Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed ¢'
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process C,

Special Instructions
The approximate cost of the services based on damages known at the time this contract is entered is $12446.58 determined by State Farm
insurance. Monarch will perform the scope of work approved by the insurance company. Monarch will supplement for items priced low, not included or items needed to pass city/county
codes, which may increase initial dollar amount that insurance is paying out. Supplemental claims billed by Monarch that are approved by the insurance company for additional work or
increases in costs become a part of this contract. The final scope and puce agreed on between the insurance company and Monarch shall become the final contract price for the insurance
approved scope. Customer will be responsible to pay for any upgrades; deductible, non-recoverable deprecation or additional work authorized by the customer that the
insurance company determines is not covered within the scope of coverage provided by your insurance policy.
Approximate Dates of Service: Beg july these dates of service are subject to change due to unusual delays in transportation of materials,
payment delays, insurance approval, ordinary weatherdelays, or by any other causes beyond reasonable control of Monarch. Monarch will provide communication to customer
to show tentative dates.
Notice: If the property owner plans to use proceeds of a property and casualty insurance policy issued pursuant to pad 1 of article 4:10, C.R.S., to pay for the roofing work, pursuant to
section 6-22-105, the roofing contractor cannot pay, waive, rebate, or promise to pay, waive, or rebate all or pad of any insurance deductible applicable to the insurance claim for payment
on the covered residential property.

adopted by the Fire Boazd of the Wheat Ridge Fire Protection District and the City

SUBJECT: Items to be completed/New Single- or Two-Family Structures

Listed below aze a few items to consider while finalizing construction of a single-or two-family
structure. Before a certificate of occupancy can be issued for a new single-family or two-family
structure, the following items must be complied with.
1. If a driveway is proposed, the first 25 feet of drive surface azea must be paved. The
driveway may be surfaced with asphalt, concrete, brick pavers or similaz materials. The
driveway must abut the exis6ng edge of asphalt on the street, which may require paving
within the right-of-way. If paving occurs within the right-of-way, a right-of-way pernvt is
required from the Public Works Department.
2. One street tree must be installed for every 70 feet of street frontage. If the structure is a
corner lot with frontage on two streets, each frontage is subject to this provision. These
street trees must be at least 2 inch caliper specimens.
3. One hundred percent of the front yazd must be landscaped, excluding driveways and
sidewalks. The front yard is defined as the first 30 feet from the front property line
adjacent to the street.
4. At least 25 percent of the entire lot must be landscaped. The front yazd landscaping may
be counted when calculating this figure.
5. No more than 50 percent of the landscaped area may be comprised of turf. Low water turf
(such as buffalo grass, blue grama grass and tall fescue) is encouraged.
6. No more than 35 percent of the landscaped azea may be non-living material, such as rock
or bazk. Living materials planted within rock or bazk beds may be excluded from the non-
living calculations.
7. If the landscaping cannot be completed due to inclimate weather, an escrow must be
submitted for the estimated cost of landscaping installation and quantities. The escrow
shall be in the form of 125% of the value of the required landscaped azea.
8. It is possible that the Public Works Department may require either construction of curb,
gutter and/or sidewalk or that an escrow be submitted for the full amount of these public
improvements. It would be beneficiai to contact the Public Works Department to discuss
the potential for public improvements.
9. Any fence, outbuilding, shed or detached gazage will require a separate building permit.

I have completed my review of the building permit application for the proposed single family
residence located at 4569 Newland Street. The building pernut application can be approved;
however, you should be aware of the following:
1. The legal address for the property is currently 4501 Newland Street.
2. I have included an attachment which describes the items to be completed before the
Pianning Division can approve a certificate of occupancy.
3. It appears that the proposed new single family residence will be located on the eastern
half of the property. The submitted site plan shows the eastem half of the property
labeled as "lot 16". Per Jefferson County records, this lot currently exists as 4501
Newland (lot 14, Thayer Subdivision). It is the assumption of staff that half of the
property was sold to another buyer, which constitutes an illegal subdivision. In the City
of Wheat Ridge, all property divisions, or "subdivisions" must follow Article IV of the
Code of Laws. This Article states that a subdivision plat must be submitted and
ultimately approved by Planning Commission.
If the subdivision application is pursued, a public hearing before the Planning
Commission will be required. The following items must be submitted with an
application for subdivision: the applicable land use fee ($950.00), 18 full sized (24x36)
copies of the subdivision plat prepared by a licensed surveyor, a copy of the deed for the
property and a letter of explanation. If the plat is approved, a mylat copy must be
recorded at the Jefferson County Clerk and Recorder's office. You can expect the
process to take approximately 3 months. It is possible that pubiic dedication will be
required, which in tum would require a City Council hearing. You can expect the need
for a City Council hearing to add one month to the process time, and add $300 to the
submittal fee.
The submitted building permit application can be approved, provided that if another
building pemut application is submitted for the western half of the property, a
subdivision plat must be processed and approved prior to issuance of building permit.

See the enclosed report from Quantem Labs in Oklahoma City, OK Samples
were examined by polarized light microscope by normal reading and by point
wunt methods (3 samples).
Results and Discussion:
The patch texture (used like drywall mud) on the N. living room wall contained
a regulated level of asbestos. This material will have to be removed before the
house is demolished or used for fire department training.

MINOR GRADING EXCAVATION AND FILL PERMITS
1. 1-50 Cubic Yards: No permit is required for dumping or excavation of earth materials, which do not exceed
50 cubic yazds, provided, however, [hat any fill deposited is on natural terrain of less than three (3) honzontal
to one (1) vertical slope, or such fill is less than tku'ee (3) feet in depth and is not intended to support
permanent structures, and in addition, sucb fill or excavation does not obstruct or otherwise adversely affect
any drainageway. Should any of the above standards be exceeded, or a drainage way be affected, a permit
sball be required under tbe guidelines for No. 2.
2. 51-500 Cubic Yards: Dumping or excavation of earth materiais not exceeding 500 cubic yazds may be
allowed with a permit approved by the City Engineer. All applications aze to be submitted on the appropriate
completed application form and shall be accompanied by the appropriate fee and sketch plan indicating the
following information before the permit will be issued.
A. Location and dimensions of ali property boundaries and sWctures on the site.
B. Location and extent of azeas to be filled and/or excavated.
C. Location of existing and proposed drainageways, irrigation ditches, etc., and indicarion of Low and where
historic run-off will be maintained on and through the site.
D. Cross-secrion azea to be filled and/or excavated indicating original slope, new slope and depth of fill.
E. Statement that indicates the proposed use or purpwse for said fill or excavarion.
F. Relative elevation of adjacent properties.
G. Erosion control plan showing placement of control devices such as bay bales, etc.
3. 501-10,000 Cubic Yards: Operations in which earth material fill or excavation exceeds 500 cubic yazds, but
does not exceed 10,000 cubic yazds, may be allowed by a permit issued by the Ciry Engineer after review of
an application and supporting information. 1'he following information shall be submitted with tbe required
application form:
A. A site plan, prepazed and signed by a Colorado registered professional engineer, at a scale of no less ihan
1 inch to twenty feet (1:20) which illustrates the following:
B. Location and dimensions of all property boundaries and structures on the site.
C. Location and extent of areas to be filled and/or excavated.
D. Location of existing waterways and drainage courses indicating any changes. (for a site containing an
established irrigation ditch, a letter of approval &om the appropriate ditch company shall be required).
E. Location of existing and proposed points of ingress.
F. Location and extent of existing vegetation, proposed changes in such vegetauon and methods of
rebabilitation on site vegetation aRer earthwork operations aze complete.
G. Erosian control plan.

intervals of 2 feet and spot elevations.

4. Performance Standards: The following provisions shall apply to all Minor Grading, Excavation and Fill
Pemvts:
A. Rehabilitation: Within tUirty (30) days after cessation of filling and/or excavation, rehabilitation for the
site shall have been completed in accordance with the approved plaos. Rehabilitation shall consist of
leveling, grading, landscaping or any combination thereof to minimize potential erosion and be acceptable
to the City Engineer.
B. Debris: Debris and/or contaminanis shall not be used except with the approval of the City Engineer.
"For the purpose of this Section, the term debris shall have the same meaning as the te[ms "gazbage, lrash
or junk" as defined in tbe VJheat Ridge Code of Laws, Section I S-4."
In no case shall debris or contaminates identified or classified ac hazardous waste by local, state or federal
agencies be used as fill in any fill azeas within the City of Wheat Ridge.
C. Excavation and/or fill azeas shall be graded to facilitate weed control until final grades aze set and site

Dear Mr. Forrest:
The City of Wheat Ridge informed the District thal Zach Magalei was planning to demolish ihe residence
of 4501 Newland Street, Wheat Ridge, Colorado 80033 and rebuild two (2) new homes. The City
indicated that Zach Magalei was the owner of the home. Afrer further research, we found you to be the
owner. Please respond to the District when you plan to have the disconnection of your service inspected.
This letter is in response to your request for confirmation of the availability of sanitary sewer service for
the above parcel. By supplying this information, the Wheat Ridge Sanitation District is not liable for the
misidentification or misrepresentation of the subject lot. The location of the subject lot is based solely on
the description provided by the requestor, and the Wheat Ridge Sanitation District is not responsible for
the way in which this information is used or disseminated.
The subject lot at the address referenced above is entirely within the boundary and service area of the
Wheat Ridge Sanitation Dishict. Treahnent of sewage generated within the Wheat Ridge Sanitation
District is provided by the Metro Wastewater Reclamation District (Metro). Wheat Ridge Sanitation
District facilities in this azea consist of an 8-inch sanitary sewer main within Newland Street east of the
subjectproperty.
This parcel is subject to the assessment ofboth Wheat Ridge Sanitation District and Metro tap charges.
The sub-dividers and/or developers may be required to extend sewer lines to their project sites and/or
share in the cost of providing additional capacity to serve their projects.
In addition, you should be aware that the District requires approval of plans For sewer main

At this time the inspection of the demolition construction has not been completed. The District requires
this inspection before any future conshvction is conducted on the service lines proposed. The
disconnection of sewer service is required to be disconnected at the main with a mechanical plug encased

Dear Mr. Magalei:
I have completed my review of the building permit application for the proposed single family
residence located at 4569 Newland Street. The building pernvt application can be approved;
however, you should be aware of the following:
1. The legal address for the property is currendy 4501 Newland Street.
2. I have included an attachment which describes the items to be completed before the
Planning Division can approve a certificate of occupancy.
3. It appears that the proposed new single family residence will be located on the eastern
half of the property. The submitted site plan shows the eastem half of the property
labeled as "lot 16". Per Jefferson County records, this lot currently exists as 4501
Newland (]ot 14, Thayer Subdivision). It is the assumption of staff that half of the
property was sold to another buyer, which constitutes an illega] subdivision. In the City
of Wheat Ridge, all property divisions, or "subdivisions" must follow Article IV of the
Code of Laws. This Article states that a subdivision plat must be submitted and
ultimately approved by Planning Commission.
If the subdivision application is pursued, a public hearing before the Planning
Commission will be required. The following items must be submitted with an
application for subdivision: the applicable land use fee ($950.00), 18 ful] sized (2406)
copies of the subdivision plat prepared by a licensed surveyor, a copy of the deed for the
property and a letter of explanation. If the plat is approved, a mylaz copy must be
recorded at the Jefferson County Clerk and Recorder's office. You can expect the
process to take approzimately 3 months. It is possible that public dedication will be
required, which in turn would reyuire a City Council hearing. You can expect the need
for a City Council hearing to add one month to the process time, and add $300 to the
submittal fee.
The submitted building permit application can be approved, provided that if another
building permit applicarion is submitted for the western half of the property, a
subdivision plat must be processed and approved prior to issuance of building permit.

structure, the following items must be complied with.
L If a driveway is proposed, the first 25 feet of drive surface azea must be paved. The
driveway may be surfaced with asphalt, concrete, brick pavers or similar materials. The
driveway must abut the existing edge of asphalt on the street, which may require paving
within the right-of-way. If paving occurs within the right-of-way, a right-of-way permit is
required from the Public Works Department.
2. One street tree must be installed for every 70 feet of street frontage. If the structure is a
corner lot with frontage on two streets, each frontage is subject to this provision. These
street trees must be at least 2 inch caliper specimens.
3. One hundred percent of the front yazd must be landscaped, excluding driveways and
sidewalks. The front yard is defined as the first 30 feet from the front property line
adjacent to the street.
4. At least 25 percent of the entire lot must be landscaped. The front yazd landscaping may
be counted when calculating this figure.
5. No more than 50 percent of the landscaped azea may be comprised of turf. Low water turf
(such as buffalo gass, blue grama grass and tall fescue) is encouraged.
6. No more than 35 percent of the landscaped azea may be non-living material, such as rock
or bark. Living materials planted within rock or bazk beds may be excluded from the non-
living calculations.
7, ff the landscaping cannot be completed due to inclimate weather, an escrow must be
submitted for the estimated cost of landscaping installauon and quantities. The escrow
shall be in the form of 125% of the value of the required landscaped area.
8. It is possible that the Public Works Department may require either construction of curb,
gutter and/or sidewalk or that an escrow be submitted for the full amount of these public
improvements. It would be beneficial to contact the Public VJorks Department to discuss
the potential for public improvements.
9. Any fence, outbuilding, shed or detached garage will require a separate building pemiit.

This letter is in response to your request for confirmation of the availability of sanitary sewer service for
the above parcel. By supplying this information, the Wheat Ridge Sanitation District is not liable for the
misidentification or misrepresentation of the subject lot. The location of the subject lot is based solely on
the description provided by the requestor, and the Wheat Ridge Sanitafion District is not responsible for
the way in wluch this information is used or disseminated.
The subject lot at the address referenced above is entirely within the boundary and service area of the
Wheat Ridge Sanitation District. Treatment of sewage generated within the Wheat Ridge Sanitation
District is provided by the Metro Wastewater Reclamation District (Metro). Wheat Ridge Sanitation
District facilities in this azea consist of an S-inch sanitary sewer main within Newland Street east of the
subject property.
This parcel is subject to the assessment of both Wheat Ridge Sanitation District and Metro tap chazges.
The sub-dividers and/or developers may be required to extend sewer lines to their project sites and/or
share in the cost of providing additional capacity to serve their projects.
In addition, you should be aware that the District requires approval of plans for sewer main
construction, sewer service connections, determination and payment of tap fees, as well as
inspection of construction by the District's Engineer.
At this time the inspection of the demolition construction has not been completed. The District requires
this inspection before any future construction is conducted on the service lines proposed. The
disconnection of sewer service is required to be disconnected at the main with a mechanical plug encased
in concrete according to District Rules and Regulations. In addition, the District will require tap
inspections of the proposed service lines.
