Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TERMS

property owner's best Interest for property replacement or repair at a price agreeable to the property owner's insurance policy and the
contractor with no additional cost to the property owner other than the deductible of the insurance. When an agreeable price has

TERMS AND CONDITIONS
THIS CONTRACT BETWEEN LIFETIME WINDOWS AND SIDING ("THE COMPANY"), ROOFING DIVISION AND THE
CUSTOMER(S) ('THE CUSTOMER") WILL BE SUBJECT TO ALL APPROPRIATE LAWS, REGULATIONS AND ORDINANCES
AND ALSO WITH THE FOLLOWING SPECIAL TERMS AND CONDITIONS:
1. This contract is subject to the approval of the company's management and credit department, without exception.
2. All checks for payment must be payable to LIFETIME WINDOWS AND SIDING, no exceptions. Homeowner will still be
liable to Lifetime Windows and Siding for payment of this contract if check is made out to any other person or entity.
3. If any provisions of this agreement should be held to be invalid or unenforceable, the validity and enforceability of the
remaining provisions of this agreement shall not be affected.
4. The company shall not be liable for the failure of performance due to labor controversies, strikes, weather, fire, inability
to obtain material from unusual sources, or any other circumstances or Acts of God beyond the control of the company.
5. If the customer defaults in payment of this contract, charges will be added to the date of default at a rate of 1.5% per
month (18% annually). If the account goes to collections, the company will be entitled to collect all collection fees, attor-
ney's fees, legal filing fees and costs expended in enforcing the collection of any charges payable under the contract.
6. If this contract is canceled by the customer after 72 hours from receipt of the written notice identified in 6-22-104, or if
the customer cancels the contract later than 72 hours for a reason other than the receipt of the written notice in 6-
22-104, the customer shall pay the company 20% of the contract price as liquidated damages, not as penalty, and the
company agrees to accept this amount as a reasonable and just compensation for cancellation and the efforts that the
company expended in relation to the contract.
7. If material has to be reordered or restocked because of a cancellation by the customer, other than permitted by 6-
22-104, the company will charge a restocking fee equal to 15% of the contract price.
8. The contract does not include the replacement of deteriorated decking, ventilators, roof jacks, flashing or other materials
unless they are included in the contract and then will be charged on a time and material basis.
9. Lifetime warrants its workmanship for the lifetime of the original customer.
10. The work shall conform to roofing industry standards for workmanlike performance. The Contractor's warranty excludes
damage or defects caused by alterations to the work, improper or insufficient maintenance, and normal wear and tear.
11. The company is not liable for any structural movement of the walls or foundation, settling, cracks in drywall, breaking or
cracking of the driveway during the roofing or loading process, or minor scratching or denting of rain gutters,
12. The company is not responsible for any damage below the roof due to leaks caused by excessive winds, ice, or hail during
the period of the warranty. The company's warranty covers the roof only.
13. All pictures, wall hangings, lights and collectibles, both outdoors AND indoors must be secured by the customer. Any
damage to personal items during installation will not be the responsibility of the company. The company will not be
responsible for nail damage to people, pets, animals, tires or property.
14. Any alteration or deviation from the scope of work outlined in the contract that involves extra cost will be executed only
after written and signed orders and will become an extra charge over and above the estimate.
15. The customer agrees to be solely responsible for carrying the necessary insurance, including, but not limited to fire,
tornado, flood.
16. The contract and labor warranty are non-transferrable unless specified in writing and signed by both parties.
17. The customer will not receive a warranty of any kind (labor, warranty or manufacturer) if the contract is not paid in full.
18. Any discounts, referral fees, sign placement discounts will be revoked if the contract is past due.
19. This contract constitutes the entire understanding of both parties, and no understanding, written or otherwise, will be
binding unless it is in writing and signed by both parties.
20. This contract is composed of front and back pages, all materials incorporated herein by reference shall be considered the
entire agreement by both parties.
21. The company has the right to order excess materials; all materials belong to the company.
22. The company reserves the right to require the customer to file for supplemental insurance claims if the insurance adjust-
er measurements are used and prove to be incorrect, the company discovers unknown conditions or due to an increase
in costs.
23. The customer has the sole responsibility to provide a safe area for pets to remain during the work, the company is not
responsible for gates left open during or after work done by the company.
24. The final payment will not be delayed while waiting for the city, county or other building inspection departments to
inspect the work.
25. Customer is solely responsible for the Homeowner's Association (covenant approval) of roofing products and color.
26. Company is not responsible for satellite dish realignment.

(1) This permit was issued in accordance with the provisions set fiorth in your application and is.subject to the laws of the State of
Colorado and to the Zoning Regulations and Building Codes of W heal Ridge, Colorado or any other applicable ordinances of
the City.
(2) This permil shall expire if (A) the work authorized is not commenced within sixty (60) days from issue date or (B) the building
authorized is suspended or abandoned for a period of 180 days.
(3) If this permit ezpires, a new permil may be acquired for a fee of one-half the amount normally required, provided no changes
have been or will be made in the original plans and specifcations and any suspension or abandonment has not exceeded one
(1) year. If changes have been or if suspension or abandonmen[ exceeds one (1) year, full fees shall be paid for a new permit.
(4) No work of any manner shall be done that will change the natural Flow of water causing a drainage problem.
(5) Contractor shall notify the Building Inspector lwenty-four (24) hours in advance for all inspections and shall receive written
approval on inspection card before proceeding wi[h successive phases of [he job.
(6) The issuance of a permit or lhe approval of drawings and specifications shall not be construedto be a permit for, nor an
approval of, any violation of [he provisions of the building codes or any other ordinance, law, rule or regulation. All plan review
is subject to field Inspections.
