Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

The terms and conditions found on this page are an express port of the Agreement between you and INCLINE ROl FIIVG. Please rend
this page carefully.

1. All work to be done by INCLINE ROOFING is outlined with specificity on the front of this Agreement ("Work") and includes all
labor, materials, equipment, apparatus, tools, transportation and services necessary for, and incideapi to, the proper
installation and completion of the Work.
2. All Work will be completed in compliance with applicable municipal and state codes. INCLINE ROOEfNG will obtain all
necessary permits for the Work. INCLINE ROOFING, license information is available upon request.
1 INCLINE ROOFING will perform all Work in a workmanlike manner according to standard practices. All trah will be removed
from the yard, gutters and roof. Magnets will be used to comb the yard for nails, staples and other metailicidebris.
4, Grass, shrubs, plants, pools, yard fixtures, etc. will be covered by plywood and/or tarps as needed. Nstwithstanding the
foregoing, you acknowledge that given the nature of root & siding replacement; damage to grass, shrubs,) plants, pools, yard
fixtures, etc. often occurs. You agree that INCLINE ROOFING is not responsible for any loss or damage to any grass, shrubs,
plants, pools, yard fixtures or other property in the yard. i
S. In the course of completing the Work, INCLINE ROOFING may discover pre-existing conditions, mold, underlying damage or
other defects or damage to the property. You agree that fixing any such issues is outside the scope fof Work. INCLINE
ROOFING will identify any such situations it encounters and upon your request, provide an estimate to fix such issues.
6. You agree to pay the "Agreed Price" set forth on the front of this Agreement for the Work performed by INCLINE ROOFING
You will pay the first deposit upon execution of this Agreement and promptly pay INCLINE ROOFING any and all monies you
receive from your insurance company relating to the Work, including amounts paid as general contractor", overhead or profit
and/or cost increases. The balance will be due promptly upon completion of the Work. You will be respo+ible for all costs of
collection, including reasonable attorneys' fees and court costs. Overdue payments will incur interest at 1.S/month.
7. In certain circumstances, the insurance company's estimate may be revised after INCLINE ROOFING estimates the Work. In
such circumstances, you may receive additional payments from your insurance company for the increased 4stimate. These are
referred to as "Supplements". You agree to pay to INCLINE ROOFING all Supplements you receive the Work and/or trades
done by INCLINE ROOFING under the Agreement.
B. INCLINE ROOFING will not be liable for any delays due to weather, strikes, material availability or any other reason beyond its
control.
The Agreement: (a) is governed by Colorado Law; lbi embodies the whole and complete agreement of the parties; and (e)
supersedes all discussions, representations, oral agreement and negotiations between the parties. ,Any modifications,
amendments, change orders, alterations or supplements to the Agreement must be made in writing and sfgned by authorized
representatives of each party. INCLINE ROOFING will carry appropriate liability insurance and workers corm. insurance.
9. THERE ARE NO WARRANTIES THAT EXTEND BEYOND THOSE SET FORTH IN THIS AGREEMENT. EXCEPT AS PROHIBITED BY LAW,
INCLINE ROOFING DISCLAIMS ALL EXPRESS AND IMPLIED WARRANTIES, INCLUDING ANY WARRANTY OF MERCHANTABILITY
AND ANY WARRANTY OF FITNESS FOR A PARTICULAR PURPOSE. INCLINE ROOFING AGGREGATE LABILITY FOR ANY AND ALL
CLAIMS OR DAMAGES UNDER THIS AGREEMENT IS LIMITED TO THE AMOUNT PAID UNDER 'THIS AGREEMENT.
NOTWITHSTANDING THE FOREGOING, INCLINE ROOFING, WILL NOT, IN ANY EVENT, BE LIABLE TO YO�"ti� FOR ANY SPECIAL,
INDIRECT, CONSEQUENTIAL OR INCIDENTAL DAMAGES IN CONNECTION WITH OR OTHERWISE ARI..NGi OUT OF THIS

Preparin&for V"r roof replacement
During your roof replacement, the exterior of your home will be considered a "Construction Zone". INCLINE ROOFING
will take all necessary steps to ensure that your job runs smoothly. Below is a check list to help you prepare for the job.
It is the homeowner's responsibility to meet the requirements outlined below.
• During your roof replacement, slight shaking may occur inside of your home. Any glass, antiques, art work,
photographs or any other items, either hanging or touching the walls, should be removed before work begins.
• It is likely that soot will fall In your attic. Cover your exposed property with old sheets, towels or tarps before
work begins.
• The dumpster will be delivered to your home the day before or the morning of your roof replacement. if you
job is scheduled for Monday, it could be delivered as early as the Friday before. Remove your vehicle(s) from
the garage / driveway prior to delivery.
• When shingles are removed from a roof, all loose materials will fall to the ground. Any outdoor furnishings,
fountains, ceramic pots or any other breakable items should be removed and stored in a safe place.
• A thorough clean-up of your homes exterior will be done when your roof replacement is complete. Adults,
children and pets should not play or walk in the work zone until the clean-up is complete.
• Take photos of your drive way before the dumpster/materials are delivered
• Place a cover or tarp over your pool.

• Jobs may be delayed at times due to weather conditions or problems with material deliveries. Your Project
Manager will keep you informed should your job be rescheduled.
• Your insurance company will not cover the cost of rotted decking. We cannot determine if/or how much rotted
wood is present until the shingles are removed from the home. if decking needs to be replaced, your Project
Manager will provide you with photos and cost to do the work. This is not included in the cost of your roof
replacement. i
• Any loose nails or debris around your property will be cleaned up immediately upon completi6n of the job. Alt
left over materials will be picked up with -in 48 hours of job completion.
• Allow 1 to 3 business days for removal of the dumpster,
Permits — Some cities require permits for roof replacements. Review the below if this applies In your

• Only sign the areas that are marked or highlighted for your signature.
• Do not fill out any areas of the permit application. This is handled by the contractor.
• Permits must be posted on the exterior of the home and cannot be removed until the "in pr rens" and "final"
inspections have been passed by the city or county.
• A driver's license, state identification card or passport may be required when signing the per4t application.
You acknowledge that given the nature of roof & siding replacement damage to gross, shrubs, plants,ols, yard fixtures,
driveways, lightingetc, often occurs. You agree that JA Edwards of Americo Inc. is not responsible foran oSS or domage to
