Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

COMMERCIAL PROPERTY RESTORATION AND REPAIR CONTRACT
This Commercial Property Restoration And Repair Contract (this "Contract") is made effective as of <
2017, between Great Roofing & Restoration, LLC; Contractor") of J09 Wy LitttonB vd. TE 290,
Littleton, CO, 80120pnd IVarnaJion Residents HOA of 5750, 5760,'5780,"5790, 5850, 5860, 58705880, 5890, 5898
W. 39th PI., & 4005, 4015, 4025, 4035 Fenton Ct., Wheat Ridge, CO, 80212 ("Owner") Contractor and Agent(s) are
collectively referred to in this Contract as the "Parties" and each individually as a "Party".
Now, therefore, in consideration of the mutual promises and covenants herein contained, and for other valuable
consideration, the receipt and sufficiency of which are hereby acknowledged, and desiring to set forth their mutual
understandings and obligations in writing, the Parties hereby acknowledge and agree as follows:
DESCRIPTION OF WORK
Contractor agrees to provide all the materials and labor required to perform the proposed work (collectively, the
"Work") described in the scope of work attached as Exhibit A (the "Scope of Work") at the property location
referenced herein (the "Jobsite Location"), which Scope of Work is expressly incorporated in and made a part of this
Contract by reference. Contractor agrees to obtain all necessary permits and to comply with all applicable state and
federal laws and regulations. Additionally we have included the insurance scope and agreed price by line item for
applicable trades.
ESTIMATE, PRICING, AND SUPPLEMENTS
Agent(s) agrees to pay the total contract price of Insurance Proceeds(Including deductible) $318,527.97. Agent(s)
is intending to make payment for such services from the proceeds of a property and casualty insurance policy.
Agent(s) further agrees and acknowledges that Contractor has the right to directly negotiate with the insurance
company to obtain approval for supplemental items that will increase the initial price estimate, but will also be paid by
the insurance company as part of the final approved claim amount. In the event that Agent(s) elects to have additional
repairs above and beyond the repairs approved by the insurance company as part of the final approved claim amount,
owner assumes personal responsibility and shall pay Contractor, at Agent(s) own expense, for the cost of such repairs.
PAYMENT SCHEDULE
Agent(s) agrees to pay Contractor the sum of actual costs, supplemental costs, and profit based off the amounts
approved by Owner's insurance company for the Work described in the Scope of Work attached as Exhibit A as
follows:
1) $107,000 payment upon delivery of materials to the job site. (1/3 of total project)
2) Payment in full for each trade as it is completed in accordance with agreed insurance pricing per trade
3) Final payment to be due less 10% at completion of the project in it's entirety.
4) The final 10% shall be paid once the punch list items have been completed to Agent(s) satisfaction
PLEASE NOTE: PAYMENT IS DUE FOR EACH TRADE UPON COMPLETION OF THAT TRADE
AND DEPRECIATION HAS BEEN RELEASED.
All checks shall be made payable to Great Roofing & Restoration, LLC. Any payments not received within 30 days
after completion of the Work described in the Scope of Work shall be charged interest at an annual rate of Seven
Percent (7.0%). Contractor reserves the right to turn any past due account over to a collection agency and Owner
agrees to pay for all collection -related costs and expenses, including reasonable attorneys' fees. Returned checks will
be charged a fee of $35.00.
PUNCH LIST ITEMS
Agent(s) agrees that all detail items ("Punch List") required to complete the Work shall be performed at the end of the
time of performance and that Agent(s) and Contractor shall cooperate in good faith to identify and communicate such
Punch List items.
TIME FOR COMPLETION
Contractor agrees that time is of the essence and that the Work under this Contract shall be substantially completed
"as soon as practicable," and in a workmanlike manner, but under no circumstance is Contractor committing to a fixed
completion date. Agent(s) understands and acknowledges that unfavorable weather or encountering unexpected issues
may cause performance delays.
WARRANTY
Contractor agrees to warrant labor for 2 (two) years and materials for the length of the Manufacturer Warranty
beginning from the date of final completion of the Work and only after full payment is received in accordance with the
Payment Schedule, provided that none of the following warranty exclusions occur: a) Agent(s) is responsible for
damaging the Work by failing to properly maintain the Work or allowing any third party to interfere with the Work, b)
Agent(s) opts out of work recommended by Contractor or c) damage occurs below the roof due to leaks caused by
excessive wind greater than 55 mph, ice damming, hail, or any other act of God at any time during the construction
process or at any time during the warranty period. In addition, per GAF, the GAF Golden Pledge warranty will cover
the shingles for an additional 25 years of workmanship and material warranty after the contractor's first 2 year
required labor warranty. The flat roofing, per Mulehide, will have coverage for both workmanship and material for a
total of 20 years.
ENTIRE CONTRACT
This Contract, together with the Scope of Work attached as Exhibit A (and any drawings or other exhibits attached
hereto or thereto), forms the entire Contract and shall constitute the only agreement between the Parties regarding the
subject matter referred to herein, and any prior understanding or representation of any kind preceding the date of this
Contract shall not be binding upon either Party except to the extent expressly incorporated in this Contract.
THIRD PARTY BENEFICIARIES
This Contract is intended to be for the exclusive benefit of the Parties and shall not be construed to create any rights or
benefits whether expressed, implied, or incidental in or for the benefit of any other person or entity.
INSURANCE
Agent(s) agrees to insure the buildings and Work against loss or damage by fire [with extended coverage
endorsement] during the progress of the work in an amount to cover the replacement cost of the buildings and Work
with any loss being made payable to the parties in accordance with their respective interests.
INDEMNITY
Owner shall indemnify, defend and hold harmless Contractor and its employees, agents and affiliates from and against
any and all suits, actions, liabilities, legal proceedings, claims, demands, losses, costs and expenses (including
attorneys' fees and expenses) arising from or related to the performance of this Contract unless caused by the
negligence of Contractor. Contractor shall conversely, indemnify, defend and hold harmless all members of the board
of the Incarnation Residences Homeowners Association and its employees, agents and affiliates from and against any
and all suits, actions, liabilities, legal proceedings, claims, demands, losses, costs and expenses (including attorneys'
fees and expenses) arising from or related to the performance of this Contract unless caused by the negligence of the
board and its members.
FORCE MAJEURE
No Party shall be liable or be deemed in default under this Contract for any delay or failure to perform caused by an
act of God, war or terrorism, disasters, strikes, shortages of materials or supplies, mechanical failure of the equipment,
computer network infrastructure, labor shortages, or any similar cause beyond the control of either Party.
DAMAGES
In no event shall Contractor be liable for any consequential, liquidated, special, indirect, punitive, or extended
damages suffered by any person. If Agent(s) fails to pay when due any sums payable to Contractor, Contractor may
suspend work upon written notice to Agent(s). If Agent(s) default continues for five (5) days after such notice,
Contractor may terminate this agreement and remove its equipment and tools from the Jobsite Location and Agent(s)
shall pay the cost of all work done up to the time of the termination and the entire profit fee of Contractor as an early
termination fee. Additionally, in the event of a termination, Agent(s) shall pay Contractor for the actual cost of any
materials that have been delivered to the Jobsite Location, but are not returnable to the supplier. In the event that any
materials that have been delivered to the Jobsite Location are returnable to the supplier, Agent(s) shall pay Contractor
a fifteen percent (15%) re -stocking fee (calculated as fifteen percent (15%) multiplied by the actual cost of the
materials).
DISPUTE RESOLUTION
If a dispute, other than a collection action for unpaid amounts owed to Contractor by Agent(s) arises out of or relating
to this Contract or the breach of this Contract, and such dispute cannot be settled through direct discussions, the
Parties agree to submit such dispute to mediation procedures before submitting such dispute to binding arbitration.
The Parties agree to share the cost of mediation and/or arbitration equally. If any action is filed in a judicial forum in
relation to this Contract, the unsuccessful Party in the action shall pay to the successful Party, in addition to all the
sums that either Party may be called on to pay, a reasonable sum for the successful Party's attorneys' fees.
GOVERNING LAW
This Contract shall be governed by, construed, and enforced in accordance with the laws of the State in which such
Work is completed without regard to principles of conflicts of law.
SEVERABILITY
If any clause or provision of this Contract is illegal, invalid, or unenforceable, then it is the intention of the Parties that
the remainder of this Contract shall not be affected thereby, and in lieu of each clause or provision of this Contract
that is illegal, invalid or unenforceable, there shall be added as a part of this Contract a clause or provision as similar
in terms to such illegal, invalid, or unenforceable clause or provision as may be possible and be legal, valid and
enforceable.
HEADINGS
The titles to the sections of this Contract are solely for the convenience of the Parties and shall not be used to explain,
modify, simplify, or aid in the interpretation of the provisions of this Contract.
COUNTERPARTS
This Contract may be executed in one or more counterparts (including by means of facsimile or other electronic
transmission), each of which shall be deemed an original, and both of which together shall be deemed one and the
same instrument.
MISCELLANEOUS
The Parties agree that any excess materials remaining after the completion of the Work shall be the property of
Contractor.
Each Party to this Contract has caused it to be executed effective as of the date first written above.
EXHIBIT A
SCOPE OF WORK
1.0 SCOPE OF WORK
Contractor shall furnish all labor, materials, and equipment required to complete the Work in accordance
with the Section 2.0 schedule of work "Schedule of Work." In addition, a project manager will be on
site at all times.
PLEASE NOTE: ALL QUANTITIES ARE APPROXIMATE. IF MORE MATERIAL/LABOR IS REQUIRED,
GREAT ROOFING WILL SUPPLEMENT FOR THE CORRECT FIGURES WITH INSURANCE.
2.0 SCHEDULE OF WORK: FLAT ROOFING: Approximately 75.42 SQ of 60mi1, Mulehide EPDM.
1. Tear off ALL layers of flat roofing all the way to the wood decking.
2. Inspect wood decking and replace any rotted decking.
3. Mechanically fasten 1/4" Densdeck to the existing deck using screws and plates.
4. Fully adhere 60mil EPDM to the Densdeck.
5. Re -flash existing penetrations and pipes.
6. Install water stop behind all walls.
7. Repair stucco over the EPDM along walls.
SCHEDULE OF WORK: SHINGLES(Pitched Roofing): Approximately 147.36 SQ of GAF, Armorshield II,
Color is TBD
S. Tear off ALL layers of roofing all the way to the wood decking.
9. Inspect wood decking and replace any rotted decking.
10. Install 2" by 4" oven baked enamel coated, aluminum drip edge around the entire perimeter on ALL
buildings to match shingles color chosen by board members.
11. Install Ice/Water Shield on all eaves and in all valleys (per Wheat Ridge code requirements).
12. Fasten a 15# felt moisture barrier on all areas not covered by ice and water shield.
13. Install an adhesive starter strip around the entire perimeter on all buildings.
14. Remove and replace existing vents.
15. Re -flash all necessary sloped, roof -to -wall penetrations with step flashing.
16. Replace all braun vents.
17. Replace all pipe flashings with a neoprene boot.
18. Paint all roofing accessories to match.
19. Install GAF, Armorshield II(Class 4 IR) shingles w/ 1 1/4" standard coil nails.
20. Install ridge cap on all hips and ridges.
21. Furnish building permit.
