Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Safety:
• Set up for re -roofing operation as approved by the Property Owner.
■ Provide all necessary protection needed to guard all walls and adjacent structures.
• Porta potty and dumpster will be placed in an agreed upon area on premises. (If needed)
• Set up all required safety lines and guard rails as required by OSHA requirements. .
• All power tools on site will have adequate cords with no rips or tears whatsoever.
Tear Off:
• Remove and dispose of ALL layers of existing roofing materials on all roof sections.
■ Remove and dispose of all existing underlayment.
• Lower all debris from the roof surface in an approved manner. Protect any building faces where needed to assure that
no damage occurs.
Installation of Complete Roof Systems:
• Install fully adhered 60 mil, Veirsgard EPDM rubber
• Install ISO rigid foam insulation as required by energy code. This will include raising roof edges as needed to
accommodate ISO rigid foam insulation.
• Install tapered insulation to provide positive drainage at all roof edges
• Install fiber board or gypsum board as required by manufacture to meet min fire code requirements.
• Flash all walls, roof edges, and roof projections as required by manufacture and code.
• Remove and replace all gutters & downspouts
Law and Ordinance:
• Permits and fees are to be determined by the appropriate building jurisdiction and will be and the cost will be in
addition to this proposal
• All proposed materials, installation procedures and specifications are subject to the building jurisdiction's approval.
Any and all specifications that are changed by any jurisdiction official will result in a written change order.
Exclusions:
• Sheathing/ roof decking that needs to be replaced as part of re -roof project will be an added expense to this
proposal. This expense, if needed, will be invoiced at $2.12/sf. I do not anticipate that we will have to
replace any decking as we will be laying insulation and gypsum board over existing decking and these
items should work to bridge any small areas of inadequate decking.
• Fascia, soffits or trim. These areas are not included in this proposal. If there are areas that homeowner
would like to address these items can be added as a change order.

THE CONTRACT ON THE FACE HEAROF AND ANY AGREEMENT MADE PURSUANT THERE TO BETWEEN ELITE ROOFING (THE
"COMPANY") AND THE CUSTOMER(S) (THE "CUSTOMER") WILL BE SUBJECT TO ALL APPROPRIATE LAWS, REGULATIONS AND
ORDINANCES, AND TO THE FOLLOWING SPECIAL TERMS AND CONDITIONS:
1. All contracts subject to approval of Management & our Credit Department without exception.
2. ALL CHECKS FOR PAYMENT MUST BE MADE PAYABLE TO ELITE ROOFING. THERE WILL BE NO EXCEPTIONS. BUILDING OWNER
WILL STILL BE LIABLE TO ELITE ROOFING FOR PAYMENTS OF THIS CONTRACT IF CHECK 15 MADE OUT TO ANY OTHER ENTITY OR
PERSON.
3. If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of remaining
provisions of this agreement shall not be affected thereby.
4. The customer further agrees that the equity in this property is security for the contract.
5. The Company shall not be liable for failure of performances due to labor controversies, strikes, fire, weather, inability to
obtain material from usual sources, or any other circumstances beyond the control of the Company, whether of a similar or
dissimilar nature.
6. Should default be made in payment of this contract, charges shall be added from date thereof at a rate of one and one half
percent per month (18% per annum) with a minimum charge of $2.00 per month, and if placed in the hands of an attorney for
collection, all attorney's fees and legal filling fees shall be paid by Customer accepting said contract.
7. This contract cannot be cancelled once insurance negotiations begin or work is commenced except by mutual written
agreement of the parties.
8. You have the right to rescind this contract and obtain a full refund of any deposit within 72 hours of entering the contract or
72 hours after you receive written notice that your claim has been denied.
9. In addition to paragraph 8, if material has to be reordered or restocked because of cancellation by the customer, there will
be a restocking fee equal to fifteen percent (15%) of the contract price.
10. Replacement of deteriorated decking, roof jacks, ventilators, flashing, or other materials, unless otherwise stated in this
contract are not included and will be charged as an extra on a time and material basis.
11. The Company's Limited Warranty does not cover damage to roofs caused by lightning, gale (50 MPH), hurricane, tornado,
hailstorm, impact of foreign object or other violent storm.
12. The Company is not responsible for any damage below the roof due to leaks by excessive wind, ice, or hail during the period
of the warranty. The Company's warranty covers the roof only.
13. The Company will not be held liable for any structural movement, settling, cracks in dry -wall, cracking or breaking of
driveway during the roofing or loading process, minor scratching or denting of rain gutters.
14. All pictures, wall hangings, lights, collectables etc., both indoors & outdoors MUST be secured by homeowner. Any damage
to personal items during installation will not be the responsibility of the Company. The Company will also not be responsible for
nail damage to tires, pets, etc. Pets should not be left unattended during installation.
15. Any alteration or deviation from specifications involving extra cost will be executed only upon written orders, and will
become an extra charge over and above the estimate.
16. The customer is to carry fire, tornado and other necessary insurance.
17. This contract and labor warranty shall not be assigned and are nontransferable unless specified in writing.
18. The Company shall revoke labor warranty if contract is not paid in full.
19. This contract constitutes the entire understanding of the parties, and no other understanding, collateral or otherwise, shall
be binding unless in writing signed by both parties.
20. This agreement is composed of front and back page and all materials incorporated herein by reference and shall be
considered the entire agreement by the parties. Any representation, statements, or other communications not written in this
contract are agreed to be immaterial, and not relied on by either party, and do not survive the execution of this contract.
21. The Company has the right to order excess materials. All excess materials belong to the Company
