to be installed in every sleeping room, in hallways outside of sleeping rooms, and on every
level of the structure.

I . This permit was issu d based on the information provided in the permit pplication and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, po plans

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermrt fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€T"s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 24-48 hours, subject to change based on volume.

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Existing permits that require amendments (for example, to add
redecking) must be completed in person at City Hall. All other non -roofing permits 2�
must be completed in person at City Hall. The Building Division will be open from /
7:30-10:30 a.m., Monday through Friday to process these types of requests.

Clean job site thoroughly removing all trash and debris.
All flashings shall be replaced or sealed as necessary including pipe jacks, step flashing, chimneys and skylights.
All roof installations shall be in accordance with required manufacturer specifications.
Upon request, customer shall be provided with written invoice which includes lien waiver and 5 year roof certification.
In addition to the manufacturer warranty, a 7 2 ,?'5 year transferable workmanship warranty is included for all installations.
It is understood that any decking which requires replacement shall be installed for an additional charge of $50 per 8x4 sheet.
In the event that there are "hidden layers" found during tear off, an additional charge of $25 per 100 sgft per layer may apply.

GENERAL
This category covers the investigation of asphalt shingles surfaced with mineral granules which are designed to
resist wind forces by affixing the shingle above to the surface of the shingle below with a sealant (factory or
field applied) applied in a pattem aligned parallel to the windward edge of the shingle. The invesrigation
process involves three steps: 1) the determination by test of a wind uplift coefficient, 2) the calculation of the
potential uplift force (Ibf) acting on the tab sealant, utilizing the wind uplift coefficient and specific wind speed
selected for the investigation and 3) the determination of resistance force (Ib fl capability of the tab sealant as
determined by mechanical test.
Wind Uplift Force Requirements
~wind uplift coefficient is determined based on air flow perpendicular to and across the shingle surface. The
'~'~wind uplift coefficient is calculated by measuring pressures above and below the shingle surface at the
windwazd and leeward sides of the sealant. The uplift force (Ib fl acting on the sealant is then calculated to
coirespond to the wind speed classificarions as tabulated below:
~ Class D - 90 mph
Class F - 110 mph
Class G - 120 mph
Class H - 150 mph
The wind uplift coefficient is determined and the corresponding uplift force (lbfl is calculated in accordance
with UL 2390, "Test Method for Wind Resistant Asphalt Shingles with Sealed Tabs."
Tab Sealant Uplift Resistance Requirements
The up1iR resistance (lbfl capability is determined by ASTM D6381, "Standard Test Method for Measurement
of Asphalt Shingle Mechanical Uplift Resistance." The uplift resistance force determined from this mechanical
test method is equal to or greater than the calculated uplift force for the coiresponding wind speed classification.
UNEVALUATED FACTORS
The effects of weathering on the wind resistance of these materials have not been investigated.

In order to be eligible for Classification under this category, asphalt shingles must also be Listed under Prepazed
Roof Covering Materials (TFWZ).
The Classifications for uplift force are derived from tests conducted in accordance with iJL2390, "Test Method
for Wind Resistant Asphalt Shingles with Sealed Tabs."

The Classification Mark of Underwriters Laboratories Inc. on the product or package of products is the only
method provided by UL to identify products manufactured under its Classification and Follow-Up Service. The
Classification Mark for these products includes the Listing Mazk for Prepared Roof Covering Materials

Fasteners
All fasteners must be driven flush with me shingie surtace and penetrate at least inio the wood deck. Where the
deck is less than thick, the fastener should be long enough to penetrate fully and extend at least 1/8" through
the raof deck.
Owens Corning recommends the use of nails as the preferred method of attaching shingles to
wood decking or other nailable substrates. If staples aze used they must be, corrosion resistant, zinc-
coated, 16-gauge minimum with minimum I S/16" crown width. Staples must be bng enough to
penehate at least into solid decking, or extend a minimum of 1/8" tluough the APA-rated sheathing.

INTRODUCTION:
This report summarizes the xesults of data obtained during a soIls test of 2875 NEWLAND STREET,
JEFFERSON COUNTY, COLORADO. Investigation occutxed on Septembet 13, 2004 and was
pezformed in undistutbed soIl in accoxdance with ASTM 1586-99. The putpose of the investigation
was to zeveal the engineexing pLOpexties for foundarion putposes fox the pxoposed consriucrion of a
tesidential addidon. The site is cutxendy occupied, and we do not know the type foundauon the
eicisting stsuchue beazs upon. Othei aspects xelating to the proposed construction aze pxesented.
A component docutnent to this plan will be an Open Hole Repoxt that will be pzoduced aftet the Open
Hole Inspection, which is not billed with this teport. The purpose of the Open Hole Report is to
detennine whether any significant variation fiom this report and the obseroed condirion exists and
whethex the foundation design will tequite modification.
FINDINGS:
The soil encountexed in test hole one consisted of tan to light brown, fine-gtained clayey sand grading
to sandy silt with medium density (Unified Soil Classification System symbol SC-ML) to the ten foot
depth; brown, fine to couse-grained sandy clay with medium consistency from the ten foot to the
twenty-four foot depth and gxay, fine-gzained clayey sandstone with very lugh density (Unified SoIl
Classification System symbol SC) at the twenty-four foot depth. Potential foz foundation movement is
moderate to high.
The soil encounteLed m test hole two consisted of bxown, fine to coazse-gxained sandy clay with
medium consistency (Unified Soil Classificarion System sytnbol CL-SC) to the eighteen foot depth.
Potential fox foundation movement is modexate to high.
Attached aze gtaphic summaries of the boring logs of the field condirions encountexed and a summacy
of laboxatory testing xesults. It should be noted that the soIl descriptions shown on the boring logs aTe
Uased on the geologist's visual classification of the samples in the field at the depths indicated. Actual
subsurface soil conditions may vary between samples and location tested.
Gioundwatex was encountered in test boring one at the twenty-foux foot depth and filled in to the
twenty-tliree foot depth iimnediately at the time of this investigation. The piesence of shallow bedrock
beneath suzficial soIls is favoxable for the fortnauon of "pexched" gxoundwatex. The depth and
occuxrence of gtoundwatet can vary ovet time depending on hydtologic condirions such as
precipitation, suxface dxainage, ixrigarion, and othex condidons not appazent at the time of this teport.

RECOMMENDATIONS:
The foundation shall beaz according to one of the following designs:
1. Helical PieLS: Due to the ptesence of modexately expansive clay and potentially collapsible silt soils
and the pxesence of gtoundwater pexched on the bedrock, the pxeferted foundauon design fot the
addition will utilize helical piexs. The specific bearing capacity of the piexs shall be designed by the
foundarion engineet, but shall be set to a minitnum of 10,000 pounds and a maximum of 25,000
pounds of vertical capacity. A ininimum void or cleax space of foux inches shall be ptovided under the
concxete gtade beams and between the piets to allow fox soil expansion. If an interior slab is planned
foz the addirion, diffexential movement is likely to occut, and a structutal wood floox is xecommended.
2. Drilled Piers and Gxade Beams: If the existing home beazs upon caissons, the addition may be
supported on gsade beams and stsaight shaft piets (caissons) drilled a mi.,imutn length of twenty-eight
feet and a minimum penetration into the sandstone of s'uc feet, ot to refusal. These drilled piets should
be designed fot a maxixnum end bearing pxessute of 25,000 psf, a side sheax of 2,500 psf fox the
portion of the pier in fum sandstone, and a mi.,;mum dead load of 5,000 psf ox as high as possible
based on pier end axea. The bottom six feet of the holes shall be artificially zoughened cvith a minimlun
1 1/2" auget tooth in ordex to anchot the pier into the sandstone. Muiiinutn pier diatnetet shall be ten
inches. A minimiun void or cleat space of foar inches shall be pxovided undet the concrete giade
beams and between the dtilled piers to allow fox soil expansion. If wateT is encountexed during the
drilling of the piers, casing will be xequiTed and Type II cement shall be used. The drilled pier holes
should be thoxoughly cleaned of all loose soil and filled with a low sluxnp concxete nnmediately aftex
drilling to ptevent infiltrarion of water ox loose soil. The concxete piexs shall be ieinforced fox their full
length with one numbex five steel tebar for every sixteen inches of pier perimetex, (a +n+*+i,,,utn of three
numbex five steel zebazs). The ieinfoxcing steel shall extend into the foundarion wall a minimum of two
feet. Equivalent fluid pxessures of the soil, assuming normal consolidarion, may be taken as 55 pcf
acrive and 210 pcf passive. These values aze based on literatuce and wexe not established by site-
specific labaratoxy sample analysis of sheaz strength. If floot movement due to expansive soil is not
acceptable, finished xoom floot azeas should be supported on wooden or steel joists ovet a cxawl space
asea. No foundarion wall shall exceed twenty-five feet in length without utilizing countecforts ox
buttxesses unless otherwise designed by the foundadon engineex.
3. Accepted Risk: If the existing home beats upon footings and the owner is willing to accept the risk
of diffetential foundation settlement up to one and a half inches within and beyond the fixst yeaz aftex
complerion of construction, the foundation design may consist of a zeinforced concxete stem wall
xesting on a concrete footex of a size deterinuied by the foundarion engineeL. Depth of foundarion
elements shall be detemiined by the foundarion engineer, but should be at least as deep as the
nvtumum depth xequited by the governing building authority. Concrete slabs shall be fxee floating and
isolated from load-bearing elements. Partition walls, utility and HVAC connections shall be capable of
a minimurn of 1-1/2 inches of movement of the foundation and slab. An attached detail sheet
diagtams a typical floating wall design.
If this design is utilized, maximum allowable bearing strength of the soil shall not be taken as
greater than 1,000 psf for elements resting on undisturbed soil. Equivalent fluid pressures of the
soil, assuming nonnal consolidarion, may be taken as 55 pcf acrive and 210 pcf passive. These values
aze based on litexatuce and were not established by site-specific laboratory sample analysis of sheac
strength.

FOUNDATION DESIGN:
This report is not a foundadon design. The foundation design engineu will detemiine the exact
configuxation of foundation elements, to include footex (if any) width and thickness, wall thickness and
height, pad and piet sizes and teinfoxcement schedules. Howevex, the foundation should be designed
fox a ten-foot fxee span.
This ieport does not addxess genetal hillside stability, landslide potential, ot othex natucal hazaxds.
Sevetal ateas of the Coloxado Ftont Range have known geologic hazazds associated with them. We
xecommend that readexs of this Teport furtheL educate themselves as to the eacistence of geologic
hazazds on ox axound theix specific pxoperty of intexest. ColoLado Engineering can assist in the
development of site-specific assessments of Geologic hazazds. The Colozado Geologic Survey
{www.dnr.state.co.us/geosutve~ or 303-866-26111 is a good soutce for publications (maps, repotts,
etc.) dealing with specific geologic issues and/ot issues telated to specific geogxaphic axeas.
FIELD INVESTIGATION PROCEDURE:
Explotatoiy borings wexe advanced using a four-inch nominal diameter, truck-mounted continuous-
flight augeL. The appxoximate boring locarions axe indicated on the attached site diagram and wexe
established in the field by pacing and measuring angles fTOm idenufiable site Lefetences. The buildei
designated the locarion fox explotation. At depths detetinuied by the superoising geologist, samples
wexe obtained by means of a two-inch diameter split-spoon samplet, advanced by a 140-pound dtop-
haininer falling a distance of 30 inches, in genexal compliance with ASTM D-1586. The numbet of
hammer blows tequized to advance the samplet ptovides an indication of the in-situ selative density of
the soil, and in combination with the laboratory analysis of the soil, pxovides data zequixed fot
derivarion of the engineering ptopezties sought by this explotadon.
EXISTING SITE CONDITIONS AND WEATHER:
The genexal topography azound the site of this investigarion is relatively flat. Vegetation consists of
grasses. Weathex was wartn and cleaz at the time of the investigation. The elevarion measuxed at 5,300
feet above sea level.
SUBSURFACE DRAINAGE:
Foundation dcain requixements shall be deteimined at the open-hole obsetvadon; howevex, drains
axound any below-grade useable space, induding cxawl spaces, should be anriupated. See the attached
detail sheet.

SURFACE DRAINAGE:
The excavation shall be protected from suxface runoff and excess precipitation during construction.
After backfilling, the soil azound the foundation shall be gcaded away from the structure at a ininimum
slope of six inches ovex ten feet. Guttexs and downspouts shall be installed to carxy watet actoss the
area distutbed by construction. ?.reas that settle shall be backfilled to pievent ponding of watex. No
vegetation requiring ixrigation shall be installed within five feet of the foundadon. Watet ftom uphill
shall be diverted axound the structute. The futute ownexs axe caudoned against the installation of a
lawn spxinklez system within 5 feet of the foundation walls. If a sprinklet system is installed, the
sprinklex heads should be placed so that watex spxay fxom the heads undet full ptessure does not fall
within five feet of the foundation walls. Lawn urigation should be controlled to pxevent excess wetting
of subsuxface soIls. Lawn, flowers, shrubs, and other plantings within five feet of the foundation walls
should be hand wateted and this watering should be nvniuvzed. If a drip irrigation system is used, one
should limit the amount of watei to sustain the plantings. One is also advised that any ixrigarion line
can leak and/or bxeak, teleasing excessive amounts of watex neaz foundations and can cause damage to
slabs and foundadon walls.
BACKFILL:
Any soIl disriubed adjacent to bearing foundation components shall be te-compacted to a**++*++**+um of
85% Standazd PTOCtoz Density. Soil suppoxting concxete slabs shall be compacted to 95% Standazd
Pxoctox Density. Mechanical compaction methods shall be utilized; water-flooding techniques aze
pxohibited. See Compaction Section fox moxe infonnation xegaxding compaction tequiuements and
techniques.
CONCRETE:
All concxete shall be Type II, Sulfate Resisrive, with 28-day strength tequixements determuied by the
foundadon design engineet.
EXTERIOR AND INTERIOR CONCRETE SLABS:
Excess moistute in the soil and 'unpxopex compaction undet slabs will incxease the possibility of slab
heave, settiement and cxacking. The buildex and futuxe owners should be awaze of and undexstand that
thexe is a definite risk of futute damage with any slab-on-gtade constnxction. If Iloor movement due to
expansive soils is not acceptable, finished zoom floox areas should be supported on wooden or steel
joists ovex a ctawl space axea, ox on a xeinforced concxete slab. If the buildet and future owners axe
willing to accept the de&nite risk of some damage due to concxete floot slab movement, the following
xecommended details cvill help pxevent, but not guasantee, wetring of the soils undemeath the slabs and
minimize damage when wetting of the soil does occut.
Do not place a gxavel layer beneath the concxete slab, unless specified by the foundadon engineer. The
slab should be placed on the undistucbed native sofls, oi well-compacted fill.

Interiox concrete slabs shall be isolated fsom the foundarion, utility lines and non-bearing pastirion
walls. Sepazate the slab fxom all load beazing members and urility lines of at least one-half inch to allow
for independent movement of the slab. Ptovide a positive control ox slip joint at the construction joint
between the slab and foundation walls
Slabs shall have conttol joints at apptoacitnately ten feet on center each way to lessen the possibility of
Tandom ctacking. These control joints may be installed as construction joints, saw cuts and/ot scoring
of the slabs. Cuts should be made at least the total thickness of the slab. A good quality concrete
sealant should be installed in these joints to ptevent suxface dischazges of water from penetrating slab
subgtades.
A min;mum void ox cleai space of one and a half inches should be provided at the bottom of all
non-bearing partitions and fuxring strips placed ovex the concxete slabs (see Floating Wall
Detail). One should also piovide a one-half inch space at the bottom of all dooz-jambs to allow
for limited movement of the floox slab. The ownex is hexeby noufied that it is theiz
tesponsibility to maintain these void spaces.
In the event a hot watet heating system is used, the pipe should not be placed beneath the
concrete floox slab. In the event a fosced au fumace is used, a two-inch collapsible connection
should be ptovided between the fumace and the heat ducts.
The so$s that will support the concrete slabs should be kept moist during construction by
occasionally spruikliug water and especially a day ot two prior to pouxing of the slab. This
ptocedute will help maintain the moistute content of the undezlying soil.
In addirion to the above xecommendations, anothec means with which to reduce the risk of
movement and cracking to slab-on-gxade construcrion would be to ovet-excavate the below
slab soil two ox moie feet, teplace with compacted structuxal fill material, pour the concTete slab
six inches thick with a Leinfotcing mat of #4/60 baxs at 18 inches each way.
RADON:
There is not believed to be any unurual hazazd from natueally occurring souxces of xadioacrivity at the
site. Howevex, most counries in Coloxado have avexage radon levels (measured in homes) above the
LJ.S. EPA xecommended "action level" of 4 picoCuries pex liter of air (pCi/1). Fot instance, TelleT
County avexages 31.94 pCi/1, Pazk County avetages 8.28 pCi/1, El Paso County avetages 4.73 pCi/l,
and Douglas County avexages 7.53 pCi/1. Results of a 1987-1988 EPA-supported radon study for
Coloxado indicate that the granitic xocks, in pazriculaz, genexally have elevated levels of uraniuxn. These
zocks have the potenrial of producing highet than avetage tadon gas levels in homes. Ptoviding
incxeased ventilarion of basements and ctawl spaces and sealing of joints can tnitigate build-ups of
xadon gas. This mirigarion is best implemented during the design and consteuction phases of
tesidences. The Coloxado Geologic Suxvey {www.dnx.state.co.us[geosurvey} and the U.S. EPA
{www.eoa.o~v/iaq/xadon} axe both good soarces for additional infonnarion xegarding xadon.

COMPACTION:
If compaction of the narive or boLrow soils is tequited, it is highly tecommended that you pick up an
informative btochure pxovided at any Coloxado Engineering and Geotechnical Gxoup, Inc. office. In
genexal, to compact clayey or silty materials:
Placing Fill.• No bxush, sod, frozen material, or othex perishable or unsuitable material, or stones
of fout inches ot gzeatex in maximum dunension shall be placed in the fill. The distribudon of
the matenal on the fill shall be such as to avoid the fortnarion of lenses, or layets, of material
differing substantially in chazacteiistics from the sucrounding material. 'I'he materials shall be
deliveted to the backfill suxface at a uniform zate, and in such quantity as to pexmit a
sausfactoiy construcrion proceduce. IInnecessary concenteation of ttavel tending to cause ruts
and othex hollows moxe than six inches in depth, shall be regazded and compacted. Aftex
dumping of 6ll material on backfill suxfaces, the material shall be spread by appzoved methods
in appzoxixnately 6 inches compacted thickness.
Moirture Contmd.• The material in each layex shall be compacted by xolling and shall contain the
optimum moisture tequixed fot maximum compaction, as neaxly as pxacricable and as
detezinined by the soIls engineer. The moishxte content shall be unifortn thxoughout the layeLS.
The contcactor may be required to add the necessaxy moistute to the backfill material in the
bortow atea, If in the opuuon of the soils engineei, it is not possible to obtain moistuce content
by adding watex on the fill suxface.
Compactian: When the moisture content and condirion of each sptead layex is satisfactory, it
shall be compacted by a method apptoved by the soils engineei to 90% ASTM D698 (Standaxd
Pioctox Density) fox slab aseas, and 100% ASTM D698 for footing and/or pad azeas. A
Standazd Pxoctox test shall be pesformed fox each typical fill material and frequent tests of the
density of the fill must be taken. Noimally, eight passes of a sheepsfoot xollex loaded to 4,000
pounds pex lineal foot on a suc inch soil layez is sufficient to achieve 90% ASTM D698.

When compacting cohesionless free-dtaining materials such as sands and gravel, the materials
shall be deposited in layers and compacted by tteads of a ctawlex type ttactot, sutface of internal

appxoved by the soils engineex. The thickness of the horizontal layex aftet compacrion shall not

an approved testing facility. Colozado Engineering & Geotechnical Group, Inc., if zequested, will
perform this test(s) for you, provided a sample of the material(s), 5-gallon bucket(s) fixll, be supplied to
Colocado Engineering & Geotechnical GLOUp, Inc. a rninimutn of 72 hxs priox to testing. The
sample(s) will be used to determuie the maximuxn proctox density and optimum moistute content for
that representarive sample. Additional chatges will be incuned for each sample tested and for
field compaction testing opetations if required.
