the Company, its owners,
agents, and employees and agree to release them from all liability, indemnify and hold them harmless, and agree to defend them if suit is brought by
any interested party.
Unless we expressly agree otherwise, I agree that myQinsurance deductible or0deposit in the amount of $95'000.00 is due prior to the
Company commencing emergency mitigation services and that the balance is due upon completion of the Work, or any portion thereof. And, unless
we expressly agree otherwise, if the Work is not covered by my insurance company, I agree that 30% of the estimated cost of reconstruction is due
prior to the Company commencing such work, 30% is due upon the completion of 30% of the Work, 30% is due upon the completion of 60% of the
Work, and the balance is due upon the completion of the Work.
I agree that all rights to any insurance proceeds paid in connection with the Work performed by the Company are hereby assigned to the Company. I
hereby direct any insurance company to include the name "All Phase Restoration" on any checks or drafts issued in connection with the Work
performed by the Company and that such checks shall be mailed directly to the Company at the above address. In the event that an insurance
company includes other parties on checks or drafts, I agree to endorse and/or obtain necessary endorsements within three days of receipt of the
payment. I fully understand that I am responsible for all charges not paid by my insurance carrier or other materially interested party and agree to
pay any unpaid balance immediately upon demand or upon completion of the work.

UNDERSTANDING BY THE CUSTOMER
Prices: I understand that water damage is a progressive condition and that drying times vary so it is impractical to give an
exact quote for services before completion.
Completion of Work: The Company will diligently pursue substantial completion of the Work (the time at which the affected
area may be used for its intended purpose) but shall not be held liable for delays due to late deliveries, weather,
restricted access to the Property or any other conditions beyond the Company's direct control.
Effect of Abnormal Water: I understand that uncontrolled water indoors is abnormal and may cause several problems that
should be addressed: (1) Water may migrate into other areas of the Property so it is highly recommended that all
potentially affected areas be evaluated; (2) Materials swell as they absorb moisture and rapid drying will help reduce
permanent damage; and (3) High humidity from evaporation can damage materials if allowed to continue and these
problems get worse the longer materials remain wet.
Reducing microbe growth: I understand that the following steps can be taken to reduce microbe growth: (1) Water can be
removed as quickly and as thoroughly as possible; (2) Rapid air -movement can increase the rate of evaporation; (3) The
relative humidity indoors can be reduced; and (4) If necessary, antimicrobials may be applied.
Drying Equipment: I understand that drying equipment will be installed. I will not interfere with this equipment, move it, turn
it off, or leave the windows open without first notifying the Company. I will also minimize my entry into affected areas
and will use extreme care when doing so. I have been informed that it may take several days to complete the drying
process. I understand that I am responsible for loss or theft of drying equipment while in my care and will take necessary
precautions to secure and protect this equipment for service rendered.
Carpet cleaning and Other Repairs: I understand that the Company does not schedule carpet cleaning or other- repairs
until drying is complete.
Hazardous Substances: I agree to inform the Company, prior to the commencement of work, of any known hazardous
substances or unsafe conditions within the Property. I understand that the Company, upon or after encountering such
conditions, may terminate this Agreement and I agree to pay for services to date, including costs incurred related to the
discovery, testing and disposal of such hazardous or unsafe substances. I agree to indemnify and hold the Company
harmless from any and all claims by any person arising from the existence of and/or exposure to hazardous substances
or unsafe conditions.

"I FLOOR PLAN REQUIREMENTS:
1. Submit two (2) complete sets of required information, including plans and equipment specifications.
2. Provide two (2) copies of a floor plan, drawn to scale, a minimum of I I" x 17", with room dimensions
and use indicated. For basements that are currently partially finished, provide two (2) copies each of
floor plan showing current condition and proposed new or altered condition.
3. Plans must show the layout and type of electrical outlets, lighting, smoke alarms and carbon monoxide
detectors. Smoke alarms are required to be hard -wired and interconnected in new and existing
bedrooms, halls and each level of the house unless removal of interior wall or ceiling coverings would
be required. If removal of interior wall or ceiling coverings would be required, battery operated
devices may be substituted. Carbon monoxide detectors may be hard -wired or battery powered and
are required to be installed within fifteen (15) feet of each bedroom door in the residence.
4. Plans must show the location and size of all windows and identify emergency escape and rescue
windows, including accompanying wells. Window sill height, ladder dimensions and clear space
dimensions of wells is required to be listed.
5. Plans must show plumbing isometric and fixture unit count, and location of cleanouts and fixtures,
including fixture spacing.
G. Plans must identify any modifications to the existing structural components such as walls, beams, posts
and floor joists.
7. Plans must show methods for heating, cooling and ventilation of spaces to be finished.
8. Plans must indicate any area in basement where finished ceiling height is less than 7 feet 0 inches.

Soilogic, Inc. (Soilogic) personnel have completed the residential site observations you
requested for the referenced project. The results of our observations are included with
this report.
ASoilogic field engineer observed the foundation bearing materials for the footing
foundations for 5720 and 5730 W 39th Avenude of the referenced multi -family residential
development. We understand the residence will be a two or three-story wood -frame
structure constructed over a full basement, At the time of our site observation, the
basement and garage areas had been excavated to approximate foundation bearing
elevation,
The exposed materials observed at the approximate foundation bearing level in the
basement area consisted ofnatural, moist brown lean clay with sand, One (1) hand auger
boring extended to a depth of approximately 3 feet below basement foundation bearing
level revealed similar materials extending to the depth exp)ored, Free groundwater was
not encountered to the depth explored at the time of our observation. Hand probe tests
indicated the lean clak with sand was medium stiff in consistency. The soils obka-nLis&&_
approximate foundation bearing elevation are consistent with those encountered as part of
our original subsurface exploration performed at this site and outlined in a report dated
March 18, 2013, Soilogi c Project # 134 03 3 and would be expected to exhibit no to low
swell potential.

Based on results of the completed field testing, it is our opinion the natural, medium stiff

footing foundations at the design maximum net allowable soil bearing pressure of 1,500
psf As a precaution, these footings should be sized to maintain a minimum dead -load
pressure of 500 psf on the foundation bearing soils.
Soilogic personnel observed the installed perimeter drain system and damp -proofing for
the referenced residence prior to backfilling. The perimeter drain system observed
consisted of rigid 4 -inch diameter 'Perforated pipe bedded in gravel and placed at
approximate foundation, bearing level around the interior and exterior perimeter of the
basement area of the structure. Plastic sheeting was observed to be glued to the
foundation wall extending below the perimeter drain trench and a filter fabric installed
over the free draining gravel. The perimeter drain was spot checked with a hand level
and observed to be sloped to drain and positively connected to the interior sump pit(s),
At the time of our site observations, the perimeter drain systern appealed to have been
installed in accordance with generally accepted construction practices.
The damp -proofing observed consisted of a grey spray -on coating that appeared to cover
the exterior of all basement foundation walls, At the time of our site observations, the

wall reinforcement size, and location and to confirm the vertical reinforcement spacing in
foundation forms, The reinforcement appeared to be in general compliance with the retaining
wall drawing and details by'172 Structural Engineering, project number: 14-182, dated September
3, 2014,
There are two 'uter' bars, one for each side of the duplex. The 'ufers' are located in the
intermediate basement foundation wall between the units approximately three feet into the wall
(north) from the rear (south) basement foundation wall,
